Effect of Vitamin C Supplementation on Iron Status of Adolescent Girls with Low Hemoglobin Levels

  • This study investigated the effects of vitamin C supplementation on the nutritional iron status of 31 adolescent girls, aged 12-15 years, with low hemoglobin levels. They were randomly divided into four groups, and for two groups daily150mg or 900mg of L-ascorbic acid(AsA) was given in three equal doses at three meals during 9 weeks. To another group daily 60mg iron as ferrous sulfate was given in the same way as AsA. The control group was given sugar placebo. Body iron status was monitored through the determination of Hb, Hct, MCHC, and serum ferritin concentrations. Dietary AsA and iron intakes were measured from food consumption surveys performed by 3-day 24-hour recalls. The amount of absorbed iron was estimated from the model of Monsen et al. The average amounts of food iron for four groups were 12.3- 15.0mg and 11.1 - 18.9mg at initial and at final period of the supplementation trial, respectively. The tentatively estimated amount of absorbed iron was significantly increased in the 900mg AsA and iron supplementing groups, but not in the 150mg AsA and placebo groups. Both Hb and MCHC were improved to above normal levels in all groups except the placebo group. Hct was elevated only in the AsA 900mg group whose Hct was relatively lower than the other groups. Serum ferritin concentrations of the four groups, which were as low as 8.50 - 14.39ng/mL on average at the intial periods, augmented significantly to 20.18ng/mL and 26.63ng/mL in the 900mg AsA and iron groups, respectively. Serum ferritin was not elevated in either the AsA 300mg group or the placebo group. The above data indicated that the daily supplementaion of 150mg AsA to the meals containing 12-15mg iron per day promoted Hb levels of adolescent girls with low Hb, and the 900mg AsA supplementing improved not only Hb level but also body iron store. A supplementation of 60mg iron per day appeared to be slightly more effective in improving the iron status in comparison to the 900mg AsA supplement. 블렌디드 러닝을 활용한 중학교 기술·가정 '청소년기 영양과 식행동' 단원의 교수·학습과정안 개발 (Development of the Teaching-Learning Process Plan for 'Adolescent Nutrition and Dietary Behavior' of Middle School Technology and Home Economics through the Use of 'Blended Learning' Teaching Method)

  • 본 연구는 청소년의 바람직한 식생활 형성을 위해 온·오프라인을 연계할 수 있는 '블렌디드 러닝'의 수업 방식을 활용하여 중학교 기술·가정① '청소년기 영양과 식행동' 단원으로 교수·학습과정안을 개발하는데 목적이 있다. 본 연구에서 개발된 교수·학습과정안은 한국교육과정평가원의 '분석-설계-개발-평가 및 수정'모형을 적용하였다. 블렌디드 러닝에 적합한 교과서 내용을 분석하고, 차시별 주제, 교수전략, 매체 및 평가도구를 선정하여 교수·학습과정안을 설계하였으며 이를 활용하여 교수·학습과정안, 활동지, 평가도구를 개발하였다. 또한, 개발된 교수·학습과정안은 전문가 검증을 받아 수정·보완하였다. 본 연구의 결과를 살펴보면, 블렌디드 러닝을 적용한 '청소년기 영양과 식행동' 단원의 교수·학습과정안은 현시점에 부합성이 있는 것으로 나타났으며 블렌디드 러닝을 적용한 온라인 수업에서는 학생들이 모둠 및 발표 활동을 제시하여 학습자들의 참여도 및 흥미가 저하되지 않도록 구성되었다. 그리고 개발된 교수·학습과정안은 수업 의도에 맞게 패들렛, 띵커벨과 같은 원격수업 도구를 사용하여 온라인 환경에서도 원활하게 수업이 진행되도록 개발되었음을 확인하였다. 본 연구에서 개발된 블렌디드 러닝을 활용한 교수·학습과정안이 온라인 수업에서 청소년기인 학생들의 올바른 식생활을 실천할 수 있는 수업으로 활용될 수 있고, 더불어 다양한 수업환경을 띠는 블렌디드 러닝 수업에서도 효과적으로 활용될 수 있기를 기대한다.

저체중인 남자 청소년의 영양상태과 식행동 특성 - 2001년 국민건강.영양조사 대상자를 중심으로 - (A study on Nutritional Status and Eating Behaviors of Underweight Adolescent Boys Using 2001 NHANS of Korea)

  • This study was performed to identify nutritional status and eating behaviors of underweight male adolescents aged 15 to 19 years. The subjects selected by using the data of KNHANS-2001 were 32 underweight boys and 135 normalweight ones. We found that weight, waist and hip circumference of the underweight group were significantly different to normal-wight group, but height and waist-hip ratio were not. Their serum indices belonged within normal ranges and showed no difference between the two groups. Their energy and nutrient intakes were mostly poor. The level and proportion of the subjects below EAR, NAR and INQ of each nutrient were not significantly different between the two groups. There were no difference of frequencies of skipping meals, snacking and eating-out between the two groups, either. When comparing frequencies of food intakes, the underweight group consumed significantly more of fermented fishes and less milk than the normal-weight group. And the former had significantly more rest/sleep, no-does and supplementation and less regular excercise than the latter. The underweight group perceived more correct self-images than the normal-weight group and they tried more to increase their body weight during weight control practice (p<0.001). It was concluded that the underweight group showed no different biochemical indices, nutrient intakes, and dietary behaviors to the normal-weight group, but they revealed significantly higher non-active activities like rest and supplementations.

