Percutaneous screw fixation and external stabilization as definitive surgical intervention for a pelvic ring injury combined with an acetabular fracture in the acute phase of polytrauma in Korea: a case report

  • Unstable pelvic ring injuries are potentially life-threatening and associated with high mortality and complication rates in polytrauma patients. The most common cause of death in patients with pelvic ring injuries is massive bleeding. With resuscitation, external fixation can be performed as a temporary stabilization procedure for hemostasis in unstable pelvic fractures. Internal fixation following temporary external fixation of the pelvic ring yields superior and more reliable stabilization. However, a time-consuming extended approach to open reduction and internal fixation of the pelvic ring is frequently precluded by an unacceptable physiologic condition and/or concomitant injuries in patients with multiple injuries. Conservative treatment may lead to pelvic ring deformity, which is associated with various functional disabilities such as limb length discrepancy, gait disturbance, and sitting intolerance. Therefore, if the patient is not expected to be suitable for additional surgery due to a poor expected physiologic condition, definitive external fixation in combination with various percutaneous screw fixations to restore the pelvic ring should be considered in the acute phase. Herein, we report a case of unstable pelvic ring injury successfully treated with definitive external fixation and percutaneous screw fixation in the acute phase in a severely injured polytrauma patient.

Effects of exercise on sleep EEG following caffeine administration (카페인 투여 후 운동이 수면에 미치는 효과)

  • The purpose of this experiment was to examine influence of acute exercise on nocturnal sleep which had been disrupted by caffeine(400mg$\times$3) thought the daytime. Six healthy young males aged 21.0$\times$0.2 yr with a history of low caffeine use. Subjects completed three conditions in a within-subject. At three conditions Sleep EEG were investigated: (1) nocturnal following quiet rest, (2) nocturnal sleep following the consumption of 1200mg of caffeine (3) nocturnal sleep following cycling at 60 min of 60% V $O_{2peak}$ with 1200mg of caffeine consumption. Sleep data were calculated for REM sleep, REM latency, sleep onset latency, sleep efficiency, sleep stages, SWS. Those data were analyzed using repeated-measures ANOVA of change scores. A main effect to, drug(caffeine) indicated that caffeine elicited sleep disturbance that is, TST and sleep onset latency increase and sleep efficiency and stage 4 decrease. The effects of exercise on sleep following caffeine intake generally improve sleep that is, stage 2, 3 and SWS increase and sleep onset latency decrease. A condition effect for sleep indicated sleep improvement after exercise Therefore The data supported a restorative theory of slow-wave sleep and suggest that acute exercise may be useful in promoting sleep and reducing sleep disturbance elevated by a high dose of caffeine.

Analysis of Autonomic Nerve System by Analyzing the Herat Rate Variability(HRV) in Chronic Musculoskeletal Pain (만성통증환자의 심박변이도(Heart Rate Variability) 분석을 통한 자율신경계 특징에 관한 연구)

  • Objectives : Chronic pain affects and is affected by physical and psychosocial factors. It also has a complicated and indefinite mechanism. This research is for clarifying relations with chronic pain, stress, and autonomic nerve system by comparing HRV and Stress Reaction Index(SRI) of chronic pain patients with those of acute pain patients. Methods : The subjects were 32 patients who visited Kangnam Kyunghee Hospital for nuchal pain or low back pain treatment from March 2008 to September 2008. Among them, 16 patients who suffered over 6 months are classified chronic pain group and 16 patients who suffered under 6 months as acute pain group. All subjects had their HRV(SA-2000E: Medicore Co..Ltd. Korea) and SRI measured at first visit. We studied the difference of HRV between two groups.(Statistics by Student t-test, p<0.05) Results : R-MSSD, TP, VLF, LF, HF and LF/HF ratio of the chronic pain group were significantly lower than those of the acute pain group. Compared with those of the acute pain group, total SRI of the chronic pain group were low but it's not significant except frustration. Conclusion : The results of HRV of the chronic pain group patients show that chronic pain is related to psychosocial factor and autonomic disturbance.

  • Delirium is an acute syndrome of disorientaion caused by dysfunction of brain and has many various symptoms. The characteristic symptoms of delirium are conscious disturbance with disorientation, dysarthria and emotional disturbance. We experienced 84-year-old man who had Wi syndrome(痿證) as well as delirium during the treatment of pneumonia, and whose condition was improved through Oriental medical treatment. The patient was diagnosed as Yangmyeong Disease(陽明病) that was caused by heat in the stomach and stool in the colon, and was treated with Daeseungki-tang (Dachengqi-tang, 大承氣湯). We were able to improve the delirium caused by high fever infection through Oriental medical treatment.

  • Delirium is a acute syndrome of disorientation caused by dysfunction of brain tissue and has a many varied symptom. The characteristic symptoms of delirium are conscious disturbance with disorientation and dys-mnesia and emotional disturbance. We experienced a 77 year-old male who had a hallucination and disorientation as well as delirium caused by antipsychotic medication, and whose condition was improved through discontinuing medication and traditional Korean medical treatment. We had given herb medication, acupuncture treatment and decreased medication to this patient, and observed. The patient's delirium symptom ad improved through oriental medical treatment and decreasing medication. It was able to improve a delirium caused by not specified origin and side effect of drug abuse through oriental medical treatment and decreasing medication.

