Extended Low Anterior Resection with a Circular Stapler in Patients with Rectal Cancer: a Single Center Experience

  • Background: to evaluate the outcome of stapled colo-anal anastomoses after extended low anterior resection for distal rectal carcinoma. Materials and Methods: A retrospective study of fifty patients who underwent coloanal anastomoses after extended low anterior resection was conducted at Imam Hospital from September 2007 up to July 2012. Results: The distance of the tumor from anal verge was 3 to 8 cm. Anastomotic leakage developed in 6% of patients and defecation problems in 16%. One-year local recurrence was 6% while three-year local recurrence was 4%. One-year systemic recurrence was seen in 22% while three-year systemic recurrence was seen in 20%. Conclusions: Colo-anal anastomoses after extended low anterior resection for distal rectal carcinoma can be conducted safely.

  • Avulsion fracture of medial cuneiform by tibialis anterior tendon is quite rare. It has been reported about the avulsion fracture and surgical repair of tibialis anterior tendon rupture at distal insertion site of medial cuneiform in Korea. We report a case of right foot medial cuneiform avulsion fracture by tibialis anterior tendon after autobike accident and describe this case with a review of literature.

  • Anterior maxillary teeth play an important role in determining a person's first impression and facial profile. Implant surgery in esthetic area requires more careful diagnosis, treatment planning, surgery, and prosthetic restoration than in posterior area. To avoid complications in surgery and prosthetic restoration for implants in esthetic area, accurate diagnosis and appropriate case selection become very important. If you have decided to restore the area with implant prosthesis, you have to know exactly where to place an implant. I will discuss the ideal implant position in terms of mesio-distally, apico-coronally, labio-palatally, and implant angulation. And I would like to point out the selection of fixture diameter & length for anterior implant. Finally, a clinical implant prosthesis case in maxillary central incisor will be shown. In conclusion, for superior esthetic outcome in anterior implant prostheses, we must understand the patient's anatomic condition and know our ability.

  • As the demand for natural and beautiful smiles increases, the demand for anterior aesthetic treatment is increasing. Orthodontic treatment is often necessary for esthetic, healthy and natural treatment outcome. Particularly, in the case of middle-aged patients, minor tooth movement limited to anterior teeth is more effective than comprehensive orthodontic treatment which requires a long-term treatment period. Clinician who is in charge of aesthetic dentistry should have the ability to select a case that can be treated with partial orthodontic treatment and to determine the most effective treatment method. This article provides decision flowchart for case selection and choosing the best treatment modality for anterior teeth alignment.

전치부 수직피개율에 관한 모형 및 두부방사선 계측학적 연구 (A Model and Cephalometric Study on the Percentage of Anterior Overbite)

  • The purpose of this study was to investigate the factors which cause the differences in the percentage of anterior overbite in spite of normal molar relationship in terms of skeletal, dental and dentoalveolar relations. The models and cephalograms taken from 154 subjects, 89 of shallow overbite and 65 of deep overbite ranging from 18 to 29 years of age were studied and analyzed statistically. The results were as follows. 1. In determining the percentage of anterior overbite, the significant differences were higher in the dental and dentoalveolar factors than in the skeletal factors and were higher in the proportional and angular measurements than in linear measurements. 2. The factor which had the greatest influence on the percentage of anterior overbite was the proportional parts of the dental and dentoalveolar heights. 3. The most influencing factor which determined the percentage of anterior overbite was the SN-MP angle among the skeletal factors.

  • 전방골간신경 증후군은 전방골간신경의 질환으로서, 장무지 굴근, 심수지 굴근 및 방형 회내근의 마비가 나타나고, 감각은 정상인 것이 특징적이다. 전방골간신경 증후군은 많이 알려져 있으나 자기공명영상 소견에 대한 보고는 적으며, 신경 분포의 해부학적 변이를 가지는 전방골간신경 증후군에 관한 보고는 더욱 드물다. 저자들은 정상변이의 신경분포를 보이는 전방골간신경 증후군 증례를 1예 경험하였기에 방사선학적 소견을 문헌고찰과 함께 보고하고자 한다.

