Effects of Femoral Head Anterior Glide Fixation during Active Straight Leg Raise on Hip Flexor Strength in Subjects with Femoral Anterior Glide Type

  • Purpose: In this study, subjects with femoral anterior glide type were examined to investigate the effects of femoral head anterior glide fixation during active straight leg raise on the strength of the hip flexor in a supine position. Methods: Fifteen subjects participated in this study. All subjects were classified through an evaluation form for femoral anterior glide type (FAGT). The strength of the hip flexor was measured during active straight leg raise test (ASLR test), and compared with and without femoral anterior glide fixation in a supine position. The fixation of the femoral head was achieved as per the therapist's manual guidelines. Paired t-test was applied to compare changes in the strength of the hip flexor according to fixation conditions. The level of statistical significance was set at α=0.05. Results: The strength of the hip flexor was lesser during the ASLR test with fixation compared to without fixation (p=0.007). Conclusion: The strength of the hip flexor decreases with fixation. Results of this study revealed a difference between hip flexor strength, with and without femoral anterior glide fixation during ASLR, in subjects with femoral anterior glide type.

Comparative Effects of Novel Modified Sleeper and Cross-body Stretching on Scapular Anterior Tilting and Shoulder Internal Rotation in Subjects With Anterior Tilted Scapular and Shoulder Internal Rotation Deficits

  • Background: Posterior capsule tightness (PCT), commonly seen in overhead athletes, is a soft tissue adaptation that is also noted in non-throwers. PCT is associated with scapular and humeral kinematic alterations, significant restriction of shoulder internal rotation (IR) range of motion (ROM), and significant scapular anterior tilting. Sleeper and cross-body stretches (CBS) are suggested for PCT and IR deficits, and have been modified since introduction. A novel modified sleeper stretch (NMSS) was designed in this study to prevent the risk of anterior translation of the humeral head. Though the effects of posterior shoulder stretching exercise have been widely studies, to the best of our knowledge, no previous studies have investigated the effectiveness of posterior shoulder exercises in decreasing scapular anterior tilting. Objects: To compare the immediate effects of two posterior shoulder stretching exercises (NMSS and CBS) on scapular anterior tilting and shoulder IR ROM. Methods: Thirty-two subjects with anteriorly tilted scapula and IR deficits [mean age: 24.3 ± 2.5 years; 15 males and 17 females] participated in this study. Subjects were randomly assigned to either the NMSS or CBS groups. Scapular anterior tilting (at rest and at shoulder 60° active IR) and shoulder IR ROM were measured before and immediately after intervention. Results: Scapular anterior tilting significantly decreased, while the shoulder IR ROM significantly increased in both groups. However, there was no significant group-by-time interaction effect or significant difference between the groups. Conclusion: Both stretching exercises were effective in restoring shoulder IR ROM and decreasing scapular anterior tilting.

교합고경 증가로 발생한 전치부 공간을 자연 폐쇄시켜 새롭게 유도한 전방유도: 증례 보고 (New anterior guidance induction through spontaneous gap closure after an increase in vertical dimension: a case report)

  • 구치부 교모나 소실로 인해 발생하는 교합붕괴에서는 수복공간이 불충분하고 심미적 문제가 발생하는 경우가 많다. 이를 해결하기 위해 고경을 증가시키면, 하악이 후하방으로 회전함으로써 상하악 전치부 사이에 공간이 발생하여 전방유도각이 상실되는 문제점이 생긴다. 이런 이유 때문에 통상 고경 증가 후에 생긴 상하악 전치부 사이 공간은 교정을 통해 전치 치축 변화를 유도하거나 보철적 수복으로 피개하여 적절한 전방유도를 얻는 것이 일반적이었다. 그러나 구치부 교모나 소실 후 발생한 전치 과개교합 형식은 하악이 전상방 회전하며 전치부에 교합력이 과도하게 집중되고 있는 상태이기 때문에, 교합력이 제거되면 전치는 원래의 치축으로 돌아갈 수 있을 것이라고 추정할 수 있다. 이런 사실에 착안하여 이 증례보고에서는 덜 침습적인 치료 방법을 고안, 교합 수직 고경 증가를 통한 전치부의 자연스러운 전방유도 재획득을 의도하였다. 이후 디지털 중첩 방법으로 전치부의 치축 개선을 확인할 수 있었다. 새로운 치료법의 적절한 적응증을 정의하며 임상에 적용할 수 있도록 증례보고 하는 바이다.

