Purpose : Cerebrovascular disease is the most frequent cause of death in Korea and it remains severe disabilities disturbing normal life, According to the previous studies, mortality of the stroke in the first one week is up to the 20% and 95% of stroke patients in the acute stage are accompanied by more than one complications. These complications affect not only the acute stage mortality but also the late stage rehabilitations. In Korea the oriental medicine is preformed in the treatment of stroke. Therefore it is important to recognize thoroughly the complications in the acute stage of stroke and to prevent them. But studies about complications of acute stage stroke is rarely presented in the fields of oriental medicine. So this study is prepared for investigating the characteristics and frequency of complications in the acute stage of stroke. And we are to assess the importance of theses acute complications by systemic reviewing the previous studies. Methods : Fifty one patients are included who had been admitted to ICU(intensive care unit) of Chenju Hospital of Wonkwang Oriental Medicine. Twenty nine patients are diagnosed as ischemic stroke and twenty one patients are diagnosed hemorrhagic stroke. Medical and neurological complications were investigated retrospectively based on medical notes excluding primary symptoms of stroke Le. motor weakness, sensory disturbance and speech disorder. And risk factors of stroke such as D.M. or hypertension are excluded. Results : Medical complications are more frequent than neurological ones. Most frequent medical complication is dysuria(61%) and constipation(45%), fever(30%) and aspiration pneumonia(22%) are followed in order of frequency. In Neurological complication dysphagia(56%), the exacerbation of infarction due to increased intracranial pressure(24%), irritability or insomnia(21%) is most frequently complicated in order of frequency. Conclusions : These complications are mostly caused by bed rest state in acute stage stroke. It is supposed that more aggressive management can prevent theses ones. And it is possible to improve the medical and neurological conditions by sticking theses study results.
Single subcutaneous injection to SD rats of both sexes was performed to investigate the acute toxicity of new skin allergy-remedy ointment, Banaron. Banaron is composed of lidocaine hydrochloride, chloro-pheniramine maleate, prednisolone acetate, chlorohexidine hydrochloride, methyl salicylate, 1-menthol and d-camphor. The results were as fellows. $LD_{50}$, /TEX> values of Banaron were 8373.6 mg/kg for male and 8260.1 mg/kg for females. Death occurred within 24 hours after administration at doses up to 6600 mg/kg. The main cause of deaths seemed to be respiratory disturbance. General symptoms decreased of activity and respiratory rate, salivation, tremor and loss of consciousness which were commonly observed by some survived animals and all dead animals. No significant gross findings of internal organs and body weight changes in treatment groups in comparison with these of control group were observed at the maximum dose levels in Banaron.
The hydrodynamic instability of the three-dimensional boundary-layer over a rotating disk has been numerically investigated for three cases flows using linear stability theory (i.e. Rossby number, Ro = -1, 0, and 1). Detailed numerical values of the disturbance wave number, wave frequency, azimuth angle, radius (Reynolds number, Re) and other characteristics have been calculated for $K{\acute{a}}rm{\acute{a}}n$, Ekman and $B{\"{o}}ewadt$ boundary-layer flows. Neutral curves for these flows are presented. Presented are the neutral stability results concerning the two instability modes (Type I and Type II) by using a two-point boundary value problem code COLUEW that was based upon the adaptive orthogonal collocation method using B-spline. The prediction from the present results on both instability modes among the three cases agrees with the previously known numerical and experimental data well.

ntermediate syndrome (IMS) typically occurs at 24-96 hours following organophosphate (OP) poisoning, after an acute cholinergic crisis, but before OP-induced delayed polyneuropathy. It is characterized by proximal muscle weakness and respiratory insufficiency, which is a major contributing factor of OP-related morbidity and mortality. We report an atypical IMS case showing rapid-onset ascending paralysis and respiratory disturbance with an acute cholinergic crisis occurring 4-5 days after skin exposure to OP.
