This study was designed to evaluate the effects of an oriental medicine(Siryung-tang) on pain caused by Post-herpetic neuralgia. We treated a 70 year-old male patient who suffered left flank pain by Post-herpetic neuralgia. As for treatment, we used Siryung-tang. After seven-day of treatment, all symptoms started to improved. This study suggests that Siryung-tang is effective in treatment of pain by Post-herpetic neuralgia.

Purpose: The aim of this study is to compare the short-term effectiveness of hard and soft stabilizing appliances (H-A and S-A) in treating patients with acute temporomandibular joint (TMJ) disc displacement without reduction (DDwoR). Methods: Eighty one patients diagnosed as acute TMJ DDwoR at oral medicine of Chosun University Dental Hospital from November 2016 to September 2017 were included in this study. The subjects were divided into three groups: The control group (34 patients treated without occlusal appliance), the H-A group (31 patients treated with H-A) and the S-A group (16 patients treated with S-A). The pain intensity using Visual Analog Scale and maximum mouth opening (MMO) were evaluated every week for 8 weeks. Results: The improvement of the pain intensity and MMO in the H-A group and the S-A group compared with the control group were noted (p<0.05). Conclusions: This study suggests that hard and S-As may be equally useful in treating patients with TMJ DDwoR. S-A could be recommended to reduce the symptoms of TMJ DDwoR in short period.

The most common and cumbersome complication of herpes zoster is postherpetic neuralgia, which typically presents as neuropathic pain. However, the painful symptoms of the postherpetic period might be associated with other causes, such as skin lesions of the herpes zoster. We report a case of a hypertrophic scar that developed in the lesion of an acute herpes zoster patient and was accompanied by pain.
Background: This study was conducted to compare the effectiveness of low-dose ketamine versus ketorolac in pain control in patients with acute renal colic presenting to the emergency department (ED). Methods: This is a double-blind randomized clinical trial. The initial pain severity was assessed using the numerical rating scale (NRS). Then, ketamine or ketorolac was administered intravenously at a dose of 0.6 mg/kg and 30 mg respectively. The pain severity and adverse drug reactions were recorded 5, 15, 30, 60, and 120 min thereafter. Results: The data of 62 subjects in the ketamine group and 64 patients in the ketorolac group were analyzed. The mean age of the patients was $34.2{\pm}9.9$ and $37.9{\pm}10.6\;years$ in the ketamine and ketorolac group, respectively. There was no significant difference in the mean NRS scores at each time point, except for the 5 min, between the two groups. Despite a marked decrease in pain severity in the ketamine group from drug administration at the 5 min, a slight increase in pain was observed from the 5 min to the 15 min. The rate of adverse drug reactions, including dizziness (P = 0.001), agitation (P = 0.002), increased systolic blood pressure (> 140 mmHg), and diastolic blood pressure (> 90 mmHg) was higher in the ketamine group. Conclusions: Low dose ketamine is as effective as ketorolac in pain management in patients with renal colic presenting to the ED. However, it is associated with a higher rate of adverse drug reactions.