전치부 접촉과 측두하악 관절강폭과의 관계에 관한 연구 (A Study on the Relationship between Anterior Tooth Contact and Temporomandibular Joint Space)

  • The aim of this study was to investigate the relationship between the presence or absence of anterior tooth contact and the changes in temporomandibular joint space. The study sample consisted of 32symptom-free dental students and 79 craniomandibular disorders patients with unilateral joint dysfunction. The two groups were categorized into control group or experimental group, respectively. Recordings of the number and distribution of occlusal contacts were made by T-Scan system. Transcranial radiographs were taken with using of accurad-100 head positioner. Measured items in transcranial radiographs were anterior, superior, posterior joint space and relative condylar position to deepest position of glenoid folla. According to the presence or absence of anterior tooth contact, each group was subdivided and compared with each other with respect to TM joint space. Data were processed and tested with SPSS/PC + package. The results of the study showed that the joint space in control group were wider than those of experimental group and the difference of the width of joint space was more remarkable in subjects with anterior contact between control group and experimental group. However, in same group whether the presence of anterior tooth contact could hardly affect the difference of the width of joint space. And anterior tooth contact in grouip are more frequent than in experimental group.

Failed Back Surgery Syndrome에서 전방 요추체간 유합술의 치료성적분석 (Clinical Analysis of Anterior Lumbar Interbody Fusion for Failed Back Surgery Syndrome)

  • Objective : To evaluate the role of anterior lumbar interbody fusion in treatment of failed back surgery syndrome, the authors retrospectively analyzed the result of anterior lumbar interbody fusion performed in our institute. Methods : Fifteen FBSS patients due to variable causes have been treated with anterior lumbar interbody fusion in our institute from April 1994 to June 1999. We analyzed clinical changes in 15 patients who were followed up for an average of 23 months. Results : The etiologies of FBSS were post operative discitis(6 cases), post operative instability(3 cases), post operative adhesion(5 cases), and recurrence(1 case). These fifteen FBSS patients were treated with anterior lumbar interbody fusion. The overall treatment outcome was satisfactory(excellent and good) in 11 cases. Three patients were slightly improved, but post operative low back pain was remained. One patient who had underwent nerve root injury due to pedicle screw insertion showed no improvement. Conclusion : We conclude that the anterior lumbar interbody fusion for FBSS seems to be safe and favorable treatment in selective patients, because low incidence of nerve injury risk and post-operative infection.

경추 신경근병증에 대한 전방 터널링 수술 : 초기 32례에 대한 보고 (Anterior Tunnelling Operation for Cervical Radiculopathy : A Report of First 32 Cases)

  • Objectives : Anterior tunnelling technique consist of anterior cervical fractional interspace decompression without fusion. This method provides sufficient space for adequate neuroforaminal decompression but avoids the need for fusion or fixation. We report early clinical results of 32 cases that underwent anterior tunnelling operation for treatment of cervical radiculopathy. Methods : This method is identical to conventional approach until the exposure of anterior cervical body and bilateral retraction of longus colli is made. A vertical window is then made at the vertebral bodies and disc space lateral to the insertion site of the longus colli. The window is deepened with drilling that follows a tunnelling fashion down to the compressive lesion. We analyzed clinical results from 32 patients who treated between December 1998 and August 2000. Results : Satisfactory results were obtained in 87% of the patients. Two patients required revision surgery. None revealed surgical spinal instability on last follow-up. Conclusion : Anterior tunnelling operation is an acceptable surgical option for the treatment of cervical radiculopathy. Its advantages are short hospitalization, minimal postoperative discomfort, and technical feasibility.

Treatment of anterior open bite by posterior maxillary segmental osteotomy and miniplates: a case report

  • Background: Anterior open bite is a challenging malocclusion to correct orthodontic treatment. Anterior open bite associated with over-erupted posterior teeth and long lower facial height should be treated by reduction of posterior dimension for esthetic results. Although the possibility of orthodontic treatment of an anterior open bite has increased with the introduction of skeletal anchorage, there are still cases requiring surgery for various reasons. Case presentation: This case report covers an anterior open bite of a 25-year-old man successfully treated with the posterior maxillary segmental osteotomy (PMSO) and miniplates. After the pre-surgical orthodontic treatment, the PMSO between canines and first premolars was performed under local anesthesia and miniplates were placed on the zygomatic buttress. As a result of 28 months of treatment, an impaction amount of 3.5 mm was obtained in the maxillary posterior teeth, and the facial esthetics improved at rest and smile. Conclusion: The impaction of the posterior dentoalveolar segment using the PMSO can be a good treatment option in patients with anterior open bite showing long lower facial height.

