Endoscopic Third Ventriculostomy for Adult Aqueduct Stenosis : Double Fenestration - A Case Report and Technical Note - (성인 수도관 폐쇄증에 대한 내시경적 제3뇌실 누공술 : 이중개창술 - 증례보고 및 수술수기 -)

  • Objective : Endoscopic third ventriculostomy is gaining popularity as a minimally invasive surgical option for certain types of hydrocephalus as an alternative to shunting. The authors have tried to fenestrate down to the subdural space passing through the prepontine cistern to lessen or avoid the chance of redoing due to healing. Materials and Method : A 48-year-old male patient with several years of intractable headache was presented. Magnetic Resonance Image(MRI) of the brain revealed marked ventricular dilatation with stenotic cerebral aqueduct. A 2.3mm flexible steerable endoscope($Neuroview^{(R)}$) was introduced via precoronal route and accessed to the third ventricular floor. Using 3-French Fogarty balloon catheter, thin third ventricular floor and the arachnoid membrane of the prepontine cistern were fenestrated, so called "double fenestration". To confirm the fenestration, subdural compa-rtment of the left abducens nerve was identified during the procedure. Forceful pulsating flow through the orifice convinced the patency of the opening. Results : The patient was discharged on the third postoperative day without any postoperative complications. The postoperative follow-up MRI of the brain, at second and sixth months, clearly demonstrated the flow void through the third ventricular floor. Conclusions : Endoscopic third ventriculostomy was successfully performed on an adult hydrocephalus patient with aqueduct stenosis. The third ventricular floor and arachnoid membrane of the prepontine cistern were fenestrated to achieve double fenestration to minimize the chance for failure. The details of this procedure and results are described.

The Effects of Self-efficacy Promoting Program on Self-efficacy, Self-care, and Anxiety of Cancer Patients (자기효능증진 프로그램이 암환자의 자기효능, 자가간호와 불안에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effects of self-efficacy promoting program on self-efficacy, self-care, and anxiety in cancer patients receiving chemotherapy. Method: A quasi-experimental nonequivalent control group pre and post design was used. Subjects were 52 cancer patients who received chemotherapy in a hospital. Twenty-six patients were assigned to the experimental and the control groups, respectively. For the experimental group, the CD image for fifteen minutes and two consultations for about ten minutes each were applied. For the control group, ordinary hospital care was applied. The data was collected before and 2 weeks after chemotherapy. Result: The scores of self-efficacy and self-care of the experimental group was higher than those of the control group while the state of anxiety of the experimental group was not lower than that of the control group at posttest. Conclusion: The self-efficacy promoting program about prevention of infection and hemorrhage increased cancer patients' self-efficacy and self-care but did not decrease anxiety. Further study is needed to find other factors to decrease anxiety and nurses need to consider the concept of self-efficacy in cancer patients receiving chemotherapy when they plan to increase cancer patients' self-care.

  • Purpose: This study was conducted to explore nursing education modality for facilitating undergraduate students' critical thinking within the Korean nursing education context. Methods: Data were collected from four group interviews from two focus groups, which were composed of six nursing professors in each group. Data were analyzed using qualitative content analysis. Results: Five themes with 13 sub-themes involving 10 categories were delineated. The five themes were managing shared resources, a supportive system in preparing qualified faculty, reflective thinking (self-directed learning), theory courses for understanding nursing resources, and clinical practicum for exercising critical thinking. Conclusion: A nursing educational model for facilitating critical thinking within Korean context was suggested. This could be used as an accelerator in developing nursing undergraduate course programs for critical thinking in Korea.

  • Purpose: This study was conducted to describe qualitatively the entities of nurse's experiences in general hospitals and to suggest basic data guiding research on developing Standards of clinical nursing practice in Korea. Method: Fourteen nurses working at general hospitals with over 300 beds in Seoul were interviewed in-depth until saturation using tape-recorders and transcription. Result: The central theme of clinical nursing practice experienced by subjects was "being with clients" that means accepting client's personal character, solving client's needs and providing client-centered nursing. A also "being with clients" was felt to be the responsibility of nurses which was learned from their nursing schools. The nursing strategies performed in order to be with patients were proving skillful nursing techniques, accepting, educating, emotional support, advocating, and self-reflecting, the subjects experienced somewhat problematic affects such as difficulties in interpersonal relationship, work overload, negative image of nursing, deficit of self-confidence for nursing actions, poor working conditions, and unfair treatment. Nurses at the hospital practiced with pride when they felt that they were accepted by clients. Conclusion: Further research is needed to analysis problems in clinical practice and the comparison of nurses' experiences of clinical practice, with nurses' experiences in various settings.

