Lobaplatin Combined Floxuridine/Pirarubicin-based Transcatheter Hepatic Arterial Chemoembolization for Unresectable Primary Hepatocellular Carcinoma

  • Purpose: To assess the effect and safety of lobaplatin combinated floxuridine /pirarubicin in transcatheter hepatic arterial chemoembolization(TACE) of unresectable primary liver cancer. Patients and Methods: TACE combined with the chemotherapy regimen was used to treat 34 unresectable primary liver cancer patients. DSA/MRI/CT/blood routine examinations were used to evaluate short term activity and toxicity after 4-5 weeks, the process being repeated if necessary. Results: Among the 34 cases, 1 (2.9%) showed a complete response, 21 (61.7%) a partial response, 8 (23.5%) stable disease, and 4 progressive disease, with a total effective rate of 67.6%. The content of alpha fetoprotein dropped by over 50% in 20 cases (58.8%). The rate of recovery was hepatalgia (88.2%), ascites (47.1%), appetite (55.9%), Performance Status(30.4%). The median follow-up time (MFT) was 281 days (63-558 days), and median progression-free survival was 118.5 days (95%, CI:88.8-148.2days). Adverse reactions (III-IV grade) were not common, with only 4 cases of vomiting and 2 cases of thrombocytopenia (III grade). Conclusions: Lobaplatin-based TACE is an effective and safe treatment for primary liver cancer.

  • Objectives : To evaluate the clinical effectiveness of Flexible brace(SpineCor Brace). Methods : CNKI, PubMed databases and Korean Journal of Oriental Medicine in 1999-2014 were searched for studies on SpineCor. This study researched 4 randomized controlled trial studies. Results : The results suggested that the SpineCor brace reduced the probability of the progression of early idiopathic scoliosis comparing with its natural history or rigid brace. But some studies showed that there is no significant difference between SpineCor and rigid brace. Conclusions : These results indicate that SpineCor have obvious effects on treatment of scoliosis and no adverse reactions, therefore we need to study more researches on SpineCor.

Optimization of longitudinal viscous dampers for a freight railway cable-stayed bridge under braking forces

  • Under braking forces of a freight train, there are great longitudinal structural responses of a large freight railway cable-stayed bridge. To alleviate such adverse reactions, viscous dampers are required, whose parametric selection is one of important and arduous researches. Based on the longitudinal dynamics vehicle model, responses of a cable-stayed bridge are investigated under various cases. It shows that there is a notable effect of initial braking speeds and locations of a freight train on the structural responses. Under the most unfavorable braking condition, the parameter sensitivity analyses of viscous dampers are systematically performed. Meanwhile, a mixing method called BPNN-NSGA-II, combining the Back Propagation neural network (BPNN) and Non-Dominated Sorting Genetic Algorithm With Elitist Strategy (NSGA-II), is employed to optimize parameters of viscous dampers. The result shows that: 1. the relationships between the parameters of viscous dampers and the key longitudinal responses of the bridge are high nonlinear, which are completely different from each other; 2. the longitudinal displacement of the bridge main girder significantly decreases by the optimized viscous dampers.

  • Methotrexate (MTX) is a chemotherapeutic agent that is used to treat a host of malignancies. But recently, MTX has also been used as a therapeutic agent for chronic inflammatory disorders such as rheumatoid arthritis, psoriasis, and systemic lupus erythematosus. However, MTX is an antimetabolite that affects rapidly dividing normal cells such as oral mucosal epithelial cells, gastrointestinal epithelial cells, and bone marrow cells-which explains why oral mucositis is often an initial manifestation of MTX toxicity. Because oral lesions are frequently initially presented in dental clinics, dentists should consider the possibility of adverse drug reactions in the differential diagnoses of oral lesions through a meticulous collection of patients' medical histories. In this report, we examine patients who suffered from oral ulcerative lesions upon diagnosis of MTX-induced oral mucositis. Then, we suggest approaches for the diagnosis and treatment of MTX-induced oral mucositis through a review of literature.

A Study of Side Effects from Korean Herb Medicine on Children (한약을 복용한 소아에게 발생한 부작용에 대한 연구)

  • Objectives: The purpose of this study is to investigate the frequency of side effects, and the range of the side effects from Korean herb medicine on children. Methods: The study has been carried out from 212 children who took Korean herb medicine in Department of Pediatrics, $\bigcirc\;\bigcirc$ Oriental Medical Hospital from September 2009 to February 2010. The study was completed through patients chart review and telephone survey. Results: 1. There were 6 side effect cases on male, and 3 side effect cases on female out of 212 children. The incidence of side effect was 4.2%. 2. The most common side effect was abdominal pain. The most common related organ was digestive organ. 3. The side effect occurred within one to three days after taking Korean herb medicine, and the symptoms were mostly disappeared after taking the Korean herb medicine into divided dose or less dose. Conclusions: The symptom of side effects on children was not as severe as other adverse drug reactions often seen in Western medication. The most common symptom was abdominal pain. Further studies are needed.

