The Current Status of BCG Vaccination in Young Children in South Korea

  • Background: Delivery of Bacille Calmette-Gur$\acute{e}$in (BCG) Tokyo vaccine, with the multipuncture device, has been much preferred over BCG Pasteur, with the intradermal method, possibly due to the easier manner of administration, a desire to avoid any trouble with scars, as well as side effects and higher profits to providers in South Korea. Methods: To determine BCG scar status in 0~6 year old children vaccinated with two BCG vaccines (Pasteur BCG vaccine with intradermal method and BCG Tokyo vaccine with percutaneous method), the data from the national BCG scar survey in 2006 was analyzed. Results: Based on the national survey, the high proportion that were vaccinated with BCG Tokyo vaccines with the multipuncture method (64.5%) was noted in 0~6 year old Korean children. From inspection of scar formation, as an indicator of vaccination, the median number of the visible pin scars from the percutaneous method was 16 (interquartile range, 12~18) in the Korean children, and pin scars decreased as the age of the children increased (p<0.001). Conclusion: The findings in this survey clearly showed a growing preference of parents for the BCG Tokyo vaccines by the multipuncture method in South Korea.

Early Outcomes of COVID-19 Lung Transplantation Recipients in Korea: A Single-Center Study

  • Background: Coronavirus disease 2019 (COVID-19) has been found to cause life-threatening respiratory failure, which can progress to irreversible lung damage. Lung transplantation can be a life-saving treatment in patients with terminal lung disease (e.g., acute respiratory distress syndrome caused by infection). This study aimed to present the clinical course and results after initial lung transplantation in patients with severe COVID-19 who did not recover even with optimal medical care. Methods: From August 2019 to February 2022, this study enrolled 10 patients with COVID-19 (5 men; median age, 55.7 years) who underwent lung transplantation at a single center in Korea. All patients' characteristics, clinical pathway, overall survival, complications, and operative data were collected and analyzed. Results: Veno-venous extracorporeal membrane oxygenation or an oxygenator in a right ventricular assist device circuit was applied to 90% of the patients, and the median length of extracorporeal life support before operation was 48.5 days. There were no cases of mortality after a median follow-up of 372.8 days (interquartile range, 262.25-489 days). The major complications included the requirement for postoperative extracorporeal membrane oxygenation support in 2 cases (20%), re-transplantation in 1 case (10%), and re-exploration due to bleeding in 2 cases (20%). During the follow-up period, 3 out of 10 patients died. Conclusion: Excellent early outcomes were observed for patients who underwent lung transplantation. Thus, lung transplantation can be an effective and feasible treatment for patients with end-stage lung disease caused by COVID-19.

인플루엔자 바이러스에 의한 소아 급성 호흡기 감염증의 유행 및 임상 양상 (Clinical Analysis of Acute Respiratory Tract Infections by Influenza Virus in Children)

