Objectives : The warm needling technique is a combination of acupuncture and moxibustion. The purpose of this study was to find the physical and thermal characteristics in order to identify the effects and mechanisms of the warm needling technique. Methods : In this study, the thermal changes were observed with a testo 845 device, which is an infra-red thermometer specifically designed for measuring a combustion of corn shaped moxibustion(moxa-corn). The thermal changes at the apex of the moxa-corn placed on the top of the an acupuncture were observed at the level of 1 cm and 2 cm from the apex to understand heat conduction through acupuncture needle for combustion of moxa-corn. Results and Conclusions : The thermal conduction through acupuncture needle from the moxa-corn was relative to the weight of moxa-corn and was inversely relative to the distance of the moxa-corn and acupuncture needle length. And the value of thermal conduction to the apex of the acupuncture needle from the moxa-corn was about $3{\sim}5^{\circ}C$. The above results suggest that the present study may be useful in finding the mechanism and effects of the warming needling technique.
Objectives : The aim of this study is to review the current trends in experimental studies on the acupuncture moxibustion treatment for visceral hypersensitivity. Methods : PubMed was searched for experimental studies about visceral hypersensitivity and acupuncture/moxibustion. Data were extracted and tabulated from the selected articles about experimental method, intervention, result and mechanism. Results : Total 23 articles were reviewed. Chronic visceral hypersensitivity animal model was applied in 17 studies (74%). Visceral hypersensitivity was measured by abdominal withdrawal reflex scoring or/and abdominal electromyogram. Acupoints like ST25, ST36, ST37, BL25, LI11, BL32 and PC6 were treated by electroacupuncture or moxibustion. All articles reported that electroacupuncture or moxibustion treatment is significantly effective in reducing visceral hypersensitivity. Treatment mechanisms were studied, related to mast cell, serotonin (5-HT) and receptor (5-HT3R and 5-HT4R), substance P (SP), vasoactive intestinal polypeptide (VIP), c-fos positive cell, corticotropin-releasing hormone (CRH), purinergic 2X (P2X)2, P2X3, P2X4, P2X7, N-methyl-D-aspartate (NMDA) receptor (NR1 and NR2B), prokinectin (PK) 1 and PK2. Conclusions : Evidences on acupuncture/moxibustion treatment for visceral hypersensitivity in animal studies warrant more research on effective acupoins, electro-acupuncture methods and treatment durations.
목적 : 침술은 동북아시아 지역의 중심에서 시작하여 2천년 이상의 역사와 함께 아시아뿐만 아니라 근-현대 전 세계의 임상의료 현장에서 크게 번창하고 있다. 이와 함께 침술의 객관화를 위하여 의생명과학 여러 방면에서 검증하는 노력이 진행되어 오고 있다. 그 과정을 통해 각종 동물을 비롯해 사람에서 침술의 다양한 효과와 기전이 점차 밝혀지고 있다. 필자는 침술 치료에서 의사와 환자가 주관적으로 경험하는 것을 설명하고, 그것의 임상적 유의성을 심신의학적 측면에서 이해하고자 하였다. 방법 : 침술과 명상에서 각각 진행되었던 뇌신경과학 분야의 여러 실험 결과들을 비교함으로써 심신 상관성이란 의미를 찾고자 하였다. 그밖에 한의학 임상영역에 시도되는 한의학과 심리학이 접목된 새로운 치료법을 정리하였다. 결과 및 결론: 침술의 치료효과와 관련해 "조기치신(調氣治神)"을 매우 중요한 개념으로 표현하고 있다. 이는 인체에 다양한 기능의 균형을 조절하고, 환자의 의식과 심리상태를 조절한다고 설명한다. 전통의학과 현대의학의 큰 차이는 현대의학은 환자와 질병을 대상화시켜 객관화 하는 노력을 추구하였고, 전통의학은 환자와 의사가 경험하는 대상을 자신과 분리하지 않는 주관적 체험을 추구하고 있다는 것이다. 그리고 전통의학은 주관적 체험을 배제하지 않고 진료에서 활용한다는 것이다. 그래서 전통의학의 역할과 의미를 이해하고자 한다면, 객관적 효과와 수량적 결과뿐만 아니라 환자와 의사의 주관적 체험과 이야기도 중요하게 다뤄져야 한다. 침술의 역할과 의미는 의사와 환자 사이에서 나타나는 행위의 모든 과정을 고려해 설명해야 할 것이다. 아마도 그 과정에서 침술이 찾는 해결의 길은 명상이 찾는 해결의 길과 만날 것이라 믿는다.
