자기공명영상사진 상에 나타난 측두하악관절원판의 형태 및 그에 따른 치료 효과의 분석 (Configuration of Temporomandibular Joint Articular Disc in Magnetic Resonance Images and its Relationship to Treatment Response of Anterior Disc Displacement Without Reduction)

  • The purposes of the study were (1) to see if there are various types of disc configuration in normal temporomandibular joint and, if so, (2) to examine whether a specific type of disc configuration is susceptible to disc displacement, and (3) to see if a specific type of disc configuration and position is related to early relief of clinical symptoms from the anterior disc displacement without reduction to conservative treatment. From the magnetic resonance (MR) images taken for the patients with symptoms of temporomandibular disorders, 235 images as normal joint group, 255 images as anterior disc displacement with reduction group, and 249 images as anterior disc displacement without reduction group were selected, After the TMJ image was scanned and processed with Photoshop program, the disc configuration was determined, The incidence of various types of disc configuration was analyzed according to age, sex, diagnostic group, and the promptness of symptom relief. The disc position was also examined in relation to the promptness of symptom relief. The results were as follows : 1. In the normal joint group, biconcave type appeared most frequently and reversed, biplanar type in order, Posterior band enlarged and folded type didn't appear at all. On the other hand, in the anterior disc displacement without reduction group, folded type appeared most frequently and also posterior band enlarged type were found often. 2. There were statistically significant differences between sex and configuration of disc in the normal joint group and no statistically significant differences in the anterior disc displacement with reduction group and anterior disc displacement without reduction group, 3. There were no statistically significant differences between age and configuration of disc in the normal, anterior disc displacement with reduction group and anterior disc displacement without reduction group. 4. In anterior disc displacement without reduction group, rapid response was observed in biconcave and reversed type and delayed response was observed in folded type.

  • Objective: Disc displacement may affect the joint space narrowing between condylar head and glenoid fossa. This study was designed to evaluate the correlation between the joint space change and the severity of anterior disc displacement. Materials and Methods: Two hundreds temporomandibular joints MR images of TMD patients(170 joints) and asymptomatic volunteers(30 joints) were evaluated for this purpose. Anterior disc displacement was divided into 3 stages(normal, little to mild, and moderate to severe displacement) based on sagittal images, then joint spaces were measured at medial, central and lateral parts of condyle head on coronal MR images, respectively The joint spaces of 3 groups divided according to the severity of anterior disc displacement were compared. Results: The reduction of joint space was related to the severity of the anterior disc displacement at lateral, medial side and especially at center Conclusion: The temporomandibular joint space was affected by the severity of the anterior disc displacement.

  • Recently, rapid palatal expansion technique is widely used for the correction of the skeletal imbalance in Cl III malocclusion patients. There were many studies about the cephalometric changes to rapid palatal expansion but quantitative analysis were small. The purpose of this study was to analysis the stresses and displacement of the maxilla in human dry skull to rapid palatal expansion. The results were as follows: 1. The anterior portion of palate show more lateral and inferior displacement than the posterior portion. But the posterior portion show more anterior displacement. 2. In transpalatal suture area, the medial portion show more anterior and inferior displacement than the lateral portion. But the lateral portion show more lateral displacement than the medial portion. 3. In mid-sagittal plane, the lower portion (palatal area) of maxilla show more anterior, lateral, inferior displacement than the upper portion (frontamaxillary stuture area). 4. In zygomatic arch, the adjacent area to maxilla show tonsil. stresses and the adjacent area to frontal bone show compressive stresses. 5. The sequence of stress bearing area to R.P.E. is upper retromolar area, upper 1st molar, 1st premolar, 2nd premolar, anterior segment of teeth.

