목적: 기존의 CT 영상 촬영에서 이용되고 있는 고용량의 경구용 조영제는 복용과정에서 위장계통의 불편함이 나타나고 또한 PET/CT에 이용할 경우에는 이로 인해 인공물(artifact)이 생길 우려가 있다. 본 연구의 목적은 $^{18}F-FDG$ PET/CT에서 저용량의 경구용 조영제 사용의 유용성에 대해 알아보고자 한다. 대상 및 방법: 총 435명의 전신 PET/CT 영상을 후향적으로 분석하였다. 모든 환자들은 $^{18}F-FDG$. 주사 직전에 200 ml의 경구용 조영제(bariumsulfate)를 복용하였다. 장에서의 FDG 섭취 양상을 가로단면, 관상면, 시상면에서 시각적으로 판독하였고, 섭취양상을 반정량화하였다. 결과: 435명의 영상 중 70명(16%, 113부위) 에서 높은 FDG 섭취(peak SUV > 4)를 보였으며, 이 중 미만성 섭취를 보인 경우가 50명(74%, 84부위), 국소적 섭취를 보인 것은 20명(26%, 29부위)이였다. 경구용 조영제가 가장 흔하게 분포된 부위는 소장(n=27, 39%)이었다. PET/CT 영상에서 FDG섭취부위와 조영제 분포 부위가 일치하는 경우 중에서 미만성 섭취를 보인 것은 26명(54%, 38부위), 국소적 섭취를 보인 것은 9명(45%, 9부위)이였다. 이들을 부위별로 보면 미만성 섭취를 보인 것은 38개(45%), 국소적 섭취를 보인 것이 9개(31%)였다. FDG 섭취와 조영제의 분포가 일치하는 부위는 소장에서 61% (29/47부위)로 가장 많이 관찰되었다. 감쇠 보정을 하지 않은 PET 영상을 본 결과, 검토가 가능했던 27개의 섭취 일치 부위에서 조영제로 인한 인공물(artifact) 영향은 관찰되지 않았다. 결론 저용량 경구용 조영제는 FDG PET/CT 영상에서 인공물 영향 없이 복부의 FDG 섭취를 판별하는데 도움을 줄 수 있다.T은 민감도 85.7%(6/7), 특이도 92.3%(12/13), MR은 민감도 100%(7/7), 특이도 76.9%(10/13)를 보였다. 그 외에도 간전이 병변의 평균 maxSUV는 $6.7{\pm}3.8$로 간전이와 양성병변을 최적으로 감별할 수 있는 maxSUV의 cutoff value는 3.1 이었다. (AUC=0.897, p<0.001, 민감도 83.3%, 특이도 94.1%) 결론: FDG PET은 간전이를 진단하는데 MR과 대등한 민감도와 특이도를 보였다. 통계적으로 유의한 차이를 보이지는 않았지만 간전이에 대한 FDG PET의 음성예측도는 MR보다 높았으며, 직경 10 mm 미만의 작은 병변에 대해서도 FDG PET은 우수한 성적을 보였다. 향후 대장암의 간전이 진단에 FDG PET이 유용하게 이용될 것이다.CA-tk 에서 확연한 집적의 차이를 보여주었다. 결론: 간암세포주에서 HSV1-tk 유전자의 발현 정도와 지속성 그리고 위치를 확인하기 위한 비침습적 PET 영상을 위한 기질로서 $[^{18}F]FHBG$는 매우 유용할 것으로 기대된다.EX>의 수용체 결합능의 감소율과 혈중 니코틴의 축적 농도와의 상관관계를 스피어만 상관분석법(Spearman's correlation)에 의하여 알아보았다. 결과: 흡연에 의한 선조체에서의 평균 $[^{11}C]raclopride$의 수용체 결합능의 변화는 미상핵에서 4.7%, 전피각에서 4.0%, 복측 선조체에서 7.8% 의 감소를 보여 흡연에 의한 선조체내 도파민 유리를 정량화 하였다. 특히 선조체에서의 도파민 유리에 의한 수용체 결합능의 감소는 흡연에 의한 혈중 니코틴의 축적 농도와 양의 상관관계를 보였다(rho=0.9, p=0.04). 결론: $[^{11}C]raclopride$ PET을 이용하여 비흡연 정상인에서 흡연에 의한
