Objective : Cervical ossification of the posterior longitudinal ligament (OPLL) can be treated via anterior or posterior approach, or both. The optimal approach depends on the characteristics of OPLL and cervical curvature. Although most patients can be successfully treated by a single surgery with the proper approach, renewed or newly developed neurological deterioration often requires repeat surgery. Methods : Twenty-seven patients with renewed or newly developed neurological deterioration requiring salvage surgery for multi-segment cervical OPLL were enrolled. Ten patients (group AP) underwent anterior approach, and 17 patients (group PA) underwent posterior approach at the initial surgery. Clinical and radiological data from initial and repeat surgeries were obtained and analyzed retrospectively. Results : The intervals between the initial and repeat surgeries were $102.80{\pm}60.08months$ (group AP) and $61.00{\pm}8.16months$ (group PA) (p<0.05). In group AP, the main OPLL lesions were removed during the initial surgery. There was a tendency that the site of main OPLL lesions causing renewed or newly developed neurological deterioration were different from that of the initial surgery (8/10, p<0.05). Repeat surgery was performed for progressed OPLL lesions at another segment as the main pathology. In group PA, the main OPLL lesions at the initial surgery continued as the main pathology for repeat surgery. Progression of kyphosis in the cervical curvature (Cobb's angle on C2-7 and segmental angle on the main OPLL lesion) was noted between the initial and repeat surgeries. Group PA showed more kyphotic cervical curvature compared to group AP at the time of repeat surgery (p<0.05). Conclusion : The reasons for repeat surgery depend on the type of initial surgery. The main factors leading to repeat surgery are progression of remnant OPLL at a different segment in group AP and kyphotic change of the cervical curvature in group PA.
The BARC flow is studied via Direct Numerical Simulation at a relatively low turbulent Reynolds number, with focus on the geometrical representation of the leading-edge (LE) corners. The study contributes to further our understanding of the discrepancies between existing numerical and experimental BARC data. In a first part, rounded LE corners with small curvature radii are considered. Results show that a small amount of rounding does not lead to abrupt changes of the mean fields, but that the effects increase with the curvature radius. The shear layer separates from the rounded LE at a lower angle, which reduces the size of the main recirculating region over the cylinder side. In contrast, the longitudinal size of the recirculating region behind the trailing edge (TE) increases, as the TE shear layer is accelerated. The effect of the curvature radii on the turbulent kinetic energy and on its production, dissipation and transport are addressed. The present results should be contrasted with the recent work of Rocchio et al. (2020), who found via implicit Large-Eddy Simulations at larger Reynolds numbers that even a small curvature radius leads to significant changes of the mean flow. In a second part, the LE corners are fully sharp and the exact analytical solution of the Stokes problem in the neighbourhood of the corners is used to locally restore the solution accuracy degraded by the singularity. Changes in the mean flow reveal that the analytical correction leads to streamlines that better follow the corners. The flow separates from the LE with a lower angle, resulting in a slightly smaller recirculating region. The corner-correction approach is valuable in general, and is expected to help developing high-quality numerical simulations at the high Reynolds numbers typical of the experiments with reasonable meshing requirements.
Objective: To investigate the physical appearance and therapeutic changes that occur with the performance of Schroth exercise in patients with scoliosis. Design: Randomized controlled trial. Methods: Fifteen subjects with maximum curvature of the lumbar who were diagnosed with idiopathic scoliosis had volunteered to participate in the study. Eight subjects were included in the experimental group where they performed the Schroth Therapeutic Exercise and the other seven were included in the control group. The experimental group underwent 2 hours of weekly treatment for 12 weeks, while the control group did not during the same period based on the decisions of patients or guardians. The Mann-Whitney rank test was carried out to compare the treatment results of the two groups, and the comparison within the group was done by Wilcoxon signed-rank test. The vertebral rotation angle (VRA) was by Scoliometer, and difference of rotated and curved portion volume (DV) between both sides on the major curvature portion measured by 3D human body scanning system. Results: In the experimental group, 12 weeks of Schroth exercise therapy has significant improved in correction rate (CR) in Cobb's angle (CA), VRA, and DV between both sides on the major curvature portion (p<0.05), while significant differences were not found between the groups regarding weight bearing difference in both feet (WD) and DV (p<0.05). Conclusions: Schroth exercise performance showed significant changes in the patient's therapeutic changes (CA, VRA), but the physical appearance (DV, WD) was not significant, indicating that external changes in the treatment goal setting are more difficult goals to achieve.
