Objectives : The purpose of this study was to evaluate effects of taping therapy on recovery of behavioral symptoms and neural excitability of the lumbar spinal cord in rat model for ankle sprain. Methods : Adult Sprague-Dawley rats was used and divided into 3 experimental groups: normal group(n=6), ankle sprain(n=6), and ankle sprain with taping treatment(n=6). In order to induce ankle sprain the right ankle joint was injured with 4~5 repetitive over-flexions and over-extensions manually. The severity of joint pain was evaluated by measuring foot weight bearing force ratio(FWBRF) of the hind limb and the injury-induced edema formation by diameter of the joint following ankle sprain. The changes of neural excitability in the lumbar spinal cord was tested by observation of cFos protein expression, a metabolic marker for neural excitation. Results : Severity of ankle injury induced in this experiment coincided with Grade 1 ankle sprain. Compared with ankle sprain group, ankle sprain+taping showed a significant reductions of joint pain as well as of edema formation at the ankle joint following ankle sprain. There was significant upregulation of cFos-immunoreactive neurons in the lumbar spinal cord 24 hours after ankle sprain. In contrast, taping therapy resulted in significant inhibition of cFos-immunoreactive neurons in the lumbar spinal cord. Conclusions : Collectively, these results suggest that taping therapy may be an alternative therapeutic intervention for symptom recovery of the mild ankle sprain.
Objectives : The purpose of this study was to determine whether electroacupuncture(EA) is effective in reducing pain on the severe ankle sprain classified as grade 3 in rats. Methods : The severe(grade 3) ankle sprain model was induced surgically by ankle ligament injury(the anterior talofibular, the calcaneofibular and the posterior talofibular) in the Sprague-Dawley rats(180~250 g). The effects of EA on weight bearing forces(WBR) of the affected foot were examined in a rat model of ankle sprain. EA was applied to either SI6, ST37, GB34, GB39 or GB42 acupoints by trains of electrical pulses(2 Hz, 1 ms pulse width, 2 mA intensity) for 15 min. Results : Cutting of the lateral ankle ligament complex produced the severe ankle sprain symptoms as grade 3. EA of the contralateral SI6 resulted in more analgesic effect than one of ipsilateral SI6 even though there was significant effect. EA of the ipsilateral GB34 and GB39 produced potent analgesic effects on the surgical ankle sprained pain behaviors. However, there were no significant analgesic effects in the contralateral GB34 and GB39 EA groups. In addition, both side of ST37 and GB42 did not result in analgesic effect on the surgical ankle sprained rat. Conclusions : The data suggest that EA induced analgesia shows point specificity on the severe ankle sprained pain model classified as grade 3.
Objective : Frutus gardeniae, seed of Gardenia jasminoides Ellis is one of the crude drugs used for the treatment of inflammatory condition in oriental medicine. Methodes : The present study aimed to examine the analgesic effect and anti-inflammatory effect of Frutus gardeniae extract (FGE) on a rat model of ankle sprain pain, and the relations between FGE-induced effect and endogenous nitric oxide (NO) and inducible NO synthase (iNOS), cyclooxygenase-2 (COX-2), and c-Fos protein expression in the spinal cord. As a chronic pain model, ankle sprain pain model was used to test the effect of FCE injection applied to acupuncture point. After the induction of ankle sprain, rats subsequently showed a reduced stepping force of the affected limb for at least the next 4 days. The reduced stepping force of the limb was presumably due to a painful knee. FGE dissolved in normal saline was injected several acupoints. Results : After the treatment, behavioral tests measuring stepping force were periodically conducted during the next 8 hours. FGE produced significant improvement of stepping force of the hindlimb affected by the ankle sprain lasting at least 4 hours. FGE produced the improvement of stepping force of the affected hindlimb in a dose-dependent manner. In addition, FGE injection showed inhibitory effect on the paw edema induced by ankle sprain. Both NO production and iNOS, COX-2 protein expression increased by ankle sprain were suppressed by FGE. FGE on combination with electroacupuncture (EA) produced more powerful and longer lasting improvement of stepping force of the hindlimb affected by the ankle sprain than either FGE or EA did. The present study suggest that FGE produces a potent analgesic effect on the ankle sprain pain model of the rat and that FGE-induced analgesia modulate endogenous NO through the suppression of iNOS/COX-2 protein expression.

