  • Objectives : This study was to investigate the effect of low frequency electro-acupuncture (EA) at ST36 acupoint on the Freund's Complete Adjuvant(FCA)-induced arthritis in rats. Methods : Arthritis was induced by intradermal injection of FCA into base of tail. Experimental groups were divided into 4 groups; Normal, Control, ST36 and Non-Acupoint(NA). ST36 group was treated by 2 Hz EA at ST36 acupoint. Body weight, paw edema volun1e and ankle joint thickness were measured after treatment. And we investigated the effects of 2 Hz EA via WBC count, segment neurophil, lymphocyte, PGE2 assay and NADPH-d histochemistry. Results and Conclusion : The mean of body weight and ankle joint thickness of ST36 group was increased compared with control group. The mean of paw edema volume, WBC count, segment neutrophil and lymphocyte of were not decreased with control group. The mean of PGE2 concentration and NADPH-d positive neurons were changed significantly compared with control group.

Clinical Analysis of Intracranial Hemangiopericytoma

  • Objective : Intracranial hemangiopericytomas (HPCs) are rare tumors with aggressive behavior, including local recurrence and distant metastasis. We conducted this retrospective study to evaluate the efficacy of grossly total resection and adjuvant radiotherapy (RT) for these tumors. Methods : A total of 13 patients treated for intracranial HPC from January 1995 through May 2013 were included in this retrospective study. We analyzed the clinical presentations, radiologic appearances, treatment results, and follow-up outcomes, as well as reviewed other studies. Results : The ages of the patients at the time of diagnosis ranged from 26 to 73 years (mean : 48 years). The majority of the patients were male (92.3%), and the majority of the tumors were located in the parasagittal and falx. The ratio of intracranial HPCs to meningiomas was 13 : 598 in same period, or 2.2%. Seven patients (53.8%) had anaplastic HPCs. Nine patients (69.2%) underwent gross total tumor resection in the first operation without mortality. Eleven patients (84.6%) underwent postoperative adjuvant RT. Follow-up period ranged from 13 to 185 months (mean : 54.3 months). The local recurrence rate was 46.2% (6/13), and there were no distant metastases. The 10-year survival rate after initial surgery was 83.9%. The initial mean Karnofsky performance scale (KPS) was 70.8 and the final mean KPS was 64.6. Conclusion : Gross total tumor resection upon initial surgery is very important. We believe that adjuvant RT is helpful even with maximal tumor resection. Molecular biologic analyses and chemotherapy studies are required to achieve better outcomes in recurrent intracranial HPCs.

Role of adjuvant postoperative external beam radiotherapy for well differentiated thyroid cancer

  • Purpose: To analyze the outcome of adjuvant postoperative external beam radiotherapy (EBRT) in well-differentiated thyroid cancer (WDTC). Materials and Methods: We identified 84 patients treated with EBRT for WDTC from February 1981 to December 2010. Among them, we analyzed 39 patients who received EBRT after initial radical surgery. Twenty-four females and 15 males were included. The median age was 49 years (range, 16 to 72 years). There were 34 papillary thyroid carcinomas and 5 follicular thyroid carcinomas. Most patients showed pathologic T3/T4 stage (54%/26%). Ten patients (25.6%) had gross residual tumors. Five patients (12.8%) had tumor cells at the margin. The median EBRT dose and fraction size were 62.6 Gy and 1.8 to 2.0 Gy, respectively. Results: The median follow-up was 73 months (range, 21 to 372 months). The five-year overall survival (OS) and locoregional recurrence free survival (LRFS) were 97.4% and 86.9%, respectively. Locoregional failures occurred in 5 and all failure sites were the neck node area. In univariate analysis, OS was significantly influenced by invasion of the trachea (p = 0.016) or esophagus (p = 0.006). LRFS was significantly decreased by male (p = 0.020), gross residuum after resection (p = 0.002), close or positive tumor at surgical margin involvement (p = 0.044), and tracheal invasion (p = 0.040). No significant prognostic factor was identified in the multivariate analysis. No patient experienced the Radiation Therapy Oncology Group grade 3 or more toxicity. Conclusion: Our locoregional control rate of 87.2% is comparable to historical controls with surgery alone, even though our study had a large proportion of advanced stage. Adjuvant EBRT may an effective and safe treatment option in patients with WDTC.