정상교합자와 III급 부정교합자의 저작근 근전도에 관한 연구 (AN ELECTROMYOGRAPHIC INVESTIGATION OF MASTICATORY MUSCLES IN NORMAL OCCLUSION AND CLASS III MALOCCLUSION)

  • The purpose of the present study was to investigate the differences of EMG activity of the masticatory muscles between normal occlusion and Class III malocclusion during various jaw functions. 46 subjects of 18.4-25.7 years were employed in this study: 26 subjects were normal occlusions, and 20 subjects were Class III malocclusions. The EMG data from the anterior and posterior temporal, anterior and posterior masseter muscles in both sides as mandibular elevators and supra-hyoid muscle group (close to the anterior belly of digastric muscle in right side) as mandibular depressor were recorded with the Medelec MS 25 electromyographic machine. The EMG recordings were analyzed during mandibular rest position, maximal biting, mastication with chewing gum, and swallowing of peanuts. All data were recorded and statistically processed. 1. The maximal mean amplitude of the anterior temporal muscle was stronger significantly in Class III malocclusion than in normal occlusion, and then the posterior temporal was weaker during mandibular rest position. 2. The maximal mean amplitudes in the anterior and posterior temporal muscles and the anterior masseter muscle of Class III malocclusion was weaker significantly than that of normal occlusion during maximal biting. 3. During mastication of the chewing gum, the maximal mean amplitudes of Class III malocclusion was weaker significantly than normal occlusion in the anterior and posterior temporal muscles of the working side, and the duration of Class III malocclusion was longer in the anterior temporal muscles of both aides, and the posterior temporal and the anterior masseter muscle of the balancing side. There were significant increasings of the latency in balancing anterior temporal, working posterior temporal muscles and supra-hyoid muscle group of Class III malocclusion. The silent period durations was 16.36 ms in Class III malocclusion while 10.76 ms in normal occlusion, which was statistically different (P < 0.05). 4. At swallowing of peanuts, the maximal mean amplitude of Class malocclusion was weaker significantly in the posterior temporal muscle than that of normal occlusion. There was no significant difference of duration between normal occlusion and Class III malocclusion. 5 The muscle activities of Class III malocclusion had a tendency of decrease less than normal occlusion. And then the muscle activities of the anterior temporal and anterior masseter muscles in Class III malocclusion showed the tendency of the increase more than other muscles of Class III malocclusion.

Pressure Distribution in Stump/Socket Interface in Response to Socket Flexion Angle Changes in Trans-Tibial Prostheses With Silicone Liner

  • This study examined the effects of socket flexion angle in trans-tibial prosthesis on stump/socket interface pressure. Ten trans-tibial amputees voluntarily participated in this study. F-socket system was used to measure static and dynamic pressure in stump/socket interface. The pressure was measured at anterior area (proximal, middle, and distal) and posterior area (proximal, middle, and distal) in different socket flexion angles ($5^{\circ}$, $0^{\circ}$, and $10^{\circ}$). Paired t-test was used to compare pressure differences in conventional socket flexion angle of $5^{\circ}$ with pressures in socket flexion angles of $0^{\circ}$ and $10^{\circ}$ (${\alpha}$=.05). Mean pressure during standing in socket flexion angle of $10^{\circ}$ decreased significantly in anterior middle area (19.7%), posterior proximal area (10.4%), and posterior distal area (16.3%) compared with socket flexion angle of $5^{\circ}$. Mean pressure during stance phase in socket flexion angle of $0^{\circ}$ increased significantly in anterior proximal area (19.3%) and decreased significantly in anterior distal area (19.7%) compared with socket flexion angle of $5^{\circ}$. Mean pressure during stance phase in socket flexion angle of $10^{\circ}$ decreased significantly in anterior proximal area (19.6%) and increased significantly in anterior distal area (8.2%) compared with socket flexion angle of $5^{\circ}$. Peak pressure during gait in socket flexion angle of $0^{\circ}$ increased significantly in anterior proximal area (23.0%) compared with socket flexion angle of $5^{\circ}$ and peak pressure during gait in socket flexion angle of $10^{\circ}$ decreased significantly in anterior proximal area (22.7%) compared with socket flexion angle of $5^{\circ}$. Mean pressure over 80% of peak pressure ($MP_{80+}$) during gait in socket flexion angle of $0^{\circ}$ increased significantly in anterior proximal area (23.9%) and decreased significantly in anterior distal area (22.5%) compared with socket flexion angle of $5^{\circ}$. $MP_{80+}$ during gait in socket flexion angle of $10^{\circ}$ decreased significantly in anterior distal area (34.1%) compared with socket flexion angle of $5^{\circ}$. Asymmetrical pressure change patterns in socket flexion angle of $0^{\circ}$ and $10^{\circ}$ were revealed in anterior proximal and distal region compared with socket flexion angle of $5^{\circ}$. To provide comfortable and safe socket for trans-tibial amputee, socket flexion angle must be considered.

  • Therapeuticre rehabilitation of young children has often included the use of walker as mobility aids. Traditional walkers have four vertical legs, a horizontal bar in front of child, and horizontal bars on either side which also serve as hand grip. Child pushes it. It is called 'anterior walker'. 'Posterior walker' have been introduced which the child pull along behind himself during ambulation. The purpose of this investigation was to compare specific gait characteristics of children with developmental disability, while they ambulated with anterior and posterior walker. Using the posterior walker, specific gait characteristics improve than using anterior walker.
