  • Objectives : Routine vaccination against influenza is recommended for all people aged 65 years or more. Without active encouragement to receive the vaccine, the rate of compliance is generally low. A study was undertaken to assess and compare the effectiveness of two reminder systems in improving the influenza vaccination rate. Methods : A total of 2,017 patients aged 65 or over in a tertiary care hospital family practice center were randomly assigned to a control, a telephone reminder, or a postcard reminder group before the influenza season in 1998. The outcome measure, receipt of vaccination, was determined by telephone interview. Demographic and clinical characteristics were checked through the medical chart review. Results : Among the 1,312 patients who actually received reminders and interviewed, the vaccination rate was 46.7% in the control group, 56.3% in the postcard reminder group, and 63.3% in the telephone reminder group. Vaccination compliance was significantly higher in people reminded by telephone (Odds ratio [OR], 2.00; 95% confidence interval [CI], 1.52-2.64) and postcard (OR, 1.55; 95% CI, 1.18-2.02) compared to that in the control group. Of the characteristics investigated, number of high risk co-morbidity was positively associated with vaccination compliance while current smoking was negatively associated. Conclusions : This result suggests that telephone and postcard reminders can significantly improve compliance with influenza vaccination in this group of Korean elderly. However, additional strategies need to be developed to encourage vaccination among noncompliant.

  • Purpose: The purpose of this study was to investigate homebound status and significant related factors for community-dwelling female elders according to age. Methods: The participants were female elders over 65 years of age registered in public health centers. Data were collected by interviewing the elders, who voluntarily completed the entire survey. For data analysis, descriptive statistics and multiple logistic regression were performed using SPSS version 18.0. Results: There were statistically significant differences in homeboundness and related factors between the young-old (65 to 74 years of age) and the old-old (75 years of age or older). The level of homeboundness of the old-old was higher than that of the young-old. Multiple logistic regression showed, timed "up and go", depression, and fear of falling as significantly associated with homebound status of the young-old, while hand grip strength (right), timed "up and go", static balance ability, severity of urinary incontinence, and fear of falling as significant for the old-old. Conclusion: The findings of this study indicate that homebound status and related factors for elders are different according to age, and therefore, interventions to prevent and help homebound elders get over being homebound should be developed according to age.

  • This research study was to examine the relationships of social supports and depression among long-term care center residents. Depression of long-term care center residents is the most common mental health problem confronting older adults. It is estimated that 13 percent of people aged 65 years and over have a major depressive disorder in Korean Society of Welfare for the Aged. Social support is a key ingredient in dealing with emotional distress and a critical in helping people to cope with all kinds of extreme circumstance. As the result of this research, Perceived social support was found to be related to depression. However, network size was not a predictor of depression, and high health status is accounted for a lower portion of the variance in depression., the poor health status was one of the most powerful predictors of depression, From results of this research study, for social workers who are employed institutional-based agencies.

  • This study aims to explore the lived experiences of middle-aged people with disabilities after a stroke. Using van Manen's phenomenological qualitative method, this study obtained information through interviews with middle-aged people with disabilities after a stroke. The following main themes were extracted from the lived experiences of middle-aged people with disabilities who were interviewed: 'a life broken in half', 'discontinuity and isolation of life', 'being born again', 'fear of stroke relapse', and 'overcoming all difficulties through harmony in a family'. Therefore, the core theme on the lived experiences of middle-aged people with disabilities after a stroke is a shadow of relapse over lives that have to protect families. The implication of this study's results on policy making and actual practices are discussed focusing on the well-being of people with disabilities and their families.

  • This study investigated the differential interactive effects of the combination of driving confidence and sensation-seeking on driving anger among different age groups, by using correlation analysis, a hierarchical regression and ANOVAs for the data gathered through the questionnaires administrated in a form of face-to-face interview. The results showed the followings. First, males tended to show higher level in driving anger than females. Second, whereas sensation-seeking was positively correlated with driving anger, age and driving experience were negatively correlated with driving anger, respectively. Third, although there was no significant relationship between driving confidence and driving anger among the drivers aged under 40 years, the drivers aged over 40 years showed higher level of driving anger as their driving confidence increased. Forth, level of sensation-seeking was found to be a strong predictor of driving anger in all age groups. Fifth, driving confidence and sensation-seeking appeared to affect the level of driving anger interactively among drivers aged under 40 years or over 65 years, resulting in higher driving anger only when both the levels of driving confidence and sensation-seeking were high. In contrast, driving confidence and sensation-seeking affected driving anger independently among the drivers aged 30-49 years. Implication and suggestion were discussed.

