방아쇠 무지 환자를 A1 활차 박리없이 근골격계에 기반하여 침과 전침으로 호전시킨 1례: 증례보고 (Dry Needling and Electro-Dry Needling Improved the Trigger Thumb in a Patient Based on Anatomical Structure Without A1 Pulley Release: A Case Report)

  • Objectives Most of the acupuncture or dry needling points for trigger finger were limited around the metacarpophalangeal joint, A1 pulley, and flexor muscle tendon. Thus, this study aimed to report a case of a patient with trigger thumb which improved using dry needling on additional muscle points. Methods The author has investigated why additional points are needed including what its accompanying effects are. Dry needling and electro-dry needling have been conducted based on anatomical structure. Moreover, a follow-up observation was performed twice to evaluate if the effects of this treatment have been maintained. Treatment progress was evaluated using a numeric rating scale (NRS) and Quinnell's classification of trigger finger. Results After 28 days of treatment, NRS and Quinnell's trigger grade decreased significantly without adverse effects. The treatment effect has been maintained until follow-up observation. Conclusions Additional points are recommended for the radical treatment of trigger finger.

Evaluation of the morphology of the canalis sinuosus using cone-beam computed tomography in patients with maxillary impacted canines

  • Purpose: The nasopalatine canal is a well-known, important anatomical structure in the anterior maxilla, but this region contains many accessory canals. The canalis sinuosus (CS) is one of these canals; it contains the anterior superior alveolar nerve, along with veins and arteries. The purpose of this study was to evaluate the CS using cone-beam computed tomography (CBCT) in patients with maxillary impacted canines. Materials and Methods: A total of 111 patients admitted to the Istanbul Medipol University School of Dentistry for the exposure, orthodontic treatment, and/or extraction of an impacted canine were included in this study. CBCT images were obtained for these patients under standard conditions. Axial, coronal, and sagittal sections were evaluated to assess the prevalence of CS, the direction and diameter of the canal, its relation with the impacted canine, and its distance from the alveolar crest. Further, possible correlations with patient gender and age were analyzed. Results: The CS could be detected bilaterally in all the evaluated tomography images. The mean canal diameter was significantly larger in males than in females(P=.001). The CS ran significantly closer to the impacted canine when the canal was located horizontally (P=.03). Variations of the canal, such as accessory canals, were identified in 6 patients. Conclusion: CS is an anatomical entity that may resemble periapical lesions and other anatomical structures. Evaluation with CBCT prior to surgical procedures in the anterior maxilla will help to prevent overlooking such anatomical structures and to decrease possible surgical complications.

  • Purpose: The mental foramen (MF) is an important anatomical structure during local anesthesia and surgical procedures in terms of achieving effective mental nerve blocks and avoiding injuries to the neurovascular bundles. Thus, understanding the anatomic features of the mandibular canal and accessory mental foramen in Korean could contribute to the surgical anatomic assessment. This study was to elucidate frequency, position and course of AMF (accessory mental foramen) in Korean using 3D cone beam computed tomography. Materials and Methods: The CBCT (Conbeam computed tomography) DICOM data (Alphard, Asahi, Japan) from 540 patients in korean were analyzed. We investigated images of 3D CBCT using Ondemand (CyberMed, Korea) software program on the incidence and anatomical characteristics of accessory foramen. Results: The accessory mental foramina were found in 17 patients. Accessory mental foramina exist predominantly in the apical area of the second premolar and posteroinferior area of the mental foramen. The accessory branches of the mandibular canal showed common characteristics in the course of gently sloping posterosuperior direction in the buccal surface area. The size of most AMF was obviously smaller than that of MF. Conclusion: We could identify frequency, position and course of AMF (accessory mental foramen) by the anatomical study of the accessory mental foramen using 3D cone beam CT in Korean.

