성대구증 환자의 공기역학적 검사 방법에 따른 치료 전과 후의 평균호기류율 비교 (Comparison of mean airflow rate before and after treatment in patients with sulcus vocalis according to aerodynamic analysis methods)

  • 성대구증은 불완전 성대폐쇄부전으로 평균호기류율(mean airflow rate, MFR)이 높은 것이 특징이다. MFR은 phonatory aerodynamic system(PAS)의 두 공기역학적 검사 방법(MXPH, VOEF)으로 측정하며, 두 검사 방법에 따라 결과가 다르게 나타날 수 있다. 본 연구는 성대구증 환자 30명을 대상으로 PAS의 MXPH, VOEF 프로토콜에 따른 MFR의 치료 전과 후 차이, 치료 전후 변화량 간의 차이가 있는지 비교하였다. 그리고 치료 전과 후의 Voice handicap index(VHI), GRBS 및 기본주파수(F0)를 비교하였다. 연구결과, 성대구증 환자의 치료 전과 후 모두에서 검사 방법 간에 유의한 차이가 나타났으나 치료 전후의 변화량은 유의한 차이가 없었다. VHI와 GRBS는 치료 전에 비해 치료 후에 유의하게 감소하였으나 F0는 치료 전과 후에 유의한 차이가 없었다. 본 연구를 통해 성대구증 환자의 치료에 따른 MFR 변화를 평가할 때, 두 공기역학적 검사 방법(MXPH, VOEF) 중 어떠한 검사 방법을 사용해도 된다는 것을 알 수 있다.

  • The purpose of this study is to compare and analyze the air infections in middle and small hospitals with the facilities of large national hospitals that have air-borne infection isolation (AII) wards through actual condition investigation and airflow analysis simulation (CFD) and to provide basic data for prevention. The method and scope of the study are as follows. First, through literature review, data related to prevention of infection spread in domestic medical institutions were investigated. Second, we conducted a survey on the status of isolation facilities to prevent the spread of infectious diseases in large hospitals and small and medium - sized clinics in Korea. Third, airflow analysis simulation (CFD) was carried out using the isolation ward of the nationally designated inpatient ward and the data of the plane and facility system of the small clinic. As a result of the study, it is found that regulations applicable to small and medium-sized clinics are insufficient. In addition, the simulation results show that the infectious disease virus is likely to spread to other patients in the hospital.

Evaluation of Filtration Efficiency and Inhalation Airflow Resistance of Uncertified Masks in Asian Countries

  • Background: During the coronavirus pandemic, masks played a critical role in preventing respiratory infections. While the performance of masks such as KF-certified masks and N95 masks was evaluated and managed by the authorities, the performance of common masks was not. Objectives: This study aimed to evaluate the performance of uncertified masks in four Asian countries against certification standards (Korean KF80, KF94, and US N95). Methods: Thirty uncertified mask products from Indonesia, 20 from South Korea, 26 from Taiwan, and 30 from Thailand were purchased to perform performance evaluations. The uncertified masks included disposable dental masks, cloth masks, and children's masks. Filtration efficiency and inhalation airflow resistance tests were conducted according to Korean KF80, KF94, and US N95 protocols. Results: None of the 106 identified masks complied with the KF94 standard. A few complied with the KF80 standard: four from Indonesia, four from South Korea, 13 from Taiwan, and 16 from Thailand. Some of the masks met the N95 standard: one from Indonesia, three from South Korea, two from Taiwan, and one from Thailand. Conclusions: Since many uncertified masks did not comply with performance standards, wearing them might not have provided sufficient protection. Performance of uncertified masks could provide critical information for next pandemic management.

전동차내 쾌적성 평가에 관한 기초연구 (A Basic Study on Evaluation of Comfortableness in Electric Rolling Stock)

  • Ventilation system on passenger rooms should be designed for the health and comfort of the passengers. One of the main aim is to create an acceptable thermal environment without draught problems. The draught sensation increases when the air temperature decreases and the air velocity increases. Airflow in passenger rooms is turbulent. Lateral temperature and humidity gradients in the electric rolling stock have been studied. And, the difference in the mean temperatures measured at 0.7, 0.9, 1.2, 1.7 m above the floor. It has been found that temperature with large fluctuations caused more draught complains.