측두하악관절내장 환자의 진단분류에 따른 하악운동 특성의 분석 (Analysis of the Mandibular Movements in Patients with Internal Derangement of the Temporomandibular Joint According to Diagnostic Subgroups)

  • The purpose of this study was analyse the mandibular movements in patients with internal derangement of the temporomandibular joint according to diagnostic subgroups. The author classified patients with internal derangement of the temporomandibular joint into 4 diagnostic subgroups by means of the magnet resonance imagings, and evaluated the clinical signs and the mandibular movements with Mandibular Kinesiograph(MKG) in each subgroups. The mandibular movements, measured in this study, were the types of movement in frontal and sagittal plane, velocities in opening and closing movement, and the opening and closing movement, and the opening and closing velocity pattern. The data were compared between the 5 groups including the normal group. The results were as follows : 1. Pain was more frequently observed in the anterior disc displacement without reduction group than in the anterior disc displacement with reduction group. Sound of joint was more frequently observed in the anterior disc displacement with reduction group, and limitation of mandibular opening movement was more frequently observed in the anterior disc displacement without reduction group. Duration of the anterior disc displacement without reduction group was significantly short compared to that of the anterior disc displacement with reduction group, and duration of the unilateral anterior disc displacement without reduction group was shortest in the experimental group. The frequency of Angle's classifications had not significant correlations between the experimental groups. 2. Active and passive range of the opening movement, maximum protrusive movement, maximum lateral movement toward left side were significantly decreased in the experimental groups compared to the control group, but there was no significant difference in the range of the maximum lateral movement toward right side between the control and experiment groups. In unilateral anterior disc displacement without reduction group, the range of maximum lateral movement toward unaffected side was no significant difference in the range of the maximum lateral movement between toward affected side and toward unaffected side. 3. Maximum opening velocity, maximum closing velocity, average opening velocity, average closing velocity and maximum velocity of terminal tooth contact were significantly decreased in the experimental groups compared to control group. There was no significant difference in maximum opening velocity and maximum velocity of Terminal tooth contact between the subgroups of the experimental group each other, but there was significant difference in maximum closing velocity, average opening velocity and average closing velocity between the subgroups each other. 4. In the frontal plane of the MKG, the frequency of complex deviation type(F-2)pattern was significantly increased in the anterior disc displacement with out reduction group compared to the anterior disc displacement with reduction group and the control group. In the sagittal plane, the frequency of coincident type(S-1)was decreased in the same group. 5. In the maximum opening velocity pattern, the frequency of no-peak type (OV-3)in the unilateral anterior disc displacement with reduction group was significantly increased compared to the control group. The frequency of 1-peak type (OV-1) and 2-peak type (OV-2) was decreased in the anterior disc displacement with out reduction group, but the frequency of no-peak type (OV-3)was increased in the same group. In the maximum closing velocity pattern, the frequency of no-peak type (CV-3) was significantly increased in the anterior disc displacement without reduction group. Compared to the anterior disc displacement with reduction group and the control group. The frequency of 1-peak type (CV-1) and 2-peak type (CV-2) in the anterior disc displacement with reduction group was decreased than that in the control group.

Effects of Sensorimotor Training Volume on Recovery of Knee Joint Stability in Patients following Anterior Cruciate Ligament Reconstruction

  • Purpose: The purpose of this study is to examine the effects of sensorimotor training on knee joint stability after anterior cruciate ligament reconstruction. Methods: The subjects were sixteen 16 adults who received anterior cruciate reconstruction by arthroscopy, and underwent sensorimotor training for which was to have them maintenanceain of a standing position with a step Balance ball on the affected side over 30 degrees knee flexion with 100% weight bearing for 15-20 seconds. Before the genuine experiment commenced, the Lysholm scale was had been used to assess functional disorders on the affected knee joint. KT-2000 Arthrometer measurement equipment was used to measure anterior displacement of tibia against to femur before and after the sensorimotor training. Results: There was significant relaxation on the affected side in tibia anterior displacement of the affected and sound sides on in supine position before the sensorimotor training. There was little significant difference in tibia anterior displacement of the affected knee joints on in the supine position before and after the sensorimotor training. The results also showed that there was a reduction in the difference of tibia anterior displacement of the affected knee joints on in the standing position. These results suggest that the effects of sensorimotor training on knee joint stability after anterior cruciate ligament reconstruction is to induce the change of tibia anterior displacement against femur and the variation of muscles activation. Conclusion: The sensorimotor training may contribute to the improvement of joint functional stability in people who are in post-operation state and with orthopedic musculoskelectal injuries.