  • 목 적 : 국내에서도 매년 인플루엔자 바이러스가 소아기 급성 호흡기 감염증의 주요 원인 바이러스로 분리되며, 매년 인플루엔자 유사 질환 환자가 발생하고 있으나, 아직 바이러스 배양이 보편화되지 못하고 진단까지 많은 시간이 필요하여 임상 진단에 의존하는 경우가 많고 확진된 환아의 임상 연구도 부족하다. 따라서 인플루엔자 바이러스가 배양된 급성 호흡기 감염 환아에서 그 유행 및 임상 양상을 분석하였다. 방 법: 1995년 2월부터 2001년 8월까지 한림대학교 한강성심병원, 강남성심병원, 동산성심병원 소아과에 급성 호흡기 감염으로 입원한 환아들을 대상으로 비인두 흡인물을 채취하여 인플루엔자 바이러스가 분리된 환아들의 의무 기록을 후향적으로 분석하였다. 결 과 : 1) 호흡기 바이러스 배양 검사를 시행한 4,533명의 비인두 흡입물 중 997명에서 호흡기 바이러스가 배양되었고, 인플루엔자 바이러스는 164례(3.6%)로 A형이 117례(71.3%), B형이 47례(28.7%)였으며, A형과 B형이 중복 감염된 예도 1례 있었다. 다른 호흡기 바이러스와 중복 감염은 10명이었으며, RSV 4명, 파라인플루엔자 바이러스 4명, 아데노 바이러스 2명이었다. 이중 129명(A형 90명, B형 39명)의 의무 기록 고찰이 가능하였다. 2) 바이러스 분리 시기는 1995년 2월부터 3월까지 2례(B형 2례), 1995년 11월부터 1996년 6월까지 10례(A형 9례, B형 1례), 1996년 12월부터 1997년 6월까지 58례(A형 43례, B형 15례), 1997년 11월부터 1998년 4월까지 14례(A형 14례), 1998년 12월부터 1999년 4월까지 17례(A형 14례, B형 3례), 1999년 12월부터 2000년 4월까지 46례(A형 20례, B형 26례), 2000년 12월부터 2001년 4월까지 17례(A형 17례)이었다. 3) 남아 85(65.9%)명, 여아 44명(34.1%)이었고, 연령 분포는 17일부터 8년 7개월까지로 중앙 연령은 15개월(A형 13개월, B형 17개월)이었다. 12개월 미만의 영아가 51명(39.5%), 2세 미만의 영유아가 86명(66.7%)으로 대부분을 차지하였다. 4) 기저 질환을 가진 환아는 25명(19.4%)이었으며, 천식 15명, 기관지 폐이형성증을 동반한 미숙아 4명, 선천성 심장 질환 2명 등이었다. 5) 임상 진단은 폐렴 47례(36.4%), 크룹 30례(23.3%), 세기관지염 19례(14.7%), 기관기관지염 17례(13.2%), 합병증을 동반하지 않은 인플루엔자 17례(13.2%), 중이염 12례(9.3%), 천식 악화 12례(9.3%), 부비동염 7례(5.4%), 열성 경련 6례(4.7%) 순이었다. 6) 발열과 기침은 대부분의 환자에서 있었고, 가래, 비루, 식욕부진, 구토 등이 흔하였으며, 통계적으로 유의한 차이는 없었으나 A형 인플루엔자에서 더 심한 증상을 보였다. 진찰 소견은 인두 발적 70.5%, 편도비대 31.8%, 고막 충혈 12.4%, 수포음 35.7%, 천명 26.4%, 건성 수포음 24.8%, 협착음 14.0%, 흉곽 함몰 10.9%이었고, 경부 강직, 간 비대, 결막 충혈을 동반한 경우도 있었다. 7) 말초 혈액 백혈구 수는 37명(28.7%), C-반응 단백은 55명(42.6%), 적혈구 침강 속도는 78명(60.5%), AST는 30명(23.3%), ALT는 12명(9.3%), LDH는 74명(57.4%)에서 증가되었다. 8) 흉부 방사선 소견은 정상(64명, 54.2%), 폐문 주위 침윤(26명, 22.1%), 폐경화(14명, 11.9%), 과팽창(11명, 9.3%), 무기폐(6명, 5.1%), 하인두 확장(6명, 5.1%) 순이었다. 9) 평균 입원 기간은 $6.8{\pm}3.95$일(A형 $7.1{\pm}3.10$일, B형 $6.2{\pm}2.51$일)이었고, 항바이러스제를 투여한 예는 없었으며, 대부분의 환아에서 항생제 치료를 받았다. 심실 중격 결손을 동반한 다운 증후군 환아 1명이 A형에 의한 폐렴으로 기계 환기 요법을 받았으나 사망하였고, 그 외의 환아들은 모두 회복되어 퇴원하였다. 결 론: 인플루엔자 바이러스는 매년 겨울 유행이 시작되어 봄까지 지속되었고, 3년을 주기로 환자가 많이 발생하였으며, A형은 매년 분리되나 B형은 매년 분리되지 않았고, A형의 유행 이후에 B형이 유행하였다. 임상 양상은 A형에서 B형보다 더 심한 경향을 보였고, 어린 영유아나 기저 질환을 가진 소아는 심한 합병증이 발생하였기에 인플루엔자 감염의 예방과 조기 진단 및 치료가 중요하다고 하겠다.

High Incidence of Staphylococcus aureus and Norovirus Gastroenteritis in Infancy: A Single-Center, 1-Year Experience

  • Purpose: The etiology of acute gastroenteritis (AGE) has changed since the introduction of the rotavirus vaccination. The aim of this study was to clarify which common pathogens, both bacterial and viral, are currently causing AGE in infants. Methods: Infants with acute diarrhea were enrolled. We tested for 10 bacterial pathogens and five viral pathogens in stool specimens collected from infants with AGE. The clinical symptoms such as vomiting, mucoid or bloody diarrhea, dehydration, irritability, and poor oral intake were recorded, and laboratory data such as white blood cell count and C-reactive protein were collected. The clinical and laboratory data for the cases with bacterial pathogens and the cases with viral pathogens were compared. Results: Of 41 total infants, 21 (51.2%) were positive for at least one pathogen. Seventeen cases (41.5%) were positive for bacterial pathogens and seven cases (17.1%) were positive for viral pathogens. Staphylococcus aureus (13 cases, 31.7%) and Clostridium perfringens (four cases, 9.8%) were common bacterial pathogens. Norovirus (five cases, 12.2%) was the most common viral pathogen. Fever and respiratory symptoms were common in the isolated viral infection group (p=0.023 and 0.044, respectively), whereas other clinical and laboratory data were indistinguishable between the groups. Conclusion: In our study, S. aureus (41.5%) and norovirus (12.2%) were the most common bacterial and viral pathogens, respectively, among infants with AGE.

