Initial Management of Radiation Injuries

  • The increasing utilization of radioactive isotopes in industry, medicine and research has raised the question, 'How should hospitals deal with radiation injuries when they occur?' A system for initial management of radiation injuries has been developed by Radiation Management Corporation. Radiation injuries are classified and a treatment plan outlined for each at the emergency and short term medical care phase. This system includes clinical prognosis as well as a detailed plan for quick set up or a Radiation Emergency Area in any hospital. Procedures for patient admission, preparation of the facility, general decontamination, sample taking, and wound decontamination are included.

횡경막 이완증 1례 보 (Diaphragmatic eventration: a case report)

  • Diaphragmatic eventration is a rare condition in primary diaphragmatic diseases and is found rarely in clinical experience. Diaphragmatic eventration means abnormally high position of diaphragm, which is caused acquired, paralytic or congenital, nonparalytic etiologic origins. This report is presented a symptomatic diaphragmatic eventration of 50 years old woman, who had complained coughing and left chest pain since I year ago prior to admission in Kosin Medical College, Gospel Hospital. A patient who had established accurate diagnosis at pre-operative period. There had been post-operative course uneventfully.

외상성 하행대동맥류 수술치험 1례 (Traumatic descending aortic aneurysm -Report of one case-)

  • Rupture or laceration of the aorta is a more common result of nonpenetrating traumatic injury than is generally appreciated. If the lesion is promptly diagnosed, a appropriate surgical treatment may be life-saving. Diagnosis may be difficult and at times the rupture may remain clinically silent for variable period.< A 34 - year old male patient had sustained steering wheel injury to his chest during automobile accident 8 weeks prior to admission. The diagnosis of traumatic aneurysm of the aorta was delayed as he was asymptomatic. Surgical repair of false aneurysm of the descending aorta was successfully performed by partial cardiopulmonary bypass through the femoral artery and vein.

폐 동정맥루[치험 1례] (Pulmonary Arteriovenous Fistula - A Case Report -)

  • Pulmonary arteriovenous fistula is a rare congenital vascular malformation resulting from abnormal capillary development with incomplete formation of vascular septum normally dividing the primitive connections between the venous and arterial plexuses. Recently we have experienced a case of the bilateral pulmonary arteriovenous fistula in 7 years-old female patient. On admission, clinical manifestations were cyanosis of lips, clubbing and cyanosis of digits, and exertional dyspnea. The PO2 in arterial blood gas analysis was 43.3mmHg. In left upper and right lower lobe pulmonary arteriovenous fistulas were confirmed by bilateral pulmonary angiography. Left upper lobectomy and wedge resection of right lower lobe were performed respectively. Postoperative results were good.

빠르게 성장한 거대 종격동 양성기형종 (Rapidly Grown Huge Mediastinal Benign Teratoma ; one case report)

  • The benign teratoma is usually slow growing tumor, but we expirienced a case of primary huge mediastinal benign teratoma that had grown very rapidly, maximally during 3 years. The 14-year-old female patient was admitted to our hospital because of abnormal chest X-ray that showed 10$\times$10cm sized well definded mass with multiple calcificactions. but the mass was not present in chest X-ray perfomed on 3 years prior to admission. Under the diagnosis of teratoma, complete surgical resection was done by the left thoracotomy. The result of pathology was benign teratoma.

복강동맥류 수술치험 1례 (Aneurysm of Celiac Artery - A Report of Case -)

  • A rare case of aneurysm of the celiac artery due to arteriosclerosis was presented. The patient was 56-year-old female and had suffered from hypertension for 4 years, and recently, from dyspepsia prior to admission for 2 months. The operation was operated upon by interposition of an autogenous tubular saphenous vein graft between the proximal celiac artery and the common opening of the hepatic and the splenic arteries in the opened aneurysmal sac with inclusion technique. The postoperative course was uneventful.

  • Cervical subcutaneous emphysema usually occurs as a result of surgery or trauma. However, when it occurs spontaneously, the patient may Present with clinically impressive and dramatic features. Spontaneous subcutaneous cervical emphysema is very rare. A 20-month-old boy diagnosed bronchial asthma had been admitted and all symptoms has improved On 5th hospital admission day, sudden subcutaneous crepitation of cervical, chest and axillary area had developed, and roentgenograms of chest and neck revealed subcutaneous cervical emphysema. He was treated needle aspiration. and recovered rapidly.

식도천공 -1례 보고- (Esophageal Perforation Predisposed by Cervical Spur - 1 Case Report -)

  • The 50-year-old female patient was admitted to our hospital because of dysphagia and foreign body sensation on the neck after swallowing of solid foods 5 days ago. Esophagoscopic findings, performed on 2 days prior to admission, revealed no pathology. She had no history of preexisting esophageal disease. Under the diagnosis of the cervical esophageal perforation by routine studies such as simple chest, neck x-ray films and clinical findings, incision and drainage on the retropharyngeal space was done. Postoperatively we found the protruded degenerative spur on the 5th and 6th cervical vertebral bodies, and we considered that esophageal perforation in this case was predisposed by cervical spur. The postoperative course was uneventful.

Incarcerated pericecal hernia: a rare form of the internal hernia

  • A pericecal hernia is a rare form of an internal hernia, which has been described in several case reports. We present a case of a 32-year-old woman who complained of vague abdominal pain a day prior to admission. Abdominal computed tomography revealed the presence of a pericecal hernia without bowel ischemia. The patient underwent manual hernia reduction and was discharged without complications. We describe this case in detail and provide a review of the pertinent literature.

간호강도에 의한 환자분류도구의 신뢰도 및 타당도 검증 (Reliability and Validity Tests of Patient Classification System Based on Nursing Intensity)

  • Purpose: This study is to verify the validity and reliability of classified items and criteria of the patient classification system(PCS) based on Park's definition of nursing intensity. Methods: An expert group of 8 persons verified the content validity of the tools. The 1817 inpatients at a tertiary hospital in Seoul, Korea were classified into 4 groups according to two tools for verifying concurrent validity and interraters' reliability. These verifications were performed from September to October, 2004. Results: Nursing domains of the tools have been divided into 12 items: hygiene, nutrition, elimination, exercise & activity, education & counseling, emotional support, communication & consciousness, treatment & examination, medication, measurement & observation, coordination of multidisciplinary team, admission & discharge & transfer management. Content validity was verified by the content validity index(above 0.75 in all 12 areas). Interraters' reliability was no significant difference in the results of the patient classification between the two raters(A group 93.75%. B group 88.24%). Concurrent validity was also verified by the agreement of two tools(73.7%). Conclusion: These results showed that the reliability and validity of the PCS based on the nursing intensity were verified. These will use an data for nursing productivity in the future.
