정상 좌심실을 가진 대동맥 판막 폐쇄증에서의 일차성 완전교정술 (Single Stage Anatomic Repair of Aortic Atresia with Normal Left Ventricle)

  • Four to seven percent of infants born with aortic atresia have a normal-sized left ventricle in association with a ventricular septal defect and a normal mitral valve. In contrast to the more common group of infants with aortic atresia whose left ventricle is hypoplastic, this important subgroup has potential for complete operative correction involving both the right and left ventricle. Our approach was to perform complete repair at one stage. The patient who underwent single-stage repair was discharged from the hospital in good condition. Achieving a physiologically normal circulation simplifies the postoperative management of this condition.

Ruptured Abdominal Aortic Aneurysm after Endovascular Aortic Aneurysm Repair

  • In treating uncomplicated abdominal aortic aenurysm, endovascular aortic aneurysm repair (EVAR) has been employed as a good alternative to open repair with low perioperative morbidity and mortality. However, the aneurysm can enlarge or rupture even after EVAR as a result of device failure, endoleak, or graft migration. We experienced two cases of aneurismal rupture after EVAR, which were successfully treated by surgical extra-anatomic bypass.

Anatomic fit of precontoured extra-articular distal humeral locking plates: a cadaveric study

  • Background: Extra-articular distal humerus locking plates (EADHPs) are precontoured anatomical plates widely used to repair distal humeral extra-articular diaphyseal fractures. However, EADHPs frequently cause distal protrusion and resulting skin discomfort. The purpose of this study was to predict the occurrence of anatomic fit mismatch. We hypothesized that the smaller the humerus size, the greater the anatomic fit mismatch with EADHP. Methods: Twenty humeri were analyzed in this study. Humeral length and distal humeral width were used as parameters of humeral size. Plate protrusion was measured between the EADHP distal tip and the distal humerus. We set the level of unacceptable EADHP anatomic fit mismatch as ≥10 mm plate protrusion. Results: A significant negative linear correlation was also confirmed between humeral size and plate protrusion, with a coefficient of determination of 0.477 for humeral length and 0.814 for distal humeral width. The cutoff value of humeral length to avoid ≥10 mm plate protrusion was 293.6 mm (sensitivity, 88.9%; specificity, 81.8%) and for distal humeral width was 60.5 mm (sensitivity, 100%; specificity, 81.8%). Conclusions: Anatomic fit mismatch in distal humeral fractures after EADHP fixation has a negative linear correlation with humeral length and distal humeral width. For patients with a distal humeral width <60.5 mm, ≥10 mm plate protrusion will occur when an EADHP is used, and an alternative implant or approach should be considered.

  • Purpose: Arthroscopic repair of rotator cuff tears are many techniques that have been developed to improve the initial postoperative strength of the repair. There was a doubt that current arthroscopic cuff repairs using a single row of suture anchors reproduce insufficient area of the anatomic cuff insertion, and concerns about failure of fixation often lead surgeons to limit early motion. Newer technique of double-row repair in arthroscopic treatment may provide initial stronger fixation and more contact with bone at the repair site than single-row repair did. We studied the comparison between clinical outcomes of arthroscopic single- and double-row repair in cuff tears at 1year postoperatively. Materials and methods: We retrospectively analyzed 40 shoulders with single-row repair and 38 shoulders with double-row repair of full-thickness rotator cuff tears between May 2002 and October 2004. Out of total 78 shoulders, 42 (54%) were male patients and 36 (46%) were female patients and the mean age at surgery was 56 years. All patients were diagnosed by physical examination and MRI. At 1year' follow-up after operation, we evaluated with the ASES and the Constant scoring system, and measured muscle power of abduction, internal and external rotation of the affected shoulder then compared with each other. Results: Mean ASES scores and Constant scores in double-row repair group improved more than single-row repair group significantly at 1year postoperatively. Muscle power of abduction and internal rotation, especially abduction power, improved more significantly in double-row repair group than in single-row repair. Conclusion: Arthroscopic double-row repair for the full thickness rotator cuff tear may be a superior technique, which showed better clinical outcomes and restoration of muscle power compared with single-row repair at relatively short period of postoperative follow-up. Restoration of footprint close to normal anatomy by double-row repair seems to play an important role in the recovery of muscle strength.