최근 대형 건설회사의 해외시장 비중 확대와 함께 해외 프로젝트에서의 공기지연에 따른 경영 손실에 대한 이슈가 지속적으로 제기되고 있다. 일반적으로 대형 프로젝트의 수행 과정에서 변경은 필연적으로 발생한다. 대부분의 변경은 공기지연과 연결되기 때문에 변경으로 인하여 공기연장이 필요한 경우 건설사는 계약에서 정한 기준에 따라 공기지연을 관리해야 공기지연에 따른 리스크에 효과적으로 대응할 수 있다. 공기지연을 유발하는 변경에 대한 계약적 대응이 부족할 경우 클레임이 해결되지 않고 분쟁으로 확대되는 경우가 많다. 프로젝트 초기 단계에 계약에서 요구하는 공기지연 관리 절차와 방법을 분석하는 것이 중요하나 이에 대한 방법 및 도구를 제시하지 못하고 있었다. 본 논문에서는 국내 주요 건설사들의 공정관리 실무 및 해외 공사에서의 클레임 및 분쟁사례에 대한 고찰을 통해 문제를 제시하고 해외 건설 계약조건상에 포함된 공기지연 관리 리스크를 평가할 수 있는 방법을 제시하였다. 공기지연의 합리적 대응과 관련된 계약조건상의 리스크 평가 틀을 제시하기 위하여 FIDIC Red Book(1999), PSSCOC(2014) 및 SIA 9th Edition(2010) 등 국제 표준 계약조건 뿐만 아니라 ARAMCO 및 QP를 포함한 해외 대형 발주자의 표준계약조건에 나타나 있는 공기지연 관리 요건 및 수준을 검토하여 전체 7개 영역, 62개 항목으로 구성된 계약조건 리스크 평가모델을 제안하였다. 계약조건 리스크 평가 모델은 계약조건상의 공기지연리스크 확인을 위한 구조적 틀을 제공하며 개별 리스크 항목별 표준적인 수준을 제시함으로써 해당 프로젝트의 리스크 수준을 확인하고 이에 효과적으로 대응할 수 있는 단초를 제공한다. 제안된 모델의 유효성 검증을 위하여 실제 제안한 모델을 실제 프로젝트에 적용하여 그 효과를 입증하였다.
유비쿼터스 시스템의 발달에 따라 시스템이 다루는 상황정보의 숫자 또한 크게 증가하고 있다. 이러한 상황정보 중에는 정보보호 관점에서 중요한 데이터들이 다수 존재한다. 이러한 중요한 상황정보가 다른 사용자 또는 서비스에게 제공됨에 따라 개인정보의 과도한 노출 가능성 또한 크게 증가되고 있다. 이러한 과도한 정보의 노출을 위하여 여러 시스템은 접근 제어 기법을 주로 이용하나 이러한 기존 기법은 허가되지 않은 정보의 접근을 막을 수는 있으나 허가된 정보의 제공에서 발생하는 과도한 정보의 노출은 막을 수 없다는 문제점을 가지고 있다. 본 논문은 이러한 개인정보의 과도한 노출을 막기 위하여 상황정보를 추상화하여 제공하는 접근 제어 프레임워크를 제안한다. 상황정보의 과도한 노출을 막기 위하여 협상 프로토콜과 RDF를 이용한 상황정보 추상화를 제공하며, 이를 통하여 개인정보의 보호와 동시에 서비스의 연속성을 유지한다.
