Implant-supported fixed prosthesis in patient with severe defects using staged GBR via 2-step augmentations: A case report (심한 결손부의 단계적 골증대술을 통한 임플란트 지지 고정성 보철 수복 증례 보고)

  • The treatment of patients with severe periodontitis should be proceeded step-bystep through an accurate diagnosis of each patients' individual tooth and with a strategic treatment plan. Implant-supported fixed prosthetic restoration has the advantage of high patient satisfaction and stable vertical dimension compared to the removable partial denture. However, multiple teeth defect areas lacking hard tissue may be disadvantageous in aesthetic failure and longer treatment time. In addition, it takes a certain period of time to manufacture and install a conventional fixed prosthesis, and during this process, the provisional prosthesis must satisfy the mechanical, biological, and aesthetic requirements of teeth. The purpose of this article is to describe the fabrication of implant-supported fixed prosthesis through a step-by-step approach in a partially edentulous patient.

Correction of Lower Eyelid Retraction with Autogenous Hard Palate Mucosa: 2 Case Report (자가 경구개점막 이식을 이용한 하안검 퇴축의 교정: 2례 보고)

  • Purpose: Grave's disease is an autoimmune disease with chronic and systemic features. It affects the orbital fat and muscle bringing about defect in extrinsic eye motility, diplopia, optic nerve defect and lid retraction. In patients with lagopthalmos and resulting facial deformity, treatment can be done by rectus muscle recession or filling with various material. Autogenous auricular cartilage graft is often used and synthetic material such as synthetic acellular dermis, polyethylene meshs are also used for filling of the depressed area. Nevertheless, autogenous auricular cartilage grafts are difficult to utilize and synthetic materials sometimes result in protrusion or infection. Therefore, hard palate mucosa was considered as an alternative. We report two cases of patients with lower eyelid retraction corrected with autogenous hard palate mucosa. Methods: We performed this operation in two patients of Graves' ophthalmopathy. The capsulopalpebral fascia was incised and elevated through an incision on the conjunctiva. Then, the harvested hard palate mucosa was sutured to the inferior border of the tarsus and covered with the conjunctiva. Results: The lower eyelid retraction was corrected successfully. No hypertrophy or deformation of the transplanted hard palate mucosa was noted 6 months after the surgery. Conclusions: From the results above, we may conclude that the hard palate mucosa serves as an ideal spacer for the curvature and the inner lining in lower lid retraction. Hard palate mucosa is as sturdy as the autogenous cartilage but is much easier to utilize. It can be also used for lid retraction after lower lid aesthetic surgeries or traumas.

Selection of Implants in Unilateral Prosthetic Breast Reconstruction and Contralateral Augmentation

  • Background In breast reconstruction using implants after unilateral mastectomy, it is challenging to create a natural, ptotic contour, and asymmetry is a potential drawback. To achieve breast symmetry and an ideal shape for both breasts, we performed contralateral augmentation in patients undergoing breast reconstruction with implants. Methods Patients underwent unilateral mastectomy and 2-stage reconstruction. During the second stage of the procedure, contralateral augmentation mammoplasty was performed. Preoperatively, we obtained the patients' demographic information, and we then assessed breast volume, the volume and dimensions of the inserted implants, and complications. Breast symmetry was observed by the surgeon and was assessed by measuring the disparity between the final volume of each breast. Results Contralateral augmentation was performed in 52 cases. When compared to patients who did not undergo a contralateral balancing procedure, patients who received contralateral augmentation were younger, thinner, and had smaller breasts. During implant selection for contralateral augmentation, we chose implants that were approximately 1 cm shorter in width, 1 level lower in height, and 1 or 2 levels lower in projection than the implants used for reconstruction. The postoperative breast contours were symmetric and the final volume discrepancy between each breast, which was measured by 3-dimensional scanning, was acceptable. Conclusions We demonstrate that contralateral augmentation can be recommended for patients who perceive their breasts to be small and not beautiful in order to achieve an ideal and beautiful shape for both breasts. Furthermore, this study offers guidelines for selecting the implant that will lead to the optimal aesthetic outcome.

