철보충이 납중독된 흰쥐의 ${\delta}-Aminolevulinic$ Acid Dehydratase 활성과 해독에 미치는 영향 (Effects of Dietary Irons Supplementation on ${\delta}-Aminolevulinic$ Acid Dehydratase Activity and Detoxification of the Pb-administered Rats)

  • 철보충이 납종독된 흰쥐의 체내 무기질 함량과 DALAD activity 및 해독에 미치는 영향을 관찰하기 위해, 흰쥐에게 납 2,000ppm을 함유한 식수를 제한없이 주면서, 철분 100ppm, 500ppm을 보충한 식이로 5주동안 사육하여 얻을 결과는 다음과 같다. 체중증가량, 식이섭취량 및 식이효율은 납급여군이 대조군보다 낮게 나타났으며, 철보충납급여군은 납단독급여군보다 높은 것으로 나타났다. 간과 심장의 무게는 차이를 보이지 않았으나, 신장은 납급여군이 대조군보다 증가를 보였다. 비장은 납단독급여군에서만 증가를 보였고, 철보충납급여군에서는 대조군과 차이를 보이지 않았다. 헤마토크릿치는 철보충납급여군에서는 대조군과 차이를 보이지 않았으나, 납단독급여군은 유의적인 감소를 보였다. DALAD activity는 혈액과 간 모두 납급여군에서 대조군보다 유의적인 감소를 보였으나, 납단독급여군보다 철보충납급여군이 높게 나타났고, 철보충수준이 높을수록 높게 나타났다. 혈청과 간의 amino-transferase activity는 철보충납급여군에서는 대조군과 비슷한 경향을 보였으나 납단독급여군에서는 유의적인 증가를 보였다. 혈청 납함량은 납단독급여군과 철보충납급여군 사이에 차이를 보이지 않았으나, 간의 납함량은 철보충납급여군이 납단독급여군보다 높게 나타났다. 혈청 철함량은 납급여군이 대조군보다 감소했으나, 간의 철함량은 납급여군에서 높게 나타났으며, 철보충납급여군이 납단독급여군보다 증가를 보였다. 혈청과 간의 구리함량은 납단독급여군은 대조군과의 차이를 보이지 않았으나, 철보충납급여군은 감소를 보였다.

Blood Vessel Regeneration using Human Umbilical Cord-derived Endothelial Progenitor Cells in Cyclophosphamide-treated Immune-deficient Mice

  • Endothelial cells are a vital constituent of most mammalian organs and are required to maintain the integrity of these tissues. These cells also play a major role in angiogenesis, inflammatory reactions, and in the regulation of thrombosis. Angiogenesis facilitates pulp formation and produces the vessels which are essential for the maintenance of tooth homeostasis. These vessels can also be used in bone and tissue regeneration, and in surgical procedures to place implants or to remove cancerous tissue. Furthermore, endothelial cell regeneration is the most critical component of the tooth generation process. The aim of the present study was to stimulate endothelial regeneration at a site of acute cyclophosphamide (CP)-induced endothelial injury by treatment with human umbilical cord-derived endothelial/mesenchymal stem cells (hEPCs). We randomly assigned 16 to 20-week-old female NOD/SCID mice into three separate groups, a hEPC ($1{\times}10^5$ cells) transplanted, 300mg/kg CP treated and saline (control) group. The mice were sacrificed on days 5 and 10 and blood was collected via the abdominal aorta for analysis. The alanine transaminase (ALT), aspartate aminotransferase (AST), serum alkaline phosphatase (s-ALP), and albumin (ALB) levels were then evaluated. Tissue sections from the livers and kidneys were stained with hematoxylin and eosin (HE) for microscopic analysis and were subjected to immunohistochemistry to evaluate any changes in the endothelial layer. CP treatment caused a weight reduction after one day. The kidney/body weight ratio increased in the hEPC treated animals compared with the CP only group at 10 days. Moreover, hEPC treatment resulted in reduced s-ALP, AST, ALT levels compared with the CP only group at 10 days. The CP only animals further showed endothelial injuries at five days which were recovered by hEPC treatment at 10 days. The number of CD31-positive cells was increased by hEPC treatment at both 5 and 10 days. In conclusion, the CP-induced disruption of endothelial cells is recovered by hEPC treatment, indicating that hEPC transplantation has potential benefits in the treatment of endothelial damage.

