  • Compared to the specialist program, the advanced general dentistry program (AGD) is designed to guarantee that dentists dealing with primary care services possess overall dental care proficiency and contribute to offering better care to customers. This program is also believed to help resolve the imbalance among the number of candidates for specialization, which came to the authorities' attention throughout the implementation of the dental specialist program. KDA will continue to implement AGD while pushing for the establishment of the program by complementing it with adjustment measures and regulations provided the dental community agrees to such actions.

  • Dental treatment under general anesthesia is considered for behavioral control of disabled patients who have severe anxiety or involuntary movement. However, in case of simple treatment, inhalation or intravenous sedation, which has earlier onset and recovery, is preferred. Conventionally, nitric oxide is used for inhalation sedation, nowadays sevoflurane can also be used due to easily titratable for controllable effect and less complications. In this case report, two patients with mental retardation required simple dental treatment. Deep sedation with inhaled sevoflurane were successfully employed and patients were discharged without any complications.

  • Even though Korean Dentistry practically have adapted the specialty system, the execution of regulation was tentatively delayed during the past 30 years. Recently a group of dentists advocated that the legislation must be executed. The Korean Dental Association announced the new legislation for specialty program to accommodate the desires of almost of dentists. It is prospected to restrict the specialist as low as 8% or up to 15%. But in fact, during the past 10 year, the applicant to the specialty program increased rapidly and about 40% of graduate are in the specialty programs these days.(omitted)

  • Patients with disabilities have difficulties tolerating in-office dental treatment due to limitations relating to cooperation and/or physical problems. Therefore, they often require general anesthesia or sedation to facilitate safe treatment. When deciding on dental treatment under general anesthesia, the plan should be carefully determined because compared to general patients, patients with disabilities are more likely to experience anesthetic complications because of their underlying medical conditions and potential drug interactions. Clinicians prefer simpler and more aggressive dental treatment procedures, such as extraction, since patients with impairment have difficulty maintaining oral hygiene, resulting in a high incidence of recurrent caries or restorative failures. This study aimed to review the available literature and discuss what dentists and anesthesiologists should consider when providing dental treatment to patients with severe disability under general anesthesia.

  • In adult patients with dental phobia, dental treatment may be difficult, or may not be possible. Depending on the level of fear or anxiety, non-pharmacological or pharmacological behavior management techniques are used in the dental treatment of such patients. Among the pharmacological behavior management techniques, minimal sedation, that is, the lowest depth of sedation, can be easily obtained in adult patients using oral sedatives, does not require special equipment or tools, and does not affect ventilatory and cardiovascular function. Diazepam is an anxiolytic drug belonging to the benzodiazepine family that, in addition to relieving anxiety, produces muscle relaxation, and is a representative drug used in adult patients with fear of dental treatment. Herein, we report the case of a 50-year-old woman with severe dental fear who successfully underwent long-term dental treatment in approximately 20 visits with minimal sedation using oral diazepam. In addition, we reviewed the considerations for the use of benzodiazepines for minimal sedation.

  • PURPOSE. The aim of this retrospective study was to evaluate cumulative survival rate (CSR) of Implantium implants followed for 5 years and association between risk factors and the CSR. MATERIALS AND METHODS. A total of two hundred forty-nine Implantium Implants System (Dentium, Seoul, Korea) placed in ninety-five patients from 2004 to 2009 were investigated with several identified risk factors (sex, systemic disease, smoking, alchohol, reason of tooth loss, length, arch (maxilla or mandible), replace tooth type (incisor, canine, premolar or molar) Kennedy classification, prosthodontic type, prosthodontic design, opposite dentition, abutment type, occlusal material, occlusal unit, splint to tooth, cantilever, other surgery). Clinical examination (mobility, percussion, screw loosening, discomfort, etc.) and radiographic examination data were collected from patient records including all problems during follow-up period according to protocols described earlier. Life table analysis was undertaken to examine the CSR. Cox regression method was conducted to assess the association between potential risk factors and overall CSR. RESULTS. Five of 249 implants were failed. Four of these were lost before loading. The 5-year implant cumulative survival rate was 97.37%. Cox regression analysis demonstrated a significant predictive association between overall CSR and systemic disease, smoking, reason of tooth loss, arch, Kennedy classification and prosthodontic design (P<.05). The screw related complication was rare. Two abutment screw fractures were found. Another complications of prosthetic components were porcelain fracture, resin facing fracture and denture fracture (n=19). CONCLUSION. The 5-year CSR of Implantium implants was 97.37 %. Implant survival may be dependent upon systemic disease, smoking reason of tooth loss, arch, Kennedy classification and prosthodontic design (P<.05). The presence of systemic diseases and combination of other surgical procedures may be associated with increased implant failure.

  • PURPOSE. The aims of this study were to suggest a method of fabrication of the record base using a light-polymerized resin by applying the two-phase fabrication method for the improvement of the fit of the record base and to compare the degree of fit according to the separation site. MATERIALS AND METHODS. In the edentulous cast of maxilla, four test groups were considered. In the first, second, third, and fourth test groups (n = 12 in each group) the separation was done at 0, 5, 10, and 15 mm, respectively below the alveolar crest along the palatal plane. For the control group, the record base was made without separating the two sections. The light-body silicone material was injected into the fitting surface of the record base. It was then placed onto the cast and finger pressure was applied to stabilize it in a seated position followed by immediate placement onto the universal test device. Finally, the mass of the impression material was measured after it was removed. ANOVA was performed using the SAS program. For the post-hoc test, the Wilcoxon Rank-Sum test and the Tukey-Kramer HSD test were performed ($\alpha$ = 0.05). RESULTS. The control group and Group 3, 4 showed significant differences. The Group 3 and 4 showed significantly smaller inside gaps than the control group which was not made with the two-phase fabrication method. CONCLUSION. The two-stage polymerized technique can improve the fit of the denture base particularly when the separation was made at 10 to 15 mm from the alveolar crest.
