Morphometric Study of the Anterior Thalamoperforating Arteries

  • Objective : To evaluate the morphometry of the anterior thalamoperforating arteries (ATPA). Methods : A microanatomical study was performed in 79 specimens from 42 formalin-fixed adult cadaver brains. The origins of the ATPAs were divided into anterior, middle, and posterior segments according to the crowding pattern. The morphometry of the ATPAs, including the premammillary artery (PMA), were examined under a surgical microscope. Results : The anterior and middle segments of the ATPAs arose at mean intervals of $1.75{\pm}1.62$ mm and $5.86{\pm}2.05$ mm from the internal carotid artery (ICA), and the interval between these segments was a mean of $3.17{\pm}1.64$ mm. The posterior segment arose at a mean interval of $2.43{\pm}1.46$ mm from the posterior cerebral artery (PCA), and the interval between the middle and posterior segments was a mean of $3.45{\pm}1.39$ mm. The mean numbers of perforators were $2.66{\pm}1.19$, $3.03{\pm}1.84$, and $1.67{\pm}0.98$ in the anterior, middle, and posterior segments, respectively. The PMA originated from the middle segment in 66% of cases. A perforator-free zone was located >2 mm from the ICA in 30.4% and >2 mm from the PCA in 67.1% of cases. Conclusion : Most perforators arose from the anterior and middle segments, within the anterior two-thirds of the posterior communicating artery (PCoA). The safest perforator-free zone was located closest to the PCA. These anatomical findings may be helpful to verify safety when treating lesions around the PCoA and in the interpeduncular fossa.

  • One of the important things in the full mouth reconstruction is the determination of therapeutic position. Centric jaw relation is used as a therapeutic position for the full mouth reconstruction. There are several techniques associated with recording this position. Five clinically acceptable techniques are as follows: 1) Swallowing or free closure, 2) Chin point guidance, 3) Bimanual method 4) Myo-monitor technique, 5) Anterior deprogrammer. Centric relation obtained utilizing the anterior acrylic resin platform in this case. Another important thing in full mouth reconstruction is provisional restoration. Provisional restorations are an excellent diagnostic instrument, especially in full remain esthetics, phonetics, function, parafunction, and dysfunction after evaluation and acceptance through clinical trial with the provisional restorations should be accurately transferred to the final restorations to ensure the same clinical success. Especially, anterior guidance should be accurately transferred to the final restorations. An accurate anterior guidance is critical for optimal esthetics, phonetics, comfort, function, stress minimization, and longevity of teeth and restorations. To record optimum anterior guidance, customized anterior guide table is used in this case. Considering previously mentioned points, we did successive treatment. And it resulted in a better situation esthetically and functionally. Followings are what we cared in treating a patient in this case. 1) Accurate centric relation recording 2) Accurate transference of anterior guidance to the final restorations.

Anomalous Arterial Supply to the Serratus Anterior Muscle (전방거근으로 분지되는 혈관경의 해부학적 변이 증례보고)

  • Purpose: The latissimus dorsi flap and the serratus anterior flap have been used as combined flaps to reconstruct extensive defects. Because these two muscles are usually supplied by the subscapular-thoracodorsal vessels, the two flaps can be based on vascular pedicle that is long and anatomically reliable. In this case, we reported that serratus anterior possessed an anomalous arterial supply totally independent from the subscapular pedicle while raising combined latissimus dorsi and serratus anterior flap. Methods: A 35-year-old male with extensive soft tissue defect in the left perineum and thigh visited. Muscle defects of the medial thigh were observed, and femoral nerve and vessels were exposed. Combined latissimus dorsi and serratus anterior free flap was raised to reconstruct defect. On raising flaps, artery supplying the serratus anterior muscle originated from the axillary artery directly, was lying on the undersurface of the serratus anterior muscle. Results: Because two flap pedicles had no communication and latissimus dorsi muscle was large enough to cover soft tissue defect, we transferred only latissimus dorsi free flap with 1 : 3 meshed skin graft. Patient had limb salvage and satisfactory functional outcome. Conclusion: There are many variations of arterial pedicles of flaps. However, most of these variations remain within known anatomical consistence, thus is an indicator in planning the dissection of the vessels. According to documents, arterial pedicle to the serratus muscle not originated from the thoracodorsal artery is rarely reported, and in most of these cases, the arteries are originated from the subscapular artery. Thus pedicle directly originated from the axillary artery to serratus muscle is a very rare variation in its vascular anatomy.

