  • 원위부 대동맥궁에 발생한 대동맥류에 대한 수술을 위해서는 정중흉골 절개술 또는 측방개흉을 통한 수술방법이 있으나 문합 부위의 시야 확보가 용이하지 않은 경우가 자주 있다. 수술 대신 중재시술로 스텐트 그라프트를 삽입하는 방법 또한 가능하지만 동맥류보다 근위부에 충분한 고정 거리를 확보하기 위해서 대동맥궁 분지 일부를 결찰하고 우회로를 조성해야 할 필요가 있다는 제한이 있다. 저자들은 이들 방법의 장점을 취하고자 정중흉골절개술 후 완전 순환 정지 상태에서 대동맥궁을 치환하는 수술 중 외과적 문합 대신 스텐트 그라프트를 이용하여 인조혈관 원위부를 고정하는 방법을 취하여 양호한 결과를 얻은 경험을 보고한다.

Meningeal Layers Around Anterior Clinoid Process as a Delicate Area in Extradural Anterior Clinoidectomy : Anatomical and Clinical Study

  • Objective : Removal of the anterior clinoid process (ACP) is an essential process in the surgery of giant or complex aneurysms located near the proximal internal carotid artery or the distal basilar artery. An extradural clinoidectomy must be performed within the limits of the meningeal layers surrounding the ACP to prevent morbid complications. To identify the safest method of extradural exposure of the ACP, anatomical studies were done on cadaver heads. Methods : Anatomical dissections for extradural exposure of the ACP were performed on both sides of seven cadavers. Before dividing the frontotemporal dural fold (FTDF), we measured its length from the superomedial apex attached to the periorbita to the posterolateral apex which connects to the anterosuperior end of the cavernous sinus. Results : The average length of the FTDF on cadaver dissections was 7 mm on the right side and 7.14 mm on the left side. Cranial nerves were usually exposed when cutting FTDF more than 7 mm of the FTDF. Conclusion : The most delicate area in an extradural anterior clinoidectomy is the junction of the FTDF and the anterior triangular apex of the cavernous sinus. The FTDF must be cut from the anterior side of the triangle at the periorbital side rather than from the dural side. The length of the FTDF incision must not exceed 7 mm to avoid cranial nerve injury.

The Quality of Life of Patients with Good Outcomes after Anterior Circulation Aneurysm Surgery Assessed by the World Health Organization Quality of Life Instrument-Korean Version

  • Objective : Even in the patients with neurologically good outcome after intracranial aneurysm surgery, their perception of health is an important outcome issue. This study aimed to investigate the quality of life (QOL) and its predictors of patients who had a good outcome following anterior circulation aneurysm surgery as using the World Health Organization Quality of Life instrument-Korean version. Methods : We treated 280 patients with 290 intracranial aneurysms for 2 years. This questionnaire was taken and validated by 99 patients whose Glasgow Outcome Scale score was 4 and more and Global deterioration scale 3 and less at 6 months after the operation, and 85 normal persons. Each domain and facet was compared between the two groups, and a subgroup analysis was performed on the QOL values and hospital expenses of the aneurysm patients according to the type of craniotomy, approach, bleeding of the aneurysm and brain injury. Results : Aneurysm patients showed a lower quality of life compared with control patients in level of independence, psychological, environmental, and spiritual domains. In the environmental domain, there were significant intergroup differences according to the type of craniotomy and the surgical approach used on the patients (p<0.05). The hospital charges were also significantly different according to the type of craniotomy (p<0.05). Conclusion : Despite good neurological status, patients surgically treated for anterior circulation aneurysm have a low quality of life. The craniotomy size may affect the QOL of patients who underwent an anterior circulation aneurysm surgery and exhibited a good outcome.

Angiographic Features and Clinical Outcomes of Intra-Arterial Nimodipine Injection in Patients with Subarachnoid Hemorrhage-Induced Vasospasm