  • Objectives : This study was designed to investigate the effects of extruded Acanthopanax Folium extracts on Complete Freund's Adjuvant (CFA) induced arthritis of rats. Methods: To induce arthritis in the ankle joint of rats, CFA was injected in the proximal part of the tail subcutaneously. After CFA injection, arthritic conditions were examined with macrography. The volume of paw edema and thickness of the ankle joints were checked regularly within 20 days. At 20 days, histopathological examination was performed on the ankle joint. Inflammation levels were determined by total WBC counts and differential WBC counts using a blood analyzer. Tumor necrosis factor-$\alpha$ and interleukin-1$\beta$ concentration in paw exudate were measured by ELISA method. Results: Several arthritic conditions induced by CFA were alleviated by Acanthopanax Folium treatment. Morphologically, reduction of arthritic conditions were observed and the volume of paw edema and thickness of the ankle joints were significantly decreased. Additionally, cytokines in paw exudate were diminished and histopathological improvement was observed. Conclusions: This study showed that the extruded Acanthopanax Folium extracts have the beneficial effects on the CFA induced arthritis of rats and might be used for chronic arthritis patients.

액와 림프절에 전이된 유방암 환자에서 수술 후 방사선치료 및 항암 약물 요법의 치료 성적 (Treatment Results of Adjuvant Radiotherapy and Chemotherapy in Breast Cancer Patients with Positive Axillary Nodes)

  • 목적 : 1983년부터 1988년 사이에 유방암으로 진단받고 근치적 유방절제술을 시행한 환자로써 조직학적으로 액와림프절 전이가 확인된 218명의 환자에서 수술 후 보조적 방사선치료 및 항암약물치료를 시행하였으며 이들 환자를 대상으로 한 치료 결과를 후향적 연구를 통해 분석하고자 하였다. 대상 및 방법 : 대상환자는 3개 군으로 분류하였는데, 1군은 항암약물치료 단독으로 치료한 80명의 환자가 포함되었고 2군은 방사선치료 단독으로 치료한 52명을 포함하였으며 3군은 방사선치료 및 항암약물치료를 병용하여 치료한 86명의 환자를 포함하였다. 이들의 평균 연령은 44세, 27$\~$70세의 연령분포를 보였으며 추적관찰 기간의 중앙값은 51개월이었다. 결과 : 7년 무병생존율 및 전체생존율은 각각 56$\%$와 67$\%$이었으며, 각 치료군에 따른 생존율은 1군에서는 50$\%$와 56$\%$, 2군은 51$\%$와 65$\%$, 3군은 62$\%$와 75$\%$이었다. 국소재발율은 13$\%$이었으며 원격전이율은 33$\%$ 이었다. 방사선치료가 포함되었던 2군과 3군에서는 국소재발 위험이 낮았으나(p<0.05) 원격전이율의 차이는 없었다. 단변량분석에서 무병 생존율에 영향을 주는 인자로서는 액와 림프절 양성 비율이었으며 전체생존율에 있어서는 원발 병소 크기, 액와 림프절 전이 개수 및 양성 비율, 병기가 관련있는 인자임을 알 수 있었다. 그러나 다변량분석에서는 치료방법만이 의미있는 예후인자임을 알 수 있었다. 전체 치료실패율과 원격전이율에 영향을 주는 예후인자는 액와 림프절 양성 비율이었으며 국소재발율은 치료방법이 의미있는 예후인자임을 알 수 있었다. 결론 : 결론적으로 이러한 결과는 폐경전 환자, 액와 림프절 전이 개수가 4개 이상이거나 양성 비율이 1/3이상인 환자에서 수술 후 방사선치료가 시행된 경우 국소재발율을 감소시킨다는 사실을 보여주고 있다. 또한 종양의 크기가 2$\~$5 cm, IIB 병기 및 액와 림프절 전이 개수가 4개 이상이거나 양성 비율이 1/3이상인 환자에서 수술후 보조적 치료로써 방사선치료/항암약물치료 병용요법이 가장 효과적인 치료방법임을 알 수 있었다.

Anti-inflammatory, Anti-arthritic and Analgesic Effect of the Herbal Extract Made from Bacopa monnieriis, Cassia fistula and Phyllanthus polyphyllus

  • Anti-inflammatory, anti-arthritic and analgesic activity of each herbal extract, which is extracted from Bacopa monnieriis, Cassia fistula and Phyllanthus polyphyllus, respectively. The treatment of herbal extract exhibited anti-inflammatory effect as a dose-dependent manner, from 1.25 mg/kg to 12.5 mg/kg, in acute inflammatory models (carrageen and egg-albumin induced rat hind paw edema). It also elicited significant anti-inflammatory activity in chronic inflammatory models (cotton pellet granuloma and Freund's adjuvant induced polyarthritis in rat). In cotton pellet granuloma test, the extract exhibited the inhibitory effect of 23 and 57% at the dose of 6.25 and 12.5 mg/kg, respectively. In Freund's adjuvant induced model, the treatment of the extract of 1.25, 6.25 and 12.5 mg/kg showed the inhibitory effect of 23, 56 and 66% at 8 days, respectively. In the acetic acid-induced model, the extract significantly reduced abdominal writhing in mice when compared to the control group, reducing the mean number of writhing from $41{\pm}2$ in the control group to $17{\pm}3$ and $15{\pm}2$ at the dose of 6.25 and 12.5 mg/kg. From these experiments, the extract, which was extracted from the combination of Bacopa monnieriis, Cassia fistula and Phyllanthus polyphyllus, (w/w/w = 1/2/1) is surprisingly found a significant analgesic and anti-inflammatory activity.

