Normothermic Cardiac Surgery with Warm Blood Cardioplegia in Patient with Cold Agglutinins

  • Cold agglutinins are predominately immunoglobulin M autoantibodies that react at cold temperatures with surface antigens on the red blood cell. This can lead to hemagglutination at low temperatures, followed by complement fixation and subsequent hemolysis on rewarming. Development of hemagglutination or hemolysis in patients with cold agglutinins is a risk of cardiac surgery under hypothermia. In addition, there is the potential for intracoronary hemagglutination with inadequate distribution of cardioplegic solutions, thrombosis, embolism, ischemia, or infarction. We report a patient with incidentally detected cold agglutinin who underwent normothermic cardiac surgery with warm blood cardioplegia.

  • Cold agglutinins are a potential danger to patients who must be subjected to hypothermia. A patient with a cold agglutinin of moderate titer but broad thermal amplitude was to undergo hypothermia during double valve replacement. She was managed preoperatively with plasmapheresis 5 times. There was no complication during and after operation.

  • In boar spermatozoa, the head-to-head agglutinability changes in parallel with the development of the fertilizing ability. Namely, both abilities gradually increase in the distal caput and corpus epididymides, but are subsequently suppressed in the cauda epididymidis. It has been postulated that these changes of the agglutinability are controlled via sperm interaction with specific epididymal plasma factors including agglutination mediators (agglutinins) and inhibitors (anti-agglutinins). Expression of these abilities (sperm agglutination and capacitation) is hardly observed in spermatozoa immediately. after ejaculation, but it occurs during incubation in a capacitation medium. Recently, we have purified and characterized epididymal plasma anti-agglutinin for boar spermatozoa. Moreover, we have conducted a series of experiments to reveal biological significance and mechanism of the head-to-head agglutination and have accumulated data indicating that boar sperm agglutination is mediated by capacitation-supporting factors including calcium, bicarbonate and sterol acceptors. This review introduces our recent data and discusses a possible mechanism for suppression of the agglutinability in the distal epididymidis and relationship between agglutinability and fertilizing ability.

Preparation of equine blood typing reagents by isoimmunization (동종면역(同種免疫)에 의한 말항혈청(抗血淸)제조에 관한 연구)

  • This study was purposed to produce blood typing reagents for classifing Cheju horse's blood group factors. The blood typing reagents were prepared by immunization of sixteen pairs out of fourty eight heads of Cheju horses, and seventeen different blood typing reagents (anti-$A_1$, A', H, Z, $ZZ_2$, C, J, K, $P_1$, Q, R, S, $T_1$, $N_1$, $U_2$, X and $E_2$) were prepared from equine isoimmunization. The result for the reagent production and the variation of agglutinin and hemolysin titer are as follows: 1. Anti-K, R, S, $N_1$, $U_2$ and X acted exclusively as hemolysins whereas anti-J, $P_1$, $T_1$ and $E_2$ acted exclusively as agglutinins, however, Anti-$A_1$, A', H, Z, $ZZ_2$, C and Q reacted both as hemolysins and agglutinins. 2. Among the antibodies in most of the reagents reacted both as hemolysins and agglutinins, But anti-$A_1$, H, Z, $ZZ_2$ and Q elevated higher agglutin titers than hemolysin titers, Anti-A' and anti-C elevated higher hemolysin titers than agglutinin titers. 3. Complex antibody $A_1U_2$ was adsorbed $U_2$ and unified to $A_1$, while complex antibody X $N_1$ went through an adsorption process and produced simple antibody X and $N_1$, respectively. 4. In the hemolytic reaction, anti-K and anti-R showed the highest titer, In the agglutination reaction, anti-Z and anti-$ZZ_2$ showed the highest titer. 5. In the case of No.138~No.140 horse immunization combination and No.133~No.146 horse immunization combination, Although production of complex antibody $CE_2$ and $JP_1$ were expected, only anti-$E_2$ and anti-J were produced respectively. 6. The degree of elevation of antibody titer was varied by the blood types and by the types of donor and recipient combination. The fast elevating types, slow elevating types and types that never elevate the antibody titer were identified.

  • Mycoplasma pneumoniae is a common cause of community-acquired pneumonia in children, with a peak incidence at 5-14 years. Extrapulmonary manifestations occur in 20-25% of patients with M. pneumoniae infection. Most auto-antibodies that cause immune hemolytic anemia in humans are cold agglutinins. The formation of cold agglutinins is frequently observed during M. pneumoniae infections, and cold agglutinin disease usually occurs during M. pneumoniae infections. Nevertheless, severe hemolysis is exceptional. If a patient has any underlying disease related to hemolysis, it is possible to accelerate hemolysis. Hereditary spherocytosis is a common cause of hereditary hemolytic anemia resulting from red blood cell membrane defects. Hemolysis of red cells may result from corpuscular abnormalities or extracorpuscular abnormalities, such as immune or non-immune mechanisms. We report a case of hereditary spherocytosis associated with severe hemolytic anemia due to Mycoplasma pneumonia.

