Validation of the Short Form Bobath Memorial Hospital Fall Risk Assessment Scale at a Specialized Geriatric Hospital in Korea (단축형 노인 낙상위험 사정도구의 타당도)

  • Purpose: This study was conducted in order to evaluate the reliability, validity, sensitivity, and specificity of the Short Form of Bobath Memorial Hospital Fall Risk Assessment Scale (BMFRAS-SF). Methods: A validation study was conducted on 207 elderly patients aged over 65 who were admitted to Bobath Memorial Hospital. Fall risk scores of BMFRAS, composed of eight subscales (age, fall history, physical activity, consciousness level, communication, fall risk factors, underlying disease, and medications) were assessed from the electronic medical record. BMFRAS-SF was derived from eight subscales of the BMFRAS representing the significance between fallers and non-fallers (fall history, physical activity, fall risk factors, underlying disease, and medications). Internal consistency reliability and interrater reliability were assessed by Cronbach's alpha and kappa coefficient. Validity was assessed by Spearman correlation analysis, factor analysis. Sensitivity, specificity, positive predictive and negative predictive values, and a receiver-operating characteristic curve (ROC) were generated. Results: Fallers had significantly higher risk scores than non-fallers in fall history, physical activity, fall risk factors, underlying disease, and medication scales. The BMFRAS-SF demonstrated acceptable Cronbach's alpha (.706) and kappa coefficients of .95. The BMFRAS-SF subscales showed good convergent validity and construct validity. The BMFRAS-SF presented good sensitivity(86.7%), specificity(67.9%), positive predictive value(42.9%) and good negative predictive value(94.8%) at a cut-off score of 5. Areas under the ROC curves were .860 for the BMFRAS and .861 for the BMFRAS-SF. Conclusion: The BMFRAS-SF was proved to be reliable and valid. It could be used for time-saving assessment and evaluation of the high risks for falls in clinical practice settings.

The Impact of University Knowledge Capital on the Local Business Activity in Korea (대학의 지식자본이 지역의 기업활동에 미치는 영향)

  • The purpose of this study is to investigate the impact of university knowledge capital on the local business activity in the Korean local governments in the year 2015. We found that the increase of the standard deviation of university patent, scientific publications, incumbent firms, and industrial property rights raised the number of new knowledge-intensive firms to 15%, 11%, 54%, and 22%, respectively, in the local economy. In addition, the coefficient which reflects the interaction effects of the knowledge capital between universities and local industries was significant in the given area in the year. On the other hand, in the control variables, the coefficients for the size of local government, population density, and percentage of 20s~40s out of the total population in the local government were significant, showing above 90% level except for the variable of the distance between a local government and Seoul. In particular, when the knowledge capital of the university and industry coexists, the synergistic effect which has influence on the foundation of knowledge-intensive firms was meaningful despite the small scale, and these two types of knowledge capital could be complementary in creating new knowledge-intensive firms in the Korean local economy.

Synthesis and Biopharmaceutical Properties of Ceftezole Butyrolactone Ester, a Novel Prodrug of Ceftezole (세프테졸 부티로락톤 에스텔의 합성 및 생물약제학적 특성)

  • Butyrolactone ester of ceftezole (CFZ-BL) was synthesized by esterification of ceftezole (CFZ) with ${\alpha}-bromo-{\gamma}-butyrolactone$. The synthesis was confirmed by spectroscopic analysis. CFZ-BL was more lipophilic than CFZ when the lipophilicity was assessed by partition coefficients between n-octanol and water at various pH. CFZ-BL itself did not show any microbiological activity in vitro, but serums taken after oral administration of CFZ-BL showed substaintial microbiological activity indicating that CFZ-BL is converted to microbiologically active metabolite, probably CFZ, in the body. The conversion was confirmed by in vitro incubation study, in which CFZ-BL was incubated in some body tissues of rabbit. Liver homogenate showed fastest conversion of CFZ-BL among the tissues tested (blood and intestine). Thus, CFZ-BL appeares to be rapidly metabolized in the liver to CFZ following oral administration. The metabolism process appears to be hydrolysis of the ester to CFZ, the parent drug of CFZ-BL. In vivo metabolism of CFZ-BL to CFZ was confirmed by analying CFZ by HPLC. CFZ concentration in the serum samples taken after oral administration of CFZ-BL were higher than those in the serum samples taken after oral administration of equivalent amount of CFZ. Oral bioavailability of CFZ-BL, a prodrug of CFZ, was 1.45-fold higher than that of CFZ in rabbits possibly due to enhanced lipophility and absorption of the prodrug.