  • Objective : The aim of this study was to determine the role of intra-arterial (IA) nimodipine injections for cerebral vasospasm secondary to ruptured subarachnoid hemorrhage (SAH) and to investigate the factors that influence vasodilation and clinical outcomes. Methods : We enrolled 29 patients who underwent aneurysm clipping for ruptured cerebral aneurysms between 2009 and 2011, and who received IA nimodipine after subsequently presenting with symptomatic vasospasm. The degree of vasodilation shown in angiography was measured, and the correlation between the degree of vasodilation and both the interval from SAH to cerebral vasospasm and the interval from clipping to cerebral vasospasm was determined. The change in blood flow rate after IA injection was assessed by transcranial Doppler ultrasound. Multiple clinical parameters were completed before and after IA nimodipine injection to evaluate any improvements in clinical symptoms. Results : For eight patients, Glasgow Coma Scale (GCS) scores increased by two or more points. The regression analysis demonstrated a positive correlation between the change in GCS scores after IA nimodipine injection and the change in blood vessel diameter (p=0.025). A positive correlation was also observed between the interval from SAH to vasospasm and the change in diameter (p=0.040); and the interval from clipping to vasospasm and the change in diameter (p=0.022). Conclusion : IA nimodipine injection for SAH-induced vasospasm led to significant vasodilation in angiography and improvement in clinical symptoms without significant complications. Our findings suggest that IA nimodipine injection should be utilized when intractable vasospasm develops despite rigorous conservative management.

대동맥류의 수술적 치료에 관한 임상적 연구 (Clinical Study of Surgical Treatment of Aortic Aneurysm)

  • 인제대학교 부속 부산백병 원 흉부외과학교실에서는 지난 87년 10월부터 96년 1월까지 총25례의 흉부 및 복부 대동맥류를 수술로 치험하였다. 환자의 연령은 최소 26세에서 최고 73세(평균연령 52. 5세)였으며 남자가 13명 여자가 12명이였다. 발생 부위에 따라흉부 대동맥류가 18례 복부 대동맥류가7례였다. 원인에 따른 분류에서 16례가 박리성이 였으며 9례가 비박리성 대동맥류였다. 동맥류의 위험 인자로는 고혈압이 가장 많았으며 기타 고지질증, Marfan증후군등이였다. 흉부 대동맥류는 침범 부위에 따라 동맥류 절제술과 인조혈관 대치술 및 관상동맥 이식술등을 증례에 따라적절히 시행하였다. 복부 대동맥류는 동맥류 절제후 침범 부위에 따라 일자 또는 역Y형 인조혈관 을 대 치시켰다. 총 5례의 술후 사망이 있어서 수술 사망률은 20%였으며 생존 환자는 특별한 합병증 없이 퇴원하였다.

만성 대동맥 박리증(DeBakey typeI) 환자의 전 대동맥 치환술 1례 보고 (Total Replacement of Aorta in Chronic Type 1 Aortic Dissection)

  • 최근 흉부 대동맥의 수술적 치료가 과거에 비해 좋은 성적을 얻고 있다. 전 흉부 대동맥을 포함하는 대동맥류나 대동맥 박리증의 경우대개 두단계나세단계로나누어 수술하였다. 본원에서는 숲 대동맥을 포함한 드베키 타입 I 만성 대동맥 박리증 환자에 있어서 한번에 출 대동맥을 치환하는 수술을 시행하였다. 환자는 호흡 부전과 Marfan 환자의 특징적 외모를 갖고있는 25세 남자로 심한 대동맥 부전증과 만성 드베키 타입 I대동맥 박리증 진단하에 수술시행하였다. 수술소견상상행 대동맥이 매우 커져 있었으며 상행대동맥에 두 개의 내막 파열이 관찰되 었고 가혈관으로 부터 늑간 동맥들과 우측 신동맥이 혈공급을 받고있었다. 벤탈 술식과 함께 술 대동맥 치환술을 성공적으로 시행하였으며, 환자는 현재까지 큰 합병증없이 외래를 통해 추적 관찰중이다.

A Comparative Study of Abdominal Aortic Aneurysm: Endovascular Aneurysm Repair versus Open Repair

  • Background: Endovascular aneurysm repair (EVAR) has dramatically changed the management of abdominal aortic aneurysms (AAAs) as the number of open aneurysm repairs have declined over time. This report compares AAA-related demographics, operative data, complications, and mortality after treatment by open aneurysm repair or EVAR. Methods: We retrospectively reviewed 136 patients with AAAs who were treated over an 8-year time period with open aneurysm repair or EVAR. Results: The mean age of the EVAR group was higher than that of the open repair group (p=0.001), and hospital mortality did not differ significantly between groups (p=0.360). However, overall survival was significantly lower in the EVAR group (p=0.033). Conclusion: Although EVAR is the primary treatment modality for elderly patients, it would be ideal to set slightly more stringent criteria within the anatomical guidelines contained in the instructions for use of the EVAR device when treating younger patients.