The occurrence of muscle weakness in patients with sepsis or multiple organ failure managed in the intensive care unit has been recognized with increasing frequency in the last two decades. The difficulty in examining critically ill patients may explain why this complication has been only recently recognized. This weakness is due to an axonal polyneuropathy which is called critical illness polyneuropathy(CIP). It must be differentiated from myopathy or neuromuscular junction disturbance that can also occur in the intensive care setting. Neither the cause nor the exact mechanism of CIP has been elucidated. Electrophysiological studies demonstrated an acute axonal damage of the peripheral nerves. Before the recognition of CIP, these cases were usually misdiagnosed as Guillain-$Barr{\acute{e}}$ syndrome. Clinical recovery from the neuropathy is rapid and nearly complete in those patients who survive. Thus, neuropathy acquired during critical illness, although causing a delayed in weaning from ventilatory support and hospital discharge, does not worsen long-term prognosis.
A comparative study of acid-base balance has been made between acidemia-induced hyperkalemia and hyperkalemia-induced acidemia. A group of rabbits was infused 0.1 N hydrochloric acid solution and metabolic acidosis was induced. Another group was administered 20 mM potassium chloride solution and hyperkalemia was induced. The third group was infused 0.1 N hydrochloric acid and 20 mM potassium chloride solution, simultaneously. Acid-base data and plasma potassium ion concentration were monitored every thirty minutes in these three groups of rabbits. Following results were obtained: 1 ) Along with the infusion of hydrochloric acid, acute metabolic acidosis was induced in the rabbits. Plasma bicarbonate ion concentration decreased primarily in this group. As a respiratory compensation, there was a tendency of reduction of arterial $Pco_{2}$. The alteration of data became larger along with the amount of administration and the time elapsed. However, hyperkalemia was not so severe compared with the second group. 2) In potassium chloride infused group, plasma potassium ion concentration increased along with the time elapsed and the amount of infusion. And the alteration of acid-base data was parrallel to the level of potassium ion concentration, above all depression of pH was prominent. 3) Above data suggest that when acute metabolic acidosis was induced, exchange of intracellular potassium ion with extracellular hydrogen ion seems significant for the regulation of extracellular acid-base balance. And when hyperkalemia was induced with the infusion of potassium chloride solution, the exchange of intracellular hydrogen ion with extracellular potassium ion also seems significant for the regulation of extracellular potassium balance. 4) In the group of rabbits infused hydrochloric acid and potassium simultaneously, disturbances of acid-base balance and potassium balance were much more severe than two other groups. In these mixed disturbances, the process of compensatory mechanism might be inhibited and one disturbance might aggregate each other. 5) Through above data it has been postulated that in acid-base disturbance potassium balance can be sacrificed as a compensatory mechanism, and vice versa in disturbance of potassium balance. And our data also suggest that hydrogen ion and potassium ion are compensatory pair, one another.
Cerebrovascular disease(CVD) is marked by loss of consciousness. facial palsy and hemiparesis. Although being treated properly in acute stage. CVD is accompanied by various sequelae. So treatment of sequelas is great importance as much as in acute stage. Man Geum Tang(MGT) and Bo Yang Whan O Tang(BYWOT) are representative general prescription for sequelae of CVD. This study is to clarify comparatively the efficiency of MGT and BYWOT. 110 Patients who were administrated with either MGT or BYWOT over 2 weeks for treatment of CVD divided into 2 groups:53 patients(27 males and 26 fomales. average $63.58{\pm}9.48$ years old) adminstated with MGT and 57 patients (32 males and 25 females. average $65.84{\pm}8.93$ years old) with BYWOT. In the patients administrated with MGT. 15 patients were with cerebral hemorrhage and 38 patients were with cerebral infarction. 57 BYWOT group consisted of 27 patients with cerebral hemorrhage and 30 patients with cerebral infarction. MGT with cerebral hemorrhage and 30 patients with cerebral infarction. MGT was administrated for $15.18{\pm}2.43$days and BYWOT for $14.24{\pm}1.24days$. Motor improved significantly(9〈0.005) from grade after administration in upper and lower extremities respectively. In view of comparative effeciency. the grade of motor disturbance after administration in upper extremities showed insignificant(p〉0.005) change to $1.19{\pm}1.18\;and\;0.89{\pm}1.13$ in MGT and BYWOT group respectively. In lower extremities. the grade changed insignificantly (p〉0.05) to $1.26{\pm}1.08\;and\;1.18{\pm}1.10$ respectively. In the patients group with cerebral infarction. the grade of motor disturbance in MGT and BYWOT administration groups respectively were $0.84{\pm}0.14\;and\;0.60{\pm}0.15$ in upper exemities and $0.89{\pm}0.14\;and\;0.90{\pm}0.17$ in lower extremities. The significant difference between two groups didn't appeared. In cerebral hemorrhage patients the grade of motor disturbance were in upper extremities $2.07{\pm}0.33$ in MGT group and $1.59{\pm}0.29$ in BOWOT group and $2.20{\pm}0.26\;and\;1.59{\pm}0.28$ in lower extremities respectively. The difference between two groups were insignificant(p〉0.05).
