Objectives : Child sexual abuse is a highly prevalent societal problem that cuts across all ethnic, racial, educational, and socioeconomic groups. There has been mounting empirical evidence for the efficacy of manual-based, cognitive behavioral treatment(CBT) for sexually abused children. Therefore, the aim of this review was to address the rationale and efficacy of CBT for sexually abused children, and to introduce a broad outline of a typical CBT program for young people suffering the emotional and behavioral consequences of sexual abuse. Methods : A selective literature review was undertaken. Results : To date, trauma-focused CBT is the most effective treatment for the sexually abused child. The rationale and efficacy of CBT was reviewed and a typical CBT program was outlined session by session. Conclusion : Based on trauma-focused CBT for sexually abused children, the results demonstrate the need for future development of a standard CBT program for the Korean population.

Study Design: Retrospective study. Purpose: Lumbar intervertebral disc degeneration is an important cause of low back pain. Overview of Literature: Spinal fusion is often reported to have a good course for adolescent idiopathic scoliosis (AIS). However, many studies have reported that adjacent segment degeneration is accelerated after lumbar spinal fusion. Radiography is a simple method used to evaluate the orientation of the vertebral column. magnetic resonance imaging (MRI) is the method most often used to specifically evaluate intervertebral disc degeneration. The Pfirrmann classification is a well-known method used to evaluate degenerative lumbar disease. After spinal fusion, an increase in stress, excess mobility, increased intra-disc pressure, and posterior displacement of the axis of motion have been observed in the adjacent segments. Methods: we retrospectively secured and analyzed the data of 15 patients (four boys and 11 girls) with AIS who underwent a spinal fusion surgery. We studied the full-length view of the spine (anterior-posterior and lateral) from the X-ray and MRI obtained from all patients before surgery. Postoperatively, another full-length spine X-ray and lumbosacral MRI were obtained from all participants. Then, pelvic tilt, sacral slope, curve correction, and fused and free segments before and after surgery were calculated based on X-ray studies. MRI images were used to estimate the degree to which intervertebral discs were degenerated using Pfirrmann grading system. Pfirrmann grade before and after surgery were compared with Wilcoxon signed rank test. While analyzing the contribution of potential risk factors for the post-spinal fusion Pfirrmann grade of disc degeneration, we used generalized linear models with robust standard error estimates to account for intraclass correlation that may have been present between discs of the same patient. Results: The mean age of the participant was 14 years, and the mean curvature before and after surgery were 67.8 and 23.8, respectively (p<0.05). During the median follow-up of 5 years, the mean degree of the disc degeneration significantly increased in all patients after surgery (p<0.05) with a Pfirrmann grade of 1 and 2.8 in the L2-L3 before and after surgery, respectively. The corresponding figures at L3-L4, L4-L5, and L5-S1 levels were 1.28 and 2.43, 1.07 and 2.35, and 1 and 2.33, respectively. The lower was the number of free discs below the fusion level, the higher was the Pfirrmann grade of degeneration (p<0.001). Conversely, the higher was the number of the discs fused together, the higher was the Pfirrmann grade. Conclusions: we observed that the disc degeneration aggravated after spinal fusion for scoliosis. While the degree of degeneration as measured by Pfirrmann grade was directly correlated by the number of fused segments, it was negatively correlated with the number of discs that remained free below the lowermost level of the fusion.

Objectives : This study was conducted in order to investigate the influence of sexual trauma on the psychopathology and quality of life of children and adolescents in Korea. Methods : Twenty-seven children and adolescents and their caretakers who visited the Kangwon Sunflower Center participated in a cross-sectional study. Participants completed the Korean version of Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version, Child Behavior Checklist (CBCL), and Child Health Questionnaire-Parent Form-50 (CHQ-PF-50). Their scores were compared with those of a age and sex-matched control group of 27 healthy children and adolescents. Results : Victims of sexual violence showed higher t score in Withdrawn, Social problems, Delinquent behavior, Externalizing problems, and Total problems in CBCL, compared with control group. In CHQ-PF-50, there were lower scores on Bodily pain/discomfort, Behavior, Mental health, Time impact in parents, and Family activities subscales in victims of sexual violence. While Behavior and Family activities subscale showed negative correlations with Delinquent behavior, Externalizing problems, and Total problems in CBCL. Mental health subscale showed negative correlations with Social problems, Delinquent behavior, Externalizing problems, and Total problems. In addition, Time impact on parent subscale showed a negative association with Delinquent behavior in CBCL. Conclusion : The current study provided evidence suggesting that victims of sexual violence had a higher level of psychopathology and lower level of quality of life.