An Experimental Study on the Bactericidal Activity of Chlorhexidine Gluconate Solution (Chlorhexidine Gluconate Solution의 殺菌效果에 관한 實驗的 연구)

  • Owing to the modification of testing methods of disinfectants or antiseptics, variations of bacteria according to characteristics of regions and resistance changes of bacteria, it is necessary that the bactericidal activities of disinfectants or antiscptics should be reevaluated nowadays. This study was carried out to reevaluate in the vitro bactericidal activity of Chlorhexidine gluconate solution. The results of experiment were summarized as follows. 1. For Chlorhexidine gluconate solution, minimal inhibitory concentrations of total bacteria taken from sewage water and Legionella bozemanii were $2.0\times 10^{-3}$%, $1.0\times 10^{-2}$%, respectively and were comparatively high. Minimal inhibitory concentration of Shigella flexneri was $1.6\times 10^{-4}$%, and was comparatively low. 2. For total bacteria taken from sewage water, it was killed within 15 minute in 0.1% Chlorhexidine gluconate solution when number of cells was $1.6\times 10^7$/ml. 3. For 0.0125% Chlorhexidine gluconate solution, decimal reduction times of Ps. aeruginosa, S. typhi, E. Coli were 45 sec, 25 sec, 18 sec repectively. For 1%, 0.125% Chlorhexidine gluconate solution, decimal reduction times of Legionella bozemanii were 10 sec, 45 sec respectively. 4. There was significant difference in the bactericidal activity of Chlorhexidine gluconate solution according to temperattire. Phenol coefficient of Chlorhexidine gluconate solution as using Staph. aureus was 100 and comparatively higher than that of other disinfectants. In comparison with other disinfectants, Legionella bozemanii was killed within 5 minutes in 0.02% KMnO$_4$ and 0.125% Chlorhexidine giuconate solution but was not killed within 3 minutes in 1% 0-cresol, 1% Phenol.

Correlation between the Pressure Pain Threshold and Sonography and Spontaneous Electrical Activity in Myofascial Trigger Points

  • Purpose: This study was designed to investigate possibilities for quantitative analysis using the electromyography and sonography. For better understanding, we evaluated the correlation between the pressure pain threshold and sonography, spontaneous electrical activity in trigger points located in the upper trapezius muscle. Methods: Thirty three active subjects volunteered to participate in this study (n=33). They had a palpable taut band, exquisite spot tenderness of a nodule in a taut band, spontaneous pain, referred pain, jump sign, local twitch response, and a painful limit to full stretch range of motion. We measured Pressure pain threshold, density, white area index, root mean square, and reaction. Pearson’s correlation coefficient was calculated to estimate the relationship between the pressure pain threshold and other variables including density, white area index, root mean square, and reaction time. Results: There were significant correlations between pressure pain threshold and density (r=-0.75, p<0.01), and between pressure pain threshold and white area index (r=-0.74, p<0.01). A significant correlations between pressure pain threshold and root mean square (r=-0.59, p<0.01). The significant correlation was found between pressure pain threshold and reaction time (r=-0.64, p<0.01). Conclusion: These should indicate whether quantitative analysis can be done using the characteristics of electromyography and sonography.