Intramuscular Activation of Scapular Stabilizing Muscles During Shrug Exercise and PNF Scapular Pattern Exercise (으쓱 운동과 PNF어깨뼈패턴 시 어깨뼈 안정근의 근활성도)

  • Purpose: The purpose of this study was to compare the intramuscular activation of the scapular stabilizing muscles and the upper trapezius/middle serratus anterior (UT/MSA) ratios during shrug exercise and proprioceptive neuromuscular facilitation (PNF) scapular pattern exercise. Methods: The participants of this study were 13 young adult men who voluntarily consented to participate in this experiment after listening to its purpose and methods. All participants were instructed on maximal voluntary isometric contraction and scapular exercises. The intramuscular activation of the upper trapezius (UT), lower trapezius (LT), middle serratus anterior (MSA), and lower serratus anterior (LSA) muscles while performing scapular exercises in a side-lying position were measured using surface electromyography. To analyze the muscle activation and UT/MSA ratio between the two exercises, a one-way repeated ANOVA was performed. Post-hoc analyses were conducted using Tukey's multiple comparison and analysis. Hamstring flexibility for each group was measured by a passive straight leg raising test. Results: The shrug exercise showed significantly higher UT activation compared to PNF anterior elevation and posterior elevation scapular patterns. The PNF scapular anterior elevation pattern showed significantly higher serratus anterior activation than the shrug exercise. Additionally, the UT/MSA ratios were significantly lower in the PNF scapular anterior elevation pattern than in the two exercises. Conclusion: Although shrug exercise was effective for strengthening UT, the PNF scapular anterior elevation pattern may be effective for strengthening MSA and improving the UT/MSA ratio.

AN ACOUSTIC ANALYSIS OF ABNORMAL PRONUNCIATION IN CHILDREN WITH ANTERIOR CROSSBITE (전치부 반대교합아동의 발음장애에 관한 음향학적 연구)

  • It has widely known for speech problem in patients with malocclusion, but there have been insufficient studies on degrees of severity. Patients with openbite and Cl III malocclusion usually have speech problem. It has widely known that fricative /s/is pronunciated most abnormally in Cl III malocclusion than any other consonant. Therefore 20 children with anterior crossbite were selected for study groups and 40 children with normal anterior occlusion were selected for control group. 7 sounds such as / 사(sa), 서($s{\delta}$), 소(so), 수(su), 스($s{\omega}$), 시(si), 세(se) / were recorded, and the formants and formants ratios were measured by using Sensimetric Speech Station, which is speech analyzing program in IBM PC. The results were as follows : 1. In anterior crossbite group, F1 of all 7 sounds were significantly higher than normal anterior relationship group(p<0.05). 2. There were no significant difference in F2 between two groups except / 소(so), 수(su)/(p>0.05). 3. In anterior crossbite group, F2/F1 ratio of all 7 sounds were significantly smaller than normal anterior relationship group(p<0.05). 4. There were no significant difference in accordance with tongue position(p>0.05).

A STUDY OF HOLOGRAPHIC INTERFEROMETRY ON THE INITIAL REACTION OF MAXILLOFACIAL COMPLEX TO THE INTERMAXILLARY FORCES ON THE ORTHODONTIC ARCHWIRES (교정용 호선에 악간 교정력 적용시 악안면골의 초기반응에 관한 Holographic Interferometry 연구)

  • This study was performed to evaluate the initial reaction of maxillofacial complex to the Class II intermaxillary and the anterior vertical elastic forces on the six types of archwires including multiloop edgewise arch wires(MEAW). A human dry skull was used for this purpose and this investigation was done by holographic interferometry. Based on such investigation, the fringe pattern and the number of fringes of each condition were compared and analyzed. The findings of this study were as follows: 1. As the orthodontic forces increased, the amount of displacement increased. 2. As the orthodontic forces were applied, the fringes were shown not only in the teeth and the maxilla but also in the adjacent bones, i.e., temporal bone, zygomatic bone, nasal bone, frontal bone and sphenoid bone. And the direction of fringe pattern and the number of fringes were different from each other by the sutures. 3. As the long Class II elastic forces were applied, the backward-downward displacements of the anterior teeth and the maxilla were shown, and backward displacement of the former were grater than those of the latter. And backward displacements were greater by the long Class II elastic forces than by the short Class II elastic forces. 4. As the anterior vertical elastic forces were applied, downward displacements of the anterior teeth and the maxilla were shown, and the downward displacements of the former were greater than those of the latter relatively. 5. The downward displacements of the anterior area to the anterior vertical elastic forces of the MEAW were greater than those of other archwires. In addition, the more tip-back bend was applied, the more displacement was seen. 6. As the Class II intermaxillary forces and the enough anterior vertical elastic forces were applied on the MEAW with tip-back bend, there was an intrusive effect of the posterior teeth.