흉벽에 발생한 거대한 유건종 1례 (A Large Dumb Bell Shaped Desmoid Tumor in Chest Wall: A Case Report)

  • Desmoid tumor is found most often in the anterior abdominal wall of parous women. Although it may originate in virtually any musculotendinous structure, those of the chest wall are rare. We experienced a case of large dumb bell shaped desmoid tumor originated in intercostal muscle and invaded anterior chest wall, pericardium, pleura and the lung. The patient was healthy in appearance except a painful swelling on the anterior chest wall. Roentgenographic studies demonstrated a huge homogenous mass in the right anterior chest cavity. He was treated with resection of the tumor including .anterior chest wall, a portion of the pericardium, middle lobe, and part of upper & lower lobes of the right lung because of tumor invasion. The tumor composed with two parts, one [$5{\times}4{\times}3$cm in size] is over the rib cage and another [$10{\times}15{\times}10$cm in size] is in the right chest cavity. Postoperative course was uneventful and there was no evidence of recurrence until last visit, 5 months after surgery.

Combined periodontal regenerative and prosthetic treatment of pathologic migration of anterior teeth

  • Purpose: Pathologic tooth migration (PTM) commonly occurs in the anterior region and is associated with periodontal disease. The treatment of PTM of anterior teeth can be complex and time consuming, and a multidisciplinary approach is often required. Materials and Methods: The patient was a 38-year-old woman with a chief complaint of saving and realigning her elongated maxillary left central incisor. This paper describes the successful combined periodontal regenerative (guided tissue regeneration) and prosthetic treatment and a 2-year follow-up of maxillary central incisor with pathologic tooth migration, deep intrabony defect, and poor prognosis. Results: The right maxillary central incisor was restored by laminate veneer and the left by all-ceramic crown. The patient had no pain and discomfort and was satisfied with the outcomes of her treatment for 2 years. She has maintained her recall program at the Department of Periodontology at 3 months interval. Conclusion: The key step in the successful treatment of PTM in anterior region is to obtain a high level of cooperation from the patient. Maintenance of the treatment result of PTM is dependent on the continuous preservation of periodontal health.

Anterior Cranial Fossa Dural Arteriovenous Fistulae Presenting as Subdural Hematoma

  • Anterior cranial fossa dural arteriovenous fistulae (DAVFs) are very rare and the bleeding rate is very high, especially in the presence of leptomeningeal draining vein and aneurysmal varix formation. A 85-year-old male patient presented with subdural hematoma (SDH). Magnetic resonance image (MRI) and transfemoral carotid angiography (TFCA) disclosed DAVF at the anterior cranial fossa with bilateral arterial feeders and leptomeningeal draining vein with varix formation. The lesion was treated by simple ligation of pial connecting vein using low frontal craniotomy. In comparison with DAVFs of the other sites, the anterior cranial fossa DAVF is difficult to manage by endovascular treatment due to not only the difficulty of transvenous access but the risk of visual impairment when using transarterial route. Surgical ligation of pial connecting vein is feasible and effective treatment.

Infraoptic Course of Both Anterior Cerebral Arteries

  • A 28-year-old woman was referred to our hospital with a sudden, very severe headache. Brain computed tomographic angiography showed a saccular cerebral aneurysm at the bifurcation of the left middle cerebral artery and infraoptic courses of both anterior cerebral arteries. The anterior cerebral arteries were seen to arise from the ipsilateral internal cerebral arteries at the level of the origin of the ophthalmic artery, passed underneath the ipsilateral optic nerve, and turned upward at ventral portion of the optic chiasm. Infraoptic course of the proximal anterior cerebral artery is an extremely rare anomaly and is often associated with cerebral aneurysms. We report the clinical features, radiological findings, and possible genesis of this anomaly with a literature review.