외과적 자연기흉의 임상적 고찰 (Clinical Investigation of Surgical Spontaneous Pneumothorax)

  • A clinical investigation was reported on 17 cases of spontaneous pneumothorax requiring surgical mana-gement. Males outnumbered females 15:2. Determination of the etiology in this series showed that the majority were pulmonary tuberculosis and paragonimiasis. Several others had pneumonia, lung abscess, cyst and blebs. It is of particular interest that the acute inflammation of respiratory system was younger age group, pulmonary tuberculosis & paragonimiasis were between 2 nd and 3 rd decades, and lung abscess, cyst, blebs were above 4 th decade. Pulmonary tuberculosis was far advanced bilateral and active. The ratio of right to left side was 13:6 and both side involved in 2 cases. In about half cases of patients, above 50%-collapsed lung associated with mediastinal shifting developed. The complications were pleural effusion and bronchopleural fistula. The former was 13 cases [76.4%] in which 3 cases combined with mixed infection, and latter was 5 cases. As the management, 11 cases were subjected to intercostal or rib resection drainage with continuous suc-tion. Among 11 drainage cases, 8 cases were successful in acute stage and 3 cases failed in chronic stage. This faiure was due to interference with re-expansion of collapsed lung for peel formation and broncho-pleural fistula. The open thoractomy was applied in 9 cases, among which primary operation were 5 cases and drainage failure were 4 cases. Among 11 cases subjected to the open thoracotomy, wedged resection was performed in 3 cases including paragonimiatic cyst, and pneumonectomy in 1 case-tuberculosis, and decortication only was performed in 2 cases in paragonimiasis. Decortication & lung resection was carried out in 2 patients among which ruptured lung abscess 1 case and ruptured multiple blebs 1 case. There was no case of death but prognosis of the tuberculosis may be poor because of far advanced bilateral and active pulmonary tuberculosis.

Sub-Health Status Survey and Influential Factor Analysis in Chinese during Coronavirus Disease 2019 Pandemic

  • Purpose: This study aimed to investigate sub-health status (SHS) of people living in China during the Coronavirus disease 2019 (COVID-19) COVID-19 pandemic. COVID-19 is a severe acute respiratory syndrome coronavirus (SARS-CoV) infection-induced acute infectious disease, which is featured by universal susceptibility and strong infectivity, and SHS (a status of low quality health) refers to a status of low-quality health. COVID-19 has gradually developed into a global pandemic, making the public in a high stress situation in physiological, psychological and social states in the short term. Methods: From March 6 to 11, 2020, a large-scale cross-sectional survey was conducted by convenient sampling, and SHS assessment scale was used in the questionnaire. The ordinal logistic regression analysis was used to identify the factors affecting SHS. Results: In this study, 17,078 questionnaires were delivered with 16,820 effective questionnaires collected, and 10,715 subjects (63.7%) were found with SHS, with moderate SHS primarily. Physiological sub-scale scored the highest, followed by psychological and social sub-scales. Ordinal logistic regression analysis indicated that man, only-child, workers and farmers were risk factors of SHS. Protective factors of SHS included living in rural areas and townships, laid-off retirees and education degree. Conclusion: It shows many people in China place in a poor health status during COVID-19 pandemic. It is necessary that relevant departments pay more attention to people with poor health such as men, only-child, urban people, workers and farmers, and groups with high education degree during and after pandemic stabilization.

