Endoscopic Ultrasound-Guided Transgastric Puncture and Drainage of an Adrenal Abscess in an Immunosuppressed Patient

  • Adrenal gland infection is a clinical entity of great importance, but it is a largely unrecognized pathology. Immunosuppressed individuals are at a higher risk of presentation. Herein, we describe a young female patient, recently diagnosed with HIV, who presented with severe sepsis due to methicillin-resistant Staphylococcus aureus, associated with a left adrenal abscess. She was initially treated with antibiotics; however, due to the persistence of the systemic inflammatory response and bacteremia, endoscopic ultrasound-guided drainage was performed. This procedure was successful in resolving the clinical situation. Endoscopic ultrasound-guided adrenal gland drainage can be a safe, efficacious, and minimally invasive option for managing antibiotic-refractory adrenal abscesses in immunosuppressed patients.

  • To investigate the changes of adrenal gland associated with follicular cyst in Korean native cattle, adrenals ans ovaries were collected from 54 Korean native cattle at abbatoir. Adrenals were stained by H-E stain and they were examined under light microscope. Enlargement of adrenal cortex affecting fascicular and reticular zone appeared in cows associated with follicular cyst.

Mucoepidermoid Carcinoma with Distant Metastases to the Kidney, Adrenal Gland, Skull and Gluteus Maximus Muscle: a Case Report

  • Introduction: Distant metastases of mucoepidermoid carcinoma (MEC) are reported with the most common sites being the soft tissue of skin, lung, liver, and bone. We report here a very rare case of MEC with multiple metastases to the kidney, adrenal gland, skull and gluteus maximus muscle. Case report: A 63-year-old male patient presented with left-sided headache. Radiologic evaluations including CT and MRI showed ill-defined soft tissue lesion involving the left infratemporal fossa and left sphenoid sinus, and multiple enlarged lymph nodes in neck and mediastinum. PET-CT demonstrated multiple hypermetabolic lesions in and around the left kidney, left adrenal gland, right ischium, right gluteus maximus and skull base. These lesions were confirmed as MEC with multiple metastases through biopsy. Discussion: Only one case of metastasis to the skull has been previously reported, and moreover, there has not been a case of metastatic MEC to the kidney, adrenal gland and gluteus maximus muscle so far in the medical literature. It is important to acknowledge the possibility of every unusual MEC metastases, since the presence of metastasis has statistically significant influence on the survival of MEC.

  • Anaplastic transformation of differentiated thyroid cancer at distant metastatic sites is extremely rare and has a poor prognosis. It usually occurs in the thyroid gland or cervical lymph nodes. Here we report a case of anaplastic transformation arising at multiple distant metastatic sites including the lung, liver, adrenal gland, bone, and lymph nodes in a patient 3 years after total thyroidectomy for follicular thyroid cancer.

Studies on Secretion of Catecholamine Evoked by Caffeine from the Isolated Perfused Rat Adrenal Gland

  • The influence of caffeine on secretion of catecholamines (CA) was examined in the isolated perfused rat adrenal gland. Caffeine (0.3 mM) perfused into an adrenal vein of the gland produced a marked increase in secretion of CA. This secretory effect of CA evoked by perfusion of caffeine for one minute was considerably prolonged, lasting for more than 90 minutes. The tachyphylaxis to releasing effect of CA induced by caffeine was observed by repeated perfusion of this drug. The caffeine-evoked CA secretion was markedly inhibited by pretreatment with ouabain, trifluoperazine, TMB-8 and perfusion with calcium-free Krebs solution containing 5 mM EGTA, but was not affected by perfusion of calcium-free Krebs solution without other addition. CA secretion evoked by caffeine was not reduced significantly by pretreatment with chlorisondamine but after the first collection of perfusate for 3 min was clearly inhibited. Interestingly, the caffeine-evoked CA secretion was considerably potentiated by pretreatment with atropine or pirenzepine, but after the first collection for 3 min it was markedly decreased. These experimental results suggest that caffeine causes a marked increase in secretion of CA from the isolated perfused rat adrenal gland by an extracellular calcium-independent exocytotic mechanism. The secretory effect of caffeine may be mainly due to mobilization of calcium from an intracellular calcium pool in the rat chromaffin cells and partly due to stimulation of both muscarinic and nicotinic receptors.