Treatment of glabellar frown lines using selective nerve block with radiofrequency ablation (고주파절제술을 통한 선택적 신경차단법을 이용한 미간주름의 개선)

  • Purpose: Corrugator supercilii muscle pulls eyebrow to inferomedial direction and produces the vertical component of the glabellar line formation. Current techniques for eliminating of glabellar frown include direct resection of corrugators and botulinum toxin injection. Muscle resection in endoscopic face lift procedure is relatively complex and has many disadvantages ranging from possible nerve injury, postoperative edema, pain and a long recovery period. The Botox treatment on the other hand is much more simple in technique but has a short duration of action. The authors have attempted new ways of finding improved treatment of the glabellar frown by selectively blocking of motor nerves innervating the corrugator supercili muscle by using radiofrequency ablation technique. Methods: A total of 80 patients were recruited in our study during the period between Feb. 2007 to June 2008. A probe was introduced from the supraorbital ridge and advanced to the corrugator supercilii muscle. Nerve stimulator was then used to locate the nerve innervating the corrugator and radiofrequency ablation of the nerve was done. Results: In all patients, there were marked improvement in glabellar frown after treatment. There were no reported cases of any relapses during the follow up period. No complication was noted such as facial nerve injury. No patient complained of any adverse symptoms other than slight discomfort due to swelling of the operation site. Conclusion: The treatment of glabellar frown lines using selective nerve block with radiofrequency ablation was not only less invasive but also excellent in surgical outcomes.

  • Stone cultural heritage will either be damaged by composite damage or will lose its original historical authenticity starting with the moment it is created. Various artificial interventions to restore them to their original state have been described, centering on the conservation treatment case of the Stupa of State Preceptor Jigwang from the Wonju Beopcheonsa Temple Site. Restoration of the Jigwang stone pagoda was carried out after securing all scientific and technological means for the research and protection of the cultural heritage in question. Since its restoration was promoted to retain its aesthetic and historical value and was based on a careful understanding of the original materials and prototypes, extensive restoration was sought, which contributed to the restoration of the original form, resulting in both preservation and authenticity.

  • Excessive pathological attrition over the entire dentition can cause decreased masticatory function, and pathological problems of the temporomandibular joint and the muscular nervous system. In this case, 71 year-old male patient with severe attrition and fracture across the entire tooth was treated by full mouth rehabilitation for regaining space of restoration. In order to determine the appropriate vertical dimension of the patient's occlusion, facial appearance, aesthetics, restoration space were evaluated, and provisional restoration of the raised vertical dimension was made. After 4 months of evaluation, the entire restoration of monolithic zirconia crown using CAD-CAM was completed. Through the above process, satisfactory aesthetic and functional results were obtained.

  • A patient with severe periodontitis has causative factors that cause pathological tooth movement, the occlusion is disintegrated, and the vicious cycle of worsening periodontitis is repeated. In particular, when pathological tooth movement occurs in the maxillary anterior region, the patient has an aesthetic sense of atrophy, and the quality of life was reduced. Therefore, when orthodontic treatment was added to patients with severe periodontitis, it promotes the formation of new bone, reduces periodontal cysts, and obtains clinical attachment, which leads to favorable results in prosthetic restoration, thereby enabling ideal occlusion, function and aesthetics. Periodontal treatment, orthodontic treatment, natural tooth restoration, and implant prosthesis were planned for patients with pathological tooth movement in the anterior region due to loss of occlusal support in the posterior region. As a result, an ideal restoration space was secured, a stable restoration of occlusal contact was formed, and the maxillary anterior teeth were aesthetically improved.