Prognostic Factor Analysis of Overall Survival in Gastric Cancer from Two Phase III Studies of Second-line Ramucirumab (REGARD and RAINBOW) Using Pooled Patient Data

  • Purpose: To identify baseline prognostic factors for survival in patients with disease progression, during or after chemotherapy for the treatment of advanced gastric or gastroesophageal junction (GEJ) cancer. Materials and Methods: We pooled data from patients randomized between 2009 and 2012 in 2 phase III, global double-blind studies of ramucirumab for the treatment of advanced gastric or GEJ adenocarcinoma following disease progression on first-line platinum- and/or fluoropyrimidine-containing therapy (REGARD and RAINBOW). Forty-one key baseline clinical and laboratory factors common in both studies were examined. Model building started with covariate screening using univariate Cox models (significance level=0.05). A stepwise multivariable Cox model identified the final prognostic factors (entry+exit significance level=0.01). Cox models were stratified by treatment and geographic region. The process was repeated to identify baseline prognostic quality of life (QoL) parameters. Results: Of 1,020 randomized patients, 953 (93%) patients without any missing covariates were included in the analysis. We identified 12 independent prognostic factors of poor survival: 1) peritoneal metastases; 2) Eastern Cooperative Oncology Group (ECOG) performance score 1; 3) the presence of a primary tumor; 4) time to progression since prior therapy <6 months; 5) poor/unknown tumor differentiation; abnormally low blood levels of 6) albumin, 7) sodium, and/or 8) lymphocytes; and abnormally high blood levels of 9) neutrophils, 10) aspartate aminotransferase (AST), 11) alkaline phosphatase (ALP), and/or 12) lactate dehydrogenase (LDH). Factors were used to devise a 4-tier prognostic index (median overall survival [OS] by risk [months]: high=3.4, moderate=6.4, medium=9.9, and low=14.5; Harrell's C-index=0.66; 95% confidence interval [CI], 0.64-0.68). Addition of QoL to the model identified patient-reported appetite loss as an independent prognostic factor. Conclusions: The identified prognostic factors and the reported prognostic index may help clinical decision-making, patient stratification, and planning of future clinical studies.

임신 중 살충제 amitraz에 노출된 랫드의 모독성 평가 (Evaluation of maternal toxicity in rats exposed to the insecticide amitraz during pregnancy)

  • The present study was carried out to investigate the potential adverse effects of amitraz on pregnant dams after maternal exposure during the gestational days (GD) 1 through 19 in Sprague-Dawley rats. The test chemical was administered orally to pregnant rats at dose levels of 0, 3, 10, or 30 mg/kg/ day. During the test period, clinical signs, mortality, body weights, food consumption, serum biochemistry, gross findings, organ weights and reproductive findings on GD 20 were examined. In the 30 mg/kg group, an increase in the incidence of abnormal clinical signs and death, a suppression in the body weight gain, and a decrease in the food consumption were observed. A decrease in the liver weight and increases in the kidneys, adrenal glands and heart weights were also found. Serum biochemical investigations revealed increases in the aspartate aminotransferase (AST), total bilirubin, and chloride. In addition, an increase in the fetal death and decreases in the litter size and fetal body weight were seen at caesarean section. Inthe 10 mg/kg group, an increase in the incidence of abnormal clinical signs, decreases in the food consumption and liver weight, increases in the total bilirubin and chloride, and a decrease in the fetal body weight were observed. There were no adverse effects on clinical signs, mortality, body weights, food consumption, serum biochemistry, gross findings, organ weights and reproductive findings in the 3 mg/kg group. Based on the results, it was concluded that the 19-day repeated oral dose of amitraz to pregnant rats caused increases in the clinical signs, kidneys, adrenal glands and heart weights, AST, total bilirubin and chloride and decreases in the body weight gain, food consumption and liver weight at the dose levels of above 10 mg/kg/day. Under the present experimental conditions, the no-observed-adverse-effect level (NOAEL) of amitraz was considered to be 3 mg/kg/day.