Nocturnal enuresis is a heterogeneous disorder with various underlying pathophysiological mechanisms and causes a mismatch between the nocturnal bladder capacity and the amount of urine produced during sleep at night. It is associated with a simultaneous failure of conscious arousal in response to the sensation of bladder fullness. Generally, a complete history and physical examination, with a specific focus on the genitourinary, gastrointestinal, and neurologic systems, is sufficient to evaluate a patient with enuresis. The therapeutic focus is directed toward a differential approach based on the underlying mechanism and toward combination therapies such as alarm devices and desmopressin as well as anticholinergic agents and desmopressin. Children with increased nocturnal urine production usually have a good response to desmopressin therapy. Patients with a small bladder generally show a poor response to desmopressin treatment, but they would benefit more from combination therapy with enuretic alarm, urotherapy, and antimuscarinic agents in addition to desmopressin. Different types of bladder dysfunction, which result in a small nocturnal bladder capacity, probably contribute significantly to the pathogenesis of nocturnal enuresis, particularly in those with treatment failure and refractory symptoms. Because different clinical subgroups may show different responses to treatment, it is necessary to distinguish these subgroups before a decision on the specific treatment protocol can be made.
Purpose: The purpose of this study was to develop a scenario and evaluate students' performance in simulation learning of care for patients with asthma in emergency units. Methods: Meetings of experts were used to develop a scenario based on actual patients and textbook material. An evaluation protocol was developed to evaluate the simulation learning. The scenario was used in 2006 with six groups of 26 senior nursing students who participated voluntarily. Results: The scenario was developed according to the nursing process for 15 minutes of simulation learning. The nursing students were able to demonstrate their knowledge and skills. The results showed a need to improve problem solving ability. In the self-evaluation, the students reported that simulation learning helped them to integrate their knowledge to practice and recognize their weaknesses and strengths. However, the scores for self-confidence about patient care after the simulation learning were low (4.8/10). Conclusion: The scenario in this study gave the students the experience of providing qualified and secure nursing care under conditions similar to reality. Further development of a variety of scenarios for simulation learning is needed.

Purpose: Although standard radical gastrectomy is recommended after noncurative resection of endoscopic submucosal dissection (ESD) for early gastric cancer in most cases, residual tumor and lymph node metastasis have not been identified after surgery. The aim of this study is to evaluate the feasibility of sentinel node navigation surgery after noncurative ESD. Materials and Methods: This trial is an investigator-initiated, multicenter prospective phase II trial. Patients who underwent ESD for clinical stage T1N0M0 gastric cancer with noncurative resections were eligible. Qualified investigators who completed the prior phase III trial (SENORITA 1) are exclusively allowed to participate. In this study, 2 detection methods will be used: 1) intraoperative endoscopic submucosal injection of dual tracer, including radioisotope and indocyanine green (ICG) with sentinel basins detected using gamma-probe; 2) endoscopic injection of ICG, with sentinel basins detected using a fluorescence imaging system. Standard laparoscopic gastrectomy with lymphadenectomy will be performed. Sample size is calculated based on the inferior confidence interval of the detection rate of 95%, and the calculated accrual is 237 patients. The primary endpoint is detection rate, and the secondary endpoints are sensitivity and postoperative complications. Conclusions: This study is expected to clarify the feasibility of laparoscopic sentinel basin dissection after noncurative ESD. If the feasibility is demonstrated, a multicenter phase III trial will be initiated to compare laparoscopic sentinel node navigation surgery versus laparoscopic standard gastrectomy in early gastric cancer after endoscopic resection.
Purpose: Extravasation of diagnostic and therapeutic materials might occur when the intravascular solution leaks into the surrounding tissues. Injury associated with extravasation depends on various factors. It may range from mild skin reaction to severe necrosis. However, the incidence rate for extravasation is largely unknown because of the limited reporting in Korea. Therefore, this study was conducted to identify the incidence of extravasation and nurses' attitude and knowledge of extravasation for providing high-quality nursing care. Methods: Three acute care hospitals were surveyed to estimate the occurrence of extravasation. Knowledge and attitude toward extravasation were investigated from 793 nurses working in six hospitals. Results: The incidence rate of extravasation was 0.5%. Extravasation commonly occurred in elderly patients aged 66 or older (59.9%) and internal medicine (48.2%), and it happened 13.73±20.68 days after hospitalization on average. It mostly occurred in the forearm site (52.9%) and was mainly caused by parenteral nutrition (33.6%). The mean scores of nurses' knowledge and attitude were 14.63±2.86 and 28.91±36.00, respectively. There was a significant negative correlation between the subjects' knowledge and attitude (r=-.11, p=.002). Conclusion: It is necessary to have a reporting system that can accurately monitor the occurrence of extravasation for patient safety management. In addition, it is necessary to develop a protocol that can be applied to clinical practice and a nurse education program.
