청소년기의 구조적 척추측만증에 대한 추나치료 결과 고찰 (A Clinical Study on the Outcome of Chuna Treatment of Adolescent Structural Scoliosis)

  • Objectives The present study retrospectively investigated clinical outcome of patients with structual scoliosis during Chuna treatment. Methods : The study population consisted of 34 patients(14 patients of adolescent and 20 patients of adult) who were diagnosised as scoliosis with radiological finding. All patients were treated with Chuna treatment, and after treatment, evaluated with radiological measurement by calculating the scoliosis correction angle from the Initial and follow-up spine AP and lateral views. Also the evaluation of clinical outcome was done twice pre and post treatment during this study. Results : The results were summarized as follows ; 1) The 85.3 percent of patients complained physical pain. and adult group had various painful lesion compared with adolescent group. 2) Adult group had treated $18.0{\pm}7.2$ times and adolescent group treated $13.5{\pm}5.7$ times with Chuna treatment. 3) Scoliotic angle is reduced in 92.9 percent of adolescent group and had all of adult group after Chuna treatment, especially, statistically significance appeared in adult group. 4) Adult group was superior to adolescent group in correctability and scoliotic angle change. 5) Correction effect was better in the group which initial scoliotic angle was lessor, or had more time of treatment. 6) According to grade of vertebral body rotation, scoliotic angle is larger and correctability was lower. Conclusions : Chuna treatment was efficacious against scoliosis. And it is necessary for adolescent scoliosis patient to be more carefully treated and observed.

  • Pulmonary surfactant is a lipoprotein complex composed primarily of phospholipid and lung specific apoproteins that reduces surface tension in the alveolus and maintains alveolar stability at low lung volume. Adult respiratory distress syndrome still carries a very high morbidity and mortality. The surfactant system is vital to the maintenance of proper lung function, any type of surfactant deficiency, whether primary or secondary, will contribute significantly to the development of pulmonary pathophysiology. Various mechanisms in adult respiratory distress syndrome may be responsible for such alterations in the surfactant system. Surfactant replacement is now an established treatment for neonatal respiratory distress syndrome, reducing both incidence of complications and mortality. With the current knowledge of surfactant physiology and the pathophysiology of the adult respiratory distress syndrome exogenous surfactant treatment or stimulation of endogenous surfactant synthesis and secretion will prove to be beneficial in preventing and treating the adult respiratory distress syndrome. The study of clinical surfactant therapy for adult respiratory distress syndrome is just beginnig and this can be viewed as an area with exciting potential. As soon as surfactant preparations become more widely available trials should begin to define the role of surfactant treatment in the adult respiratory distress syndrome as an adjunct to available treatment techniques.

Minimal sedation using oral sedatives for multi-visit dental treatment in an adult patient with dental phobia

  • In adult patients with dental phobia, dental treatment may be difficult, or may not be possible. Depending on the level of fear or anxiety, non-pharmacological or pharmacological behavior management techniques are used in the dental treatment of such patients. Among the pharmacological behavior management techniques, minimal sedation, that is, the lowest depth of sedation, can be easily obtained in adult patients using oral sedatives, does not require special equipment or tools, and does not affect ventilatory and cardiovascular function. Diazepam is an anxiolytic drug belonging to the benzodiazepine family that, in addition to relieving anxiety, produces muscle relaxation, and is a representative drug used in adult patients with fear of dental treatment. Herein, we report the case of a 50-year-old woman with severe dental fear who successfully underwent long-term dental treatment in approximately 20 visits with minimal sedation using oral diazepam. In addition, we reviewed the considerations for the use of benzodiazepines for minimal sedation.

  • Orthodontic treatment for middle-aged patients has become more commonplace with various reasons including improved socioeconomic status. Understanding of oral status and treatment modalities of middle-aged patients is mandatory for accurate diagnosis and proper treatment planning. This study investigated 100 consecutive patients aged 40s and 50s and 100 aged 20s who had been examined and diagnosed at the Department of Orthodontics, Chonnam National University Dental Hospital. The results were obtained as follows; 1. Gender distribution showed female outnumbered male patients in young-aged adult patients, but middle-aged patients showed similar male and female distribution. 2. The major concern seeking orthodontic treatment was esthetics not only in young-aged but also in middle-aged adult patients, and a number of middle-aged patients were concerned about oral health as well. 3. Considerable number of middle-aged patients were referred by other dental specialties while young-aged adult patients were more self-motivated for orthodontic treatment. 4. Middle-aged adult patients had more missing teeth and periodontal disease than young-aged adults. 5. The most frequently-observed problem was dental spacing in middle-aged patients while dental crowding in young-aged adult patients. Middle-aged patients showed higher prevalence of deep overbite and overjet while most of young-aged adults presented opposite direction of problem in overbite and overjet. 6. Limited orthodontic treatment was required rather than comprehensive treatment in middle-aged patients, and the most common tooth moving area was anterior part of dentition in case of limited treatment. Need of interdisciplinary therapy with other dental specialties was more common in middle-aged patients. 7. Intervention of specific technique such as invisible TP, passive bracketing, passive wire bonding, and lingual orthodontics was more required in middle-aged patients. Considering that middle-aged patients have different characteristics than young-aged adults, the results of the present study suggest that different treatment modalities are required in middle-aged orthodontic patients in order to manage them properly and efficiently.