교정형 대혈관 전위증의 수술적 치료 (Operative Treatment of Congenitally Corrected Transposition of the Great Arteries(CCTGA))

  • 배경: 본 연구에서는 교정형 대혈관전위증으로 양심실교정이 가능하여 개심술을 시행받은 65례의 환아를 대 상으로 질환의 해부학적 특성, 수술방법 및 결과를 분석하였다. 대상 및 방법: 1984년부터 1998년 9월까지의 교정형 대혈관전위증으로 양심실교정을 받은 65례의 환자들을 대상으로 하였으며 주로 환자의 의무기록을 참조하여 후향적으로 동반기형에 따른 수술법의 차이 및 각각의 수술법에 따른 합병증등 단기 및 중장기 성 적을 분석하였다. 결과 평균 연령은 5.4$\pm$4.8세(2개월~18세)이었으며 남:녀비는 39:26이었다. 형태로는 {S,L,L}이 53례{I,D,D}가 12례였다. 폐동맥 협착이나 폐쇄가 동반되지 않았던 경우가 13례(20%)였으며 심실 혹은 심방중격결손증과 폐동맥 협착증이 동반된 경우가 26례(40%)였고 심실중격결손증과 폐동맥 폐쇄증이 동반된 례가 26례(40%)였다. 술전 경도이상의 삼첨판막 폐쇄부전을 보인 환자는 38.5%(26/65)였다. 완전교정 술전 단락술은 22명의 환자에서 24차례 시행하였다. 교정술의 방법으로 폐동맥심실유출로 협착이 없던 13례 에서는 7례에서 심실 혹은 심방 중격결손증만을 폐쇄하였고, 3례에서 삼첨판막치환술을, 3례에서 좌심실을 체순환으로 사용하는 중복치환술(Senning+Rastelli, Senning+REV, Senning+ASO)을 시행하였다. 심실,혹은 심방 중격결손증과 폐동맥 협착이 있던 26명의 환자에서는 24례에서 우심실을 체순환으로 사용하는 기존의 방법 (Rastelli술식 9례, 심실중격결손증 폐쇄후 폐동맥판막절개술 7례, REV형 술식 4례, 중격결손 폐쇄후 폐동맥 판막하 근육절제가 3례, 삼첨판막치환술이 1례)으로 수술하였으며 2례에서 중복치환술(Senning+Rastelli, Mustard+REV)을 시행하였다. 심실중격결손증과 폐동맥폐쇄증이 있었던 26명의 환자에서는 18명에서 심실중 격결손증을 폐쇄하고 Rastelli술식을 시행하였고, 7례에서 중복치환술(Senning+Rastelli)을 시행하였으며, 1례는 심실중격결손증을 폐쇄하고 REV형 술식을 시행하였다. 술후 추적 기간동안 우심실을 체순환으로 사용한 환 자들에서의 삼첨판막폐쇄부전은 술전 평균 1.3$\pm$1.4도에서 2.2$\pm$1.0도로 통계적으로 의미있게 증가하였다 (p<0.05). 그러나 중복치환술을 시행받은 환자들은 술후 삼첨판막의 폐쇄부전의 증가가 없었다. 술전 완전 방 실차단을 보인 환자는 2례(3.1%)있었으며 술후 새롭게 발생한 완전방실차단은 7례(10.8%)있었다. 술후 기타 장,단기 합병증으로는 폐동맥심실유출로도관(conduit) 재협착이 10례, 혈전증(판막: 2례, 인조혈관: 1례, 폐동 맥: 1례)이 4례, 2주 이상의 지속적 흉관배액이 4례, 유미흉이 3례, 출혈에 의한 재수술이 3례, 기타 급성 신 부전, 종격동염, 횡경막신경 마비가 각각 2례씩 있었으며, 중복치환술을 받은 환자들과 전통적 술식으로 수 술받은 환자에서 술후 합병증의 차이는 없었다. 65명의 환자를 평균 54$\pm$49개월(0~177개월)간 추적관찰하였 으며, 수술 초기에 사망한 환자는 13명으로 20.0%(13/65)의 수술사망율을 보였으며 3명의 환자가 추적기간중 사망하여 24.6%(16/65)의 전체사망율을 보였다. 중복치환술을 받은 환자의 수술사망율은 33.3%(4/12)였다. 술 후 1년, 5년, 10년 누적생존율은 각각 75.0$\pm$5.6%, 75.0$\pm$5.6%, 69.2$\pm$7.6%였다. 가장 흔한 사망원인으로는 술 후 저심박출증후군으로 8례였으며 삼첨판막 폐쇄부전이 심해져 심부전으로 사망한 경우도 5례로 사망의 중 요 원인이었다. 결론 저자들은 본 연구를 통하여 기존의 형태학적 우심실을 체순환으로 사용하는 방법과 형태학적 좌심실을 체순환으로 사용하는 해부학적 교정법인 중복치환술중 어느 것이 우월하다고 결론지을 수는 없었으나 해부학적 교정술이 삼첨판막의 폐쇄부전의 진행을 막을수 있다는 면에서 이점이 있으리라 사 료되며 보다 세분화된 적응증의 결정과 아울러 장기적인 추적관찰 및 비교가 필요하리라 생각된다