A common source of persistent pain in humans is the lateral ankle sprain. Methods : To model this condition, the rat's right ankle was bent repeatedly, overextending lateral ligaments, for 4 min under enflurane anesthesia. The rat subsequently showed swelling of the ankle and a reduced stepping force of the affected limb for the next several days. The reduced stepping force of the limb was presumably due to a painful ankle. EA was applied to the several acupuncture point on the contralateral forelimb for 30 min under gaseous anesthesia. After the termination of EA, behavioral tests measuring stepping force and Paw volume were Periodically conducted during the next 4 h and 18 h respectively. Results : EA applied to $SI_6$ with won-rak point produced more powerful improvement of stepping force of the sprained foot than to $SI_6$ alone lasting for at least 4 h. However, neigher $KI_4$ point nor $BL_{64}$ point produced any significant increase of weight bearing force. The improvement of stepping pressure was interpreted as an analgesic effect. The analgesic effect was specific to the acupuncture point since the analgesic effort on the ankle sprain pain model could not be mimicked by EA applied to $KI_4$ or $BL_{64}$. In addition, EA applied to $SI_6$ with won-rak combination point showed inhibitory effect on the paw edema induced by ankle sprain. Also, COX-2 protein expression increased by ankle sprain were suppressed by the EA stimulation. Conclusion : These data suggest that EA with won-rak combination point produces a more potent analgesic effect on the ankle sprain pain model in the rat and that EA with won-rak combination point induced anti-inflammatory effect through the suppression of COX-2 protein expression.

Objectives: Pharmacopuncture which is a combination of acupuncture and herbal medicine helps to prevent and treat the diseases and symptoms including various pains. However, little was known about the therapeutic effects and its mechanisms on acute pain, although pharmacopuncture has been used frequently in acupuncture clinics. Acupuncture is known for producing analgesia for persistent ankle sprain pain in human. Furthermore, it also produces analgesia in a rat model of ankle sprain pain. Methods: To illuminate the underlying mechanisms of capsaicin pharmacopuncture-induced analgesia, weight bearing force (WBF) was observed on the acute ankle sprained rat model. Ankle sprain was induced in the rat by manually hyper-extending ligaments of the right ankle. Capsaicin pharmacopuncture was applied to SI6 (Yanglo) on the left forelimb (contralateral to the sprained ankle). Results: In behavioral test, capsaicin pharmacopuncture produced marked analgesic effects on acute ankle sprained animals as measured by WBF of the affected limb similar to manual acupuncture. Capsaicin pharmacopuncture was also suppressed by serotonin (5-HT) receptor antagonist methysergide (2 mg/kg, Lp.), but not by opioids receptor antagonist naltrexone (10 mg/kg, Lp.) and alpha adrenoceptor antagonist phentolamine (5 mg/kg, Lp.). Conclusion: The data suggest that capsaicin pharmacopuncture-induced analgesia is accomplished by activating the descending serotonergic inhibitory systems.

The reduced stepping force of the limb was presumably due to a painful ankle. EA was applied to the several acupuncture point on the contralateral forelimb for 30 min under gaseous anesthesia. After the termination of EA, behavioral tests measuring stepping force were periodically conducted during the next 4 h. EA applied to SI-6 point produced a significant improvement of stepping force of the sprained foot lasting for at least 2 h. However, neigher LI-4 point nor TE-3 point produced any significant increase of weight bearing force. The improvement of stepping pressure was interpreted as an analgesic effect. The analgesic effect was specific to the acupuncture point since the analgesic effect on the ankle sprain pain model could not be mimicked by EA applied to a nearby point, LI-4 or TE-3. The analgesic effect of EA applied to SI-6 was more powerful when EA was applied by low-frequency and high-intensity stimulation. In addition, this effect need to be stimulated more than 15 min. Conclusions : These data suggest that EA produces a potent analgesic effect on the ankle sprain pain model in the rat. This analgesic effect is produced by applying EA to a Tae-Yang meridian at opposite side from the painful area in a stimulus point-specific way.
