  • Taiwan has experienced rapid economic growth during the past two decades. As a result, the demand for health care in Taiwan has increased rapidly. To meet the rising demand, Taiwan implemented a National Health Insurance (NHI) program on March 1, 1995. This program now covers more than 96 percent of Taiwan's citizens. Implementation of the NHI in 1995 represents fulfillment of a primary social and health policy goals of Taiwan. The goals of the NHI program is to eliminate financial barriers of health care for the citizens, to improve the quality of care. To achieve these goals, the NHI was designed on the following principles: 1. All Taiwan citizens are compul내교 joined the NHI program by law; 2. The NHI program provides comprehensive services; 3. The NHI is run by one single govt' subsidy; 5. The NHI adopt fee-for-services scheme to pay medical expenses and copayment to avoid abouse of medical services. However, the scheme did not bring in the efficient use of health care C. National Health Council, 1986 NARC, Aging in Japan, International Publication Series 1991;2 Kahana EF. Kiyak HA. • This study was carried out by using questionnaires with 126 insurance societies from Sept. 30, 1995 to Oct. 18, 1995. The primary data collected bythe survey have been significantly supplemented by secondary data obtained from sources such as health insurance statistical year books and internal data in the Ministry of Health and Wolfare. Major findings were summarized as follows: Two financial coordinating programs have significantly improved financial status of regional health insurance societies: the catastrophic program for high cost medical care that was initiated in 1991 and the program for hospitalization cost of the aged in 1995. Another finding is that there existed ambiguity and inconsistency of equity index that had been used by stabilization programs and its side effects could not be ignored. Regression analyses were made to identify factors that affect financial transfers. Inde pendent variables in the regression include utilization frequency, dependancy ration, insurance contribution per insured and medical expense per insured. All these variables were statistically significant in the equations of applying distribution rate (distribution/contribution) and transfer rate (transfer/contribution) as dependent variables. Policy suggestions for the catastrophic program for high cost medical care are modifying the definition of catastrophic case and setting the maximum amount of subsidies for each society based on distribution rates. To solve the problems of the financial coordinating program for the aged, we could consider reimbursing more than 50% of the copayment incurred by the aged 65 or more and determining the maximum amount of outpatient copayment at 10,000 Won per day or per visit for the elderly. More fundamental improvement could be made by amending the Welfare Benefit Act to establish and expand medical and welfare facilities for the elderly.

  • Purpose: The purposes of this study were to assess the oral health knowledge of nursing staff in long-term care institutions and to identify predictors of oral health knowledge. Method: For this descriptive correlation study, a self-administered and structured questionnaire was used. Respondents were 111 nursing staff in two long-term care facilities and two long-term care hospitals located in G metropolitan area and C province in the Southern part of Korea. Descriptive statistics, t-test or ANOVA, and stepwise multiple regression analysis were used to analyze the data. Results: Participants in this study did not have many opportunities to learn about oral health care for elders in long-term care institutions. The percentage of correct answer for oral health knowledge questionnaire was 64.5%. Predictors of oral health knowledge among nursing staffs were education on oral health in long-term care institutions, type of institution, and length of time working with elders. These three variables accounted for 24.2% of variance in oral health knowledge. Conclusions: Nursing staffs should make an effort to improve their knowledge of oral health. Moreover, oral health educational program for nursing staffs working with elders in long-term care institution is need to be developed and the effectiveness of this education needs to be evaluated.

The survey included 592 preschool children aged 1-5 years old in 31 childcare facilities. General characteristics, feeding type, complementary feeding, food allergy, dietary behaviors based on the NQ-P questionnaire, and dietary habits of their family were considered. Exclusively breast-fed children was 32.2% of the subjects and the breast feeding duration was 6.5 months. Subjects who had complementary feeding within 6 months was 77.2% and starting time of complementary feeding was 6.1 months. Fifteen percent of the subjects had food allergies and foods that induced allergy were instant foods, eggs, milk and dairy products, nuts, seafood. Food allergy was not related to breast feeding nor complementary feeding. The NQ-P score and its 3 factors including 'balance', 'moderation', and 'environment' were 59.9, 61.1, 56.0, 62.6, respectively. There were positive relations between children's dietary behaviors and family dietary habits such as breakfast eating frequency and meal regularity. As age of children increased, instant food intake increased and breakfast eating frequency decreased. Proper nutrition education is needed to children, their parents, and their care givers at childcare facilities to improve children's dietary behavior and health.

  • This study analyzed the current status of care policies and senior citizen issues in Korea, which has entered a super-aging society, and attempted to suggest improvement measures for desirable convergence policies. The problems of the elderly in Korea have emerged as health problems, poverty problems, loss of roles, and care problems due to changes in the concept of family, values, and socioeconomic structure. In addition, poor seniors need jobs and employment policies, and healthy middle-class seniors need policy development to enable leisure activities and volunteer activities. In particular, it is necessary to establish policies to enable people to spend the rest of their lives in elderly care facilities and elderly care hospitals through community care policies. The super-aging society accounts for more than 20% of the total population among those aged 65 or older, and according to the National Statistical Office, it is expected to become a super-aging society by 2025. Elderly welfare policies must provide various programs and the needs and services of elderly care, and in the future, a super-aging society will need to secure a lot of financial resources and maintain a productive population to cover the financial resources. In other words, the human life cycle is from birth to death, and when the ratio of natural deaths of the elderly and infants born is appropriate, the society and the country can be seen as stable, vibrant, and healthy societies.

  • The purpose of this study is to elaborate the factors and strategies to revitalize the policies of elderly-friendly care industry. The analysis was carried out using the method called DEMATEL. The analytical method is based on the DEMATEL technique. The purpose of this study is to clarify the complexity and unclear relationship between activation factors and factors that constitute the elderly friendly-care industry through the prioritization of policy strengthening. As the result of analysis, it was analyzed that the most intensive activation intensity was the highest total strength of meal service(22.095). Followed by health promotion facilities(19.97) and health care(17.726). The lowest factor is sport(15.896). Therefore, in order to improve and promote these factors, continuous policy measures should be sought that are closely related to facility care services, home care services, and preventive support services.

  • Objectives : To evaluate the annual visit days, the annual prescription days and the medical costs of hypertensive patients. Methods : The medical insurance records of 40,267 incident patients with the diagnostic code of hypertension from September 1998 through August 1999 in Daegu city were reviewed. Results : The proportion of the most proper medical care pattern group (Group VIII) who visited for 6-15 days with 240 prescription days or more a year was only 6.2%. The proper care group (Group IX) who visited for more than 16 days with 240 prescription days or more a year was 9.3%. The overall proper care group (Group VIII+IX) was therefore 15.5%. The proportion of the insufficient care group (Group I, IV) in both the number of visiting days and prescription days was 57.4%. The mean prescription day of the most proper group (Group VIII) was 29 days; the mean annual medical expenses,453,587won; the mean annual amount paid by patients, 218,013won; and mean medical expenses per prescription day, 1,483won. The proportion of the overall proper care group (Group VIII+IX) was significantly higher in adults aged 50-59, those who were enrolled in industrial workers health insurance as well as government employees and private school teachers health insurance, and those who made a higher contribution per month (p<0.01). According to the type of medical facilities, the proportion of the most proper medical care pattern group was highest in the general hospitals (9.3%) but the overall proper care group was higher in the public health centers (22.1%) and private clinics (17.1%). Conclusions : The management system of hypertension should be reinforced urgently. Therefore, it is necessary to develop guidelines including the number of visiting days per year and prescription days per visit day, and make the system provide medical facilities to more properly care for hypertensive patients.
