Plunging Ranula환자의 치험례 (A Case Report of Plunging Ranula : An unusual case of mucous extravasation cyst)

  • The plunging ranula or cervical ranula is amucous extravasation cyst of the sublingual gland. It is slightly common in females, shows no side preference, and is more prevalent in the second and third decades of life. It typically manifests as a painless, nonmobile swelling in the neck. The pathogenesis of plunging ranula is the discontinuities of the mylohyoid muscle in a position that would allow extravasation of sublingual gland mucin. The histologic appearance is characteristically of a cyst, devoid of epithelium or endothelium, with a vascular fibro-connective tissue wall containing some chronic inflammatory cells and macrophages stuffed with mucin. The correct diagnosis is essential for the most effective treatment, which is exicision of the sublingual gland. The plunging ranula must be differentiated clinically and histomorphologically from thyroglossal duct cyst, dermoid cyst, branchogenic cyst, lymphangioma, laryngocele, lipoma, hemangioma, cervial thymic cyst, cysts of the parathyroid or thyroid gland, lymphadenopathy, abscess, or tumor. We report a case and review the literatures, in our case, 23-year old man were diagnosis as plunging ranula after have been taken sialogam, MRI, etc. He underwent surgery via a cervical approach. The ranula reached the anterior neck by passing through a dehiscence in the mylohyoid muscle. A pseudocyst was extirpated. Although total sublingual gland excision was not performed, no recurrence was observed during 6 months follow-up periods.

편도주위농양에 합병된 하행 괴사성 종격동염 -치험 1례- (Descending Necrotizing Mediastinitis Secondary to Peritonsillar Abscess -A Case Report-)

  • 하행 괴사성 종격동염은 구강인두와 경부 염증의 드문 합병증 중의 하나로서, 높은 사망률과 합병증 등을 낮추기 위해 조기에 적극적이고 효과적인 수술적 배농술이 필요하다. 39세 남자 환자가 연하통, 경부 종창, 연하곤란 및 호흡곤란으로 내원하여 촬영한 경부 CT상 편도주위농 양과 인두 후부 및 주위의 농양이 발견되어 이비인후과에서 경부절개를 통한 배농술을 시행하였다. 1차 수 술 이후에 발열, 양측 견갑통이 재발하였으며 재촬영한 경부 및 흉부 CT에서 종격동 농양이 발견되어 경부 배농술과 더불어 개흉술을 통한 종격동 배농술을 시행하였다. 수술시에 시행한 종격동 농양과 농흉, 그리고 객담으로부터의 균배양 결과 Klebsiella pneumona로 밝혀졌다. 2차 수술후 환자는 호전되어 술후 85일째 퇴원 할 수 있었다. 결론적으로 CT촬영으로 하행 괴사성 종격동염을 조기에 진단하고 경부 배농술 뿐만 아니라 개흉술을 통한 광범위하고 효과적인 배농술이 환자의 생존율을 높이는데 도움이 되리라 본다.

부악하선에서 유발된 양측성 몰입성 하마종 (Bilateral Plunging Ranula Arising from Accessory Submandibular Gland)

  • Purpose: Whereas oral ranula is relatively common and presents as a cyst in the mouth, the plunging ranula is rare and manifests itself as a mass in the neck with or without an associated oral lesion. The purpose of this study is to examine the clinical characteristics of rare bilateral plunging ranula arising from accessory submandibular gland in order to provide our experience for its correct diagnosis and treatment. Methods: A 13-year-old girl manifests as a slow growing painless, non-mobile swelling in the anterior neck. She underwent surgery via a cervical approach. A pseudocyst was extirpated and adjacent accessory gland tissue and related lymph node were removed. Results: The histologic appearance is characteristically of a cyst, devoiding of epithelium or endothelium, with a vascular fibro-connective tissue wall containing some chronic inflammatory cells and macrophage stuffed with mucin. Pathologic findings represented a form of myxomatous degeneration and lined by condensed connective tissue and granulation tissue. The nature of the accessory gland tissue was same as subligual gland. Although total submandibular or sublingual gland excision was not performed, no recurrence was observed during 6 months follow-up periods. Conclusion: Usually, unilateral plunging ranula develops commonly because of rupture of sublingual gland duct by trauma and extravasation of salivary secretion to the adjacent tissue. But our case developed because of bilateral congenital accessory submandibular gland. This is thought to be a result from a congenital failure of canalization of the terminal end of the duct. Finally, the correct diagnosis is essential for the most effective treatment, which is excision of the ranula and related accessory salivary gland. We performed excision of accessory submandibular gland and plunging ranula and had a good result without recurrence.