PURPOSE. The aim of this study was to evaluate the thickness of buccal and palatal alveolar bone and buccal bony curvature below root apex in maxillary anterior teeth of Korean adults using Cone-beam CT images. MATERIALS AND METHODS. The 3D image was reconstructed with dicom file obtained through CBCT from 20 - 39 year old Korean subjects (n = 20). The thickness of buccal and palatal plate, root diameter, the buccal bony curvature angle below root apex and the distance from root apex to the deepest point of buccal bony curvature were measured on maxillary anterior teeth area using OnDemand3D program. RESULTS. Mean thickness of buccal plate 3 mm below CEJ was $0.68{\pm}0.29\;mm$ at central incisor, $0.76{\pm}0.59\;mm$ at lateral incisor, and $1.07{\pm}0.80\;mm$ at canine. Mean thickness of palatal plate 3 mm below CEJ was $1.53{\pm}0.55\;mm$ of central incisor, $1.18{\pm}0.66\;mm$ of lateral incisor, $1.42{\pm}0.77\;mm$ of canine. Bucco-lingual diameter 3 mm below CEJ was $5.13{\pm}0.37\;mm$ of central incisor, $4.58{\pm}0.46\;mm$ of lateral incisor, and $5.93{\pm}0.47\;mm$ of canine. Buccal bony curvature angle below root apex was $134.7{\pm}17.5^{\circ}$ at central incisor, $151.0{\pm}13.9^{\circ}$ at lateral incisor, $153.0{\pm}9.5^{\circ}$ at canine. Distance between root apex and the deepest point of buccal bony curvature of central incisor was $3.67{\pm}1.28\;mm$ at central incisor, $3.90{\pm}1.51\;mm$ at lateral incisor, and $5.13{\pm}1.70\;mm$ at canine. CONCLUSION. Within the limitation of this study in Korean adults, the thickness of maxillary anterior buccal plate was very thin within 1mm and the thickness of palatal plate was thick, relatively. The buccal bony curvature below root apex of maxillary central incisor was higher than that of lateral incisor and canine and it seems that the buccal bony plate below root apex of central incisor is most curved.
The effect of diameter of rods or fibers on contact angle was studied in a vertical rod configuration. A contact angle measuring device described in the previous paper was optimized for the measurement of small-diameter fibers. It was shown that contact angles of water and hexadecane on nylon 6 monofilsments and glass rods increased with decrease of diameter below a critical diameter, which varied from one system to another. Beyond the critical value, contact angle of the liquid on the vertical glass rod reached to an equilibrium value which is equal to the unique value of the contact angle of the liquid drop on the horizontal glass plate.
Theoretical analysis using finite element method are peformed in order to clarify the formation of the flare-shape defect for multi-step tube sinking process. The parameters of concern were the friction between the tube and the die, and geometrical parameters, such as the die inclination angle, the diameters of the die entrance and exit, and the curvature at the corner of the die exit. The effect of the curvature at the comer of the die exit is dominant for determining the flare-shape defect. In order to minimize the flare-shape defect the curvature at the corner of the die exit should be increased up to a certain level(120mm). Using three-step tube sinking die sets which have different curvatures at the comer of the die exit, several numbers of tests were performed and its results are compared with that of theoretical analysis.
