Three-dimensional evaluation of the pharyngeal airway space in patients with anterior open bite

  • Objective: This study aimed to three-dimensionally evaluate the pharyngeal airway space (PAS) of patients with anterior open bite (AOB) by using cone-beam computed tomography (CBCT) and compare the findings with those obtained in individuals with normal occlusion. Methods: The open bite group (OBG, n = 25) consisted of patients with an anterior overbite of -3 mm or less, while the control group (n = 25) consisted of age- and sex-matched individuals with an anterior overbite of 1-3 mm, Angle Class I malocclusion (1° ≤ point A-nasion-point B angle ≤ 4°), and a normodivergent profile (22° ≤ Frankfort mandibular plane angle ≤ 28°). After the CBCT data were reconstructed into a three-dimensional image, the PAS was segmented into four parts, and the volume of each part was measured. Pharyngeal airway length (PAL) and the area and transverse width of the part showing minimal constriction were also measured. Pearson's correlation analysis was used to evaluate the correlation between changes in the PAS and the amount of anterior overbite. Results: The OBG showed a significantly narrower airway space in the nasopharyngeal, hypopharyngeal, and total airway volumes. The OBG also showed a significantly smaller area and transverse width of the part with minimal constriction. The OBG showed a significantly longer PAL, but there was no correlation between the amount of anterior overbite and the changes in PAS. Conclusions: The PAS was associated with AOB. Patients with AOB had a narrower PAS and a smaller part showing minimal constriction.

Correlation between cone-beam computed tomographic findings and the apnea-hypopnea index in obstructive sleep apnea patients: A cross-sectional study

  • Purpose: The aim of this study was to explore the correlations of cone-beam computed tomographic findings with the apnea-hypopnea index in patients with obstructive sleep apnea. Materials and Methods: Forty patients with obstructive sleep apnea were selected from the ear-nose-throat (ENT) outpatient clinic, Faculty of Medicine, Mansoura University. Cone-beam computed tomography was performed for each patient at the end of both inspiration and expiration. Polysomnography was carried out, and the apnea-hypopnea index was obtained. Linear measurements, including cross-sectional area and the SNA and SNB angles, were obtained. Four oral and maxillofacial radiologists categorized pharyngeal and retropalatal airway morphology and calculated the airway length and volume. Continuous data were tested for normality using the Kolmogorov-Smirnov test and reported as the mean and standard deviation or as the median and range. Categorical data were presented as numbers and percentages, and the significance level was set at P<0.05. Results: The minimal value of the cross-sectional area, SNB angle, and airway morphology at the end of inspiration demonstrated a statistically significant association (P<0.05) with the apnea-hypopnea index, with excellent agreement. No statistically significant difference was found in the airway volume, other linear measurements, or retropalatal airway morphology. Conclusion: Cone-beam computed tomographic measurements in obstructive sleep apnea patients may be used as a supplement to a novel radiographic classification corresponding to the established clinical apnea-hypopnea index classification.

  • Oral cancer surgery typically consists of resection of lesion, neck dissection and reconstruction, and it has an impact on the position of hyoid bone. Therefore, morphological change of airway can occur since the geometric parameter of airway is correlated with the hyoid bone. Airflow is affected by geometry of the airway. In this study, flow characteristics were compared between pre- and post-surgery models by both particle image velocimetry (PIV) and numerical simulation. 3D model of upper airway was reconstructed based on CT data. Velocity is accelerated by the reduced channel area, and vortex and recirculation region are observed in pre- and post-surgery models. For the post-surgery model, high pressure distribution is developed by significantly decreased hydraulic diameter, and the longitudinal flow stream is also interrupted.