Influence of Quinidine on Catecholamine Secretion Evoked by Cholinergic Stimulation and Membrane Depolarization from the Perfused Rat Adrenal Gland

  • The present study was designed to investigate the effect f quinidine on catecholamine (CA) secretion evoked by ACh, high $K^{+}$, DMPP, McN-A343, cyclopiazonic acid and Bay-K-8644 from the isolated perfused rat adrenal gland and to establish the mechanism of its action. The perfusion of quinidine (15-150 $\mu$M) into an adrenal vein for 60 min produced relatively dose- and time-dependent inhibition in CA secretion evoked by ACh (5.32$\times$10$^{-3}$ M), high $K^{+}$ (5.6$\times$10$^{-2}$ M), DMPP (10$^{-4}$ M for 2 min), McN-A-343 (10$^{-4}$ M for 2 min), cyclopiazonic acid (10$^{-5}$ M for 4 min) and Bay-K-8644 (10$^{-5}$ M for 4 min). Furthermore, in adrenal glands pre-loaded with quinine (5$\times$10$^{-5}$ M), CA secretory responses evoked by veratridine (10$^{-4}$ M) was time-dependently inhibited. Also, in the presence of lidocaine (10$^{-4}$ M), which is also known to be a sodium channel blocker, CA secretory responses evoked by ACh, high potassium, DMPP, McN-A-343, Bay-K-8644 and cyclo-piazonic acid were also greatly reduced in similar fashion to that of quinidine-treatment. Taken together, these results suggest that quinidine causes greatly the inhibition of CA secretion evoked by stimulation of cholinergic (both nicotinic and muscarinic) receptors as well as by membrane depolarization, indicating strongly that this effect may be mediated by inhibiting influx of extracellular calcium and release in intracellular calcium in the rat adrenomedullary chromaffin cells. Furthermore, these findings indicate strongly that this inhibitory action of quinidine appears to be associated to the blocking action of sodium channels at least in CA secretion from the rat adrenal gland.and.

  • Adrenal venous malformation is an uncommon disease. We report a case of an incidentally diagnosed adrenal cystic mass during an evaluation for gastritis in a child. A 14-year-old girl admitted to our pediatric department for epigastric pain. After the gastroscopy, she was diagnosed with hemorrhagic gastritis. A 5.5 cm-sized cystic mass was incidentally found adjacent to her left adrenal gland during an ultrasound examination for evaluating her abdominal symptoms. She underwent laparoscopic surgery for the diagnosis and treatment of this cystic mass, which was confirmed to be venous malformation at pathologic diagnosis.

Vacor 유발당뇨 모래쥐의 부신수질의 크롬친화성세포와 신경절세포의 미세구조 (Ultrastructural Changes in the Adrenal Chromaffin Cells and Ganglion Cells in the Adrenal Gland of Vacor-Induced Diabetic Mongolian Gerbil)

  • The ultrastructural changes of adrenal chromaffin cells and ganglion cells in the adrenal gland of vacor-induced diabetic Mongolian gerbils were studied by electron microscopy. After one month of vacor-induced diabetes, some chromaffin cells were filled with dense bodies and large cytosomes with formy contents. Most of degenerating axon terminals were observed on chromaffin cells. A few macrophages were found among chromaffin cells at one month after induction. Several of these macrophages were filled with numerous phagosomes. After one month of vacor-induced diabetes, the ganglion cells showed increase in numbers of dense bodies and degenerating dendrites compared with the normal ganglion cells. Both electron dense and lucent types of degenerating axon terminals were found in interstitial space of the ganglion cells. Degenerating unmyelinated and myelinted axons contained dense and lamellar bodies. The satellite cells and macrophages with engulfed degenerated axon terminals were observed. After three months of vacor-induced diabetes, the unmyelinated and myelinated axons showed degenerative changes, whereas no structural changes could be demonstrated in adrenal ganglion and chromaffin cells. The satellite cells and macrophages containing partially digested debris were still commonly observed in the interstitial space of adrenal medulla. These results suggest that the degenerative changes occur in the adrenal ganglion cells as well as adrenal chromaffin cells of vacor-induced diabetic Mongolian gerbils.

