간접심미수복을 부탁해 세라믹을 심미수복의 임상 접근 (Clinical approach with ceramic)

  • The requirements for the successful treatment of all-ceramic restorations are not so different from the ones of conventional restorations. "The provisional restoration followed by an adequate tooth reduction" and "the accurately fitting prostheses with corresponding to final impression" can be the examples of them. Nevertheless, the one which all-ceramic restorations are distinguished from conventional restorations is the additional procedure of so called "bonding". In addition to the application of resin cement between "inner surface of restoration and outer surface of abutment", bonding technology can be also applied to the treatment process of "Post and Core" in particular if the abutments are non-vital teeth. Core build-up for all-ceramic crown is conducted with fiber post and tooth colored composite by considering the properties of the restorations transmitting light. I would like to share my clinical experience about "silica based ceramic and non silica based ceramic restoration.

The creation of "Post and Core" with bonding technique is a decisive factor for a long-term success if the abutment is non-vital tooth with dentinal collapse. I would like to share my clinical experience about "post & core build-up and all-ceramic restoration bonding" out of several success strategies of all-ceramic crown with this presentation.

  • 성공적싱 완전도재관 치료를 위한 요구조건은 통상적 치료와 크게 다르지 않다. 적절한 치아삭제, 임시치아, 및 정확한 최종인상이 중요하다. 그럼에도 불구하고 차별점은 본딩에 있다. 수복물 내면과 지대치 외면에 레진시멘트를 도포하는 것 이외에도 본딩 술식은 비생활치의 포스트 코어 치료에도 적용할 수 있다. 생활치 지대치는 비생활치보다 심미와 기능 면에서 성공을 거두기가 더 쉽다. 포스트 코어 본딩 술식은 비생활치 지대치의 장기 성공에 중요한 역할을 한다. 본 논문에서 완전 도재관의 성공 전략 중에서 포스트 코어 축성 및 완전 도재관 본딩에 관하여 임상적 경험을 나누고자 한다.

전치부 ALL-CERAMIC PARTIAL CROWN 의 임상 (- Aesthetic Restoration of the Gingival Recession with All-ceramic Partial Crown -)

  • The black triangle is a common clinical finding in aged people with gingival recessions. Among other prosthetic treatment molalities such as composite resin filling and laminate venners, the all ceramic restoration procedure can lead to most successful result. With improved bonding strength and ceramic properties, the tooth preparation design for all-ceramic crown can be modified to minimize the reduction of sound tooth structure without loosing properties of conventional preparation design. Case selection is an important factor in acheiving succesful prosthesis. In this case report, the leucite reinforced pressable ceramic, $Authentic^{TM}$ [Ceramay, Germany] was used to fabricate the prosthesis.

A new manufacturing method of all ceramic restoration using alumina tape and presentation of clinical cases

  • In an effort to facilitate fabrication procedure of all ceramic crowns, a novel preparation method for all ceramic crown using alumina tape was developed. The alumina tape having a uniform thickness was cast by using Doctor blade method. The physical properties of newly introduced alumina tape has biaxial flexure strength of $500\sim600MPa$. The value of toughness is $3.18\sim3,28MPa.m^{1/2}$ which corelates with fracture and the linear shrinkage rate of the alumina tape is 0.44% during core production. The marginal fitness of the alumina tape all-ceramic restoration with $90{!`}$shoulder margin had average marginal discrepancy at $78.3{\S}$ > and average marginal gap at $44.4{\S}$ >. At the marginal preparation of $135{!`}$deep chamfer, the average marginal discrepancy at $82.1{\S}$ > and the average marginal gap at $40.2{\S}$ > had been reported. This fabrication procedure of all ceramic crowns with alumina tapes is easier and less technical sensitive for dental technicians. After restoration with new all ceramic crowns we followed the patients 2 years later, there were no complications as porcelain fractures or periodontal disease. We had good esthetic clinical results with new all ceramic crowns.

전부도재수복물을 위한 수종의 코어 세라믹의 반투명도 및 색 안정성 (Translucency and Color Stability of Various Core Ceramics for All-Ceramic Restoration)