Outcomes of Surgery for Total Anomalous Pulmonary Venous Return without Total Circulatory Arrest

  • Background: Recent developments in surgical techniques and hospital care have led to improved outcomes following total anomalous pulmonary venous return (TAPVR) repair. However, the surgical repair of TAPVR remains associated with a high risk of mortality and need for reoperation. We conducted this retrospective study to evaluate mid-term outcomes following in situ TAPVR repair without total circulatory arrest (TCA), and to identify the risk factors associated with surgical outcomes. Methods: We retrospectively reviewed 29 cases of surgical intervention for TAPVR conducted between April 2000 and July 2015. All patients were newborns or infants who underwent in situ TAPVR repair without TCA. Results: Four anatomic subtypes of TAPVR were included in this study: supracardiac (20 cases, 69.0%), cardiac (4 cases, 13.8%), infracardiac (4 cases, 13.8%), and mixed (1 case, 3.4%). The median follow-up period for all patients was 42.9 months. Two (6.9%) early mortalities occurred, as well as 2 (6.9%) cases of postoperative pulmonary venous obstruction (PVO). Preoperative ventilator care (p=0.027) and preoperative PVO (p=0.002) were found to be independent risk factors for mortality. Conclusion: In situ repair of TAPVR without TCA was associated with encouraging mid-term outcomes. Preoperative ventilator care and preoperative PVO were found to be independent risk factors for mortality associated with TAPVR repair.

Does humeral fixation technique affect long-term outcomes of total shoulder arthroplasty?

  • Background: For anatomic total arthroscopic repair, cementless humeral fixation has recently gained popularity. However, few studies have compared clinical, radiographic, and patient-reported outcomes between cemented and press-fit humeral fixation, and none have performed follow-up for longer than 5 years. In this study, we compared long-term postoperative outcomes in patients receiving a cemented versus press-fit humeral stem anatomic arthroscopic repair. Methods: This study retrospectively analyzed 169 shoulders that required primary anatomic total shoulder arthroplasty (aTSA). Shoulders were stratified by humeral stem fixation technique: cementation or press-fit. Data were collected pre- and postoperatively. Primary outcome measures included range of motion, patient reported outcomes, and radiographic measures. Results: One hundred thirty-eight cemented humeral stems and 31 press-fit stems were included. Significant improvements in range of motion were seen in all aTSA patients with no significant differences between final cemented and press-fit stems (forward elevation: P=0.12, external rotation: P=0.60, and internal rotation: P=0.77). Patient reported outcome metrics also exhibited sustained improvement through final follow-up. However, at final follow-up, the press-fit stem cohort had significantly better overall scores when compared to the cemented cohort (visual analog score: P=0.04, American Shoulder and Elbow Surgeon Score: P<0.01, Simple Shoulder Test score: P=0.03). Humeral radiolucency was noted in two cemented implants and one press-fit implant. No significant differences in implant survival were observed between the two cohorts (P=0.75). Conclusions: In this series, we found that irrespective of humeral fixation technique, aTSA significantly improves shoulder function. However, within this cohort, press-fit stems provided significantly better outcomes than cemented stems in terms of patient reported outcome scores. Level of evidence: III.