Objectives : The objectives of this study is to compare the effects of electroacupuncture(EA), warm needling(WN) and Radio Frequency warm needling(RFWN) stimulations on the acupoints at the artificially damaged ankles of Sprague-Dawley rats, which could be classified as the Grade 3. Methods : The foot weight bearing force ratio(FWBFR) of ankle sprain was measured first at 24 hours after without any other stimulations. Pain Recovery Index(PRI) represents the analgesic level, and modified Pain Recovery Index(mPRI) shows the accumulated recovery level. PRI was measured at 2 hours after each stimulus on GB34, GB39 and GB42, and mPRI was during 7 days. Results : EA stimulation of GB34 and GB39 acupoint in grade 3 ankle sprain showed a marked analgesic and recovery effect. RFWN of GB42 exhibited significant analgesic and pain recovery effect. RFWN of GB34 resulted in pain recovery effect but not analgesic effects, and RFWN of GB39 resulted in analgesic effect 2 hours after but not pain recovery. However, WN did not affect the pain recovery among three acupoints at all. Conclusions : In the Grade 3 ankle sprain model, the difference of analgesic effects were explained by the acupoints and the stimulation methods according to the accumulated recovery effects during 7 days. It is insufficient to determine that a certain acupoint has a specific analgesic effect depending on the stimulation method by the results of this study. Therefore, the effects of each stimulation on the acupoints in any other meridians at the sprained ankle should be compared and analyzed.

Objectives : This study was aimed to determine which pharmacopuncture was effective in relieving pain in standard pain model, acute grade 3 ankle sprain rat model. Methods : Percentage changes of weight bearing ratio(WBR), as pain indicator, were measured after anti-inflammation, neutro Eohyul, Hwangryunhaedock-decoction pharmacopuncture were injected to Guheo ($GB_{40}$). Results : In three kinds of pharmacopuncture, neutro Eohyul and Hwangryunhaedock-decoction pharmacopuncture represented more effect in percentage changes of WBR than surgical(non treatment), needling group. Conclusions : Comparing of pharmacopuncture in grade 3 ankle sprain rat model, these results provide basis to select pharmacopuncture in sprain disease. Furthermore, mechanism of pharmacopuncture's analgesia, anti-inflammation should be progressed.
Objectives : Electroacupuncture(EA) has been used effectively in producing analgesia on ankle sprain pain of humans and animals. Currently to examine the underlying mechanisms of the EA-induced analgesia, the effects of EA on weight-bearing forces(WBR) were examined at ankle sprain classified as grade 3 in rats. Methods : The severe ankle sprain classified as grade 3 was induced surgically by ankle ligament injury in the Sprague-Dawley rats. WBR of the affected foot were examined to evaluate effects and mechanism of EA(2 Hz, 1 ms pulse width, 2 mA intensity, for 15 min) which was applied to either SI6, GB34, or GB39 acupoints. The rats were pretreated with naltrexone(10 mg/kg, i.p.) as an opioid receptor antagonist or phentolamine(5 mg/kg, i.p) as an ${\alpha}$-adrenoceptor antagonist at 30 min before EA. Results : The daily repeat EA at either SI6, GB34, or GB39 showed significant analgesic effects on the severe ankle sprain. Particularly, daily EA at GB34 showed more potent analgesic effect than the others. In addition, the naltrexone pretreatment completely blocked the analgesic effect of EA at GB34, indicating the involvement of the endogenous opioid system in mediating the effect of EA at GB34. However, the phentolamine pretreament blocked analgesic effects of EA at either SI6 or GB39, indicating the involvement of ${\alpha}$-adrenoceptors in mediating the effect of EA at either SI6 or GB39. Conclusions : These data suggest that EA-induced analgesia on ankle sprain pain is mediated through either endogenous opioids or ${\alpha}$-adrenoceptors dependant on acupoint specific pattern.