기관협착증에 대한 기관 성형술 (Surgical Management of Trachea Stenosis)

  • Between 1975 and 1992, forty five patients with trachea stenosis received tracheoplasty for relief of obstruction. The causes of airway problem are brain contusion[19 cases, 40%], cerebrovascular disease[3 cases, 7%], drug intoxication[8 cases, 18%], psychotic problem[2 cases, 4%], trachea tumor[3 cases, 7%], adult respiratory distress syndrome[9 cases, 20%] and direct trauma[1 case, 2%]. Direct causes of trachea stenosis were complications of tracheostomy[36 cases, 80%], complications of nasotracheal intubation[5 cases, 11%], tumor[3 cases, 6%] and trauma[1 case, 2%]. Thirty one patients underwent the sleeve resection and end-to-end anastomosis. Five patients performed a wedge resection and end-to-end anastomosis. Forteen patients received the Montgomery T-tube for relief of airway obstruction. Four patients have done simple excision of granulation tissue. Two, subglottic stenosis patients were received Rethi procedure[anterior division of cricoid cartilage, wedge partial resection of lower thyroid cartilage and Montgomery T-tube molding] and the other subglottic stenosis patient underwent permanent trachea fenestration. Including cervical flexion in all patients postoperatively, additional surgical techniques for obtain tension-free anastomosis were hyoid bone release technique in two cases, and hilar mobilization, division of inferior pulmonary ligament and mobilization of pulmonary vessel at the pericardium were performed in one case. Cervical approach was used in 39 cases, cervicomediastinal in 12 cases and transthoracic in one case. Complications of tracheoplasty were formation of granulation tissue at the anastomosis site[3 cases], restenosis[9 cases], trachea-innominate artery fistula[2 cases], wound infection[2 cases], separation of anastomosis[2 cases], air leakage[3 cases], injury to a recurrent laryngeal nerve[temporary 8 cases, permanent 2 cases] and hypoxemia[1 case]. Surgical mortality for resection with primary reconstruction was 6.7%, with one death due to postoperative respiratory failure and two deaths due to tracheo-innominate artery fistula.

Surgical Outcomes According to Dekyphosis in Patients with Ossification of the Posterior Longitudinal Ligament in the Thoracic Spine

  • Objective : Ossification of posterior longitudinal ligament (OPLL) in the thoracic spine may cause chronic compressive myelopathy that is usually progressive, and unfavorable by conservative treatment. Although surgical intervention is often needed, the standard surgical method has not been established. Recently, it has been reported that posterior decompression with dekyphosis is effective surgical technique for favorable clinical outcome. The purpose of this study was to evaluate the surgical outcomes in patients with thoracic OPLL according to dekyphosis procedure and to identify predictive factors for the surgical results. Methods : A total of 25 patients with thoracic OPLL who underwent surgery for myelopathy from May 2004 to March 2017, were retrospectively reviewed. Patients with cervical myelopathy were excluded. We assessed the clinical outcomes according to various surgical approaches. The modified Japanese orthopedic association (JOA) scores for the thoracic spine (total, 11 points) and JOA recovery rates were used for investigating surgical outcomes. Results : Of the 25 patients, 10 patients were male and the others were female. The mean JOA score was 6.7±2.3 points preoperatively and 8.8±1.8 points postoperatively, yielding a mean recovery rate of 53.8±31.0%. The mean patients' age at surgery was 52.4 years and mean follow-up period was 40.2 months. According to surgical approaches, seven patients underwent anterior approaches, 13 patients underwent posterior approaches, five patients underwent combined approaches. There was no significant difference of the surgical outcomes related with different surgical approaches. Age (≥55 years) and high signal intensity on preoperative magnetic resonance (MR) image in the thoracic spine were significant predictors of the lower recovery rate after surgery (p<0.05). Posterior decompression with dekyphosis procedure was related to the excellent surgical outcomes (p=0.047). Dekyphosis did not affect the complication rates. Conclusion : In this study, our result elucidated that old age (≥55 years) and presence of intramedullary high signal intensity on preoperative MR images were risk factors related to poor surgical outcomes. In the meanwhile, posterior decompression with dekyphosis affected favorable clinical outcome. Posterior approach with dekyphosis procedure can be a recommendable surgical option for favorable results.