Outcomes of Open Repair of Mycotic Aortic Aneurysms with In Situ Replacement

  • Background: Mycotic aortic aneurysms are rare and life-threatening. Unfortunately, no established guidelines exist for the treatment of patients with mycotic aortic aneurysms. The purpose of this study was to evaluate the midterm outcomes of the open repair of mycotic thoracic and thoracoabdominal aneurysms and suggest a therapeutic strategy. Methods: From 2006 to 2016, 19 patients underwent open repair for an aortic aneurysm. All infected tissue was extensively debrided and covered with soft tissue. We recorded the clinical findings, anatomic location of the aneurysm, bacteriology results, antibiotic therapy, morbidity, and mortality for these cases. Results: The median age was $62{\pm}7.2years$ (range, 16 to 78 years), 13 patients (68%) were men, and the mean aneurysm size was $44.5{\pm}4.9mm$. The mean time from onset of illness to surgery was $14.5{\pm}2.4days$. Aortic continuity was restored in situ with a Dacron prosthesis (79%), homograft (16%), or Gore-Tex graft (5%). Soft-tissue coverage of the prosthesis was performed in 8 patients. The mean follow-up time was $43.2{\pm}11.7months$. The early mortality rate was 10.5%, and the 5-year survival rate was $74.9%{\pm}11.5%$. Conclusion: This study showed acceptable early and midterm outcomes of open repair of mycotic aneurysms. We emphasize that aggressive intraoperative debridement with soft-tissue coverage results in a high rate of success in these high-risk patients.

회전근개 파열과 동반한 TypeⅡ SLAP 병변 (Type Ⅱ SLAP Lesion with the Rotator Cuff Tear)

  • Purpose: We reviewed the SLAP lesions and associated injuries, also evaluated a hypothesis that the type II posterior SLAP lesion is related with posterior rotator cuff tear and gives rise to the postero-superior instability. Materials and Methods: The patient recording papers, MRI, video and operation sheets were reviewed with the 28 SLAP lesions confirmed by the arthroscopy among 242 cases. Among these SLAP lesions, type II was 22 cases and classified to the anterior, posterior(16 cases), combined subtype(6 cases) based on the main anatomic location. There were 14 cases of the type II accompanying rotator cuff tear. The average follow-up(13 months) results were evaluated with the ASES and Rowe rating score after repair or debridement of the SLAP lesions. Results: In the type II lesions accompanying the rotator cuff tears(14 cases), the posterior(l0 cases) and combined type(4 cases), cuff lesions were all existed posteriorly. Also We could confirm the drive-through sign in the eleven cases, though did not check the disappearance of this sign after repair because of retrospective study. We could followed up the 22 cases, 18 cases(77%) were excellent or good, fair 3 cases(14%) and poor 1 case(4%). Also, type II lesions with the rotator cuff tear(14 cases) were showed better results in the repair(8 cases) than the debridement(6 cases) of the unstable type II with the cuff repair. Conclusion: The type II lesions were frequently associated with the cuff tear in the specific location. We could presume the possibility of postero-superior instability in the SLAP lesion with the cuff injuries. Also, satisfactory results could be experienced when the unstable SLAP lesions with the cuff tear were repaired at the same time.

편측성 구순구개열 환자에 있어 구순성형술과 동반한 서골피판법 치험례 (Simultaneous Repair of Unilateral Cleft Lip and Hard Palate with Vomer Flap : a Case Report)

  • Cleft lip and palate is the most common congenital facial malformation and has a significant developmental, physical, and psychological impact on those with the deformity and their families. When treating the patients with unilateral cleft lip, many surgeons adopt the rotation advancement flap method originally developed by Millard, or the triangular flap technique developed by Tennison, Randall or the modifications of these techniques. Among these, Millard's rotation advancement flap method has its advantage in designing the flap using the patient's anatomic landmarks. For performing this rotation advancement technique, skillful operation is needed to obtain esthetically satisfactory results. Vomer flap sometimes is used to repair anterior hard palate in complete cleft lip and palate patients. Vomerine tissue is readily available in the vicinity of the palatal defect and elevation of the vomerine flap is relatively simple procedure. In this article, we will introduce the comprehensive vomer flap technique conjunction with primary lip closure and review the comparative studies of the outcome of simultaneous repair of cleft lip and cleft hard palate with Millard's rotation advancement method and vomer flap.