Objectives : Acupuncture and herbal medicine have been used to prevent and treat the cerebrovascular accident, such as a stroke, and many studies of acupuncture and moxibustion concerning to the stroke have been undertaken in the human and various animals. Recently, the new therapeutic tool, that is herbal acupuncture, has been developed since the 1950' and applied to various diseases including the cerebrovascular accident. The main characteristics of herbal acupuncture are a combination of acupuncture and herbal medicine. It was not well known the therapeutic effect and the mechanism of herbal acupuncture on transient forebrain ischemic injury, although it has been used frequently in clinics. Methods : In this study, effects of folium Artemisiae Argyi and moxa tar' herbal acupuncture on the $GV_{20}$, named Baek-Hue, on neuroprotection after the transient forebrain ischemia were investigated in Sprague-Dawely rats. Expressions of cFos, FosB and BDNF protein in the hippocampus and cortex were observed at 2 hrs and 48 hrs after transient forebrain ischemia by immunohis- tochemistry and ELISA technique. Results : Expression of cFos protein was increased slightly in the hippocampus and cortex at 2 hrs after transient forebrain ischemia, but FosB protein was increased highly comparing to cFos protein. However, pretreatment with folium Artemisiae Argyi or moxa tar' herbal acupuncture on $GV_{20}$ significantly increased expression of cFos protein and significantly decreased expression of FosB protein compared to control group, respectively. These features were observed in the motor cortex and retrosplenial granular cortex as well as the hippocampus. Also, pretreatment with folium Artemisiae Argyi and moxa tar' herbal acupuncture on$GV_{20}$ significantly increased the expression of BDNF protein in the hippocampus and the cortex compared to control group at 48 hrs after transient forebrain ischemia, respectively. Conclusions : These results suggest that pretreatment with folium Artemisiae Argyi or moxa tar' herbal acupuncture on $GV_{20}$ has neuroprotective effect on transient forebrain ischemia and theherbal acupuncture on $GV_{20}$ may be related to antioxidative function.
Objectives : In this study, we conducted a focus group interview among expatriates living in Korea, to investigate their experiences and perceptions of acupuncture and Korean Medicine in order to improve the usage of Korean Medicine in the medical tourism market. Methods : The inclusion criteria for the focus group interview was: (1) participants between the age of 20 to 75 years old; (2) those who had lived in Korea for more than 90 days, and (3) those who were able to express their thoughts freely in English. The participants were informed that the interviews will be recorded and transcribed. We ended recruitment once saturation of materials was met. Semi-structured interviews were conducted, and the data was analyzed after each interview. Results : The interviews were held between June to December 2016. Twenty-four participants were recruited but four dropped out and a total of twenty participants successfully finished the interviews. Six focus group interviews were conducted. Analysis revealed that Korean Medicine was relatively less known compared to Traditional Chinese Medicine or Complementary and Alternative Medicine. Participants automatically associated acupuncture with the management of pain or stress and replied that these were the areas that they thought acupuncture would be most effective. Positive experiences with acupuncture and with the acupuncture practitioners were factors that promoted the use of acupuncture whereas lack of awareness, accessibility and accreditation were factors that hindered its use. Other factors that hindered the use of acupuncture was needle phobia and the perception that acupuncture lacks scientific evidence. Conclusions : Awareness on Korean Medicine and acupuncture is low. Participants lack awareness on what diseases acupuncture can treat, the scientific evidence behind the mechanism of action, and the rigorous education system that Korean Medicine doctors must go through to get their license. Rigorous marketing should be encouraged, which includes greater exposure in the media, more honest reviews from patients, and promotion of the scientific evidence base.
Objective : We have studied the part, definition, diagnostic generaliztion and pathological viewpoint of Qi mouth and Renying by refering literatures Methods : I refered to records from anciant to modern. The results are followings : 1. Qi mouth and Renying is ancient diagnostic method and the origin of diagnostic method through the pulsation of radial artery. And Qi mouth is recognized as the right and left Qi mouth and Renying is located on pulsation points of both common carotid arteries. 2. The yin of five viscera, nutritional blood and viscous liquid are diagnosed by Qi mouth which is inclued lung meridian. And the yang of six organs, guarding Qi and the rise and fall of stomach energy are diagnosed by Renying which is inclued stomach meridian. 3. It is possible that the position, process and transformational condition of disease are decided by comparing and examining Qi mouth and Renying.
Objectives : The warm needling technique is the method in combining the effects of acupuncture needle with the effects of moxibustion. We need to standardize the characteristics of the warm needling technique in order to get more systematic and objective result in operation mechanism and effects and then get more clinical abilities in these fields. Methods : In this study, using of labview system on the warm needling technique we studied about measurement and comparison with partial temperature changes according to the position of ignition. Results & conclusion : When we measured the warm needling's partial temperature according to the position of ignition, the bottom ignition method got the higher result on the peak temperature measured at 2cm below the head than the apex ignition method.
