MAXILLARY SINUS GRAFT FOR ENDOSSEOUS IMPLANT PLACEMENT : REVIEW OF THE LITERATURES (임프란트 식립을 위한 상악동점막 거상술: 문헌고찰)

  • A review of the literature, provided by a MEDLINE search from 1980 through June 1999, was performed. This study was screened that 649 patients received 679 sinus lift grafts in which 2056 implants were placed. The types of grafts in sinus augmentation were autogenous bone, corticocancellous block bone, allogenic bone, and a variety of alloplastic materials. Results of these grafts are presented. The most frequent complications was the infection of maxillary sinus. Long-term follow-up is necessary to advance the sinus elevation and to support posterior maxillary restorations.

  • The ankylosis of temporomandibular joint occured by several causes directly developes TM joint disfunction, In cases with prolonged condylar ankylosis, especially at growing age the condylar ankylosis evokes malfunction of growth center area, and then developes the abnormal facial morphology and malocclusion. Therefore it must be cured. Almost authors have agreed to the necessity of surgical correction of the TMJ anylosis. but they did not decide the one surgical method to get the best result. The Tx. method suggested by many authors are the using interposition after resection of condyle to remain a lever of 3rd class in Mn. kinetics, the autogenous condylar graft and the alloplastic condylar graft. Some authors have got the satisfied results only with the condylectomy of the involved TMJ. This study also operated only the detachment of fibrous adhesion on ankylosed condylar side and then established occlusion in the case with the unilateral TMJ ankylosis and fibrous joint adhesion and facial asymmetry evoked after the fracture of condylar head at early age. This study got a improved mouth opening and a stable postsurgical result after 1 year. Also, this study reviewed many author's study about the chanracteristics, etiology, diagnosis and Tx. method for the ankylosis of TMJ.

Maxillofacial reconstruction with Medpor porous polyethylene implant: a case series study

  • Objectives: The role of alloplastic materials in maxillofacial reconstruction is still controversial. Determining the utility of porous, high-density, polyethylene implants as a highly stable and flexible, porous alloplast, with properties such as rapid vascularization and tissue ingrowth, is crucial in cases of maxillofacial deformities and aesthetic surgery. Materials and Methods: Thirty high-density porous polyethylene implants were implanted in 16 patients that had been referred to a private office over a three-year period. These implants were used for correcting congenital deformities, posttraumatic defects and improving the aesthetic in nasal, paranasal, malar, chin, mandibular angle, body and orbital areas. Results: The outcomes of the cases in this study showed good aesthetic and functional results. The majority of patients had no signs of discomfort, rejection or exposure. Two implants suffered complications: a complicated malar implant was managed by antibiotic therapy, and an infected mandibular angle implant was removed despite antibiotic therapy. Conclusion: Based on the results, the Medpor implant seems to be an excellent biomaterial for correcting various facial deformities. Advantages include its versatility and relatively ideal pore size that allows for excellent soft tissue ingrowth and coverage. It is strong, flexible and easy to shape.

  • Facial asymmetry can be most distressing for the young child and parents. It can cause functional problems as a result of malocclusion. Classification of facial asymmetry has not been yet well-organized because of its varieties on etiologic factors, involved sites and clinical expressions. Even though, we don't know its causes definitely. It is generally believed that problems with aberrant pattern of condylar growth are related to facial asymmetry. This is a case report on surgical correction of the patients who had severe facial asymmetry. One patient was diagnosed as condylar hyperplasia and the other was diagnosed as a condylar hypoplasia related to trauma. We performed a simultaneous two-jaw surgery, condylar shaving, inferior border ostectomy of affected mandible in the former case, and a simultaneous two-jaw surgery, reverse-L osteotomy and alloplastic implantation with $Biocoral^{TM}$ in the latter case. The postoperative results of the two cases were excellent functionally and esthetically.

  • Osteochondroma is a common benign tumor of the axial skeleton, especially the distal metaphysis of the femur and proximal metaphysis of the tibia. However, it occurred rarely on the facial skeleton. The coronoid and condylar processes have been considered to be the most common sites of occurrence for osteochondroma of the facial skeleton. The first treatment of osteochondroma is condylectomy, whereas extirpation was done by excision with condyle salvage. Condylectomy presents decrease of vertical dimension, jaw deviation, malocclusion. So, reconstruction is need. Methods of reconstruction are as follows: no reconstruction, condyloplasty, discectomy, costochondral graft, discplication or coronoidectomy, eminoplasty, alloplastic spacer placement, Le Fort I level maxillary osteotomy, extraoral and intraoral vertical ramus osteotomy. This is a case report of a 28-year old woman who had facial asymmetry, malocclusion and temporomandibular joint pain. We obtained moderate functional and cosmetic results with surgical removal of the osteochondroma by condylectomy and concomitant reconstruction of condyle by vertical ramus osteotomy with sliding technique.