  • The ultraviolet blocking agents used to skincare are largely classified ultraviolet scattering agent and ultraviolet absorbent agents. There are UVC, UVB, and UBA in ultraviolet ray(UV) showing shorter wavelength than visible light. Both visible light and UVA give rise to 1st dark-skinned phenomena. On exposure of the skin to UV, phenomena of skin variation are termed sunburn or suntan. There are chronic and acute adverse reactions in skin response to UV. The latter is caused by UVB, which has mainly effects on the skin. But lately due to destruction of ozone layer, UVA has more serious irritation on the skin than UVB. In this paper spectrometric properties of UV absorbent agents such as PABA, octyl-PABA, Urocanic acid, and 2-hydroxy-4-methoxy benzophenone in vitro have been investigate. As results, it was found that the three fomer were more suitable than the last. UV scattering measurements on the sample used inorganic pigments showed that pigments containing titanium dioxide had a better scattering effects than the inorganic pigments such as $Fe_2O_3$, $Al_2O_3$, etc.

A Case Report of Hypersensitivity Vasculitis Improved by Korean Medicine Treatment (과민성 혈관염의 한방치료 증례보고 1례)

  • Introduction: The aim of this study is to report the effect of Yukmijiwhang-tang-gamibang on the treatment of hypersensitivity vasculitis. Case presentation: A patient with hypersensitivity vasculitis was treated with herbal medicine (3 times a day) and dressing (once per day) for 16 days. The severity of hypersensitivity vasculitis was assessed using the numeral rating scale (NRS) and pruritus score. After treatment, pain and itching symptoms were reduced, the NRS score improved from 5 to 1, and the pruritus score improved from 2 to 0. No adverse reactions were noted and no relapse of hypersensitivity vasculitis occurred within 38 days after the end of treatment. This result shows that herbal medicine could be effective and safe for treating hypersensitivity vasculitis. Conclusion: According to the results, Korean medicine such as Yukmijiwhang-tang-gamibang can be effective and safe for the treatment of hypersensitivity vasculitis.

Use of a Combination Imidacloprid and Permethrin for Psoroptic Mite Infestation in Three Pet Rabbits (세 마리 애완 토끼의 귀진드기 감염증에 imidacloprid와 permethrin 합제의 이용)

  • Psoroptic mites can cause severe pruritic otitis externa with crusts in rabbits. In this case report, three rabbits with a psoroptic mite infestation were treated with a formulation containing 10% imidacloprid and 50% permethrin at a dose of 0.4 ml regardless of body weight. One week after treatment, the rabbits showed mild pruritus with no crusts, mites or eggs. Four weeks after treatment, there were no clinical signs, mites or eggs observed in the rabbits. The rabbits were treated successfully with this combination without adverse reactions for 8 weeks after treatment. This case report suggests that a single topical application of a combination of imidacloprid and permethrin may be an effective and practical treatment for psoroptic mite infestations in pet rabbits.

A Case Report of Neck Pain Patient with Klippel-Feil Syndrome by Cervical Manipulation Treatment (Klippel-Feil 증후군 환자의 경추 도수치료 후 발생한 부작용 치험1례)

  • The Klippel-Feil Syndrome (KFS) is characterized by congenital fusion of two or more vertebrae with hypermobile normal segment. According to this, a patient with KFS can be at risk of severe neurological symptoms after manipulation treatment. We had a KFS patient who suffered from neck pain and limited range of motion at cervical after manipulation treatment. The patient, 49-year-old woman was diagnosed as KFS through Cervical X-ray and MRI. The patient was treated by acupuncture therapy and SCENAR therapy. We measured neck pain by visual analog scale (VAS) and neck disability index (NDI) and checked range of motion at cervical before and after the treatments. After 8 times treatments, the patient's pain decreased and the range of motion increased. From this case, we can recognize the risk of Chuna manipulation treatment for KFS patient. So, we suggest that radiological examination is needed before cervical Chuna manipulation treatment to avoid adverse reactions.

Reviews on the Hepatotoxicity of Tyrosine Kinase Inhibitors (티로신 키나아제 저해제의 간독성에 대한 고찰)

  • Background: Small-molecule tyrosine kinase inhibitors (TKIs) have had major impacts on anticancer therapy by targeting the catalytic activities of dysregulated tyrosine kinases. TKIs have not presented traditional toxicities; however, some serious adverse effects, including hepatotoxicity, have been documented in clinical trials and post-marketing surveillance. Although TKI-induced hepatotoxicity can cause severe clinical complications in patients, the underlying mechanism is still unclear. Methods: Studies on TKI-induced hepatotoxicity were identified by Pubmed search, and relevant articles were reviewed. Results: Immunoallergic reaction, cytochrome P (CYP) 450 polymorphisms, and formation of reactive metabolites are under consideration as mechanisms of TKI-induced hepatotoxicity. Host protein-drug metabolite conjugates are recognized as antigens by class II major histocompatibility complexes and are believed to cause liver injuries. Polymorphisms in CYP, which influences TKI metabolism, can slow TKI metabolism and may induce development of hepatotoxicity. The formation of reactive metabolites during drug metabolism can induce hepatotoxicity by directly causing cytotoxicity, leading to cell dysfunction, and indirect toxicity by mediating secondary immune reactions. Concurrent use of various medications with TKI can also cause hepatotoxicity by affecting drug transporter or enzyme activities. Conclusion: Periodic monitoring of patients taking TKIs and risk/benefit reassessments though post marketing surveillance are necessary to prevent hepatotoxicity.