청소년에서 요추 추간판 탈출증은 드물게 발생하고, 임상적 증상이 성인과 달리 다양하게 나타난다. 또한, 청소년 환자의 주관적인 증상 호소를 분석하는 것이 어려워서 진단 및 치료가 늦어지는 경우가 많고, 성인에서와 달리 보존적 치료에 잘 반응하지 않는다. 슬괵근 강직 및 요추부 측만 변형등 청소년 요추 추간판 탈출증에서 특징적인 증상들은 수술로 잘 치료 되었고, 청소년의 추간판 부분절제술 수술 후의 중장기 추적검사에서 재수술 빈도는 15%로 성인과 큰 차이가 없었다. 그러므로, 보존적 치료에 잘 반응하지 않는 청소년 요추 추간판 탈출증은 수술적 치료를 적극적으로 고려해야 한다고 알려져 있다. 저자들은 축구를 하던 도중에 수상한 슬관절 손상으로 오인되어 수상 후 2년이 지나도록 진단이 지연되면서 슬괵근 강직과 보행 장애가 있었던 16세 청소년 요추 추간판 탈출증을 수술을 통해 치료하여 좋은 결과를 경험하였기에 문헌 고찰과 함께 보고하고자 한다.

소아기 외상이 아동의 발달에 미치는 부정적인 결과를 예방하는 resilience factor와 부정적인 결과를 더욱 증폭시키는 위험인자에 대한 지식은 이 분야의 전향적인 연구의 부재와 소아기 외상과 발달과정, 아동의 환경 등 다양한 측면들의 유기적인 관계에 대한 이해의 어려움으로 아직은 초보적인 단계이다. 이러한 소아기 외상의 연구에서의 개념적, 연구 방법론적인 어려움은 현재까지 발표된 연구들의 결과가 일관적이지 못하고 때로는 상반적이기도 한 면으로 나타나기도 한다. 이러한 어려움에도 불구하고 지금까지 발표된 연구들을 종합하여 보면 몇 가지 중요한 공통적인 결론을 도출할 수가 있다. 즉 사춘기 전에는 여아가, 사춘기 후나 영아에서는 남아가, 사회경제적 지위가 높은 가정의 아동, 기질적인 문제가 없는 경우, 쉬운 기질, 조기 이별이나 상실의 경험이 없는 경우, 외상을 받은 시기가 어릴수록, 문제 해결능력이 좋은 경우, 높은 자존감, internal locus of control, 우수한 대처 능력, 대인 관계를 인지 할 수 있는 능력, 놀이를 할 수 있는 능력, 유머 감각이 있는 경우, 유능한 부모를 가지고 있거나 혹은 적어도 한명의 보호자와 따뜻한 관계를 맺고 있을 때, 교육 정도가 높거나 조직적인 종교 활동에 참여 하고 있는 아동들은 외상을 경험하였을 때 외상의 부정적인 결과에 대하여 resilient하게 된다. 이러한 결과는 아동기 외상의 위험 및 보호인자가 서로 독립되어 있기 보다는 상호작용을 하는 유기적인 관계이며, 각 요소가 아동의 발달 단계, 아동 자신 및 그들의 가족과 환경에 따라 다양하게 작용하고, stressor의 영향은 그 강도에 따라 다양한 작용을 보이며 일부 위험요소와 보호요소는 상대방의 효과를 상승시키거나 감소시키는 등 복잡한 상호 관계를 보이는 것을 시사한다. 앞으로 소아기 외상과 발달 정신병리와의 관계, 위험 및 보호인자에 대한 포괄적인 이해 및 이를 바탕으로 효과적이며 효율적인 예방 및 치료적인 개입이 가능해지기 위해서는 stress가 정상적인 신경발달에 미치는 영향, 유전적인 소인과 부모의 요인과 같은 개개인의 소인에 대한 차이점, 신경조절의 resilience 연구, 소아기 외상의 예방과 뇌의 plasticity에 관한 연구가 같이 진행되어야 할 것이다.