청소년용 식품섭취빈도 조사지의 개발 및 타당도 검증 (The Development and Validation of a Food Frequency Questionnaire to Assess Diets of Korean Adolescents)

  • The purpose of this study was to develop and evaluate the validity of a food frequency questionnaire for Korean adolescents (FFQ-A) which could be used in clinical and epidemiological studies of the lifestyle and health of young people. The FFQ-A was designed to reflect the eating pattern of Korean adolescents, and was based on the 1998 Korean National Health and Nutrition Survey Reports. The FFQ-A had 25 (food categories. A total of 125 subjects (aged 13 to 15 years) was recruited from a randomly chosen middle school in a middle-income neighborhood in Anyang, South Korea. Each subject completed a FFQ-A, as well as a three-day dietary record. Data from 117 subjects (boys 47, girls 70) was used in the final analyses. Data on the nutrients was analyzed to estimate the Pearson correlations, Spearman rank-order correlations and agreement with categories. The validity of the FFQ-A was assessed relative to a three-day dietary record. The Pearson correlation coefficients for all the subjects were 0.94, 0.87, 0.77, 0.79, 0.49 and 0.68 for energy, carbohydrate, protein, fat, calcium, and iron, respectively. Similarly the Spearman rank-order correlation coefficients were 0.94, 0.85, 0.79, 0.81, 0.46, and 0.77 for energy, carbohydrate, protein, fat, calcium and iron, respectively. The Kappa values for energy, carbohydrate, protein, fat, calcium, and iron were 0.88, 0.67, 0.63, 0.67, 0.26, and 0.59, respectively. The percentage for misclassification of the lowest quartile into the highest quartile or vice versa ranged from 0% (energy, carbohydrate, or fat) to 16.7% (Vitamin C). Therefore the FFQ-A has a reasonable ability to assess the energy, carbohydrate, protein and fat intakes as estimated from a three-day dietary record of Korean adolescents. 여고생의 비만도에 따른 식습관과 식품섭취에 관한 연구 (A Study on Eating Habits and Food consumption pattern among High school girls)

  • This study was undertaken to assess eating habits and food consumption pattern of 365 adolescent girls in Kwangju area. Subjects were divided into 3 groups based on relative body weight as obesity index. Anthropometric data showed that mean height and weight were $161.9{\pm}19.0cm$ and $53.3{\pm}7.2kg$ respectively which are similar to those in the Korean Standard Growth data. Mean BMI and relative body weight were 20.50 and 97.1%. Age of menarche in the subjects significantly influenced the obesity index. Food habit score in the underweight group was significantly lower than that in the normal group at ${\alpha}=0.2$ level by multiple range test. Obesity was significantly associated with more frequent and irregular eating. It seems that students in the obese group were concerned on their body weight and tended to consume much vegetable. Obese group consumed more fruits, less butter and fruit juice compared to other groups. It might be suggested that more effective nutrition program might be developed and implemented to ensure good food habit of adolescent girls including obese as well as underweight group.

Lemierre Syndrome in Adolescent with Active Ulcerative Colitis

  • Inflammatory bowel disease (IBD) is a well-recognized risk factor for thrombotic events in adults but data on children are scarce. In the great majority of adult patients, thrombotic events are usually deep vein thrombosis and pulmonary embolism. Other sites such as jugular veins are extremely rare. We present a case of Lemierre syndrome in an adolescent girl with active ulcerative colitis and discuss possible risk factors. This is the first reported case of severe Lemierre syndrome with thrombus extension to cranial veins in a patient with ulcerative colitis. Early recognition of Lemierre syndrome in patients who present with rapidly worsening symptoms of neck pain, fever and signs of pharyngitis is imperative because it increases a chance of favorable prognosis. It is important for pediatricians treating IBD patients not to underestimate possible thrombotic events in children with IBD. Recognition of additional risk factors is crucial for prompt diagnosis and adequate treatment.