PURPOSE: This study was conducted to investigate the effects of pain, balance, and left / right step time difference between elastic taping and non-elastic taping in patients with acute ankle sprain. METHODS: The subjects were patients with acute ankle sprains who had been injured within 1 day. A total of 30 subjects were divided into three groups (CG: control group, EG1: experimental group 1, EG2: experimental group 2) of 10 people. The intervention period was 3 days. In the CG, only physical therapy (cryotherapy+pulsed ultrasound) was performed, while physical therapy and elastic taping were applied in EG 1 and physical therapy and non-elastic taping were applied in EG 2. RESULTS: Changes in pain, balance and left / right step time difference following intervention decreased significantly in all three groups. After intervention, the balance between the groups differed significantly between the CG and the EGs. In addition, the left / right step time difference differed significantly between the CG and EGs, as well as between the EG 1 and the EG 2. CONCLUSION: Physiotherapy is helpful for treatment of pain associated with acute ankle sprain. Taping is considered to be a way to provide more balance and gait ability.
Objectives : The purpose of this study was to compare the effects of acupuncture and medial branch block(collaborative treatment) with just medial branch block regarding acute low back pain. Methods : Forty inpatients who received treatment between January 2012 and February 2013 were divided into 2 groups. The East-West treatment group(EW group, n=20) received one treatment of medial branch block and then acupuncture afterwards. The Western treatment group(W group, n=20) received one treatment of medial branch block. Both groups continued to receive manual therapy 3 times a week. Evaluations were made before medial branch block, 7 days after, and 14 days after using the Numerical Rating Scale(NRS). Results : Compared to before treatment, the NRS score of both EW and W groups after 7 and 14 days of treatment significantly decreased(p=0.0001). But only the EW group showed additional improvement between days 7 and 14(p=0.005). Regarding group comparison, the NRS score of the EW group was significantly lower than the W group at 7 days(p=0.037), and even more at 14 days(p<0.0001). Conclusions : Although medial branch block alone significantly improved acute low back pain, collaborative treatment with acupuncture was even more effective, with increased efficacy as time passed. Further research is recommended regarding the effects of collaborative treatment with acupuncture on acute low back pain.
Objective : The purpose of this study was to investigate the effect of acupuncture, moxibustion and whuallak-tang on acute back pain and liver function. Methods : We investigated 34 acute back pain patients. We treated them with acupuncture, moxibustion and bed rest, herbal medicine (Whuallak-tang, Huoluo-tang, 活絡湯) and physical therapy and we observed clinical effects and checked the changes of AST,ALT,ALP on admission and 1week later. Results : The result obtained is as follows. The patients were 58.8% male and 41.2% female ranging in age from 29 to 76 years. after treatment, of the 34 cases, 10(29.4%) were obtained excellent results and 23(67.7%) were good, and 1(2.9%) case was obtained fair result, with the total effective rate being 97.1%. AST,ALT,ALP was within normal range on admission and 1week later. and AST and ALP were decreased but had no statically significance.(P>0.05) Conclusion : This study suggests that acupuncture, moxibustion and Whuallak-tang are effective on acute back pain and should not induce hepatotoxicity. even though there are few toxic herbs in Oriental medicine, acupuncture and general herbal medication by a oriental doctor should not injure liver function of human.
The enzyme activities of creatine kinase (CK), its isoenzyme MB (CK-MB) and of lactate dehydrogenase isoenzyme 1 (LD-1) have been used for years in diagnosing patients with chest pain in order to differentiate patients with acute myocardial infarction (AMI) from non-AMI patients. These methods are easy to perform as automated analyses, but they are not specific for cardiac muscle damage. During the early 90's the situation changed. First, creatine kinase ME mass (CK-MB mass) replaced the measurement of CK-MB activity. Subsequently cardiac-specific proteins, troponin T (cTnT) and troponin I (cTnI) appeared and displacing LD-1 analysis. However, troponin concentrations in blood increase only from four to six hours after onset of chest pain. Therefore a rapid marker such as myoglobin, fatty acid binding protein or glycogen phosphorylase BB could be used in early diagnosis of AMI. On the other hand, CK-MB isoforms alone may also be useful in rapid diagnosis of cardiac muscle damage. Myoglobin, CK-MB mass, cTnT and cTnI are nowadays widely used in diagnosing patients with acute chest pain. Myoglobin is not cardiac-specific and therefore requires supplementation with some other analyses such as troponins to support the myoglobin value. Troponins are very highly cardiac-specific. Only the sera of some patients with severe renal failure, which requires hemodialysis, have elevated cTnT and/or cTnI without there being any evidence of cardiac damage. The latest studies have shown that elevated troponin levels in sera of hemodialysis patients point to an increased risk of future cardiac events in a similar manner to the elevated troponin values in sera of patients with unstable angina pectoris. In addition, the bedside tests for cTnT and cTnI alone- or together with myoglobin and CK-ME mass can be used instead of quantitative analyses in the diagnosis of patients with chest pain. These rapid tests are easy to perform and they do not require expensive instrumentation. For the diagnosis of patient with chest pain, routinely myoglobin and CK-ME mass measurements should be performed whenever they are requested (24 h/day) and cTnT or cTnI on admission to the hospital and then 4-6 and 12 hours later and maintained less than 10% in imprecision.
Objectives : The purpose of this study was to assess the ability of Extravascular Laser System on pain treatment. Methods : On this study, we carried out with the 9 cases of children who visited in the Department of Pediatrics, OO University Oriental Hospital as pain. 9 children suffering from pain were treated with Extravascular Laser System, and two of these were treated with combined acupuncture. In order to determine the degree of pain relief, a visual analogue scale was used. Results : Extravascular Laser System for pain resulted in a significant decline in the visual analogue scale. For Extravascular Laser System combined with acupuncture, VAS score has fallen by 30% each time compared to 17.5% of Extravascular Laser System only. In 2 cases of chronic pain, VAS score decreased to 0. In 7 cases of acute pain, All of 7 children improved 50% on VAS score, On average VAS score have seen 79.2% reduction. The average treatment times of chronic pain were 5 times, while that of acute pain were 4.1 times. Conclusions : We report the good result of the efficacy of extravascular laser system on children's pain. but, this study's cases were small in number. so more study is need.
Backgroud and Objectives: Because the pain is subjective in human, we frequently need the objective tool for estimation of pain. The Visual Analog Scale (VAS) is a method for pain grading, but it is also a subjective method. The purpose of this study is to seek an objective method for measurement of pain. Author suggests that the estimation of local body temperature may be associated with the severity of pain in tonsillar disease and the change of it measured by Digital Infrared Thermographic Imaging (DITI), may also be correlated with the change of pain. Materials and Method : Four groups were selected and measured for VAS and body temperature in DITI. Group A and B were post-tonsillectomy groups divided by age. Group C was acute tonsillitis group. Group D was peritonsillar abscess group. Results : In Group A and B, the subjective VAS was significantly correlated with objective local body temperature (correlation coefficient r=0.673, 0.647) and significant correlation was also present in acute tonsillitis and peritonsillar abscess groups(r=0.596,0.642). Conclusion : The change of pain was strongly associated with that of local body temperature in tonsillar disease. DITI is a useful method for objective grading of pain and can be used for the study of postoperatve pain and effectiveness of pain control.