Study on the Ecological Restoration of Rock-exposed-cut-slope by Natural Topsoil Restoration Methods : In Case of Won-Ju Experiment (자연표토 복원공법에 의한 암절취비탈면의 생태적 복원에 관한 연구 : 원주사례지역을 중심으로)

  • This study was conducted to suggest the ecological restoration and environmentally friendly revegetation technology for the rock-exposed cut-slopes by the Natural Topsoil Restoration Methods (NTRM) with the following restoration objectives; (1) prevention or reduction of wind and water erosion, (2) provision of food and cover for variety of animal species, (3) improvement of the visual or aesthetic quality of the disturbed slopes. On Nov. in 1995, the 5cm thick layer of artificial soil and 2cm thick layer of straw-mulching was attached at rock-exposed cut-slopes by NTRM without using anchor wire and anchor pin. The main results during four years surveying on the ground-coverage effect, plant growth, species diversity and importance values were summarized as follows. 1. Artificial soil attached at rock exposed cut-slopes was not eroded until recovered by tree and herbaceous vegetation in spite of not using anchor wire and anchor pin. Also it shows low soil hardiness and has almost the same amount of bacteria and fungi with in surrounding natural topsoil. 2. In 'combination for the woody vegetation', Lespedeza cyrtobotrya, Albizzia julibrissin, Rhus chinensis, Indigofera pseudo-tinctoria occupied upper layer vegetation. Since three years after seeding, Indigofera pseudo-tinctoria had overwhelmed the other woody plants and cool season foreign grasses, Erigeron canadensis, Taraxacum mongolicum, Commelina communis, Arundinella hirta (Thunberg) and Oenothera erythrosepala grows at lower part of the vegetation, 3. The heights of the Rhus chinensis grows 1.8m, Indigofera pseudo-tinctoria 2.0m, so it seems that the objectives of woody vegetation with native plants could be accomplished. 4. After 4 years later after seeding in 'combination for the herbaceous vegetation', the most dominant plant was Indigofera pseudo-tinctoria, the next was in order of cool-season grasses, Taraxacum mongolicum, Erigeron canadensis, lxeris dentata (Thunberg), Oenothera erythrosepala, Arundinella hirta (Thunberg). The diversity index in 'combination for woody vegetation' was higher than that in 'combination for the herbaceous vegetation'. The tendency of the intrusion of secondary succession plants was more effective in 'combination for the herbaceous vegetation' than in 'combination for the woody vegetation'.

Treatment for ophthalmic paralysis: functional and aesthetic optimization

  • Facial nerve palsy has an effect on a person's well-being functionally and psychologically. Therefore, comprehensive patient management is essential. One of the most common uncomfortable and potentially debilitating features is associated with the incapacity for eye closure. Restoration of eye closure is a key consideration during the surgical management of facial palsy. In this article, we introduce simple surgical methods-which are relatively easy to learn and involve the upper and lower eyelids-for achieving eye closure. Correcting upper eyelid function involves facilitating the component of eye closure that is in the same direction as gravity and is, therefore, less complicated and favorable outcomes than correction of lower lid. Aesthetic aspects should be considered to correct the asymmetry caused by facial palsy. Lower eyelid function involves a force that opposes gravity for eye closure, which makes correction of lower eyelid ectropion more challenging than surgery for the upper eyelid, particularly in terms of effecting a sustained correction. Initially, proper ophthalmic evaluation is required, including identifying the chronicity and severity of ectropion. Also, it is important to determine whether or not lateral canthoplasty is necessary. The lateral tarsal strip procedure is commonly used for lower lid correction. However, effective lower lid correction can be achieved with better cosmesis when extensive supporting techniques are applied, including those involving cheek tissue.

Study on Digital Restoration by 3-dimensional Image for Gilt Bronze Cap Excavated from the Ancient Tomb of Andong, Goheung (고흥 안동고분 출토 금동관모의 3차원 디지털 복원연구)

  • A precision measurement and digital image restoration of the 5th century's gilt bronze cap of Baekje dynasty, excavated from the ancient tomb of Andong, Goheung in 2006, was undertaken. The objective of the scanning is to preserve precise feature of the artefact in the form of digital data by embodying it in 3 dimensional space. Acquirement of the data has been undertaken in the following process : 3D scanning to obtain 3D shape and color information(original data photographing)-3D modelling(joining original data and restoring non-photographed or damaged area)-CG image production. Production of restoration CG image was based on joined shape of original data and each part's measurement on CAD. Non-photographed part and area of loss was restored referring actual measurement and research result of excavated cap from the 5th to 8th century. 3D image restoration is one of artefact restoration methods which restores artefact without risk. It is also undertaken with historical research. As result, this method can enhance aesthetic and academic value of the artefact by successful restoration.