  • Objectives & Methods: We investigated 28 books to study etiology and pathology of Son-Bal Jeorim. Result and Conclusion 1. The eiology of Son-Bal Jeorim is same as it of Bee Jeung(痺症). 2. Generally speaking, the cause of Bee Jeung was distributed Wind(風), Coldness(寒), Wetness (濕) of meridian. Bee Jeung can be devided into SilBi(實痺) and HeoBi(虛痺). In SilBi(實痺) there are PungHanSeupBi(風寒濕痺) and YeolBi(熱痺). In HeoBi(虛痺), there are GiHyeolHeoBi(氣血虛痺), EumheoBi(陰虛痺) and YangHeoBi(陽虛痺). 3. Son-Bal Jeorim belong to peripheral neuropathy in western medicine. 4. Syndrome of acute motor paralysis with variable disturbance of sensory and autonomic function, subacute sensorymotor paralysis, syndrome of chronic sensorimotor polyneuropathy, neuropathy with mitochondrial disease, syndrome of mononeuropathy or nerve plexusopathy. 5. Peripheral neuropathy is caused by carpal tunnel syndrome, diabetic neuropathy, uremic neuropathy, hepatic neuropathy, hypothyroid neuropathy, hyperthyroid neuropathy, neuropathy due to malnutrition, neuropathy due to toxic material, neuropathy due to drug, paraneoplastic neuropathy, hereditary neuropathy, etc. 6. Cerebral apoplexy, myelopathy, peripheral circulatory disturbance, anxiety syndrome cause symptoms of peripheral neuropathy

We experienced a 91 year-old woman who had a sequela of compression fracture as well as unspecified delirium, and whose condition was improved through oriental medical treatment. The patient's delirium symptom had improved through oriental medical treatment and decreasing medication. It is able to improve a delirium caused by not specified origin and side effect on drug abuse through oriental medical treatment and decreasing medication.

  • This study was done in order to investigate the etiology and pathology of dementia in the variety literature. Dementia in elderly persons(above the age of 60) mainly classfied Alzheimer disease and Cerebral vascular dementia. The results were as follows: 1. Dementia patients have abnormal mental function, who have no mental weakness but defects of memory, verbal disturbance, behavior disturbance and loss of intellectual function. 2. Dementia regard as 'me-beng(?病)', 'jeon-gwang(癲狂)', and 'heo-ro(虛勞)' in oriental medicine and the symptom is a silence with no response, mixing, a crying or a laugh, a stranger behavior and a amnesia; disturbances of speech, emotion, behavior. 3. Dementia caused by Alzheimer disease, Multi infarct dementia, Parkinson's disease, sequelae of acute CO poisoning, head injury and alcoholism(occidental medically) and the 'Dam(痰) and Damhwa(痰火), weakness of heart and spleen(心脾虛) caused by pent up anger of seven emotions(七情鬱結), the weakness of liver and kidney(肝腎不足)(oriental medically). 4. The causes of Alzheimer disease are various; a heredity factor, a morphological factor of brain tissues, a psychological factor and a biochemical factor (occidental medically) ; the 'Dam(痰) and Damhwa(痰火) caused by weakness of the internal organs and disturbance of the emotions(oriental medically). 5. Cerebral vascular dementia caused by loss of the certain cerebral neurons and oriental medically caused by obstruction of 'dam(痰)' or 'eo heul(瘀血)'. It is recommended that further study of many sided investigations, specially against a weakness of spiritual functions and a certain neurotoxin in the future.

Improvement of Control Performance of Array-Sensor System Using Soft Computing (Soft Computing을 이용한 배열 센서 시스템의 제어 성능 개선)

  • In this paper, we propose a method to obtain a linear characteristic using soft computing for systems which have array sensors of nonlinear characteristics. Also a procedure utilizing the pattern information of array sensors without additional sensors is proposed to reduce disturbance effects. For a typical example, even a single CdS cell for CdS array has nonlinear characteristics. Overall linear characteristic for CdS array is obtained using fuzzy logic for each cell and overlapped portion. In addition, further improvement for linearization is obtained applying genetic algorithms for the parameters of membership functions. Also the effect of disturbing external light changes to the CdS array can be reduced without using any additional sensors for calibration. The proposed method based on fuzzy logic shows improvements for position measurements and disturbance reduction to external light changes due to the fuzziness of the shadow boundary as well as the inherent nonlinearity of the CdS array. This improvement is shown by applying the proposed method to the ball position measurements of a magnetic levitation system.

Acute Oral Toxicity Studies of WHS-1 and WHS-2 in Rats (경구용 복합제 WHS-1,2의 급성독성시험 연구)

  • Single oral administration to SD rats of both sexes were performed to investigate the acute toxicity of two new cough and cold remedies, WHS-1 and WHS-2. WHS-1 is composed of acetaminophen, chlorpheniramine maleate, cloperastine hydrochloride, dl-methylephedrine hydrochloride, caffein anhydrous, thiamine hydrochloride, riboflavin and serratiopeptidase. WHS-2 is composed of similar formula except that thiamine hydrochloride and riboflavin is not added. The results were as follows. $LD_{50}$ values of WHS-1 were 4295.5 mg/kg for males and 4606.3 mg/kg for females, and $LD_{50}$ values of WHS-2 were 3236.7 mg/kg for males and 4360.5 mg/kg for females. Death occurred within 2~3 hours after administration at doses up to 2900 mg/kg in WHS-1 and 2500 mg/kg in WHS-2, the main cause of deaths seemed to be respiratory disturbance. General symptoms included decreased motor activity, salivation and loss of consciousness which were commonly observed in all dead animals treated with WHS-1 and WHS-2. No significant gross finding and body weight changes were observed at any dose level in the groups treated with WHS-1 and WHS-2.