Fatty liver associated with metabolic derangement in patients with chronic kidney disease: A controlled attenuation parameter study

  • Background: Hepatic steatosis measured with controlled attenuation parameter (CAP) using transient elastography predicts metabolic syndrome in the general population. We investigated whether CAP predicted metabolic syndrome in chronic kidney disease patients. Methods: CAP was measured with transient elastography in 465 predialysis chronic kidney disease patients (mean age, 57.5 years). Results: The median CAP value was 239 (202-274) dB/m. In 195 (41.9%) patients with metabolic syndrome, diabetes mellitus was more prevalent (105 [53.8%] vs. 71 [26.3%], P < 0.001), with significantly increased urine albumin-to-creatinine ratio (184 [38-706] vs. 56 [16-408] mg/g Cr, P = 0.003), high sensitivity C-reactive protein levels (5.4 [1.4-28.2] vs. 1.7 [0.6-9.9] mg/L, P < 0.001), and CAP (248 [210-302] vs. 226 [196-259] dB/m, P < 0.001). In multiple linear regression analysis, CAP was independently related to body mass index (${\beta}=0.742$, P < 0.001), triglyceride levels (${\beta}=2.034$, P < 0.001), estimated glomerular filtration rate (${\beta}=0.316$, P = 0.001), serum albumin (${\beta}=1.386$, P < 0.001), alanine aminotransferase (${\beta}=0.064$, P = 0.029), and total bilirubin (${\beta}=-0.881$, P = 0.009). In multiple logistic regression analysis, increased CAP was independently associated with increased metabolic syndrome risk (per 10 dB/m increase; odds ratio, 1.093; 95% confidence interval, 1.009-1.183; P = 0.029) even after adjusting for multiple confounding factors. Conclusion: Increased CAP measured with transient elastography significantly correlated with and could predict increased metabolic syndrome risk in chronic kidney disease patients.

Effect of blended protein nutritional support on reducing burn-induced inflammation and organ injury

  • BACKGROUND/OBJECTIVES: Previous studies have reported that protein supplementation contributes to the attenuation of inflammation. Serious trauma such as burn injury usually results in the excessive release of inflammatory factors and organs dysfunction. However, a few reports continued to focus on the function of protein ingestion in regulating burn-induced inflammation and organ dysfunction. MATERIALS/METHODS: This study established the rat model of 30% total body surface area burn injury, and evaluated the function of blended protein (mixture of whey and soybean proteins). Blood routine examination, inflammatory factors, blood biochemistry, and immunohistochemical assays were employed to analyze the samples from different treatment groups. RESULTS: Our results indicated a decrease in the numbers of white blood cells, monocytes, and neutrophils in the burn injury group administered with the blended protein nutritional support (Burn+BP), as compared to the burn injury group administered normal saline supplementation (Burn+S). Expressions of the pro-inflammatory factors (tumor necrosis factor-α and interleukin-6 [IL-6]) and chemokines (macrophage chemoattractant protein-1, regulated upon activation normal T cell expressed and secreted factor, and C-C motif chemokine 11) were dramatically decreased, whereas anti-inflammatory factors (IL-4, IL-10, and IL-13) were significantly increased in the Burn+BP group. Kidney function related markers blood urea nitrogen and serum creatinine, and the liver function related markers alanine transaminase, aspartate aminotransferase, alkaline phosphatase, and lactate dehydrogenase were remarkably reduced, whereas albumin levels were elevated in the Burn+BP group as compared to levels obtained in the Burn+S group. Furthermore, inflammatory cells infiltration of the kidney and liver was also attenuated after burn injury administered with blended protein supplementation. CONCLUSIONS: In summary, nutritional support with blended proteins dramatically attenuates the burn-induced inflammatory reaction and protects organ functions. We believe this is a new insight into a potential therapeutic strategy for nutritional support of burn patients.

알코올성 간 및 장 손상 마우스모델에서 홍국쌀 추출물의 항산화효과 (Red Yeast Rice (Monascus purpureus) Extract Prevents Binge Alcohol Consumption-induced Leaky Gut and Liver Injury in Mice)

