왕버들과 굴참나무의 뿌리, 줄기 및 가지에 있어서 이기목부(二期木部)의 해부학적(解剖學的) 비교(比較) (An Anatomical Comparison of the Secondary Xylem in the Branch, Stem and Root of Salix glandulosa and Quercus variavilis)

  • 산공재(散孔材)인 왕버들(Salix glandulosa Seemenn)과 환공재(環孔材)인 굴참나무(Quercus variabilis BI.)의 뿌리, 줄기 및 가지에 있어서 2기(二期) 목부조직(木部組織)을 해부학적(解剖學的)으로 비교관찰(比較觀察)하였다. 굴참나무의 뿌리의 도관분포(導管分布)는 줄기와 달리 산공형(散孔型)이다. 뿌리의 연륜(年輪) 경계(境界)는 양(兩) 수종(樹種)에서 대체(大體)로 불분명(不分明)하다. 도관요소(導管要素) 및 대섬유(大纖維)는 뿌리에서 제일 길고, 가지에서 가장 짧다. 또한 도관(導管)의 직경(直徑)은 뿌리, 줄기, 가지의 순(順)으로 좁아진다. 횡단면상(橫斷面上)에서 도관(導管)의 단위면적당(單位面積當) 분포수(分布數)는 가지에서 제일 많고 뿌리에서 가장 적다. 도관요소(導管要素)의 천공(穿孔)은 두 수종(樹種) 모두 단천공(單穿孔)이고, 천공판(穿孔板)의 경사각(傾斜角)은 뿌리, 줄기, 가지의 순(順)으로 작아진다. 접선단면상(接線斷面上)에서 방사조직(放射組織)은 뿌리가 줄기나 가지에 비(比)해 높은 반면(反面), 단위면적당(單位面積當) 분포수(分布數)는 뿌리에서 줄기나 가지에 비(比)해 적다.

Anatomic reconstruction for acromioclavicular joint injuries: a pilot study of a cost-effective new technique

  • Background: The treatment for acromioclavicular joint injuries (ACJI) ranges from a conservative approach to extensive surgical reconstruction, and the decision on how to manage these injuries depends on the grade of acromioclavicular (AC) joint separation, resources, and skill availability. After a thorough review of the literature, the researchers adopted a simple cost-effective technique of AC joint reconstruction for acute ACJI requiring surgery. Methods: This was a prospective single-center study conducted between April 2017 and April 2018. For patients with acute ACJI more than Rockwood grade 3, the researchers performed open coracoclavicular ligament reconstruction using synthetic sutures along with an Endobutton and a figure of 8 button plate. This was followed by AC ligament repair augmenting it with temporary percutaneous AC K-wires. Clinical outcomes were evaluated using the Constant Murley shoulder score. Results: Seventeen patients underwent surgery. The immediate postoperative radiograph showed an anatomical reduction of the AC joint dislocation in all patients. During follow-up, one patient developed subluxation but was asymptomatic. The mean follow-up period was 30 months (range, 24-35 months). The mean Constant score at 24 months was 95. No AC joint degeneration was noted in follow-up X-rays. The follow-up X-rays showed significant infra-clavicular calcification in 11 of the 17 patients, which was an evidence of a healed coracoclavicular ligament post-surgery. Conclusions: This study presents a simple cost-effective technique with a short learning curve for anatomic reconstruction of acute ACJI. The preliminary results have been very encouraging.