Objectives : Crohn's disease is a severe chronic inflammation that is treated mainly by immunosuppression, which often has serious side effects. There is need to develop new therapeutic methods or drugs that have few side effects in order to treat this disease. Acupuncture with Moxi-tar at Cheonchu (ST25) has anti-inflammatory properties, but the mechanism of its anti-inflammatory actions is unclear. We investigated the protective effects and speculated the mechanisms of acupuncture with Moxi-tar at ST25 on trinitrobenzene sulfonic acid (TNBS) induced colitis in mice which is a well known Crohn's disease animal model. Methods : 5 % TNBS was treated at day 1 and day 7 into rectum of mice. To investigate therapeutic effects of acupuncture with Moxi-tar at ST25, acupuncture was carried out on day 3, and day 6. For the data analysis, we observed macroscopic and microscopic findings of the colon. Weight and width of the colon, degree of damage, changes of body weight, and myeloperoxygenase (MPO) activity were checked. For analysing protein expression, we carried out immunohistochemical staining and Western blot. For analysing mRNA expression, RT-PCR was carried out. Results : TNBS induced damages on the colon of mice, while acupuncture of Moxi-tar at ST25 suppressed TNBS mediated damages similar to those on the colons of mice in the control (not treated with TNBS) group. The average body weight of TNBS treated mice (77.4%) was decreased compared with that of the control mice (105%), and acupuncture with Moxi-tar at ST25 suppressed the loss of body weight caused by TNBS (from 77.4% to 95.3%). TNBS induced infiltration of immune cells in all layers of the colon while acupuncture with Moxi-tar at ST25 suppressed infiltration of immune cells caused by TNBS. Furthermore, acupunctured with Moxi-tar at ST25 suppressed macro-, micro- colonic damages caused by TNBS. Acupunctured with Moxi-tar at ST25 dramatically improved the clinical and histopathological symptoms such as the increase in weight of the distal colon and the MPO activity in TNBS-induced colitis. Acupuncture with Moxi-tar at ST25 down-regulated the nuclear transcription factor kappa B ($NF-{\kappa}B$) activity and suppressed tumor necrosis factor-a (TNF-${\alpha}$), interleukin-$1{\beta}$ (IL-1${\beta}$), and intracellular adhesion molecule-1 (ICAM-1) expressions caused by TNBS. Conclusions : Acupuncture with Moxi-tar at ST25 helps recovery from the TNBS-induced colonic damage by down-regulation of $NF-{\kappa}B$ activity and suppressing of TNF-${\alpha}$, IL-1${\beta}$, and ICAM-1 expressions. This may be an important method for the treatment of Crohn's disease.

Ramulus Uncariae Cum Uncis (釣鉤藤) has been claimed to possess sedative, anti-spasmodic actions, and treat childhood epilepsy in oriental medical preparation. Also, it has been stated that Ramulus Uncariae Cum Uncis was antioxidatvie effect and neuroprotection against glutamateinduced neuronal death. Methods : In this study, effects of Ramulus Uncariae Cum Uncis' herbal acupuncture on the $GV_{20}{\;}or{\;}LR_3$, named Baek-hue or Tae-chung, on neuroprotection after the transient forebrain ischemia were investigated in Sprague-Dawely rats. Expressions of cFos, FosB and BDNF protein in the hippocampus and cortex were observed at 2 hrs and 48 hrs after transient forebrain ischemia by immunohistochemistry and ELISA technique. Results : Expression of cFos protein was increased slightly in the hippocampus and cortex at 2 hrs after transient forebrain ischemia, but FosB protein was increased highly comparing to cFos protein. Also, pretreatment with Ramulus Uncariae Cum Uncis' herbal acupuncture on $GV_{20}{\;}or{\;}LR_3$ significantly increased the expression of BDNF protein in the hippocampus ($254.88{\pm}12{\;}pg/ml{\;}in{\;}GV_{20}$ group, $245.93{\pm}44.4{\;}pg/ml{\;}in{\;}LR_3$ group) and the cortex ($85.81{\pm}3.45{\;}pg/ml{\;}in{\;}GV_{20}$ group, $111.51{\pm}15.79{\;}pg/ml{\;}in{\;}LR_3$, group) compared to the hippocampus ($134.07{\pm}2.96{\;}pg/ml$) and the cortex ($61.16{\pm}4.11{\;}pg/ml$) in control group at 48 hrs after transient forebrain ischemia. Conclusion : These results suggest that pretreatment with Ramulus Uncariae Cum Uncis' herbal acupuncture on $GV_{20}{\;}or{\;}LR_3$ has neuroprotective effect on transient forebrain ischemia and the herbal acupunture on $GV_{20}{\;}or{\;}LR_3$ may be related to antioxidative effect and calcium channel block of Ramulus Uncariae Cum Uncis. Also, it could be mentioned there is specificity of acupoints treating ischemic injury through the difference between the herbal acupuncture of $GV_{20}{\;}and{\;}LR_3$.