  • Red rice Yeasts는 홍국 또는 붉은 코지로 불리며 오래전부터 아시아에서 기능성 식품 친 전통의학에서 사용되고 있다. 홍국쌀에는 여러가지 생리 활성 물질을 포함하여 기능적인 2차대사산물과 6가지 유형의 색소를 생성한다. 간 질환의 주요한 원인인 알코올로 인한 알코올성 간 질환(ALD)은 단순 지방증 또는 염증으로 섬유증 및 간경변에 이르기까지 다양할 수 있으며, 장 누수 및 전신 내독소혈증을 유발하여 장장벽을 파괴하고 장내 미생물 변화를 증가시킨다. 본 연구는 홍국쌀(Monascus purpureus; RYR) 에탄올 추출물이 과량의 에탄올로 유발된 간 및 장 손상 마우스모델에서 간 및 장에 미치는 영향을 조사하였다. RYR 추출물을 10일 동안 200 mg/kg 체중/일의 농도로 C57/BL/6N 마우스에 경구 투여한 후 과량의 알코올 (5g/kg/용량)을 12시간 간격으로 3회 처리하였다. 과량의 알코올 노출은 혈장의 내독소, AST 및 ALT 활성과 간 트리글리세리드 수치를 유의적으로 증가시켰지만 RYR 처리는 이들 수치를 감소시켰다. 또한, RYR 전처리는 과량의 알코올 유발 장누수 및 간 손상에서 알코올 유발 산화 인자 단백질 및 세포사멸 인자를 유의하게 감소시켰다. 이러한 결과는 RYR이 알코올로 인한 급성 장누수 및 간 손상을 예방할 수 있음을 시사한다.

Double-Chambered Right Ventricle in an Old Standard Poodle Dog

  • A 12-year-old Standard Poodle presented with intermittent weakness and occasional dyspnea at the Veterinary Medicine Teaching Hospital of Kangwon National University. A grade of 4 out of 6 systolic murmur with an irregular tachycardic rhythm was auscultated on both sides of the chest. Systolic blood pressure was 140 mmHg. Panting was noticed in the hospital, but there was no crackle sound. Blood analysis revealed mild increases in liver panel levels (alanine aminotransferase 149 [reference interval, 19-70] U/L; and alkaline phosphatase, 185 [reference interval, 15-127] U/L) and severe increases in cardiac biomarker levels (n-terminal pro-brain natriuretic peptide, 4169 [reference interval, 50-900] pmol/L; and cardiac troponin I, 0.22 [reference interval, 0.03-0.12] ng/mL). On electrocardiography, irregularly irregular supraventricular tachycardic rhythm with an f-wave and no distinct p-wave was observed. Generalized cardiomegaly with an enlarged right atrium and left ventricle was confirmed on thoracic radiography. Moreover, hepatomegaly and an enlarged caudal vena cava were observed. Echocardiographic evaluation revealed a fibromuscular diaphragm in the right ventricle. Because of the obstructive lesion in the right ventricle, the right atrium and ventricle were enlarged (right atrial area index, 38.82 cm2/m2 [reference interval, 4.2-10.2 cm2/m2]; right ventricle end-diastolic area index, 14.152 cm2/m2 [reference interval, 4.9-10.92 cm2/m2]). Accordingly, the patient was diagnosed with double-chambered right ventricle (DCRV). Pimobendan, furosemide, enalapril, diltiazem, and S-adenosylmethionine (SAMe) were prescribed, and all symptoms were relieved. DCRV is a right-sided congenital heart defect resembling pulmonic valve stenosis. If symptoms are not severe, medical therapy can be facilitated without surgery or the balloon dilation.

참마 분획물과 vitamin E 투여가 당뇨유발 흰쥐의 혈당 및 지질대사에 미치는 영향 (Effects of $H_2O$ Fraction of Dioscorea japonica Thunb with Vitamin E on Glucose and Lipid Metabolism in Streptozotocin Induced Diabetic Rats)