Mapping out the surgical anatomy of the lingual nerve: a systematic review and meta-analysis

  • Objectives: Understanding the lingual nerve's precise location is crucial to prevent iatrogenic injury. This systematic review seeks to determine the lingual nerve's most probable topographical location in the posterior mandible. Materials and Methods: Two electronic databases were searched, identifying studies reporting the lingual nerve's position in the posterior mandible. Anatomical data in the vertical and horizontal dimensions at the retromolar and molar regions were collected for meta-analyses. Results: Of the 2,700 unique records identified, 18 studies were included in this review. In the vertical plane, 8.8% (95% confidence interval [CI], 1.0%-21.7%) and 6.3% (95% CI, 1.9%-12.5%) of the lingual nerves coursed above the alveolar crest at the retromolar and third molar regions. The mean vertical distance between the nerve and the alveolar crest ranged from 12.10 to 4.32 mm at the first to third molar regions. In the horizontal plane, 19.9% (95% CI, 0.0%-62.7%) and 35.2% (95% CI, 13.0%-61.1%) of the lingual nerves were in contact with the lingual plate at the retromolar and third molar regions. Conclusion: This systematic review mapped out the anatomical location of the lingual nerve in the posterior mandible, highlighting regions that warrant additional caution during surgeries to avoid iatrogenic lingual nerve injuries.

Additional Surgical Method Aimed to Increase Distractive Force during Occipitocervical Stabilization : Technical Note

  • Objective : Craniovertebral junctional anomalies constitute a technical challenge. Surgical opening of atlantoaxial joint region is a complex procedure especially in patients with nuchal deformity like basilar invagination. This region has actually very complicated anatomical and functional characteristics, including multiple joints providing extension, flexion, and wide rotation. In fact, it is also a bottleneck region where bones, neural structures, and blood vessels are located. Stabilization surgery regarding this region should consider the fact that the area exposes excessive and life-long stress due to complex movements and human posture. Therefore, all options should be considered for surgical stabilization, and they could be interchanged during the surgery, if required. Methods : A 53-year-old male patient applied to outpatients' clinic with complaints of head and neck pain persisting for a long time. Physical examination was normal except increased deep tendon reflexes. The patient was on long-term corticosteroid due to an allergic disease. Magnetic resonance imaging and computed tomography findings indicated basilar invagination and atlantoaxial dislocation.The patient underwent C0-C3-C4 (lateral mass) and additional C0-C2 (translaminar) stabilization surgery. Results : In routine practice, the sites where rods are bound to occipital plates were placed as paramedian. Instead, we inserted lateral mass screw to the sites where occipital screws were inserted on the occipital plate, thereby creating a site where extra rod could be bound.When C2 translaminar screw is inserted, screw caps remain on the median plane, which makes them difficult to bind to contralateral system. These bind directly to occipital plate without any connection from this region to the contralateral system.Advantages of this technique include easy insertion of C2 translaminar screws, presence of increased screw sizes, and exclusion of pullout forces onto the screw from neck movements. Another advantage of the technique is the median placement of the rod; i.e., thick part of the occipital bone is in alignment with axial loading. Conclusion : We believe that this technique, which could be easily performed as adjuvant to classical stabilization surgery with no need for special screw and rod, may improve distraction force in patients with low bone density.

Analysis of Measurement Accuracy for Craniovertebral Junction Pathology : Most Reliable Method for Cephalometric Analysis

  • Objective : This study was designed to determine the most reliable cephalometric measurement technique in the normal population and patients with basilar invagination (BI). Methods : Twenty-two lateral radiographs of BI patients and 25 lateral cervical radiographs of the age, sex-matched normal population were selected and measured on two separate occasions by three spine surgeons using six different measurements. Statistical analysis including intraclass correlation coefficient (ICC) was carried out using the SPSS software (V. 12.0). Results : Redlund-Johnell and Modified (M)-Ranawat had a highest ICC score in both the normal and BI groups in the inter-observer study. The M-Ranawat method (0.83) had a highest ICC score in the normal group, and the Redlund-Johenll method (0.80) had a highest ICC score in the BI group in the intra-observer test. The McGregor line had a lowest ICC score and a poor ICC grade in both groups in the intra-observer study. Generally, the measurement method using the odontoid process did not produce consistent results due to inter and intra-observer differences in determining the position of the odontoid tip. Opisthion and caudal point of the occipital midline curve are somewhat ambiguous landmarks, which induce variable ICC scores. Conclusion : On the contrary to other studies, Ranawat method had a lower ICC score in the inter-observer study. C2 end-plate and C1 arch can be the most reliable anatomical landmarks.

