Evaluation of Cost-Effectiveness of Medical Nutrition Therapy : Meta-Analysis (메타분석을 이용한 임상영양서비스의 비용-효과성 평가)

  • Objectives: A meta-analysis of the literatures was conducted to evaluate the cost-effectiveness of medical nutrition therapy by dietitians. Methods : The 30 studies were identified from a computerized search of published research on MEDLINE, Science-Direct and the PQD database until May, 2002 and a review of reference lists. The main search terms were“dietitian”,“dietary intervention”,“nutrition intervention”, “cost”,“cost-effectiveness”and“cost-benefit analysis”. The subgroup analysis was performed by publication year, study design, intervention provider, type of patient (in/out-patient) and type of cost (total cost/direct cost). Two reviewers independently selected trials for inclusion, assessed the quality and extracted the data. Results : The 30 studies were identified using the electric database search and bibliographies. The 17 trials were eligible for inclusion criteria, then the systematic review and a meta-analysis were conducted on effectiveness and cost-effectiveness of medical nutrition therapy. The quality of the studies was evaluated using the quality assessment tool for observational studies. The quality score was 0.515 $\pm$ 0.121 (range : 0.279-0.711, median : 0.466). The meta-analysis of 17 studies based on the random effect model showed that medical nutrition therapy was highly effective in treating the diseases (effect size 0.3092 : 95% confidence interval 0.2282-0.3303). The vote-counting method, one of meta-analysis methods, was applied to evaluate the cost-effectiveness of medical nutrition therapy conducted by dietitians. Two criteria (method 1, method 2) for voting were used. The calculated p-values for method 1 (more conservative method) and method 2 (less conservative method) were 0.1250 and 0.0106, respectively. Medical nutrition therapy by dietitians was significantly cost-effective in the method 2. Conclusion. This meta-analysis showed that the effectiveness of medical nutrition therapy was statistically significant in treating disease (effect size 0.3092), and that the cost-effectiveness of medical nutrition therapy was statistically significant in the method 2 (less conservative method) of vote counting.   • The 3-dimensional grain burn-back analysis is performed using the face offsetting method for calculating the solid rocket motor performance. The grain burning configuration analysis is a moving surface problem that calculates the regression of the burning surface. In the previous study, various moving interface analysis methods were applied for the grain burn-back analysis, but the results were imperfect. In this study, a 3-dimensional grain burn-back analysis module is developed using the face offsetting method, which combines the advantages of the existing moving interface analysis methods to increase the accuracy and robustness. As a result, the face offsetting method is proved to be efficient for the grain burn-back analysis.

A study on the quantitation of asbestos by the visual estimation and point counting method (시야평가법과 포인트계수법에 의한 석면정량평가 연구)

  • While variety of cases of studies about asbestos analysis methods are released internationally, the results of Asbestos Containing Materials (ACM) according to differences in the method of the analysis is becoming an issue. In this study, homogeneity ensured ACM samples were analyzed by visual estimation method and point counting method, and the result cound be used not only to improve the reliability on asbestos analysis of the institutions and analysts but also to obtain the basic data of Polarizing Light Microscope (PLM) analysis by comparing and evaluating. Asbestos analysis were divided into qualitative and quantitative analysis method. The quantitative analysis was performed by visual estimation method and point counting method (total 400 points) of EPA 600-R-93-116 method by using PLM. Firstly, The following was the result of homogeneity of the samples by ANOVA (Analysis of variance) and the results were satisfied. The results of qualitative analysis showed that the samples were chrysotile and amosite, and about the results of quantitative analysis, asbestos concentration determined by point counting method tend to be lower than concentrations determined by visual estimation method and also, pont counting method was a little more complicated and time-consuming.

A Study on the Behavior of Welded Connections (용접 연결부의 거동에 관한 연구)

  • Welded connections have been designed on basis of allowable stresses, wherein the response to loading is assumed to be totally elastic. This is the vector analysis method, which resolves the stresses determined from the direct stress formula and the torsion formula into a vector combination to obtain a solution. It has been known that this method gives conservative answers and typically a very high factor of safety. An analytical method based on the Instantaneous Center of Rotation has been developed which predicts the ultimate strength of an eccentically loaded fillet welded connection. The method of Instantaneous Center of Rotation results in weld resistance capacities greater than the vector analysis method, by recognizing the variation in fillet weld strength with respect to the direction of the applied loading and actual load-deformation response of elemental fillet welds. The procedure of numerical analysis is iterative and complex. The relations between vector analysis method and the method of Instantaneous Center of Rotation on eccentrical distance subjected to variation of load direction are presented in this paper. Considering of the effects on configuration of weld groups, the method of Instantaneous Center of Rotation are provided a more exact results of the numerical analysis.