  • 본 연구에서는 methanol로 온시여과 후 계통분획하여 감압농축하여 얻은 참마의 $H_2O$ 분획물을 vitamin E와 함께 STZ으로 당뇨를 유발시킨 흰쥐에게 14일간 경구투여한 후 혈당과 지질대사에 미치는 영향을 실험하여 다음과 같은 결과를 얻었다. 당뇨대조군은 당뇨로 인한 대사상의 변화로 정상군과는 다른 대사적인 차이를 나타내었다. 당뇨대조군과 참마와 vit. E 투여에 따른 당뇨실험군에서는 식이의 섭취와 식이효율에는 유의적인 차이가 없었다. 체중의 변화는 14일 후 참마와 vit. E 병용투여군에서 정상군과 유사한 수준이었고 전 실험기간동안의 체중의 변화는 실험군간에 점차 낮아지는 경향을 보였으나 유의적인 차이를 보이지 않았다. 장기의 무게를 체중 100 g 당으로 환산하였을 때 심장의 무게는 당뇨실험 군 중 vit. E 투여군에서 낮게 나타났으며 췌장의 무게는 당뇨대조군에 비해 모든 실험군에 낮게 나타났으나 유의적인 차이는 보이지 않았다. 혈장 단백질 수준은 당뇨대조군에 비해 당뇨실험군 모두에서 높게 나타났다. 혈장 인슬린의 함량은 모든 실험군에서 유의적인 차이를 보이지 않았으나 참마투여군에서 약간 높은 수준이었다. 혈장 포도당 수준도 당뇨대조군에 비해 당뇨실험군에서 낮아지는 경향을 보였다. 혈장지질 수준에 미치는 영향을 보면 중성지방과 유리지방산 함량은 당뇨대조군에 비해 vit. E군에서 낮은 수준을 보였으며 HDL-cholesterol 수준은 모든 당뇨실험군에서 높게 나타났다. 혈장 cholesterol 수준의 경우도 당뇨대조군과 실험군에서 큰 차이는 없었으나 vit. E군에서 낮은 수준을 보였다. AST및 ALT활성도는 실험군 간에 큰 차이를 보이지 않았다. 본 연구 결과 참마의 $H_2O$ 분획물은 vitamin E 병용 투여시 지방대사와 인슬린활성에 작용하여 혈당강하에 영향을 주는 생리활성성분을 함유하는 식품으로 당뇨식이처방에 유용하리라 생각되며 vitamin E 보충에 의한 대사적인 변화에 대하여 더 구체적인 생화학적인 연구가 이루어져야 될 것으로 여겨진다.

  • Holstein종(種) 빈유우(牝乳牛)에 있어서 케톤증(症)이 제1위내(第1胃內)의 휘발성지방산(揮發性脂肪酸)(VFA)과 혈액화학치(血液化學値)에 미치는 영향(影響)을 알아보고저 시도(試圖)하여, 임상진단상(臨床診斷上) 건강(健康)한 빈유우(牝乳牛) 5두(頭)(제I군)과 산후(産後) 초기(初期)케톤증(症) 빈유우(牝乳牛) 5두(頭)(제II군)를 실험군(實驗群)으로 하여, 출산전(出産前) 5일(日)부터 7일(日) 그리고 출산후(出産後) 5, 10, 15, 20 및 25일(日)에의 VFA를 검사(檢査) 비교(比較)하여 본 바 출산후(出産後) 5일(日)부터 VFA수준(水準)이 점진적으로 증가(增加)됨이 관찰(觀察) 되었는데, 특(特)히 케톤증후군(症候群)의 VFA의 조성(組成)으로서 acetic acid와 caproic acid치(値)가 건강빈유우(健康牝乳牛)군 보다 낮은 것이 관찰(觀察)되었다. 혈액화학치(血液化學値)로서 혈당량(血糖量)은 양군(兩群)이 공(共)히 감소경향(減少頃向)을 보인 반면(反面)에 혈중(血中) 케톤체(體)와 3-hydroxy-butyric acid는 출산후(出産後) 현저(顯著)히 증가(增加)되고 있으며 양군간(兩群間)에는 유의(有意)한 차이(差異)가 없었다. AST수준(水準)은 출산후(出産後) 양군(兩群)에서 일시적(一時的)으로 증가(增加)되었는데, 특(特)히 케톤증후군(症候群)의 경우 건강빈유우군(健康牝乳牛群)들 보다 현저(顯著)히 높았다. 혈중(血中)콜레스테롤, 유리(遊離)콜레스테롤, 콜레스테롤-에스텔, 소지질(燒脂質) 및 총담즙산(總膽汁酸)은 출산후(出産後) 증가경향(增加傾向)을 보였으나, 양군간(兩群間)에는 유의(有意)한 차이(差異)가 없었다. 이상(以上)의 결과(結果)를 미루어 보아 건강빈유우(健康牝乳牛)에서도 출산후(出産後) VFA치(値)와 혈액화학치(血液化學値)에 조성(組成)이 케톤증후군(症候群)과 비슷한 변화(變化)를 가져 오는 것은 이들 건강빈유우(健康牝乳牛)에서도 산후(産後)에 일시적(一時的)으로 준림상적(準臨床的) 케톤증후(症候)를 유발(誘發)할 수 있다는 것으로 생각된다.