In Vivo Three-dimensional Evaluation of the Functional Length of Glenohumeral Ligaments

  • The acromioclavicular-hook-plate is one of the surgical treatments for distal clavicle fracture and traumatic acromioclavicular (AC) joint dislocation. Although this procedure can obtain rigid and accurate anatomical reduction of the AC joint, secondary widening of the hook-hole in the acromion is often seen during postoperative follow-up. This complication is owing to the high-degree of mobility of the AC joint. Therefore, it is important to evaluate the effect on these complications due to the position of the hook-hole. The purpose of the present study is to investigate three-dimensionally the effect due to the position of the hook-hole during arm abduction motion. We studied in vivo and three-dimensional kinematics of the normal shoulder joint with use of a markerless bone-registration technique. Magnetic resonance images of 14 shoulders of 7 healthy volunteers were acquired in 7 positions between $0^{\circ}$ and $180^{\circ}$ of abduction. We created three-dimensional computer models of the bones and the acromioclavicular-hook-plate. Based on the three-dimensional kinematics data, we simulated the widening of the hook-hole each different positioning of the hook-hole. The widths of the hook-holes almost linearly increased. And these widths significantly increased, when we put the hook-hole on the acromion from AC joint to 20 mm and 25 mm posterior position.

손가락끝 손상의 치료 시 인조손톱의 유용성 (The Usefulness of the Artificial Nail for Treatment of Fingertip Injuries)

  • Purpose: The fingertip is the most commonly injured part of the hand and its injury frequently results in avulsion or crushing of a segment of the nail bed and fracture of the distal phalangeal bone. Restoration of a flat and smooth nail bed is essential for regrowth of a normal nail, which is important not only for cosmetic reasons but also for tactile capability of the fingertip. It is also anatomical reduction of the distal phalanx to promote patient's cosmetics and prevent nail bed deformity. Absence or no replacement of the nail plate results in obliterated proximal skin fold. When the avulsed nail plate cannot be returned to its anatomic position or when it is absent, we use a synthetic material for splinting the nail bed and alternative reductional method for distal phalangeal bone fracture, especially, instead of hardwares. Methods: From January of 2006 to June of 2009, a total of ten patients and fourteen fingers with crushing or avulsion injuries of the fingertip underwent using the artificial nails for finger splint. We shaped artificial nails into the appropriate sizes for use as fingernail plates. We placed them under the proximal skin fold and sutured to the fold proximally and to the lateral and medial edges of the nail bed or to the distal fingertip. Our splints were as hard as K-wire and other fixation methods and more similar to anatomic nail plates. Artificial nails were kept in place for at least 3 weeks. Results: No artificial nail related complication was noted in any of the ten cases. No other nail fold or nail bed complications were observed, except for minor distal nail deformity because of trauma. Conclusion: In conclusion, in order to secure the nail bed after injury and reduce the distal phalangeal bone fracture, preparing a nail bed splint from a artificial commercial nail is a cheap and effective method, especially, for crushed or avulsion injuries of the fingertip.

The Safety and Analgesic Effect of Datura Flos Pharmacopuncture in Sprague-Dawley Rats