한국재래산양 뇌두개의 분리골에 대한 해부학적 연구 (Anatomical study on the individual bones of the cranium of Korean native goat)

  • This study was carried out to identify anatomical characteristics of the individual bones of the cranium of the Korean native goat. The pars lateralis and pans basilaris of the os occipitale were fused each other, whereas the squama occipitalis articulated with the pars lateralis, forming the sutrue. The sinus sphenoidale was formed in the corpus ossis presphenoidalis. The os prietale was observed as a single bone. In the os frontale, an fossa innominatum was found ventrolateral to the root of processus cornualis, and the orificium orbitale canalis supraorbitalis was also formed on the pars orbitalis. The meatus temporalis was formed in the medial side of the pars squama of the os temporalis. The lamina cribrosa of the os ethmoidale was membrane-like structure with many foramina. The labyrinth ethmoidalis was composed of four endoturbinalia and ten to twelve ectoturbinalia.

  • 1. The physical and anatomical properties investigated in this study are the ones, which may give more effect on absorption of preservatives, such as variation of annual ring, specific gravity, shrinkage, E.M.C. of desorption and adsorption at 70 percent relative humidity condition, general and minute properties of wood. 2. On this study, the positive effects of wood structure, in particular, the appearances of tyloses in vessel element upon the absorption of creosote were observed. 3. Thirteen percent sodium chloride solution treatment and boiling hot water treatment have shown poor results to improve the preservative absorption characteristics of tested species however, some slight difference between treated and control could be seen. 해마와 정신분열병 (Hippocampus and Schizophrenia)

  • Schizophrenics suffer not only psychotic symptoms but also cognitive deficits such as an attentional difficulty, memory impairment, poor abstraction, etc. These cognitive abnormalities have been reported to be significantly related to the social and occupational outcome in schizophrenia. Thus, it is important to explore the cause and pathophysiology for the cognitive abnormalities in patients with schizophrenia. In this regard, hippocampus is one of the most promising brain areas to search for the clue because it is closely involved in memory related function. In fact, during the past several decades, there have been extensive studies supporting hippocampal abnormalities as a cause of schizophrenia in both clinical and preclinical field. In this review, basic anatomical knowledge about hippocampus and major findings of preclinical and clinical studies which investigated the correlation between schizophrenia and hippocampus were highlighted. The contents are 1) anatomical structure of hippocampus, 2) neuronal pathway and receptor distribution in hippocampus, 3) function of hippocampus, 4) hippocampal animal model for schizophrenia, 5) hippocampus-related studies on antipsychotic drugs, and 6) clinical studies in hippocampus in patients with schizophrenia.

  • 후두악성종양의 해부병리학적 소견과 종양의 파급경로의 연구를 위해서는 후두전체조직 표본의 관찰이 필수적이다. 이에 저자들은 후두악성종양의 진단하에 후두전적출술을 시행하여 얻은 10례의 적출후두를 이용해 탈석회화후 수평절개하여 후두전체조직 표본을 제작해 같은 위치의 MRI소견과 비교함으로써 종양의 해부병리학적 구조 및 주변조직과의 관계와 종양의 파급경로를 입증해보고자 하였다.

A simplified CAD/CAM extraoral surgical guide for therapeutic injections

  • Therapeutic injections into the craniofacial region can be a complex procedure because of the nature of its anatomical structure. This technical note demonstrates a process for creating an extra-oral template to inject therapeutic substances into the temporomandibular joint and the lateral pterygoid muscle. The described process involves merging cone-beam computed tomography data and extra-oral facial scans obtained using a mobile device to establish a correlated data set for virtual planning. Virtual injection points were simulated using existing dental implant planning software to assist clinicians in precisely targeting specific anatomical structures. A template was designed and then 3D printed. The printed template showed adequate surface fit. This innovative process demonstrates a potential new clinical technique. However, further validation and in vivo trials are necessary to assess its full potential.