  • In this paper, three dimensional Equivalent Magnetic Circuit Method(3-D EMCN method) a numerical analysis method which supplements to magnetic equivalent circuit adding numerical technique, is proposed for analysis Optical Pickup Actuator. (3) This method provides better characteristics both in precision of the analysis and in computation time than other analysis method such as three-dimensional Finite Element Method. We choose the simple 2-magnet moving coil type pickup actuator model and verify upper yoke effect using this method.

  • This paper suggests a new analysis algorithm for the time historical response of three dimensional rectilinear structure which is frequently found in a pipe line system of plant by the combination of the transfer stiffness coefficient method(TSCM) and the Newmark method. The present analysis algorithm for a time historical response can improve the computational accuracy and time remarkably owing to advantages of the TSCM in comparison with transfer matrix method(TMM). The structural system is modeled as a lumped mass system in this method. The analysis algorithm was formulated far the three dimensional rectilinear structure. We confirmed the validity of the present algorithm by comparing the numerical computation results of TSCM with those of TMM.

  • This paper presents a voltage analysis method of distribution systems interconnected with DG(Distributed Generation). Nowadays, small scale DG becomes to be introduced into power distribution systems. But in that case, it is difficult to properly maintain the terminal voltage of low voltage customers by using only ULTC(Under Load Tap Changer). This paper presents a voltage analysis method of distribution systems with DC for proper voltage regulation of power distribution systems with ULTC. In order to develop the voltage analysis method, distribution system modeling method and advanced loadflow method are proposed. Proposed method has been applied to a 22.9 kV practical power distribution systems.

  • Recently, failures of equipments are linked directly to extensive damages of human lives or financial losses from the increasing uses of train control equipments utilizing computers. Then safety activities have to progress for guaranteeing safety during the system life-cycle. In this paper, we examine the methods for risk analysis and assessment of safety activities and propose optimized one method for risk assessment. There are original risk assessment methods; risk graph and risk matrix method under the qualitative analysis, IRF(Individual Risk Formula) calculations and statistical calculations method under the quantitative analysis. Best-Practice(BP) risk analysis method is proposed for combining advantages of the qualitative and the quantitative analysis. In the comparison of risk graph and risk matrix method for safety estimation, BP method has no applications published up to now, but we can expect that this method will be utilized widely for the risk assessment due to various strong points.

  • This paper presents a new time-stepping 3-D analysis method coupled with an external circuit with motion equation for dynamic transient analysis of induction machines. In this method, the magneto-motive force (MMF) generated by induced current is modeled as a passive source in the magnetic equivalent network. So, by using only scalar potential at each node, the method is able to analyze induction machines with faster computation time and less memory requirement than conventional numerical methods. Also, this method is capable of modeling the movement of the mover without the need for re-meshing and analyzing the time harmonics for dynamic characteristics. From comparisons between the results of the analysis and the experiments, it is verified that the proposed method is capable of estimating the torque, harmonic field, etc. as a function of time with superior accuracy.

Direct Earthquake Design Using Secant Stiffness (할선강성을 이용한 직접내진설계)

  • A new earthquake design method performing iterative calculations using secant stiffness was developed. The proposed design method has the advantages of convenience and stability in numerical analysis because it uses elastic analysis. At the same time, the proposed design method can accurately estimate the strength and ductility demands on the members because it performs the analysis on the inelastic behavior of structure using iterative calculation. In the present study, the procedure of the proposed design method was established, and a computer program incorporating the proposed method was developed. Design examples using the proposed method were presented, and its advantages were presented by the comparisons with existing design methods using elastic or inelastic analysis. The proposed design method, as an integrated method of analysis and design, can address the earthquake design strategy devised by the engineer, such as ductility limit on each member, the design concept of strong column - weak beam, and etc. Through iterative calculations on the structure preliminarily designed only with member sizing, the strength and ductility demands of each member can be directly calculated so as to satisfy the given design strategy As the result economical and safe design can be achieved.