The Clinical Effects of Heating-Conduction Acupuncture Therapy for Anterior Talofibular and Calcaneofibular Ligament Injury Induced by Acute Ankle Sprain (전열침(傳熱鍼)이 급성 족관절 염좌로 인한 전거비·종비 인대 손상에 미치는 임상적 효과)

  • Objectives : The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on ligament injury induced by acute ankle sprain. Methods : From september 16, 2008 to April 17, 2010, the 28 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental Medical hospital, Dae-Jeon university with acute ankle sprain were performed heating-conduction acupuncture therapy on anterior talofibular and calcaneofibular ligament injury. To evaluate the efficiency of heating-conduction acupuncture therapy, pain threshold with pressure algometer and visual analogue scale(VAS) were applied before treatment and after 1st, 2nd treatment. Results : 1. The pain threshold and the VAS score showed statistical significant improvement after 1st treatment on calcaneofibular and anterior talofibular ligament injury. 2. The VAS score after 2nd treatment on calcaneofibular and anterior talofibular ligament injury showed statistical significant improvement compared with that after 1st treatment. 3. The pain threshold after 2nd treatment on calcaneofibular and anterior talofibular ligament injury did not show statistical significant improvement compared with that after 1st treatment. 4. The difference between anterior talofibular and calcaneofibular ligament injury was not statistical significant in pain threshold and VAS score. Conclusions : Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with calcaneofibular ligament injury as well as anterior talofibular ligament induced by acute ankle sprain.

A Study of UGI Series for Improvement of Diagnosis on the Anterior Wall of the Stomach (위 전벽 병변 진단을 위한 UGI series의 실태 및 개선방안에 관한 고찰)

  • This paper is to investigate a more detailed method for the diagnosis of anterior wall of the stomach by making a comparative study with several hospitals. It has been true that there have been hospitals, that have not examined anterior wall of the stomach. However, it is very important for us to examine anterior wall of the stomach for an carly detection of gastric carcinoma. The results of th study are as follows : 1. Frequency of occurrence of the early gastric carcinoma for the anterior wall were 50 cases and 34 cases for the posterior wall out of 84 cases. 2. Only a hospitals have examined the anterior wall of stomach. 3. In case of operation, only a hospitals have used two techniques at for same time single and double contrast studies. 4. Only cue hospital used a compression pad and three hospitals hod only filing state images taloen. 5. In general, 1 chest of film was used and the number of exposures rouged from 1 to 2 times. Lesions on the anterior wall of the stomach can be shown by the combination of prone single com-pression and supine double contrast radiographs. Therefore, the conclusion came to the result that the prone single compression and supine double contract technique of the anterior wall are Indispensable methods to the routine check of the stomach.

Analysis of Noninstrumented Anterior Cervical Discectomy and Interbody Fusion in Degenerative Cervical Disease (퇴행성 경추질환에서 전방경유 추간판 절제술 및 골유합술의 결과분석)

  • Objectives : We retrospectively studied the efficacy of anterior cervical discectomy and interbody fusion without plate fixation in degenerative cervical disease. Methods : Thirty two consecutive patients with degenerative cervical disease treated by decompression and interbody fusion(Smith-Robinson technique) without anterior cervical plating were studied for postoperative complication rate as well as the clinical and radiologic outcomes and were compared the result of ours with other reported series where the anterior cervical plating was used. Results : All cases were reviewed after average period of 13 months for the purpose of this study. There were 4 postoperative complications related to grafting. A solid fusion was obtained in all cases with single-level fusion(n=21) and 81.8 % of the cases with a two-level fusion(n=11). The overall fusion rate was 93.8 % and fusion rate per level fused was 95.3%. The clinical outcome of the patients was comparable with that in the literature, with one patient having a poor result. Comparing the result of this study with others of the anterior cervical plating, clinical outcome and fusion rate were not superior in plate fixation group in single-level fusion, but increased fusion rate and decreased graft-related complication rate were noted in multilevel fusion with plate fixation. However, the clinical outcome was not superior to noninstrumented fusion group of this study. Conclusion : These results demonstrate that anterior cervical discectomy and interbody fusion(Smith-Robinson technique) without instrumentation is safe and reliable method of single-level fusion in degenerative cervical disease. Plate fixation system doesn't seem necessary in single level fusion in degenerative cervical disease.

Minimal Ablation of the Tibial Stump Using Bony Landmarks Improved Stability and Synovial Coverage Following Double-Bundle Anterior Cruciate Ligament Reconstruction

  • Purpose: To evaluate the clinical effects of using anatomical bony landmarks (Parsons' knob and the medial intercondylar ridge) and minimal ablation of the tibial footprint to improve knee anterior instability and synovial graft coverage after double-bundle anterior cruciate ligament reconstruction. Materials and Methods: We performed a retrospective comparison of outcomes between patients who underwent reconstruction with minimal ablation of the tibial footprint, using an anatomical tibial bony landmark technique, and those who underwent reconstruction with wide ablation of the tibial footprint. Differences between the two groups were evaluated using second-look arthroscopy, radiological assessment of the tunnel position, postoperative anterior knee joint laxity, and clinical outcomes. Results: Use of the anatomical reference and minimal ablation of the tibial footprint resulted in a more anterior positioning of the tibial tunnel, with greater synovial coverage of the graft postoperatively (p=0.01), and improved anterior stability of the knee on second-look arthroscopy. Both groups had comparable clinical outcomes. Conclusions: Use of anatomical tibial bony landmarks that resulted in a more anteromedial tibial tunnel position improved anterior knee laxity, and minimal ablation improved synovial coverage of the graft; however, it did not significantly improve subjective and functional short-term outcomes.