  • Background: The aim of this study was to investigate the safety and analgesic effects of Datura Flos pharmacopuncture (DFP). Methods: The analgesic effects of DFP were assessed using mechanical (hot plate), chemical (formalin test), and thermal (von Frey filament test) pain tests. Forty male Sprague Dawley rats were assigned randomly into DFP (75 mg/kg, 150 mg/kg), lidocaine 0.5%, or normal saline group for treatment on Kl3. Gross pathology, histopathology, biochemistry and hematology were performed. Results: In the hot plate test, DFP at a high dose (HDDFP; 150 mg/kg) produced a significant analgesic effect, at 10 and 20-minutes post injection (p < 0.01). Low dose DFP (LDDFP; 75 mg/kg) also showed an analgesic effect at 10 minutes post injection (p < 0.01). In the formalin test, HDDFP produced an analgesic effect, for 0-10 and 10-20 minutes (p < 0.01) post treatment, whereas LDDFP showed analgesic effects between 10-20 minutes (p < 0.05). In the von Frey filament test, DF-H produced an analgesic effect, 10 (p < 0.01) and 20 minutes post treatment (p < 0.05). LDDFP showed analgesic effect at 10 minutes (p < 0.05). In the acupuncture response test, HDDFP produced an analgesic effect at 10 minutes post treatment (p < 0.05). DF-H did not cause any anatomical changes to the liver or kidney and there were no abnormalities in biochemistry or hematology. Conclusion: DF-H was not toxic and provided short term analgesia, suggesting it may be useful in the management of pain.

Study of frontal and ethmoid sinus of sinonasal complex along with olfactory fossa: anatomical considerations for endoscopic sinus surgery

  • The Functional endoscopic sinus surgery through transnasal approach is a common modality of treatment for disorders of the nasal cavity, paranasal air sinuses as well as cranial cavity. The olfactory fossa (OF) is located along the superior aspect of cribriform plate which varies in shape and depth. This variable measurement of the depth of OF is mostly responsible for greater risk of intracranial infiltration during endoscopic procedures in and around the nasal cavity. The morphology of frontal and ethmoid sinus (ES) vary from simple to complex. This cadaveric study is planned to improve the ability of the otolaryngologist, radiologist to understand the possible morphological variations and plan steps of less invasive "precision surgery" to have a safe and complication free procedures. A total of 37 human head regions were included in the study. For classification of OF, Modified Kero's classification was used. The size, shape and cells of frontal and ES were noted. We found, type II (60.8%) OF was more common followed by type I (29.7%) than type III (9.5%). The shape of frontal sinus was comma shaped (55.4%) followed by oval (18.9%) than irregular (16.2%). Most common two cells type of ES was seen in 50.0% of both anterior and posterior ES. Out of 74 ES, 8.1% of Onodi cells and 14.9% of agger nasi cells were seen.

Evaluation of a Curtain-Type Radiation Protection Device for Veterinary Interventional Procedures

  • The standard radiation protection method in the angiography suite involves the use of a thyroid shield, a lead apron, and lead glasses. However, exposure to substantial amounts of ionizing radiation can cause cataracts, tumors, and skin erythema. A newly developed curtain-type radiation protection device consists of a curtain drape composed of a five-layer bismuth and lead acrylic head-shielding plate, with both bearing an equivalent 0.25 mm lead thickness. In this study, a quality assurance phantom was used as the patient to create radiation scatter from the radiographic source, and an anthropomorphic mannequin phantom was used as the interventionalist to measure the radiation dose at seven different anatomical locations. Thermoluminescent dosimeters were used to measure the radiation dose. The experimental groups consisted of all-sided or one-sided curtain set-ups, the presence or absence of a conventional shielding system, and the orientation of beam irradiation. Consequently, the curtain-type radiation protection device exhibited better radiation protection range and capabilities than conventional radiation protection systems, especially in safeguarding the forehead, eyes, arms, and feet, with minimal radiation exposure. Moreover, the mean shielding ratios of the conventional shielding system and curtain-type radiation protection device were measured at 51.94% and 93.86%, respectively. Additionally, no significant decrease in the radiation protection range or capability was observed, even with changes in the beam orientation or one-sided protection. Compared with a conventional shielding system, the curtain-type radiation protection device decreased radiation exposure doses and improved comfort. Therefore, it is a potential new radiation protection device for veterinary interventional procedures.