대형 강의실의 실내 열환경 실측 및 컴퓨터시뮬레이션 비교 연구 (Field Measurements and CFD Simulations of Indoor Thermal Environments in the Assembly Hall)

  • The evaluation of the indoor environment of the Assembly Hall in the University, which is designed to be a large space, requires efficient design of its heating system that takes into consideration natural convection and the characteristics of the occupant's spaces. Indoor thermal environment was measured in the field and simulated with CFD code. The estimations of temperature distribution and indoor airflow distribution must be carried out simultaneously, as the thermal stratification is induced by natural convection flows. In order to simulate the even distribution of factors affecting the indoor environment, including temperature and airflow, Phoenics is used. The turbulent flow model adopted is the RNG k- model. The inlets and outlets of the air-conditioning systems, material and thermal properties, and the size of the test room ($35m{\times}18m{\times}10m$) are used for the simulation. Since the Assembly Hall is symmetric, half of the space is simulated. A Cartesian grid is used for calculation and the number of grids are respectively $60{\times}45{\times}35$. The results of the computer simulation during winter conditions are compared with the measurements at the typical points in the assembly hall with the heating system. After evaluating the results of the computer simulations, the methods of the heating system and layout are suggested.

구개인두성형술 후 공기역학적 구개기능 평가 (The Aerodynamic Evaluation of Velopharyngeal Function after Uvulopalatopharyngoplasty)

  • Uvulopalatopharyngoplasty (UPPP) is one of the popular surgical procedure for snoring and sleep apnea syndrome. The main principle of this procedure is to reduce abundant velopharyngeal soft tissues resulting in a shortened soft palate, which may cause some alterations in speech sound. The purpose of this study is to evaluate the change of velopharyngeal function after UPPP in the view of aerodynamics. Thirty three patients who received uvulopalatopharyngoplasty for correcting snoring and sleep apnea were included in this study. The airflow, airflow rate and air pressure during the production of oral and nasal consonants were measured before surgery and 4 week and 8 week after surgery. The oral air flows and pressures for oral and nasal consonants were not changed after surgery. However, oral air pressure for nasal consonants were increased significantly after surgery. The nasal air flows for oral consonants were not changed after surgery, but for nasal consonants were decreased at 8 weeks after surgery. The nasal flow rate for oral and nasal consonants were increased at 8 weeks after surgery. The uvulopalatopharyngoplasty may result in affecting the aerodynamic air streams during speech production.

  • 본 연구는 지하철방재대책의 일환으로 터널구간에 설치된 환기기의 제연절환 운전모드 및 승강장 선로부 환기장치를 포함한 6종류의 제연운전모드를 대상으로 승강장에 정차된 열차화재 시나리오에 따라 3차원 실시간 화재 시뮬레이션을 수행하여 제연방식별 열 및 연기전파특성을 규명함으로서, 승강장에서 열차 화재 발생시, 승객이 안전하게 최적 대피가 가능한 환기기의 제연절환 조합운전의 도출을 목표로 한다.

Evaluation of the Impacts of Stack Effect in High-Rise Buildings

  • The objective of this study is to analyze and evaluate the impact of the stack effect in high-rise buildings for solving the various problems resulting from it. For the evaluation of the impacts on the stack effect, computer program simulations based on the network model were performed for a typical high-rise office building. The results of the simulations show that the impact caused by the stack effect is mainly dependent on building envelope air-tightness and internal air flow resistance. Therefore the problems due to the stack effect may be solved to some extent by installing vestibules around entrance doors and doors serving elevators, and by zoning the elevators.

The Proposal of a Quantitative Evaluation Method on Mixing Loss in the HVAC System Design

  • It is a serous subject for energy conservation to prevent the energy loss caused by the mixture of heated and cooled air jets in perimeter and interior zone of a building operated with tow kinds of air-conditioning system simultaneously. The purpose of this paper is to clarify the quantitative and qualitative mechanisms of mixing loss and to propose a evaluation method for it. By using the dynamic heat load calculation, heat extraction load of a typical office building in Busan are calculated. According to the results, numerical simulations based on CFD(Computational Fluid Dynamics) were performed in order to evaluate mixing loss in the physical size of HVAC system. Then, the distributions of air temperature and airflow patterns according to the differences of set-point temperature are analyzed to grasp relations how to influence mixing loss.

Evaluation and interpretation of transcriptome data underlying heterogeneous chronic obstructive pulmonary disease

  • Chronic obstructive pulmonary disease (COPD) is a type of progressive lung disease, featured by airflow obstruction. Recently, a comprehensive analysis of the transcriptome in lung tissue of COPD patients was performed, but the heterogeneity of the sample was not seriously considered in characterizing the mechanistic dysregulation of COPD. Here, we established a new transcriptome analysis pipeline using a deconvolution process to reduce the heterogeneity and clearly identified that these transcriptome data originated from the mild or moderate stage of COPD patients. Differentially expressed or co-expressed genes in the protein interaction subnetworks were linked with mitochondrial dysfunction and the immune response, as expected. Computational protein localization prediction revealed that 19 proteins showing changes in subcellular localization were mostly related to mitochondria, suggesting that mislocalization of mitochondria-targeting proteins plays an important role in COPD pathology. Our extensive evaluation of COPD transcriptome data could provide guidelines for analyzing heterogeneous gene expression profiles and classifying potential candidate genes that are responsible for the pathogenesis of COPD.