An acute fatal infectious disease in rabbits has been outbroken in Korea since 1985. This disease has been characterized as an acute hepatitis caused by viruses. However, viral pathogenesis in rabbit viral hepatitis leading to sudden death remain unclear. This report dealt with the electron microscopic findings on the spleen of experimentally infected rabbits, because spleen is one of the affected organs which have high titer of virus by a haemagglutination test. A typical crystalline array of virus was not found in the splenic cells of infected rabbits with acute hepatitis. Virus-like particles were seen within the phagosome of macrophages of the spleen. Ultrastructural changes in the spleen were severe with the lapse of time after inoculation. From these results, virus-like particles in the spleen were supposed to be phagocytosed by macrophage during viremia, while active replication of virus occurred in the liver. It was concluded that sudden death in this viral disease was caused by hepatic coma and/or circulatory disturbance.
Purpose: Peripheral neuropathy accompanied by sensory disturbance, such as limb paralysis and hemiplegia, is mainly caused by acute disseminated encephalomyelitis (ADEM). This case study aimed to determine the effect of ankle strengthening exercises that use proprioceptive neuromuscular facilitation (PNF) on the gait, balance, ankle-control ability, and sit-to-stand ability on a patient with ADEM. Methods: A 10-year-old male with quadriplegia and ankle-control impairment participated in this 4-week training intervention. The patient, diagnosed with ADEM, was treated with ankle strengthening exercises that used PNF. Results: The patient demonstrated improvements in balance, ankle-control ability, sit-to-stand ability, and gait performance. Outcome measures (manual muscle test, modified Ashworth scale, sensory assessment, coordination assessment, Berg balance scale, 5 time sit-to-stand test, and 10 m walk test) were taken before and after the training program. Conclusion: The results of this case suggest that an ankle strengthening exercise that uses PNF can improve the gait, balance, ankle-control ability, and sit-to-stand ability in patients with ADEM. In ADEM, the initial treatment is important, and the use of ankle strengthening exercises with PNF could lead to meaningful results. However, there is limited research due to an insufficient number of cases. In the future, more patients will need to be studied.
The purpose of this study was to introduce effects of Chukyu pharmacopuncture and Brainspotting on knee pain, headaches, and acute stress disorder of a patient with a traffic accident. We treated a patient with Chukyu pharmacopuncture and Brainspotting. Numerical rating scale (NRS), Range of Motion (ROM), Hyperextension Position (HEP), Effusion (Eff), Patella Compression Test (PCT), Frontal Flexion Position (FFP), McMurray (MCM), Lateral Joint Line Tenderness (LJLT), Medial Joint Line Tenderness (MJLT), Anterior Drawer Test (ADT), Lachman Test, Varus/Valgus stress, Beck Anxiety Inventory (BAI), Patient Health Questionnaire-9 (PHQ-9), Pittsburgh Sleep Quality Index (PSQI), Core Seven Emotions Inventory Short Form (CSEI-S), Subjective units of disturbance scale (SUDs), and Heart rate variability (HRV) were used to evaluate the patient. After the patient was treated by Chukyu pharmacopuncture and Brainspotting, the patient showed improvement in NRS, physical examination, and psychological symptom assessment. These results indicate that Chukyu pharmacopuncture and Brainspotting are effective on knee pain, headache, and acute stress disorder after a traffic accident.