수면무호흡증 환자에서 Mandibular Repositioning Device가 Airway size와 Airway Collapsibility에 미치는 효과 (Effect of Mandibular Repositioning Device on Airway Size and Airway Collapsibility in Obstructive Sleep Apnea Syndrome : Cine CT during Sleep)

  • 목 적: 수면무호흡증 환자들에서 수면 중 구강내 기구의 착용이 수면 중 기도의 크기와 airway collapsibility에 미치는 영향을 알아보기 위하여. 방 법: 9명의 폐쇄성 수면무호흡(obstructive sleep apnea) 환자들에서 구강내 기구를 착용하기 전과 후에 수면다원검사와 axial airway CT를 시행하였다. 상기도의 다섯 곳(retropalatal-high, retropalatal-low, retroglossal, epiglottis, hypopharynx levels)에서 axial CT를 시행하였는데, 각 위치에서 수면무호흡 상태에서 하나의 axial CT image를 얻고, 정상적인 수면호흡 중에 1초 간격으로 10장의 axial CT images를 연속적으로 얻었다. 이와 같은 방법으로 구강내 기구를 착용하기 전과 후에 airway CT를 각각 2회 실시하였다. 각 CT 영상에서 기도의 횡단면적(cross-sectional area)을 측정하여서 구강내 기구의 착용 전과 후에서 기도 횡단면적의 최소 크기, 최대 크기 및 airway collapsibility의 측정치를 비교하여 유의한 변화가 있는지 알아보았다. 결 과: 수면무호흡 중에 가장 흔하게 좁아지는 부위는 retropalatal-low level이었다. 정상 수면호흡 중에 retropalatallow level의 최소 기도면적이 구강내 기구에 의하여 유의하게 넓어졌다(p=0.011). 평균 airway collapsibility는 retropalatal-low level에서 가장 높았다. 구강내 기구는 retropalatal-low level(p=0.021) 뿐만아니라 epiglottis level(p=0.038)에서도 유의하게 airway collapsibility를 감소시켰다. 결 론: 이상의 결과를 종합하여 보면 구강내 기구는 일부 환자에서 수면 중 기도의 최소면적을 넓히고, airway collapsibility를 낮추어서 수면무호흡을 예방할 수 있음을 시사한다.

Three-dimensional evaluation of the association between tongue position and upper airway morphology in adults: A cross-sectional study

  • Objective: This study aimed to evaluate the association between low tongue position (LTP) and the volume and dimensions of the nasopharyngeal, retropalatal, retroglossal, and hypopharyngeal segments of the upper airway. Methods: A total of 194 subjects, including 91 males and 103 females were divided into a resting tongue position (RTP) group and a LTP group according to their tongue position. Subjects in the LTP group were divided into four subgroups (Q1, Q2, Q3, and Q4) according to the intraoral space volume. The 3D slicer software was used to measure the volume and minimum and average cross-sectional areas of each group. Airway differences between the RTP and LTP groups were analyzed to explore the association between tongue position and the upper airway. Results: No significant differences were found in the airway dimensions between the RTP and LTP groups. For both retropalatal and retroglossal segments, the volume and average cross-sectional area were significantly greater in the patients with extremely low tongue position. Regression analysis showed that the retroglossal airway dimensions were positively correlated with the intraoral space volume and negatively correlated with A point-nasion-B point and palatal plane to mandibular plane. Males generally had larger retroglossal and hypopharyngeal airways than females. Conclusions: Tongue position did not significantly influence upper airway volume or dimensions, except in the extremely LTP subgroup.

Assessment of pharyngeal airway in Korean adolescents according to skeletal pattern, sex, and cervical vertebral maturation: A cross-sectional CBCT study

  • Objective: To investigate airway volumes using cone-beam computed tomography (CBCT) by skeletal patterns, sex, and cervical vertebral maturation (CVM) stages in Korean adolescents. Methods: The sample consisted of pretreatment CBCT and cephalograms of 95 adolescents (aged 12-19) obtained out of 1,611 patients examined for orthodontic treatment from 2018 to 2020 in Kyungpook National University Dental Hospital. The samples were classified into two sex groups; three skeletal pattern groups, four chronological age groups and four CVM stages. Nasopharyngeal volumes (NPV), oropharyngeal volumes (OPV), total pharyngeal airway volume (TAV) and minimum cross-sectional area (MCA) measurements were taken from the CBCT. Multiple linear regression analyses to find out which one of the independent variables are good predictors for airway variables. Significant factors were analyzed by two-way multivariate analysis of variance (MANOVA) then multiple comparisons were analyzed using a t-test, and Fisher least significant difference. Results: Age, sex, CVM, and Sella-Nasion-B point have significant influence on airway variable. Males and females showed similar patterns of change in chronological age groups 1-3; however, males had larger NPV, OPV, and MCA at CVM in group 4. According to CVM stages, males had larger OPV, TAV, and MCA at CVM stage 6 (p-value: 0.019, 0.021, 0.015, respectively) and no sex differences at CVM stages 3, 4, and 5. Conclusions: Skeletal patterns have an effect on airway volume. Sex differences were found in CVM 6.