본 연구는 이륜차 법규위반 및 교통사고 예방을 위한 무인교통단속장비 및 표준규격 개발을 목적으로 한다. 이를 위해 현재 운영중인 무인교통단속장비의 이륜차 단속의 문제점 및 신기술 검토를 진행하였고, 설문조사를 통해 이륜차 무인교통단속장비 도입타당성 및 기술현황을 조사하였다. 또한 개발장비 현장실험을 통해 이륜차 단속기능을 구현하였고, 성능개선을 통해 단속대상 추가 및 번호인식률이 향상되었다. 현장실험 및 성능평가 결과를 바탕으로 이륜차 무인교통단속장비의 성능기준을 마련하였고 통신프로토콜에서는 차종분류코드 및 위반항목에 이륜차에 관한 사항을 신규로 구성하여 규격을 개발하였다.

Purpose: Despite recent developments in the management of trauma patients in South Korea, a standardized system and guideline for trauma treatment are absent. Methods: Five guidelines were assessed using the Appraisal of Guidelines for Research and Evaluation II instrument. Results: Restrictive volume replacement must be used for patients experiencing shock from trauma until hemostasis is achieved (1B). The target systolic pressure for fluid resuscitation should be 80-90 mmHg in hypovolemic shock patients (1C). For patients with head trauma, the target pressure for fluid resuscitation should be 100-110 mmHg (2C). Isotonic crystalloid fluid is recommended for initially treating traumatic hypovolemic shock patients (1A). Hypothermia should be prevented in patients with severe trauma, and if hypothermia occurs, the body temperature should be increased without delay (1B). Acidemia must be corrected with an appropriate means of treatment for hypovolemic trauma patients (1B). When a large amount of transfusion is required for trauma patients in hypovolemic shock, a massive transfusion protocol (MTP) should be used (1B). The decision to implement MTP should be made based on hemodynamic status and initial responses to fluid resuscitation, not only the patient's initial condition (1B). The ratio of plasma to red blood cell concentration should be at least 1:2 for trauma patients requiring massive transfusion (1B). When a trauma patient is in life-threatening hypovolemic shock, vasopressors can be administered in addition to fluids and blood products (1B). Early administration of tranexamic acid is recommended in trauma patients who are actively bleeding or at high risk of hemorrhage (1B). For hypovolemic patients with coagulopathy non-responsive to primary therapy, the use of fibrinogen concentrate, cryoprecipitate, or recombinant factor VIIa can be considered (2C). Conclusions: This research presents Korea's first clinical practice guideline for patients with traumatic shock. This guideline will be revised with updated research every 5 years.

We have always attempted to create a standard treatment protocol for patients with gastric cancer. However, many debates still exist regarding gastric cancer treatment. For the past 2 years, at the Annual Congress of the Korean Gastric Cancer Association, we have presented a grand symposium on the "Debates on the strategy for treating gastric cancer". In 2008, four major topics were discussed and voted on after discussion. The four major topics were proximal location treatment for early gastric cancer, management choices for pyloric obstruction with advanced gastric cancer, management of liver metastasis, and reconstruction methods after a distal gastrectomy. The opinions of the audience for six minor topics were expressed by an electronic voting system. In 2009, the four main topics were treatment for submucosal tumor sized around 2 cm, laparoscopic gastrectomy in T2N1 gastric cancer, choices for managing gastric lymphoma, and application of a pylorus preserving procedure for early gastric cancer at the antrum. The opinions of the audience for these six minor topics were expressed by an electronic voting system, as was conducted in 2008. It was good opportunity to identify a point of contact about the debates on managing gastric cancer. The results of these debates and studies will identify the best methods to treat patients with gastric cancer.

② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
