A Study on the Hyang-Gyo Libraries In the Yi Dynasty (조선조의 향교문고에 관한 연구)

  • The libraries of the various learning institutes in the Yi Dynasty may be categorized into four types: 1) The Hyang-Gyo library(the public school libraries in rural districts) 2) The Sabu-Hankdang library (the public school libraries in the Capital) 3) The $S\bar{o}w\bar{o}n$ library (the libraries of private learning institutes) 4) The $Jyon-Gy\bar{o}ng-Gak$ library (the library of the Sung Kyun Kwan which was the highest learning institute in the Yi Dynasty) For the historical study of Korean libraries as well as its education and culture the Hyang-Gyo libraries hold an very important position, but undeservedly its study has been neglected. In this paper, the writer made an effort to grasp the various function of the Hyang-Gyo libraries with its historical background.

A Case of Successful Resuscitation of 10,150 J Shocks and Therapeutic Hypothermia on Aconitine-induced Cardiovascular Collapse (10,150 J의 심장조율동과 치료적 저체온법으로 소생한 중증 초오 중독 환자 1례)

  • Aconitine, found in the Aconitum species, is highly extremely toxic, and has been known to cause fatal cardiac arrhythmias and cardiovascular collapse. Although several reports have described treatment of aconitine intoxication, management strategy for the patient in a hemodynamically compromised state who experienced cardiopulmonary collapse is unknown. We report here on a case of a successful cardiopulmonary resuscitation and therapeutic hypothermia in an aconitine-induced cardiovascular collapsed patient. A 73-year-old male who presented with nausea, vomiting, chest discomfort, and drowsy mental state after eating an herbal decoction made from aconite roots was admitted to the emergency department. He showed hemodynamic compromise with monomorphic ventricular tachycardia resistant to amiodarone and lidocaine. After 3 minutes on admission, he collapsed, and cardiopulmonary resuscitation was initiated. We treated him with repeated cardioversion/defibrillation of 51 times, 10,150 joules and cardiopulmonary resuscitation of 12 times, 69 minutes for 14 hours and therapeutic hypothermia for 36 hours. He recovered fully in 7 days.

  • Ovarin hyperstimulation syndrome (OHSS), an iatrogenic complication of ovarian stimulation, shows varying degrees of clinical manifestations. The pathogenesis of OHSS is an increase of vascular permeability resulting in hypovolemia, thromboembolism, ARDS, and death in sometimes. Pleural effusion is also a result of an increase of vascular permeability in the pleura. Thoracentesis is sometimes required to relieve dyspnea. We report a case of OHSS with bilateral exudative pleural effusion in a 23 year-old female with resting dyspnea. She was received clomiphen, FSH, and LH for the treatment of irregular menstruation twenty days previously. The ultrasonogram showed severe ascites and bilaterally huge ovary, and chest radiography showed bilateral effusion. Therapeutic thoracentesis and paracentesis were done for relief of the dyspnea. Two weeks later the bilataral effusion and symptoms disappeared spontaneously.

  • In this paper, the novel dielectric resonator bandpass filter for Korea personal communication service channel combiner is proposed. The equivalent circuit of the channel bandpass filter is derived using the lumped elements. Proposed channel bandpass filter provides compact size, low insertion loss, two attenuation poles in the stop band, and excellent temperature characteristics. Two channel filters are combined with proposed dielectric resonator filter configuration to implement the 2-channel combiner. Experimental results show the validity of the proposed dielectric resonator channel bandpass filter.

  • We have recently demonstrated that high glucose (50 mM D-glucose) upregulates fibronectin mRNA expression and protein synthesis by human peritoneal mesothelial cells (HPMC) through activation of protein kinase C (PKC) and suggested that this may lead to progressive peritoneal fibrosis during a long-term peritoneal dialysis (PD) using glucose as an osmotic agent.(omitted)

  • This paper presents a digital LCD(Liquid Crystal Display) backlight inverter control using an 8-bit microcontroller. The backlight uses cold cathode fluorescent lamps(CCFLs) that have the negative resistance characteristics, so it requires the ballast function. For this, the proposed LCD backlight inverter uses an 8-bit microcontroller, ATmega128. Therefore, the controller circuit is simplified and cost-minimized. It shows through the experimental results that the proposed inverter has good performance for the LCD backlight inverter.

  • Characteristics Regarding Xiang Yao (Hyang Yag) Jiu Ji Fang Life Saving Local Botanical prescriptions in Medical Research Xiang Yao Jiu Ji Fang is Korea's most ancient extant medical manuscript. It was first published between 1232 and 1251 during the Goguryeo period (Gao Zong) by the (Jianghuadao Da Cang Du Jian) Great Storage Depository Under Capital Supervision. The entire work is divided into three scrolls named the front, middle and back parts. There is also an appendix titled Fangzhong Xiang Yao Mu Cao Bu 'Catalogue of Medicines in the Local Botanicals prescriptions' in one volume. The contents comprise discussion of internal medicine diseases, external medicine diseases, commentaries on sexually transmitted diseases, diseases caused by parasites and bugs, diseases of the five orifices, gynecological diseases and pediatric diseases, in all totaling fifty three types of disease. The prescriptions record 180 types of xiangyao or hyang yag-local botanicals representing the beginning of an independent Korean path of development. Owing to the development during the Goguryeo period of medical material left to us by history, investigation and research in the area of hyang yag local botanicals has bequeathed us methods used in the contemporary period. Through the related comprehensive annotated explanatory notes and documents, much analysis and discussion is taking shape.

Case Report of Avulsion Amputation of Multiple Digits: Use of Rerouting the Transverse Digital Palmar Arch (가로손가락손바닥활을 이용한 다발성 벗겨짐 손상 손가락 재접합술 - 증례보고 -)

  • Purpose: Avulsion injuries of digits have been presented for a long time as complex management problems. Despite of microsurgical advances, it is difficult to achieve good functional results and their management remains somewhat controversial. However, in a finger there are three transverse digital palmar arches. The middle and distal transverse digital palmar arches are consistently large(almost 1 mm) and may be used for arterial vessel repairs either proximally or distally, depending on the length and direction needed. 39-year-old man presented with avulsion amputation of the ulnar three digits, was operated using only arterial anastomosis with rerouting the transverse digital palmar arches. Methods: Replantation was performed using the artery-only technique. Because the digital arteries had been damaged, we did that the transverse digital palmar arches were transposed in an inverted Y to I configuration and were lengthened with rerouting them for the purpose of direct anastomosis of the digital artery. Venous drainage was provided by an external bleeding method with partial nail excision and external heparin irrigation. Results: The authors conclude that complete avulsion amputations with only soft tissue at the distal to insertion of the flexor digitorum superficialis tendon were salvageable with acceptable functional results. All three fingers survived. Conclusion: With technical advancements, the transverse digital palmar arches play an important role for finger amputation. Three digital palmar arches give us additional treatment option for the finger amputation. In this case, replantation with only-arterial anastomosis was successful and we obtained good aesthetic and functional outcome.

A Case of Sensory Neuronopathy Caused by Pyridoxine Intoxication (피리독신 중독에 의한 감각신경원병증 1예)

  • Pyridoxine has been known as an essential cofactor in many important biological reactions of tissue metabolism including blood, skin, and central nervous system. Nowadays, vitamins are widely consumed because of the belief that they provide health benefits with no harm. We report a patient with sensory ataxia who had a 3-year history of excessive vitamin $B_6$ intake. Her clinical and electrodiagnostic findings were characteristic of sensory neuronopathy, which were probably caused by pyridoxine intoxication. Physicians should be aware of the toxicities of megavitamin therapy.