The Usefulness of 3D-CT Angiography as a Screening Tool for Vascular Abnormalities in Spontaneous ICH Patients

  • Objective : We investigated the incidence of the vascular abnormalities associated with spontaneous intracerebral hemorrhage [ICH] using three-dimensional computed tomographic angiography [3D-CTA]. Methods : We prospectively assessed consecutive 76 patients with spontaneous intracerebral hemorrhage [ICH] who underwent 3D-CTA between June 2003 and May 2005. The patients with a recent history of trauma or mainly subarachnoid hemorrhage were excluded. We investigated relationship between vascular abnormality and ICH location. The findings of 3D-CTA were classified as one of three patterns with ICH; type A [without evidence of vascular abnormality], type B [with no vascular abnormality as the source of hemorrhage, but with incidental vascular abnormality], and type C [presence of a vascular abnormality as the source of hemorrhage]. Results : Sites of ICH were lobar 26, basal ganglia 23, thalamus 17, posterior fossa 6 and dominant intraventricular hemorrhage [IVH] 4. Among 76 patients, sixteen [21.1%] vascular abnormalities were noted excluding 13 cases of stenoocclusive disease. Sixteen cases included 6 cases of cerebral aneurysms [7.9%], 5 moyamoya diseases [6.6%], 4 arteriovenous malformations [5.3%] and 1 dural sinus thrombosis [1.3%]. Lobar ICH [30.8%] had a higher vascular abnormalities than other types, and younger age [<40] group had a higher incidence of vascular abnormalities than old age group. The patterns of 3D-CTA include sixty cases [79.0%] of type A, 6 cases [7.8%] of type Band 10 cases [13.2%] of type C. The vascular abnormalities were found in 8 [13.5%] of 59 hypertensive patients and 8 [47.0%] of 17 non-hypertensive patients [p=0.006]. Conclusion : 3D-CT angiography is considered a useful screening tool for ICH patients with suspected cerebrovascular abnormalities and should be considered in such clinical settings, especially in lobar type and in non-hypertensive younger patients.

가와사끼병 환아에서 발생한 거대관상동맥류 내 혈전의 성공적 용해요법과 장기 치료 및 경과 1례 (Thrombolytic Therapy and Long Term Follow-up Study in a Child with Kawasaki Disease Complicated by Giant Coronary Aneurysm with Thrombosis)

  • 환아는 3개월된 영아로 발병 10일째에 본원에 내원하여 초기에 적절한 진단과 치료를 받지 못했으며 발병 2주에 시행한 심초음파상 거대 관상동맥류를 보였다. 이에 정맥용 면역글로불린 투여와 aspirin과 dipyridamole로 치료 중 발병 24일째 혈전증이 동반되어 urokinase와 heparin의 IV 투여를 통한 혈전용해 치료로 혈전을 완전히 제거할 수 있었다. 이후 5년간 aspirin, dipyridamole, coumadin을 투여하면서 심전도와 심초음파검사를 반복하며 외래 추적 관찰 중 점차 좌측관상동맥내 혈전이 증가하고 내경이 점차 좁아지는 소견과 함께 협착이 의심되었다. 발병 5년 후 시행한 관상동맥 혈관조영술상 관상동맥 좌측 전하지가 완전 폐색되고 우측관상동맥으로부터 발달한 측부순환에서 혈류를 공급받고 있었고, 핵의학 검사상 좌심실의 전벽과 중격, 일부 외벽에 완전 가역적 관류 결손을 보였던 가와사끼병 환아 1례를 경험하여 보고한다.

Analysis of Clip-induced Ischemic Complication of Anterior Choroidal Artery Aneurysms

  • Objective: The surgical approach for anterior choroidal artery (AChA) aneurysm is typically similar to those used for other supraclinoid internal carotid artery (ICA) lesions. However, the surgical clipping of this aneurysm is complicated and as a result. can result in postoperative ischemic complications. The purpose of this study was to clarify the risk of clip-induced ischemic complication in AChA aneurysm and to get the benefits for helping decision making. Methods: We retrospectively investigated 53 cases (4.0%) of AchA aneurysm treated surgically. We divided the AChA aneurysm to 3 subtype according to the origin of aneurysmal neck; A type originating from the AChA itself. J type from junction of AChA and ICA and I type from the ICA itself. We evaluated brain CT about 1 week post-operative day to confirm the low density in AChA territory. Results: Ruptured aneurysm was 26 cases and unruptured aneurysm 27 cases. The aneurysmal subtype of A, J, and I was 13, 17, and 23 cases. Of the 53 cases who performed surgical neck clipping, twelve (22.6%) had postoperative AChA distribution infarcts. Increased infarct after neck clipping had statistic significance in non-I subtype (r=0.005) Conclusion: AChA aneurysm surgery carries a significant risk of postoperative stroke. Don't always stick to clipping only, especially in non-I type of incidental small aneurysm, which has high risk of post-clip ischemic complications.