만성 폐쇄성 폐질환에서 기관의 단면적과 폐기능지표와의 상관관계 (Correlation of Tracheal Cross-sectional Area with Parameters of Pulmonary Function in COPD)

  • 연구배경 : 만성 폐쇄성 폐질환을 대변하는 폐기종에서 주요 기능적 장애는 호기의 장애이며, 최대 호기류량은 기도의 크기, 폐의 탄력반동압 그리고 기도의 함몰성에 의해 결정된다. 기도의 항몰성은 폐기종에서 기류를 제한하는 하나의 기전으로 작용하지만, 폐용적과 폐의 과팽창을 유지하는 기전으로 작용하여 가스교환을 증진시킨다는 주장도 있다. 따라서 폐기종에서 기관이 호흡에 미치는 생리학적 역할이 무엇인지를 알아보고자 본 연구를 수행하였다. 방 법 : 1997년 1월 1일부터 8월 31일까지 보라매병원 호흡기내과를 방문하여 단순 흉부방사선검사, 체간용적기록계(body plethysmography)를 포함하는 폐기능검사 및 HRCT를 통해 폐기종으로 진단된 20명을 대상으로 하였다. HRCT에서 대동맥궁의 정상부위에서 기관의 단면적을 호흡주기에 따라 측정하고 이를 체표면적으로 보정한 값과 동맥혈의 이산화탄소분압 및 산소분압, 기도저항, 폐유순도 등 폐기능지표와의 상관 관계를 분석하였다. 결 과 : 폐기종에서 기관의 단면적은 호기시 동맥혈의 이산화탄소분압(r=-0.61, p<0.05) 및 산소분압(r=0.6, p<0.05) 그리고 매분환기량(r=0.73, p<0.05)과 유의한 상관관계가 있었지만, 흡기시에는 상관관계가 없었다(이산화탄소분압과는 r=-0.22, p>0.05, 산소분압과는 r=0.26, p>0.05, 매분환기량과는 r=0.44, p>0.05). 매분환기량은 상시호흡량(tidal volume)과는 r=0.45(p<0.05)로 유의한 상관관계가 있었지만, 호흡수와는 r=-0.31(p>0.05)로 상관관계가 없었다. 폐기종에서 기관의 단면적은 호흡주기와 상관없이 $FEV_1$ FVC, $FEV_1$/FEC, 최대 호기류량, 잔기용적, 폐확산능, 기도저항, 폐유순도 등의 다른 폐기능지표와는 상관관계가 없었다. 결 론 : 폐기종에서 호기시 기관의 단면적은 주로 가스 교환(gas exchange)의 지표들과 유의한 상관관계가 있었지만, 폐용적이나 폐의 과팽창을 시사하는 지표와는 상관관계가 없었다. 따라서 폐기종에서는 호기시 동일 압력점의 개념에 의해 발생하는 기도압박으로 기관의 단면적이 감소하고 이러한 현상이 기류를 제한하는 하나의 기전으로 작용하는 것이지 폐용적이나 폐의 과팽창을 유지시켜 가스교환을 증진시키는 것은 아니라고 생각된다.