  • Objectives : This study investigated the actual condition of an oriental medical office in a public health center. Methods : We classified patients who visited an oriental medical office in the public health center of Gyeyang-gu over a 1-year period, according to age, sex, disease, etc. The number of patients was 6529. Results : Analyzing these 6529 persons, 65.8% were female, and 79.6% were seniors aged over 65. By disease category motor systemic disease or circulatory disease affected 84% of the patients. By medication or treatment category. O-Juk-San (五積散) at 38.3% was the greatest proportion, and Pal-Mul-Tang (八物湯) the second greatest. As to medical fee, 80.2% of the patients were not charged for treatment. Conclusions : The extract of herbal medication should be more expanded in the medical insurance, and should be improved in quality. The oriental medical office in a public health center should focus more on health promotion and disease prevention than patient care. For accomplishing this goal, the institutional position and rights of oriental medical doctors as civil officials should be equal to those of western medical doctors.

  • The purpose of this study was to promote an understanding of suicidal ideation and identify the factors that influence suicide ideation according to the life cycle of Korean adults. This study was a secondary analysis study using the Korea Health Panel 2016 data. Among adults over 19 years of age, 14,538 people with no missing values in suicidal ideation and influencing factors were classified into young adults (19-39 years old), middle-aged adults (40-64 years old), and the elderly (65 years or over). The data were analyzed by multiple logistic regression. The results showed that 2.9% (108 people) of young adults, 3.2% (181 people) of middle-aged adults, and 3.7% (80 people) of the elderly had suicide ideation over the past year. Factors influencing suicidal ideation by life cycle were anxiety, depression, drug use and quality of life for young adults, subjective health status, stress, anxiety, depression, drug use and quality of life for middle-aged adults. The factors affecting the elderly were body mass index, stress, anxiety, depression, and quality of life. Therefore, factors influencing suicidal ideation should be considered as a major factor for screening risk groups according to the life cycle, and differentiated intervention programs should be developed and provided to prevent and manage suicide in risk groups.

  • Korea is the most rapidly aging country in the OECD countries, becoming 'an aging society' in 2010, when over 12% of the population had reached the age 65 and beyond. Along with the population aging, problems of the elderly have become a social issue. Also, the elderly population in most farming villages is increasing due to the cityward concentration of younger generation and the rapid urbanization centered around the capital area. Yet no definite success in revitalization of these villages has been reported despite the acknowledgement of the problems arising from such phenomena. Therefore, by examining "marginal villages" (communities whose joint social activities are difficult to maintain as over 50% of the population is 65 years or older) in Japan, which at this point has entered "the ultra-aged society" before Korea, this study intends to compare and analyze the countermeasure plans for the population decrease in these villages. The case study has been done with the focus on the literary references published in Japan such as papers, various reports, statistics, newspapers, and journals.

  • Background: Intra-articular distal humeral fractures can be surgically challenging. It remains under discussion whether open reduction and internal fixation (ORIF) or total elbow arthroplasty (TEA) is more beneficial for treatment of the elderly. This study aimed to compare the clinical and functional outcomes of ORIF and TEA for managing intra-articular distal humerus fractures in patients aged 65 years or older. Methods: Patients who underwent ORIF (n=28) or TEA (n=43) for in intra-articular distal humerus fracture between May 2008 and December 2018 were reviewed. Range of motion, Mayo Elbow Performance Score (MEPS), Disabilities of the Arm, Shoulder, and Hand (DASH) score, radiologic outcomes, and surgical complications were evaluated at the final follow-up visit. Results: The ORIF and TEA groups showed a mean arc of flexion-extension of 97°±21° and 101°±12°, respectively. The mean MEPS and DASH scores were 94±15 and 27±12 points, respectively, in the ORIF group and 81±27 and 47±28 points in the TEA group. This difference was statistically significant. The incidence of total complications was similar between the groups. Conclusions: In patients older than 65 years with intra-articular distal humerus fracture, ORIF had better outcomes than TEA.