후천적 성인 편평족: 병태생리, 진단과 비수술적 치료 (Acquired Adult Flatfoot: Pathophysiology, Diagnosis, and Nonoperative Treatment)

  • Acquired adult flatfoot is a deformity characterized by a decreased medial longitudinal arch and a hindfoot valgus with or without forefoot abduction. The etiologies of this deformity include posterior tibial tendon dysfunction, rheumatoid arthritis, trauma, Charcot's joint, neurologic deficit, and damage to the medial spring ligament complex or plantar fascia. Among these, posterior tibial tendon dysfunction is the most well-known cause. Although posterior tibial tendon dysfunction has been regarded as a synonym of acquired adult acquired flatfoot, failure of the ligaments supporting the arch can also result in progressive deformity even without a posterior tibial tendon problem. The authors describe the pathophysiology, diagnosis, and nonoperative treatment of acquired adult flatfoot, focusing on posterior tibial tendon dysfunction.

성인 구순구개열환자의 교정치료 및 관리 (Orthodontic treatment and management of adult patient with cleft lip and palate)

  • Patients with cleft lip and palate require interdisciplinary treatment to achieve successful rehabilitation. However, there are special difficulties in orthodontic treatment of adult cleft lip and palate patients: 1. Lack of Tissue, Bone, and Soft tissue; 2. Heavy Scar Tissue, Vestibule, and Palate; 3. Severe Anteroposterior discrepancy and Impaired Maxilla; 4. Distortion of Alveolar Ridge; 5. Abnormal Eruption Path and Malalignment of Tooth. Solving these problems, orthodontist should have differential diagnosis on extent of cleft site and residual deformities of adult cleft lip and palate patient. The tooth missing area in cleft site was commonly treated with a removable or fixed prosthesis, but this method is not stable to retain maxillary arch shape. To establish the more stable arch shape in cleft lip and palate, endosseous implants in the alveolar clefts with bone graft is helpful for management of adult cleft lip and palate patient.

의료행위와 대리승낙 (A Review on Consent to the Medical Treatment in the case of Foreign Determination)

  • The right to self-determination in regard to one's body is a key element of human dignity, privacy and freedom. It is constitutionally enshrined in the guarantee of human dignity, in the general right of personality and, most concretely of all, in the right to physical integrity. In principle No-one may trespass another person's body against his will, whether this act improves his physical condition or not. This right of self-determination applies equally to healthy and to sick people. Hence everyone has the right either to permit or to refuse a medical treatment, unless he can not make a rational decision. If the person does not consent himself, for whatever reason, another one must do for him as guardian. Representation in consent to medical treatment is therefore the exception of self-determination rule. This article explored, 1. who can consent to the medical treatment in the case of the mentally incapacitated adult and the infant, 2. what kind of consent to the medical treatment can the deputy determinate for the mentally incapacitated adult and the infant, 3. when the deputy can not determinate without permission of the court, and 4. what can the doctor do in the case of conflict between minors and guardians.

Comprehensive Orthodontic Treatment in a Middle-Aged Patient with Missing Maxillary Left First Premolar: A Case Report

  • As the adult population continues to increase, orthodontic treatment for adult patients is becoming more common. This case report describes comprehensive orthodontic treatment of a middle-aged patient with closure of the extraction space without prosthetic restoration. A 55-year-old woman with her maxillary left first premolar extracted because of a periodontal problem, wanted to close the space with orthodontic treatment. Since she had generalized crowding and mild skeletal discrepancy, we planned comprehensive orthodontic treatment, including closure of the extraction space by protraction of the left maxillary molars using miniscrews and aesthetic alignment of anterior teeth. The total treatment period was 28 months. As a result of these treatments, the extraction space was successfully closed, good tooth alignment and satisfactory occlusion were achieved.

  • Fragility fractures of the pelvis (FFP) and fragility fractures of the sacrum (FFS), which are emerging in the geriatric population, exhibit characteristics that differ from those of pelvic ring disruptions occurring in the younger population. Treatment of FFP/FFS by a multidisciplinary team can be helpful in reducing morbidity and mortality with the goal of reducing pain, regaining early mobility, and restoring independence for activities of daily living. Conservative treatment, including bed rest, pain therapy, and mobilization as tolerated, is indicated for treatment of FFP type I and type II as loss of stability is limited with these fractures. Operative treatment is indicated for FFP type II when conservative treatment has failed and for FFP type III and type IV, which are displaced fractures associated with intense pain and increased instability. Minimally invasive stabilization techniques, such as percutaneous fixation, are favored over open reduction internal fixation. There is little evidence regarding outcomes of patients with FFP/FFS and more literature is needed for determination of optimal management. The aim of this article is to provide a concise review of the current literature and a discussion of the latest recommendations for orthopedic treatment and management of FFP/FFS.

소양인(少陽人) 처방을 활용한 성인형 아토피피부염 치험 1례(例) (A Case Report of Soyangin Adult Atopic Dermatitis Patient Treatment with Soyangin Formulae)

  • 1. Objectives: The purpose of this case study is to evaluate the effects of Sasang constitutional therapy and treatment of an adult atopic dermatitis patient, who was diagnosed Soyangin, Interior febrile disease induced from the stomach affected by heat(少陽人 胃受熱裏熱病). 2. Methods: The adult Atopic Dermatitis patient was diagnosed Soyangin, Interior febrile disease induced from the stomach affected by heat(少陽人 胃受熱裏熱病) which was based on his Nature & Emotion(性情), physical characteristics, symptoms. He was medicated with Prescriptions of Soyangin. 3. Results and Conclusions: The clinical evaluation was made by SCORAD index, Total eosinophil count and Total IgE. After treatment, his SCORAD results significantly decreased from 88.4 to 21.9, Total eosinophil count decreased from 1320 to 120 and Total IgE also decreased from 1406.6 to 897.3. And the patient's subjective symptom and gerneral condition considerably improved after treatment. This case study describe the effectiveness on adult Atopic Aermatitis symptom by using Prescriptions of Soyangin.