Testosterone Propionate의 투여(投與)가 어린 숫흰쥐의 부신(副腎)에 미치는 영향(影響) (Effect of Testosterone Propionate Administration on Adrenal Gland in Immature Male Rats)

  • 정소의 기능(機能)이 부신(副腎)에 미치는 영향(影響)을 구명(究明)코져 말성숙(末成熟) 숫흰쥐에 매(每) 10일간격(日間隔)으로 20 mg씩의 testosterone propionate를 투여(投與)한 후(後) 부신(副腎)의 중량(重量) 및 조직학적(組織學的) 변화(變化)를 50일간(日間) 조사하여 정상대조군(正常對照群)과 비교(比較) 검토(檢討)한바 다음과 같은 결론(結論)을 얻었다. 부신(副腎)의 중량(重量)은 처리기간(處理期間)이 경과(經過)할수록 증가(增加)되는 결과(結果)였는데 30일후(日後)부터 유의성(有意性)이 인정(認定)되었다. 부신(副腎)에서 각층(各層)이 차지하는 비율(比率)의 변화(變化)에 있어서, 색상대(索狀帶)는 20일후(日後), 망상대(網狀帶)는 30일후(日後)부터 유의(有意)하게 증가(增加)하였으며, 부신수질(副腎髓質)은 감소(減少)하는 경향(傾向)이었는데 20일후(日後)부터 유의성(有意性) 인정(認定)되었다. 부신(副腎)의 조직학적변화(組織學的變化)는 색상폐(索狀幣)와 망상대(網狀帶)에서는 20일후(日後)부터 세포(細胞)가 비대(肥大)하여지고 공포변성(空胞變性)이 인지(認知)되었으나 피질(皮質)의 구상대(球狀帶)와 부신수질(副腎髓質)에서는 특기(特記)할 만한 변화(變化)가 인지(認知)되지 않았다.

Influence of Apamin on Catecholamine Secretion from the Rat Adrenal Medulla

  • The present study was attempted to investigate the effect of apamin on catecholamine (CA) secretion evoked by ACh, high $K^+$, DMPP, McN-A-343, cyclopiazonic acid and Bay-K-8644 from the isolated perfused rat adrenal gland and to establish the mechanism of its action. The perfusion of apamin (1 nM) into an adrenal vein for 20 min produced greatly potentiation in CA secretion evoked by ACh (5.32 $ imes$ $10^{-3}$ M), high $K^+$, (5.6 $ imes$ $10^{-2}$), DMPP ($10^{-4}$ M for 2 min), McN-A-343 ($10^{-4}$ M for 2 min), cyclopiazonic acid ($10^{-5}$ M for 4 min) and Bay-K-8644 ($10^{-5}$ M for 4 min). However, apamin itself did fail to affect basal catecholamine output. Furthermore, in adrenal glands preloaded with apamin (1 nM) under the presence of glibenclamide ($10^{-6}$ M), an antidiabetic sulfonylurea that has been shown to be a specific blocker of ATP-regulated potassium channels (for 20 min), CA secretion evoked by DMPP and McN-A-343 was not affected. However, the perfusion of high concentration of apamin (100 nM) into an adrenal vein for 20 min rather inhibited significantly CA secretory responses evoked by ACh, high $K^+$, DMPP, McN-A-343, cyclopiazonic acid and Bay-K-8644. Taken together, these results suggest that the low concentration of apamin causes greatly the enhancement of CA secretion evoked by stimulation of cholinergic (both nicotinic and muscarinic) receptors as well as by membrane depolarization. These findings suggests that apamin-sensitive SK ($Ca^{2+}$) channels located in rat adrenal medullary chromaffin cells may play an inhibitory role in the release of catecholamines mediated by stimulation of cholinergic nicotinic and muscarinic receptors as well as membrane depolarization. However, it is thought that high concentration of apamin cause the inhibitory responses in catecholamine secretion evoked by stimulation of cholinergic receptors as well as by membrane depolarization from the rat adrenal gland without relevance with the SK channel blockade.