  • This study was conducted to provide information about weight control behavior in adolescent females. To explain the behavior intention of dieting, conceptual framework based on "Social Support, Control and the Stress Process Model" and "Theory of Reasoned Action" was used. The survey was carried out by self-questionnaires with 463 female high school and college students in Daegu. Analysis of data was done using mean, correlation and multiple regression analysis with the SAS computer program. A society preoccupied with thinness gives a burden to women, and this burden may stress dissatisfaction with body image. Social perception of ideal body image except parents' perception, and salient others'perception, and salient others' expectation of subjects' body image except parents' expectation, were much thinner than normal figures in this study. The influencing factors for behavior intention of dieting of the subjects were perceived stress and attitude toward diet behavior, especially beliefs of behavioral outcome. Influencing factors related to perceived stress-that is dissatisfaction of body image-were current figure, social perception of body image, effect of mass communication and others' estimation of subjects' body image with self-comparison with others, in order.th others, in order.

The subjects were selected using data from 2001 NHANS of Korea, and included 28 underweight and 160 normal-weight subjects. We found that the underweight group had significantly lower weight (9.8 kg), waist (6.9 cm), and hip circumference (7.2 cm) values than the normal-weight group; however height and waist-hip ratio were not different. Serum indices were within normal ranges and showed no differences between the two groups. The energy and nutrient intakes of most of the subjects were considered poor. For intake levels, the proportion of subjects below the EAR, NAR and INQ of each nutrient were also not significantly different between the groups. There were no differences in frequency for skipping meals, snacking, and eating-out between the groups. When comparing food frequency data for 62 food items, the underweight group consumed significantly more often of eggs, dried anchovies, mackerel, shellfish, and mushrooms than the normal-weight group. The former also had significantly less excercise/walking, more diet-control, and more rest/sleep than the latter. Subjects in both of two groups perceived their body images as over weight, so they practiced diet-control to reduce body weight, which is considered harmful as a healthy weight. In conclusion, the underweight group showed no differences in items of biochemical indices, nutrient intakes, and many dietary behaviors, but subjects revealed significantly higher food intake frequencies for several protein foods, as well as less excercise/walking, and more rest/sleep than the normal-weight group.

Pediatric Cholecystectomy: Clinical Significance of Cases Unrelated to Hematologic Disorders

  • Purpose: Cholecystectomy is rarely performed in the child and adolescent. However, it is associated with several conditions. This study was conducted to describe the characteristics of pediatric patient who underwent cholecystectomy unrelated to hematologic disorders, and then to suggest its clinical significance in management by comparing a simple and complicated gallbladder disease. Methods: We reviewed cases of cholecystectomy in pediatric patients (under 18 years old) at a single institution between January 2003 and October 2014. There were 143 cases during the study period and 24 were selected as the subject group. Results: There were 7 male (29.2%) and 17 female (70.8%) patients. The mean age was 13.1 years old, and 66.6% of patients were older than 12 years. Mean body weight was 52.7 kg, and body mass index was $21.7kg/m^2$, with 41.7% of patients being overweight or obese. We could identify a female predominance and high proportion of overweight or obesity in a complicated disease. There were also significantly increased levels of aspartate transaminase (AST), alanine transaminase (ALT), alkaline phosphatase (ALP) and bilirubin in this group. Most patients (87.5%) underwent laparoscopic cholecystectomy. Conclusion: Cholecystectomy for diseases unrelated to hematologic disorders is rarely performed in the child and adolescent. In general, female patients who are overweight or obese, and those older than 12 years old, require laparoscopic cholecystectomy owing to multiple gallstones. This condition has a tendency to show a complicated gallbladder disease and significantly increased levels of AST, ALT, ALP, and bilirubin.

Body Image, Eating Behavior, and Weight Control Practices among Korean Adolescent Girls

  • This study was designed to investigate body image, objective and subjective measurements of body size and shape, eating behavior, and weight control practices in 650 Korean adolescent girls; 500 of the subjects were 14 years old and enrolled in middle schools, while the remaining 150 were 17 years old and enrolled in high schools. Subjects responded anonymously to a self-administered questionnaire. The results showed that average height and weight of the subjects were 157cm and 49.5kg, respectively, while subjects ideal heights and weights averaged 164cm and 48kg, respectively. The ideal adult body heights and weights proposed by the subjects averaged 172cm and 55kg respectively. Over 50% of the subjects were dissatisfied with their body image; the degree of dissatisfaction with body image was higher in those who believed themselves to be fat. Most of the subjects wanted to lose weight and had tried to lose weight. Self-reported weight control practice methods included reducing food intake (31.9%), skipping meals (29.6%), exercise (25.8%), using slimming machines (6.9%), and using diet pills (5.1%). The heavier the subjects weight was, the greater the difference between the subjective and objective evaluations of body size. The body image scores of subjects who perceived themselves as fat were significantly lower than those of subjects who perceived themselves as normal or underweight. The difference between subjective and objective evaluations of body size, and the degree of obesity, were significantly higher in those subjects who had experience of weight control than those who had no experience of weight control. These results suggest that nutrition and health education programs for adolescent girls should be researched and implemented in order to establish a more realistic body image, positive habits in weight control, and healthful eating habits.