New Method for Combined Quantitative Assessment of Air-Trapping and Emphysema on Chest Computed Tomography in Chronic Obstructive Pulmonary Disease: Comparison with Parametric Response Mapping

  • Objective: Emphysema and small-airway disease are the two major components of chronic obstructive pulmonary disease (COPD). We propose a novel method of quantitative computed tomography (CT) emphysema air-trapping composite (EAtC) mapping to assess each COPD component. We analyzed the potential use of this method for assessing lung function in patients with COPD. Materials and Methods: A total of 584 patients with COPD underwent inspiration and expiration CTs. Using pairwise analysis of inspiration and expiration CTs with non-rigid registration, EAtC mapping classified lung parenchyma into three areas: Normal, functional air trapping (fAT), and emphysema (Emph). We defined fAT as the area with a density change of less than 60 Hounsfield units (HU) between inspiration and expiration CTs among areas with a density less than -856 HU on inspiration CT. The volume fraction of each area was compared with clinical parameters and pulmonary function tests (PFTs). The results were compared with those of parametric response mapping (PRM) analysis. Results: The relative volumes of the EAtC classes differed according to the Global Initiative for Chronic Obstructive Lung Disease stages (p < 0.001). Each class showed moderate correlations with forced expiratory volume in 1 second (FEV1) and FEV1/forced vital capacity (FVC) (r = -0.659-0.674, p < 0.001). Both fAT and Emph were significant predictors of FEV1 and FEV1/FVC (R2 = 0.352 and 0.488, respectively; p < 0.001). fAT was a significant predictor of mean forced expiratory flow between 25% and 75% and residual volume/total vital capacity (R2 = 0.264 and 0.233, respectively; p < 0.001), while Emph and age were significant predictors of carbon monoxide diffusing capacity (R2 = 0.303; p < 0.001). fAT showed better correlations with PFTs than with small-airway disease on PRM. Conclusion: The proposed quantitative CT EAtC mapping provides comprehensive lung functional information on each disease component of COPD, which may serve as an imaging biomarker of lung function.

전산화단층촬영상에서 하악후퇴수술 후 인두기도 공간의 변화 (Changes of Pharyngeal Airway Space after Mandibular Setback Surgery in Computed Tomography Images)

  • Purpose: This study evaluated the pharyngeal airway space changes in CT images in patients receiving bilateral sagittal split osteotomy (BSSRO) for the surgical correction of mandibular prognathism. Methods: A total of 22 patients with mandibular prognathism were treated using BSSRO. Computed tomography was performed 1 month (T0) before surgery and, 1 month after surgery (T1). The anteroposterior length (AP), lateral width (LAT) and cross-sectional area (AREA) at the level of soft palate (C2) and base of the tongue (C3) were measured using CT images. Results: The mean amount of mandibular setback was 7.41 mm (${\pm}$3.46 mm). All the AP, LAT and AREA at the C2 and C3 level were decreased significantly 1 month after surgery (P<0.001). As the amount of mandibular setback was increased, the AP, LAT and AREA levels at the level of C2 and C3 had decreased. In addition, the reduction of the AREA at the C3 level was associated with the amount of mandibular setback (P<0.05). Conclusion: A significant decrease in pharyngeal airway space was observed 1 month after the operation. The cross-sectional area at the level of base of tongue was decreased with increasing amount of mandibular setback.

두경부 감염의 천층 및 심층부로의 확산 경로: 증례 보고 (THE SUPERFICIAL AND DEEP SPREADING PATHWAYS OF INFECTION OF THE MAXILLOFACEAL AND NECK AREA: REPORTS OF 2 CASES)

  • The spreading pathways which is the cause of infection on head and neck area are submandibular space, masticatory space, parapharyngeal space, retropharyngeal space, carotid sheath and mediastinum, etc. If spread to parapharyngeal area involving airway, such infection can be life-threatening by airway obstruction, or can cause vascular injury followed by hemorrhage, nerve injuries. Also, if spread to superficially, necrotizing fasciitis and many complications may occur including gangrene of skin. The key to successful treatment of infection on head and neck area is recognition of spreading pathways, early diagnosis and following therapeutic management. Our department present two cases, one is infection progressed superficially to suprasternal space, another is spreading deep according to parapharyngeal space, subclavian space and carotid sheath followed by airway obstruction, and obtained significant results with surgical incision and drainage, administration of selected antibiotics, continuous post-operative treatment. We report these 2 cases with literatures review.