Development of Mobile-application based Cognitive Training Program for Cancer Survivors with Cognitive Complaints (암 환자를 위한 앱 기반의 인지건강훈련 프로그램의 개발)

  • Purpose: The purpose of this study was to design a mobile-application of a cognitive training program for people who have chemo-related cognitive complaints. Methods: The program was developed based on the network-based instructional system design proposed by Jung. The program consisted of several tasks centered on four cognitive domains: learning, memory, working memory, and attention. For memory learning, a target-image and all its elements (color, position, and number) were presented on the screen that had to be recognized among a number of distractor-figures. In working memory training, the previous learned target-figure according to the level of difficulty had to be remembered among many different figures. In attention training named "Find the same figure," two identical symbols in a grid-pattern filled with different images were presented on the screen, and these had to be simultaneously touched. In attention training named "Find the different figure," a different symbol in a grid pattern filled with same figures had to be selected. This program was developed to train for a minimum of 20 min/day, four days/week for six weeks. Results: This cognitive training revealed statistically significant improvement in subjective cognitive impairments (t=3.88, p=.006) at six weeks in eight cancer survivors. Conclusion: This cognitive training program is expected to offer individualized training opportunities for improving cognitive function and further research is needed to test the effect in various settings.

Active Shape Model-based Objectionable Image Detection (활동적 형태 모델을 이용한 유해영상 탐지)

  • In this paper, we propose a new method for detecting objectionable images with an active shape model. Our method first learns the shape of breast lines through principle component analysis and alignment as well as the distribution of intensity values of corresponding landmarks, and then extracts breast lines with the learned shape and intensity distribution. To accurately select the initial position of active shape model, we obtain parameters on scale, rotation, and translation. After positioning the initial location of active shape model using scale and rotation information, iterative searches are performed. We can identify adult images by calculating the average of the distance between each landmark and a candidate breast line. The experiment results show that the proposed method can detect adult images effectively by comparing various results.

  • Schistosomiasis is one of the important neglected tropical diseases (NTDs) in Tanzania, particularly in Lake Victoria zone. This baseline survey was a part of the main study of integrated control of schistosomiasis and soil-transmitted helminths (STHs) aimed at describing morbidity patterns due to intestinal schistosomiasis among adults living on Kome Island, Sengerema District, Tanzania. Total 388 adults from Kome Islands (about 50 people from each village) aged between 12 and 85 years, were examined by abdominal ultrasound according to the Niamey protocol. Liver image patterns (LIPs) A and B were considered normal, and C-F as distinct periportal fibrosis (PPF). The overall prevalence of PPF was 42.2%; much higher in males than in females (47.0% in male vs 34.4% in females, P=0.007). Abnormal increase of segmental branch wall thickness (SBWT) and dilated portal vein diameter (PVD) were also more common in males than in females. Hepatosplenomegaly was frequently encountered; 68.1% had left liver lobe hepatomegaly and 55.2% had splenomegaly. Schistosoma mansoni-related morbidity is quite high among adults in this community justifying the implementation of integrated control strategies through mass drug administration, improved water supply (pumped wells), and health education that had already started in the study area.

  • Background: This study aimed to investigate the effect of elastic band-resistive exercise using audio-visual medium on pain, proprioception, and motor function in adults with chronic neck and shoulder pain. Design: One group pretest-posttest follow-up experimental design. Method: Twenty adult women with neck and shoulder pain voluntarily participated in this study. Elastic band-resistive exercise using audio-visual medium including cervical flexion and extension, shoulder external rotation, and scapular retraction-protraction motions was conducted 5 times a week for 3 weeks. The Numerical Rating Scale, pressure threshold tool, CROM goniometer, and Image J software were used to assess subjective pain level, tenderness threshold (pain), joint position sense error (proprioception), joint range of motion, and postural alignment (motor function), respectively. Result:: The pain intensity and threshold and joint position sense error showed significant decreases after the intervention, whereas the joint range of motion angle revealed significant increases. The postural alignment including forward head posture and rounded shoulder revealed significant improvements after the intervention. Conclusions: Therefore, we suggest that elastic band-resistive exercise through audio-visual medium would be helpful in preventing and managing pain and physical dysfunction in individuals with chronic neck and shoulder pain, and then it would support the development of health management-related online education content.

A Study on the Co-relationship between Programming Ratio of Animation and the Preference of Broadcasting Channel : Focusing on the Programming Ratio of Terrestrial Broadcasting in the 1980s (방송채널의 애니메이션 편성비율과 선호채널이미지 축적간의 상관관계 연구 : 1980년대 지상파 방송채널 애니메이션 편성비율을 중심으로)

  • Animation programs are regarded as bad genre in the aspect of advertisement revenues due to relative low viewing ratios. But programming of animation can be considered positively in the aspect of channel loyalty and preference. This study is based on the assumption that if the scheduling of animation genres could bring out the improvement of channel image of broadcasting stations in the long term, the terrestrial broadcasters could reconsider the increase of animation programs. The research questions of this study are as follows: 1. What is the relationship between the programming ratio of animation genres and the concentration of viewing patterns? 2. How has the increase of animation programming influenced the channel image of broadcasters? 3. Why is it necessary to increase of reinforce the programming of animation genre in the new media platforms? The teenagers aged from 8 to 14 can generally make a decision which program they want to watch. Likewise, the adult viewers aged from 30 to 40 can make a selection which commodity they want to buy. The results of this study indicate that the adult viewers have showed the strong preference for the same broadcasting stations which they have been exposed to in their teenages. This result implies that in the new media environment, animation genre can lead the viewers' loyally and preference for the broadcasting channel for a long period.