  • Post-traumatic enophthalmos is a relatively common problem following orbitozygomatic fractures. Bony-volume expansion and soft tissue atrophy are considered the main etiological causes of this condition. Enophthalmos is corrected mostly through reducing the enlarged orbit volume. Autogenous graft and various alloplastic materials are used for this purpose. Porous polyethylene is highly biocompatible, durable, and remarkably stable. Also, the titanium plate embedded in a porous polyethylene sheet provides radiographic visibility and increased sheet strength and contour retention. We present experiences of titanium reinforced porous polyethylene for correction of the traumatic enophthalmos with literature review.

Utility of the BoneSource® in the Treatment of Craniofacial Bone Defect (두개안면골 결손 환자의 치료에서 Bonesource®의 유용성)

  • Skull or facial bone defect causes cosmetic and functional problems. On going efforts were taken to reconstruct the craniofacial bone defect with autogenous bone or alloplastic materials. Between 2001 and 2003, we reconstructed 15 cases with small to large craniofacial bone defect using Hydroxyapatite paste($BoneSource^{(R)}$, Leibinger Corp., Dallas, TX, U.S.A) and calvarial bone graft. All patients were followed up by clinical examination and periodic radiographic studies for an average of 1.6 years (range, 6 months to 2 years and 6 months). One complication case was noted with $BoneSource^{(R)}$ exposure. Otherwise, there was no evidence of adverse healing, wound infection, foreign body reaction and seroma collection in other patients. Adequate 3-dimensional aesthetic restoration of calvarial contour was noted in each case. In conclusion, $Bonesource^{(R)}$ is relatively safe and rigid material for craniofacial bone defect and reasonable method for the reconstruction of craniofacial bone defects.

  • Ankylosing spondylitis (AS) is a chronic autoimmune disease mainly involving the axial skeleton. The pathology of the disease is usually found at the sacroiliac joint, and half of the patients experience cervical spine invasion, but eventually, the whole spine is affected. The involvement of the temporomandibular joint (TMJ) in AS has not been investigated very well. A review of the literature revealed that there are only a few studies of TMJ involvement in AS that combined clinical and radiographic examinations. These studies show widely different results, ranging between 4% and 32%. We experienced Bilateral osseous ankylosis of the jaw treated by total alloplastic joint replacement in AS, and offer a case report.

  • The maxillary posterior edentulous region presents unique and challenging conditions in implant dentistry. The height of the posterior maxilla is reduced greatly as a result of dual resorption from the crest of the ridge and pneumatization of the maxillary sinus after the loss of teeth. Materials previously used for sinus floor grafting include autogenous bone, allogeneic bone, xenogenic bone and alloplastic materials. Autogenous bone is the material of choice, but its use is limited by donor-site morbidity, complications, sparse availability, uncontrolled resorption and marked volume loss. One way to overcome this problem would be to use bone substitutes alone as a osteoconductive scaffold for bone regeneration from the residual bone or in combination with allogeneic bone, which also has osteoinductive properties. The purpose of this article is to describe a double layers technique of demineralized and mineralized bone graft materials instead of autogenous bone in sinus floor augmentation of deficient posterior maxillary alveolar process and to report our experience with this technique. Our results show that maxillary sinus augmentation using mineralized and demineralized bone materials, when installed simultaneously with the implant or not, is good results for bone healing.

A STUDY ON TISSUE RESPONSES OF VARIOUS ALLOPLASTIC MATERIALS FOR RIDGE AUGMENTATION (수종 치조제 이물성형재의 조직반응에 관한 연구)

  • Loss of teeth causes the inevitable reduction of residual ridge. Among the various methods solving this problem, hydroxyapatite proved to be useful for correction of ridge defect and irregularity. The purpose of this study is to evaluate the tissue responses of two types Of hydroxyapatites and resin polymer. Calcitite 2040 (Calcitek Inc.), Interpore 200 (Interpore Int.), and HTR polymer (HTR Sciences) were implanted into the jaw of an adult dog. The procedure was designed to obtain the results of 1 week, 2 week, 4 week, and 12 week-intervals. And after 12 weeks from the first operation day, the dog was sacrificed and evaluated histologically by light microscope. The results were as follows : 1. The mucosa was healed after two weeks. 2. After 1 week, there were acute inflammatory cells, but diminished after 2 weeks and were not seen after 12 weeks. 3. The hydroxyapatites implanted in soft tissues were surrounded by fibrous connective tissue. And some foreign body giant cells were found. 4. Calcitite and Interpore particles implanted subperiosteally were surrounded by newly formed bone after 12 months. And direct contact between bone and particles was noted. 5. The HTR particles implanted in soft tissues were encapsulated by fibrous connective tissues. The sample where the particles contacted directly to bone could not obtained by some probable insufficiencies of surgical technique or care of the animal. And the residue of HTR particles was digested by macrophage.