직접외상에 의한 급성 비정복성 관절원판 변위의 치험례 (A Case Report on the Treatment of Acute Anterior Disc Displacement without Redution in TMJ Disorders by Direct Trauma)

  • The authors treated a 30-years old female patient who visited the Department of Oral Medicine, PNUH due to the chief complaint of limitation of mouth opening. The magnetic resonance imaging following clinical examination was used for establishing an accurate and reliable diagnosis and the patient was diagnosed as having anterior disc displacement without reduction in the right joint and anterior disc displacement with reduction in the left joint. For managing acute anterior disc displacement without reduction, mandible manipulation was applied first focusing on pain control and then stabilization appliance was used for maintenance of joint stabilization. With time, the sign and symptom was remarkably reduced and an active exercise program was recommended to maintain of normal muscle length, increase joint range of motion and develop normal coordination arthrokinematics. As a result of treatment, the patient did not complain discomfort of normal daily activities and it was difficult to consider that the displace disc was not reduced completely, but the improvement in range of motion and joint mobility were remarkably found. Therefore, an exercise program should be considered to maintain joint mobility and be effective as a self-care.

전방재위치 교합장치를 이용한 비정복성 관절원판변위 환자의 치험례 (Case Report : Treatment of Disc Displacement without Reduction Patient Using ARS)

  • The treatment of anterior disc displacement without reduction patients needs speicific treatment methods different from other Craniomandibular Disorders. Those are manual manipulation, anterior repositioning splint; ARS and step-back. It is well known that the use of Anterior Repositioning Splint is effective on relief of TMJ symptom. But the side effect of long- term ARS wearing, which is irreversible posterior open bite induction, has made many clinicians avoid ARS treatment. This report introduces a clinical case recently proven part- time wearing ARS method for treatment of Disc Displacement without reduction patient' that can reduce side effects with good efficacy. But now clinical statistical studies and basical histophysiological studies are more needed.

측두하악관절원판변위 환자에서 나타나는 측방횡두개방사선사진상의 과두위치에 대한 평가 (Evalutation of the Condylar Position in the Lateral Transcranial Projection for the Patients with Disc Displacements of the Temporomandibular Joint)

  • The author assessed the sagittal relationships between glenoid fossa of the temporal bone and mandibular condyle from lateral transcranial views of 74 TMJ with disc displacement and 16 TMJ with normal disc-condyle complex by the magnetic resonance image findings. All the subjects were female and also in their 3rd decades. The disc displacement group was subdivided into anterior disc displacement with reduction (ADWR) group and anterior disc displacement without reduction (ADWOR) group. The anterior, superior, and posterior joint spaces as well as anterior/posterior (A/P) ratio of the space at the closed jaw position and vertical and horizontal components of the condyle position relative to the articular eminence at the open jaw position were measured from all the subjects and the data were compared among groups. The result were as follows : 1. The mean posterior joint space of ADWR group was smaller than ADWOR group, but there were no significant differences in anterior and superior joint spaces between two groups. 2. There showed a tendency of higher A/P ratio in ADWR group which meant the condyle of ADWR was likely to take posteriorly displaced position. 3. There were higher proportion of neutral condylar position in glenoid fossa in normal group, but higher proportion of posterior condylar position in ADWR group. 4. There were no significant differences in the degree of condyle-fossa concentricity among groups.