Enterovirus 71 infection: An experience in Korea, 2009

  • Enterovirus 71 (EV71) has been recognized as a frequent cause of epidemics of hand-foot-and-mouth disease (HFMD) associated with severe neurological symptoms. In the spring of 2009, HFMD was epidemic in Korea. Severe cases with complication, including death, have been reported and it has become a public health issue. Most symptomatic EV71 infections commonly result in HFMD or herpangina. These clinical manifestations can be associated with neurologic syndromes frequently. Neurologic syndromes observed in EV71 include meningitis, meningoencephalomyelitis, poliomyelitis-like paralytic disease, Guillain-Barr$\acute{e}$ syndrome, transverse myelitis, cerebellar ataxia, opsoclonus-myoclonus syndrome, benign intracranial hypertension, and brainstem encephalitis. Examinations for EV 71 were performed from the stools, respiratory secretion or CSF of the children by realtime PCR. Gene analysis showed that most of them were caused by EV71 subgenotype C4a which was prevalent in China, 2008. Public health measures including personal and environmental hygiene, must to target daycare centers, kindergartens, and schools where highly susceptible children congregate. To prevent the spread of infection, preschools where transmission persists for more than 2 incubation periods, have been recommended for closure, and trigger criteria for voluntary closure was instituted. During closure, operators are to thoroughly clean the centers before they are allowed to reopen. In addition, parents are advised to ensure that their children adopt a high standard of personal hygiene and to keep the infected child at home until full recovery. Because the outbreaks occur in a cyclical pattern, surveillance system to predict next outbreaks and adequate public health measures to control need to be planned for future. Control of EV71 epidemics through surveillance and public health intervention needs to be maintained in Korea. Future research should focus on understanding of EV71 virulence, identification of the receptor(s) for EV71, development of antiviral agents and development of vaccine.

Regional TMPRSS2 V197M Allele Frequencies Are Correlated with COVID-19 Case Fatality Rates

  • Coronavirus disease, COVID-19 (coronavirus disease 2019), caused by SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2), has a higher case fatality rate in European countries than in others, especially East Asian ones. One potential explanation for this regional difference is the diversity of the viral infection efficiency. Here, we analyzed the allele frequencies of a nonsynonymous variant rs12329760 (V197M) in the TMPRSS2 gene, a key enzyme essential for viral infection and found a significant association between the COVID-19 case fatality rate and the V197M allele frequencies, using over 200,000 present-day and ancient genomic samples. East Asian countries have higher V197M allele frequencies than other regions, including European countries which correlates to their lower case fatality rates. Structural and energy calculation analysis of the V197M amino acid change showed that it destabilizes the TMPRSS2 protein, possibly negatively affecting its ACE2 and viral spike protein processing.

Association of Lower Socioeconomic Status and SARS-CoV-2 Positivity in Los Angeles, California

  • Objectives: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spreads heterogeneously, disproportionately impacting poor and minority communities. The relationship between poverty and race is complex, with a diverse set of structural and systemic factors driving higher rates of poverty among minority populations. The factors that specifically contribute to the disproportionate rates of SARS-CoV-2 infection, however, are not clearly understood. Methods: We evaluated SARS-CoV-2 test results from community-based testing sites in Los Angeles, California, between June and December, 2020. We used tester zip code data to link those results with United States Census report data on average annual household income, rates of healthcare coverage, and employment status by zip code. Results: We analyzed 2 141 127 SARS-CoV-2 test results, of which 245 154 (11.4%) were positive. Multivariable modeling showed a higher likelihood of SARS-CoV-2 test positivity among Hispanic communities than among other races. 코로나바이러스감염증-19 유행과 교육기관 등교 정상화 (School Closures during Coronavirus Disease 2019 Outbreak)

  • 코로나바이러스감염증-19 (coronavirus disease 2019, COVID-19)의대유행 동안 이에 대응하기 위해 학교폐쇄가 이루어졌고, 이에 대해 경제학, 사회학, 수학모델링, 역학 및 공중보건을 포함한 다양한 학문분야에서 연구가 진행되고 있다. 인플루엔자 대유행의 모형으로부터 파생된 학교 폐쇄의 COVID-19에 대한 수리 모형은 상충되는 결과들을 보였다. 초기 연구에서는 학교와 지역사회 전반에 걸친 전파를 모델링하여 개학의 위험을 평가했으나, 이 위험이 성인 또는 소아의 전파 중 어떤 측면에 인한 것인지 여부는 여전히 불분명하다. 경험적인 결과들을 종합했을 때, 학교 폐쇄가 COVID-19 유행의 통제에 명확한 효과를 보여주지 못하였으며 이는 지역사회에 따라 COVID-19 유행의 규모가 다르고, 학교 폐쇄 정책이 다양하며, 여러 가지 대응 전략이 함께 사용되었기 때문일 수 있다. 학교 폐쇄의 이익은 불분명하고 정량화하기 어려운 반면, 학교 폐쇄에 따른 교육 격차 및 사회경제적 부담 등 미래 세대에 치러야 할 잠재적인 사회적 비용이 매우 커질 것이다. 본 종설에서는 COVID-19 대유행에 대응하기 위한 학교 폐쇄의 영향에 대해 정리하고 소아청소년의 건강 영향에 대한 고찰을 통해 교육기관 등교 정상화에 대한 제언을 하고자 하였다.