기초간호자연과학의 인체구조와 기능 내용별 필요도에 대한 연구 (A Study on the Degree of Need of Human Structure and Function Knowledge in Clinical Nurses)

  • The purpose of this study was to define the content of requisite human structure and function knowledge needed for clinical knowledge of nursing practice. Subjects of human structure and function were divided into 10 units, and each unit was further divided into 21 subunits, resulting in a total of 90 items. Contents of knowledge of human structure and function were constructed from syllabus of basic nursing subjects in 4 college of nursing, and textbooks published by nurse scholars prepared with basic nursing sciences. The degree of need of 90 items was measured with a 4 point scale. The subjects of this study were college graduated 136 nurses from seven university hospitals in Seoul and three university hospitals located in Chonnam Province, Kyungbook Province, and Inchon. They have been working at internal medicine ward, surgical ward, intensive care unit, obstetrics and gynecology ward, pediatrics ward, opthalmology ward, ear, nose, and throat ward, emergency room, rehabilitation ward, cancer ward, hospice ward, and their working period was mostly under 5 years. The results were as follows: 1. The highest scored items of human structure and function knowledge necessary for nursing practice were electrolyte balance, blood clotting mechanism and anticoagulation mechanism, hematopoietic function, body fluid balance, function of plasma, and anatomical terminology in the order of importance. The lowest scored items of human structure and function knowledge necessary for nursing practice was sexual factors of genetic mutation. 2. The highest order of need according to unit was membrane transport in the living unit, anatomical terminology in movement and exercise unit, mechanism of hormone function in regulation and integration unit, component and function of blood in oxygenation function unit, structure and function of digestive system in digestive and energy metabolism unit, temperature regulation in temperature regulation unit electrolyte balance in body fluid and electrolyte unit, concept of immunity in body resistance unit, and genetics terminology in genetics unit. The highest order of importance according to subunit was membrane transportation in cell subunit, classification of tissues in tissue unit, function of skin and skin in skin subunit, anatomical derivatives of the skeleton subunit, classification of joints in joint subunit, an effect of exercise on muscles in muscle subunit, function of brain in nervous system subunit, special sense in sensory subunit mechanism of hormone function in endocrine subunit, structure and function of female reproductive system in reproductive system unit, structure and function of blood in blood unit, structure of heart, electrical and mechanical function in cardiovascular system unit, structure of respiratory system in respiratory system subunit, structure and function of digestive system in digestive system subunit, hormonal regulation of metabolism in nutrition and metabolism subunit, function of kidney in urologic system subunit, electolyte balance in body fluid, electolyte and acid-base balance subunit. 3. The common content of human structure and function knowledge need for all clinical areas in nursing was structure and function of blood, hematopoietic function, function of plasm, coagulation mechanism and anticoagulation mechanism, body fluid, electrolyte balance, and acid-base balance. However, the degree of need of each human structure and function knowledge was different depending on clinical areas. 4. Significant differences in human structure and function knowledge necessary for nursing practice such as skin and derivatives of the skin, growth and development of bone, classification of joint, classification of muscle, structure of muscle, function of muscle, function of spinal cord, peripheral nerve, structure and function of pancrease, component and function of blood, function of plasma, structure and function of blood, hemodynamics, respiratory dynamics, gas transport, regulation of respiration, chemical digestion of foods, absorption of foods, characteristics of nutrients, metabolism and hormonal regulation, body energy balance were demonstrated according to the duration of work. 5. Significant differences in human structure and function knowledge necessary for nursing practice such as classification of tissue, classification of muscles, function of muscles, muscle metabolism, classification of skeletal muscles, classification of nervous system, neurotransmitters, mechanism of hormone function, pituitary and pituitary hormone, structure and function of male reproductive organ, structure and function of female reproductive organ, component and function of blood, function of plasma, coagulation mechanism and anticoagulation mechanism, gas exchange, gas transport, regulation of respiration, characteristics of nutrients, energy balance, function of kidney, concept of immunity, classification and function of immunity were shown according to the work area. Based on these findings, all the 90 items constructed by Korean Academic Society of Basic Nursing Science should be included as contents of human structure and function knowledge.

  • Mass transfer behavior in wood was estimated through its microscopic structure. The diffusion coefficients which were decided by theoretical equations are influenced by different anatomical properties of wood. From the experiment, the moisture flux was linear to the square root of time. The diffusion coefficients had a regular tendency during the time elapse. During the modeling, it is necessary to understand the limitation of parameters and consider the particular situation to be simulated. In hardwood, because the apertures were not considered, tangential mass transfer simulation was totally different from experiment. As a result, a hardwood model design should consider the apertures which are even on the fiber walls.