중심위 교합에서 중심 교합으로 전위될 때의 변위량과 교합형태에 관한 비교 연구 (A COMPARATIVE STUDY OF THE AMOUNT OF DISPLACEMENT AND OCCLUSION FORMS IN THE CHANGE FROM CR TO CO)

  • Statement of problems: The concept of CR has also changed continuously.?In order to find out the factors that affect the centric slide, studies were carried out to compare the forms of wisdom teeth eruption, lateral movement, premature contact in CR, and anterior movement. Research and statistical methods were based on the report by the 1980 Korean dental association. Material and method: In our study, 403 dentists in their twenties and dentistry students who could understand CR and CO (and who did not receive occlusal, orthodontic treatment, without extreme caries and large prosthodontic care) were compared with the 25 year old results. A segment of line parallel to the upper incisor was marked on the lower incisor. When seen laterally, a line perpendicular to the occlusal plane was drawn on the foremost area of the upper incisor. This line was extended to the lower incisor and the two points (points at the lower and upper incisors) were used as reference points for the CO. After guiding the occlusion to the CR, two lines were marked by using the same method that was used for the CO. The point in which these lines meet became the reference point of CR occlusion Results and conclusions: Results of the experiment completed in 1980 show that all 307 research members had anterior-posterior and upper-lower displacement. Displacement measurements were $0.7{\pm}0.4mm$ for the anterior-posterior displacement, $0.99{\pm}0.50mm$ for the upper-lower displacement,0.18{\pm}0.31 mm for the lateral displacement, and $1.32{\pm}0.67mm$ for the total displacement. Results of the 2006 experiment show that all 409 research members had anterior-posterior and upper-lower displacement. The anterior-posterior displacement was $1.12{\pm}0.86mm$, the upper-lower displacement was $1.02{\pm}0.71mm$, the lateral displacement was $0.61{\pm}0.56mm$, and the total displacement was $1.80{\pm}0.99mm$. No specific differences were found between each group when comparing displacement according to the forms of wisdom teeth eruption. Since 1980, the percentage of unerupted teeth has increased from 35.16% (111/307 people) to 57.5% (236/409 people). Westernization of the Korean cranial form and intraoral structure has brought about these results. In our experiment, 26.7% (109/409 people) of the subjects were cuspid guided, while 7.3% (30/409 people) were mutually guided. No specific differences were found in the amount of displacement between the two groups. Only the subjects with anteriors coming in contact made up the largest percentage group (42.3%, 173/409 people) in our study. No specific differences were found between each group.

The effects of alveolar bone loss and miniscrew position on initial tooth displacement during intrusion of the maxillary anterior teeth: Finite element analysis

  • Objective: The aim of this study was to determine the optimal loading conditions for pure intrusion of the six maxillary anterior teeth with miniscrews according to alveolar bone loss. Methods: A three-dimensional finite element model was created for a segment of the six anterior teeth, and the positions of the miniscrews and hooks were varied after setting the alveolar bone loss to 0, 2, or 4 mm. Under 100 g of intrusive force, initial displacement of the individual teeth in three directions and the degree of labial tilting were measured. Results: The degree of labial tilting increased with reduced alveolar bone height under the same load. When a miniscrew was inserted between the two central incisors, the amounts of medial-lateral and anterior-posterior displacement of the central incisor were significantly greater than in the other conditions. When the miniscrews were inserted distally to the canines and an intrusion force was applied distal to the lateral incisors, the degree of labial tilting and the amounts of displacement of the six anterior teeth were the lowest, and the maximum von Mises stress was distributed evenly across all the teeth, regardless of the bone loss. Conclusions: Initial tooth displacement similar to pure intrusion of the six maxillary anterior teeth was induced when miniscrews were inserted distal to the maxillary canines and an intrusion force was applied distal to the lateral incisors. In this condition, the maximum von Mises stresses were relatively evenly distributed across all the teeth, regardless of the bone loss.