  • Purpose: This investigation was designed to determine the translucency and color stability of various core ceramics for all-ceramic restoration using the CIE $L^*a^*b^*$ system. Material and Methods: IPS e.max Press ceramic(Ivoclar-Vivadent, Liechtenstein), $LAVA^{TM}$ All Ceramic(3M-Espe, Germany), Cercon Smart Ceramic(Dentsply, Germany), and Z-match Ceramic(DentAim, Korea) were used for this study. For the specimens of zirconia oxide ceramics, the as-sintered cylindrical blanks($11.0{\times}25.0mm$) were machined into the shape of a disk(0.4, 0.8, 1.5 mm in thickness, 10 mm in diameters) with a diamond grind machine. The IPS e.max Press specimens ($0.8{\times}10mm$) were fabricated using the "lost wax" technique. CIE $L^*a^*b^*$ coordinates and light transmission were recorded for each specimen with a spectrophotometer(CM-2600d, Minolta, Japan). Color differences were calculated using the equation, ${\Delta}E^*ab=[({\Delta}L^*)2+({\Delta}a^*)2+({\Delta}b^*)2]1/2$. Results: The results were obtained as follows: 1. The most translucent group was IPS e.max Press ceramic that is a glass-ceramic, and $Lava^{TM}$ and Z-match ceramic were more translucent than Cercon Smart ceramic in zirconia ceramic group. 2. In the all groups, there was no significant translucent change after 6 times heat-treatments required to make a final restoration. 3. Colored zirconia was showed more yellowish and dark than uncolored zirconia. 4. After heat-pressing, IPS e.max Press ceramic was showed high ${\Delta}E^*ab$ value(4.1 of eM1, 6.8 of eM2) that means to be more whiter than before heat-pressing. However, there was no color change after additive heat treatments for final restoration. 5. In the colored zirconia groups, there was no significant color change after some heat-treatments required to make a final restoration.

치과용 CAD/CAM 전용 세라믹기반 하이브리드 레진 수복물의 변연 적합 연구 (A study on marginal fit of the ceramic-based hybrid resin restoration for dental CAD/CAM systems)

  • Purpose: This study aimed to assess and compare the marginal fit of ceramic-based hybrid resin restoration (HYB) and zirconia restoration (ZIR) for dental computer-aided design/computer-aided manufacturing systems. Methods: A stainless steel master model was produced. The impression was first made with silicone, and then stone working models were produced. A total of twenty restorations were fabricated with two different materials: ZIR and HYB. The silicone film thickness of the marginal gap was measured using a digital microscope; digital photos were taken at a magnification of ×160, and then analyzed using a measurement software. The values of the result were evaluated with the independent-sample t-test (α=0.05). All statistical analyses were performed with a statistical software. Results: The mean values for the marginal gap was 37.14±2.96 ㎛ for HYB, compared with 40.37±5.26 ㎛ for ZIR. No significant difference was found between ZIR and HYB (p=0.107). Conclusion: As a result, the marginal fit of the restoration fabricated using the hybrid resin was better than that of the restoration fabricated using zirconia. Also, the marginal fit of all groups was below the clinical acceptable range of 120 ㎛. Thus, HYB for dental CAD/CAM system in this study is expected to be suitable for clinical use in dentistry.

도재소성 시 냉각속도가 전부도재관의 색조에 미치는 영향 (Effect of Shade on All Ceramic Restoration based Zirconia according to Cooling rate on Firing)

  • Purpose: The purpose of this in vitro study was to evaluation the effect of shade on all ceramic restoration based zirconia according to cooling rate on Firing. Methods: 10 specimens applied to the dentin porcelain were made on the zirconia. After 5 specimens of the first group were burned in the furnace, these were put out rapidly. And after 5 specimens of the second group were burned in the furnace, these were put out slowly later 15 min. All specimens were measured $L^*$, $a^*$, $b^*$ using spectrophotometer, there were calculated 10 mean(SD) of descriptive statistics with SPSS program. These data were used for ${\Delta}E^*$ with color difference equation. Independent t tests were performed between 2 groups. Results: $L^*$, $a^*$, $b^*$ of 2 groups was statistically significant respectively (p<0.001), ${\Delta}E^*$ was 4.55 value. Conclusion: This study showed effect of shade on all ceramic restoration based zirconia according to cooling rate on Firing.

전부도재관에 사용되는 레진시멘트의 색안정성에 관한 연구 (THE STUDY ON THE COLOR STABILITY OF RESIN CEMENT USED IN ALL CERAMIC CROWN)

  • Statement of problem : The Cement as well as restoration required esthetics for making natural color restoration. Purpose : The purpose of this research is to evaluate color stability of restoration intermediated by resin cement which is used for cementation of all ceramic crown. Material and method : After making Empress 2 ingot into the size of $10mm{\times}10mm{\times}1mm$ according to indication, it glazed and made 48 Empress 2 blocks. Three kinds of resin cement(Rely-X, Variolink 2, Choice) having same shade cemented between Empress 2 blocks and Ivory shade tiles and made 48 specimens in the thickness of $30{\mu}m$ and $80{\mu}m$. After measureing color difference using spectorphotomenter, the result of this study were as follows. Results : The color difference of resin cement used in experiment increased in the order Rely-X, Variolink 2. As the thickness of cement increases, the color difference of all kinds of cement found statistically sifnificant difference but, this result is clinically acceptable. Conclusion : More resarch would have to be done in order to decrease the color difference as cement's thickness.