Purpose: Although trauma is the most common cause of death under age 18, Korean national pediatric trauma data has lack of clinical data. This study is to prepare manpower resources, equipment, and make a correct policy decision on pediatric trauma victims Methods: The study enrolled 528 patients under age 16 with traumatic injury visited Wonju Severance Christian Hostpital Trauma Center, from February 12, 2015 to December 31, 2016. We analyzed the distribution of gender, age, place and time of the accident, injury mechanism, injury severity, and injured organ by medical record. Results: The major injury mechanisms were blunt injury in 485 (91.90%), penetrating injury in 27 (5.10%), burn in 13 (2.50%), near drowning in 2 (0.40%), and foreign body ingestion in 1 (0.20%). Ninety-seven (18.4%) patients were injured at home and 67 (12.7%) patients were injured at school. The overall mortality rate was 1.13% (n=6). 5 mortalities were related to automobile accident and one was fall down. Mean Injury Severity Score (ISS) was 4 (2, 8). No statistical significance was observed in the mean ISS between each age group. The peak time of accident occurrence was between 16 and 17 o'clock. The mean ISS was higher in blunt injury group than penetrating injury with statistical significance ($6.50{\pm}7.60$ vs. $3.00{\pm}8.10$; p<0.05). The most common injury site was upper extremity. Mean ISS was highest in thorax injury. However, mean ISS of thorax injury was higher with statistical significance only compared with face, neck and upper extremity injury. Conclusions: We reported our pediatric trauma patients data of our hospital level I trauma center, which is the only one level I trauma center of Gangwon Province. These data is useful to prevent and prepare for pediatric trauma.

Children exposed to potentially traumatic events are at risk of developing posttraumatic stress disorder (PTSD). However, the subsequent developmental course of posttraumatic stress symptoms appears to vary considerably. In this regard, some PTSD symptoms resolve without significant interventions, while for many children and adolescents, they persist until the patient receives appropriate treatment specifically designed to address PTSD and other trauma related symptoms. Evidence-based psychotherapies represent the standard of care for children with PTSD and, while psychopharmacologic interventions are utilized for many youth with posttraumatic stress symptoms and PTSD, there is little data available to guide the use of these medications in this population. However, given the structural challenges involved in disseminating and delivering evidence-based psychotherapies in all settings, prescribing clinicians should be aware of the medications whose use in children with pediatric PTSD has been studied. Herein, we review the PTSD assessment modalities, as well as the use of pharmacologic interventions in PTSD, including antiadrenergic agents, selective serotonin reuptake inhibitors and other medications.
Moyamoya disease is a cerebrovascular condition that results in the narrowing of the vessels of the circle of Willis and collateral vessel formation at the base of the brain. Although relationships between Graves' disease and cerebrovascular accidents in Moyamoya disease are obscure, the coexistence of the two diseases is noteworthy. Moyamoya disease has been rarely reported in adolescent patients with thyrotoxicosis. Recently, we encountered two adolescent Korean patients with Moyamoya disease associated with Graves' disease who presented with episodic right-sided hemiparesis and syncope. These two girls who had Graves' disease had no history of other diseases or head trauma. A thyroid function test revealed a euthyroid state and a high thyroid-stimulating hormone (TSH) receptor antibody titer at that time. The patients were diagnosed with Moyamoya disease based on brain magnetic resonance angiography and cerebral four-vessel angiography. The patients underwent cranial revascularization by encephalo-duroarterio-synangiosis as soon as a diagnosis was made, which resulted in successful symptom resolution. They fared well and had no additional neurological symptoms as of their last follow-up visits. Here, we report these two cases of confirmed Moyamoya disease complicated by Graves' disease with a review of the literature, and discuss the possible association between the two diseases. To our knowledge, this is the first report in South Korea on Moyamoya disease associated with Graves' disease in adolescents with a euthyroid.
치아외상은 현대사회에서 점차 증가하고 있으며 특히 어린이와 청소년에서 빈발한다. 그러나 치아외상의 위험성이 부각되고 있는 것에 반해, 이에 대한 국내 연구는 미미한 실정이다. 본 연구는 청소년의 치아 외상 유병률을 조사하고 위험요인을 평가하며 외상에 대한 주관적 인지정도를 파악할 목적으로 시행되었다. 양산시 소재 중학교에 재학중인 청소년 1,371명을 대상으로 임상검사 및 설문조사를 실시하여 다음과 같은 결과를 얻었다. 임상검사 상 치아 외상 유병률은 16.8%로 남자(19.2%)가 여자(13.7%)에 비해 높게 나타났고(p < 0.05), 외상 병력이 있는 사람의 일인 당 평균 외상 치아 수는 1.34개였다. 치종 별로는 상악 중절치에서, 외상의 유형에 따른 분류에서는 법랑질 파절에서 가장 높은 비중을 보였다. Class II division 1과 8.0 mm 이상의 수평피개가 외상의 위험요인으로 판단되었다(p < 0.05). 설문조사로 드러난 본인의 외상 인지도는 임상 검사의 결과와 일치하지 않았고, 치아 외상으로 인한 치료 결과에 대하여 남자가 더 높은 만족도를 나타냈다(p < 0.05).