A numerical computation of turbulent wirling flow in a stationary pipe is presented in this work. Major concerns of this study are: 1) To approve similarity laws which were verified experimentally. 2) To investigate the effects of curvature modification for the K- .epsilon. model. To account for effects of swirl, Rodi's curvature correction and Kim & Chung's are applied. The governing differential equations for eliptic flow are discretized by control volume formulation method, and the discretized equations are calculated ay line by line TDMA and SIMPLE algorithm. The computational results also satisfy similarity laws which are based on swirl angle as in experiments. And the curvature modification of Rodi improves compuational accuacy than the standard K- .epsilon. model. But such lower order closure models are not adequate for the prediction of this complex flow.
Objectives : The purpose of this study was to investigate the correlation between cervical spine curvature and thermography on neck pain patients. Methods : The cervical spine curvature was assessed on lateral view of plain radiograph by three measurements(Depth of cervical curve, Method of Jochumsen, Angle of cervical curve), then the neck pain patients were divided into straight curvature group(Straight group), normal curvature group(Control I), and no neck pain group(Control II) was selected by random sampling. I measured temperature of the both side Pungji(風池, G20), Kyonjong(肩井, G21), Chonjong(天宗, SI11) by thermography examination on Experimental group(Straight group) and control group(Control I, II), then analyzed the temperature statically with student's t-test, ANOVA. Results : The temperature of six point on Straigt group was lower than that of Control I, but had no significant difference(p>0.05). The temperature of six point on Straigt group was lower than that of Control II. The descent of temperature on Lt. Pungji(風池, G20), Lt. Kyonjong(肩井, G21), Rt. Chonjong(天宗, SI11) had significance(p<0.05). There is no corelation between onset period and mean temperature of six points. Conclusions : Thermography represnets the presence of neck pain, but has no correlation with straight of cervical curvature.
1. The field test of our experiment comparing the traditional syckle and the new manually pulled cutter showed that the working efficiency of the manually pulled cutter is about two times higher than that of the syckle. The operator can standerect while using the manually pulled cutter, thus reducing fatigue. Because of the reduction in body fatigue, the operator is able to work more total hours, making the manually pulled harvester three times more efficient than the syckle. 2. The optimum angle for the V-shaped cutter blade varies according to the number of the hiller, but in manufacturing the cutter, the range must be controlled between $35^{\circ}{\sim}45^{\circ}$. 3. The radius of curvature of the V-shaped blade is closely related to the blade angle. The best results were obtained when the radii of curvature were r=32cm with $35^{\circ}$ angle, r=153mm with $45^{\circ}$ angle, and r=220mm with $40^{\circ}$ angle. 4. The hardness distribution test of the blade showed great differences between foreign made and domestic made blades. The heat treatment method must be greatly improved for superior hardness distribution. 5. Although the efficiency of the manually pushed cutter is equal to that of the manually pulled cutter, the manually pushed cutter is not only more expensive to produce but also has a much more complicated structure than the manually pulled cutter. Because of our rural economic conditions, the power harvester which is utilized in foreign countries can not yet be introduced here. Thus, the manually pulled cutter should be popularized in our rural areas until economic conditions will allow us to introduce power harvesters. 6. The use of the manually pulled cutter will make it possible for an operator to harvest an area in only one-third of the time that it would take to harvest the same area with a traditional syckle, and it will make it possible for an operator to earn about 8,000 won for harvesting 10 Jung-bo: such benefits will help in increasing farm income and will contribute to farm modernization.
A Femur is the longest and largest bone which supports body in human musculoskeletal structure. Therefore, it may cause heavy loss of blood when it is suffered by a simple or complex fracture, and the complication is very dangerous with a possibility of severe tissue damage. In this study, the femoral cancellous angle change is estimated in order to design the Korean femoral IM nail. Generally, it is various in the size and curvature of femoral cancellous bone depending on patient's body dimension. Therefore surgeon has difficulty in fitting this femoral IM nail to the patient in the surgical procedure. In our study, we tries to estimate femoral lateral curve angle with more precise method based on CT image of the femur and utilize this information on the design of femoral IM nail for Korean patients.