Rapid Clinical Course of Cerebral Metastatic Angiosarcoma from the Heart

  • We report here one case of rapid and aggressive course of cerebral metastatic angiosarcoma from the heart. A 36-year-old man presented with 10-days history of headache. Magnetic resonance imaging demonstrated subacute hemorrhage with a small region of enhancement in right parietal region and the pathological diagnosis was angiosarcoma. Transthoracic echocardiography demonstrated $3.2{\times}3$ cm sized mass on right atrial wall. Newly developed lesion was reoperated, three and four weeks later respectively, and whole brain radiotherapy of total 30 Gy was done. With the interval of two months, gamma knife surgery was done for new lesions two times, which were well controlled. Newly developed lesions rapidly happened even in the adjuvant treatment. He died 9 months after the diagnosis because of the aggravation of primary cancer. The cerebral metastatic angiosarcoma from the heart showed the rapid aggressive behavior and the closed follow-up could be needed for the adjuvant treatment.

The Early Detection of Recurrence of Malignant Peripheral Nerve Sheath Tumor by Frequent Magnetic Resonance Imaging

  • Surgery has a key role in the treatment of malignant peripheral nerve sheath tumors (MPNSTs), but the resectability of paraspinal MPNSTs is only 20%. Therefore, spinal MPNSTs show frequent recurrence and poor prognosis. Local recurrence is much more common than metastasis for MPNSTs, and surgery still has a key role in the treatment of local recurrence. Therefore, it is important that recurrence must be detected before resectability is lost. However, no evidence-based follow-up protocol has been established for MPNST. The authors performed gross total resection in a 34-year-old woman presented with thoracic MPNST. Adjuvant radiotherapy and chemotherapy were not administered since these adjuvant therapies generally do not improve survival in MPNST and may cause additional neurovascular damage. Instead, the authors monitored the primary site every 3 months using magnetic resonance imaging to detect local recurrence at the earliest opportunity. The tumor recurred locally on two occasions without overt symptoms at 21 and 24 months postoperatively. These recurrences were treated successfully by gross total removal.

Factors Influencing the Background Parenchymal Enhancement in Follow-Up Breast MRI after Adjuvant Endocrine Therapy

  • Purpose: To investigate factors influencing the evaluation of background parenchymal enhancement (BPE) at follow-up breast magnetic resonance imaging (MRI) after adjuvant endocrine therapy. Materials and Methods: One hundred twelve women with breast cancer and MRI of the contralateral unaffected breast before and after endocrine therapy were identified. Two readers in consensus performed blinded side-by-side comparison of BPE (minimal, mild, moderate, and marked) before and after therapy with categorical scales. Age, body mass index, menopausal status, treatment regimen (selective estrogen receptor modulator or aromatase inhibitor), chemotherapy, follow-up duration, BPE at baseline MRI, MRI field strength before and after therapy, and recurrence were analyzed for their influences on decreased BPE. Results: Younger age, premenopausal status, treatment with selective estrogen receptor modulator, MRI field strength, and moderate or marked baseline BPE were significantly associated with decreased BPE. In multivariate analysis, MRI field strength and baseline BPE showed a significant association. Conclusion: MRI field strength and baseline BPE before and after therapy .were associated with decreased BPE at post-therapy, follow-up MRI.

Clinical Implementation of Precision Medicine in Gastric Cancer

  • Gastric cancer (GC) is one of the deadliest malignancies in the world. Currently, clinical treatment decisions are mostly made based on the extent of the tumor and its anatomy, such as tumor-node-metastasis staging. Recent advances in genome-wide molecular technology have enabled delineation of the molecular characteristics of GC. Based on this, efforts have been made to classify GC into molecular subtypes with distinct prognosis and therapeutic response. Simplified algorithms based on protein and RNA expressions have been proposed to reproduce the GC classification in the clinical field. Furthermore, a recent study established a single patient classifier (SPC) predicting the prognosis and chemotherapy response of resectable GC patients based on a 4-gene real-time polymerase chain reaction assay. GC patient stratification according to SPC will enable personalized therapeutic strategies in adjuvant settings. At the same time, patient-derived xenografts and patient-derived organoids are now emerging as novel preclinical models for the treatment of GC. These models recapitulate the complex features of the primary tumor, which is expected to facilitate both drug development and clinical therapeutic decision making. An integrated approach applying molecular patient stratification and patient-derived models in the clinical realm is considered a turning point in precision medicine in GC.