  • Forty species of marine shells were collected from Korean coasts and studied extensively for their lectin activities by using erythrocytes of human blooc A, AB, B. O group and rabbit blood. In total, 7 species contained lectines :Neptunea intersculpta, Omphalius nigerrimus and Scapharca subcrenata, blood group nonspecific; Saxidomus purpuratus, human blood A and AB group specific; Lepidozona coreana, Tegilarca granosa and Neptunea polycosta, rabbit blood specific.

Sexual Reproduction in Unicellular Green Alga Chlamydomonas (수염녹두말속(Chlamydomonas) 단세포 녹조의 유성생식)

  • The sexual reproduction of the unicellular green alga Chlamydomonas is reviewed for a comprehensive understanding of the complex processes. The sexual life cycle of C. reinhardtii is distinguished into five main stages: gametogenesis, gamete activation, cell fusion, zygote maturation, and meiosis and germination. Gametogenesis is induced by nitrogen starvation in the environment. C. reinhardtii has two mating types: mating type plus ($mt^+$) and mating type minus ($mt^-$), controlled by a single complex mating type locus ($MT^+$ or $MT^-$) on linkage group VI. In the early gametogenesis agglutinins are synthesized. The $mt^+$ and $mt^-$ agglutinins are encoded by the autosomal genes SAG1 (Sexual AGglutination1) and SAD1 (Sexual ADhesion1), respectively. The agglutinins are responsible for the flagellar adhesion of the two mating type of gametes. The flagellar adhesion initiates a cAMP mediated signal transduction pathways and activates the flagellar tips. In response to the cAMP signal, mating structures between two flagella are activated. The $mt^+$ and $mt^-$ gamete-specific fusion proteins, Fus1 and Hap2/Gcs1, are present on the plasma membrane of the two mating structures. Contact of the two mating structures leads to develop a fertilization tubule forming a cytoplasmic bridge between the two gametes. Upon fusion of nuclei and chloroplasts of $mt^+$ and $mt^-$ cells, the zygotes become zygospores. It is notable that the young zygote shows uniparental inheritance of chloroplast DNA from the $mt^+$ parent and mitochondrial DNA from the $mt^-$ parent. Under the favorable conditions, the zygospores divide meiotically and germinate and then new haploid progenies, vegetative cells, are released.

Hemaggulutinin and Hemolysin in Korean Ascidians

  • Two kinds of humoral factors were observed in 2 orders, 7 families, 10 genera, and 15 different species of Korean ascidians. They are the naturally occuring hemagglutinins and/or hemolysins against human erythrocytes A, B, and 0. All but two species showed aggregative activity, although there were considerable variations in titer. The weak agglutinating and lytic activities were increased in the presence of $Ca^{++}$. Much higher activities of agglutination and/or lysis were shown in the hemolymph than extracts from tissues, and a higher response was shown in adults than in juveniles. No distinct differences from collected locations were observed. The hemolymph of Ciona intestinalis showed a strong hemolytic (cytotoxic) and weak agglutinins capacities. In addition, hemolymph of Styela plicata and Styela clava clava also showed hemoagglutining and hemolytic activities. Botryllus tuberatus had hemagglutining and weak lytic activities. Other species showed only hemagglutining activity. These agglutining activities are probably responsible for carbohydrate recognition in solitary or colonial ascidian. The lytic activity is probably responsible for antibacterial defense and nonfusion reactions between allogeneic colonial ascidians, especially the genus of Botryllus. The occurrences of humoral factors in ascidians were independent of their geographic distributions.

  • Open heart surgery with hypothermia in patients with cold agglutinin can cause severe complications by hemolysis and hemagglutination of red blood cells. A 41 year-old male patient with mitral stenosis was admitted due to fever and cough. After antibiotics treatment, he was scheduled to undergo mitral valve replacement. In the operation room, we found agglutination of blood cardioplegia during lowering temperature of cardioplegia. And then, the cardioplegia was changed to warm cardioplegia and the operation was performed under normothermia due to the suspicion of the cold reactive protein. The operation was performed uneventfully. Postoperatively, cold agglutinin was confirmed by immunochemistry of the patient\`s serum.

A Case of Henoch-Schönlein Purpura Associated with Mycoplasma Pneumoniae Pneumonia (Mycoplasma pneumoniae 폐렴에 동반된 Henoch-Schönlein purpura 1례)

  • We experienced a case of Henoch-Sch$\ddot{o}$nlein purpura associated with Mycoplasma pneumoniae pneumonia in a 28 month old male who suffered from cough, abdominal pain and both leg swelling and pain. Physical examination showed varying sized purpura, characteristic of Henoch-Sch$\ddot{o}$nlein purpura, below both knee. Laboratory test revealed Mycoplasma pneumoniae antibody titer >1:2,560 and cold agglutinins titer 1:64. Chest X-ray showed peribronchial blurring in both lung fields. The patient was treated with midecamycin and prednisolone for 7 days and responded to the treatment well. The authors report a case of Henoch-Sch$\ddot{o}$nlein purpura with Mycoplasma pneumoniae pneumonia with brief review of related literatures.