Muscle Function Path Analysis through Muscle Activity Analysis of Human Myofascial Meridians

  • Objective: The purpose of this study was to perform a muscle function path analysis of muscle function on myofascial meridians. Method: Seven male students (mean age: $22{\pm}3.46years$; mean mass: $72.71{\pm}8.19kg$; mean height: $174{\pm}4.39cm$) without a history of musculoskeletal system symptoms or injuries were recruited for this study. The measurement muscle of the myofascial line was selected along with the muscle presented in "anatomy trains (Thomas W. Myers. 2014)", and the attachment of the surface EMG (Telemyo 2400T G2, USA) pad was determined according to "EMG analysis (Kim Tae Wan et al., 2013)". The subjects underwent maximum volumetric contraction of their fascia line end muscles three times in lying and standing postures and were subjected to the maximum number of contractions of the myofascial line muscle three times in the lying and standing postures. The sampling rate of the EMG signal was set to 1,000 Hz, and the bandwidth was 20 to 350 Hz. The activity of each muscle was quantitated using the Pearson correlation coefficient, and SPSS 22.0 was used for data analysis. Results: In myofascial meridians, a positive correlation in the myofascial connection and a negative correlation in the mechanical connection were observed. Conclusion: Muscles that show significant contract correlations with one another may be expected to be used as an effective clinical marker in muscle strengthening or relaxation therapy, and rehabilitative training. In this study, the correlation of total myofascial meridians may differ without consideration of functional posture. Future studies need to consider these points.

Identification of Essential Histidines in Cyclodextrin Glycosyltransferase Isoform 1 from Paenibacillus sp. A11

  • The isoform 1 of cyclodextrin glycosyltransferase (CGTase, EC 2.4.1.19) from Paenibacillus sp. A11 was purified by a preparative gel electrophoresis. The importance of histidine, tryptophan, tyrosine, and carboxylic amino acids for isoform 1 activity is suggested by the modification of the isoform 1 with various group-specific reagents. Activity loss, when incubated with diethylpyrocarbonate (DEP), a histidine modifying reagent, could be protected by adding 25 mM methyl-$\beta$-cyclodextrin substrate prior to the modification. Inactivation kinetics of isoform 1 with DEP resulted in second-order rate constants ($k_{inactivation}$) of $29.5\;M^{-1}s^{-1}$. The specificity of the DEP-modified reaction for the histidine residue was shown by the correlation between the loss of isoform activity and the increase in the absorbance at 246 nm of N-carbethoxyhistidine. The number of histidines that were modified by DEP in the absence and presence of a protective substrate was estimated from the increase in the absorbance using a specific extinction coefficient of N-carbethoxyhistidine of $3,200\;M^{-1}cm^{-1}$. It was discovered that methyl-$\beta$-CD protected per mole of isoform 1, two histidine residues from the modification by DEP. To localize essential histidines, the native, the DEP-modified, and the protected forms of isoform 1 were digested by trypsin. The resulting peptides were separated by HPLC. The peptides of interest were those with $R_t$ 11.34 and 40.93 min. The molecular masses of the two peptides were 5,732 and 2,540 daltons, respectively. When the data from the peptide analysis were checked with the sequence of CGTase, then His-140 and His-327 were identified as essential histidines in the active site of isoform 1.

Relationships Between Leisure Competence, Leisure Flow, and Leisure Satisfaction of University Students Participating in Leisure Activities (대학생의 여가유능감과 여가몰입, 여가만족도의 관계)

  • The purpose of this study was to investigate the relationships between leisure competence, leisure flow, and leisure satisfaction of university students participating in leisure activity. The subjects were selected by stratified cluster random sampling method. They were composed of 308 students who had been leisure activity participating in university students. The Leisure competence(Ahn, 2005), Leisure flow(Lee, 2006), Leisure satisfaction(Ahn, 2009) were used for collecting data. In consequence of exploratory factor analysis, 3sub-factors(leisure competence), 5sub-factors(leisure flow), and 5sub-factor(leisure satisfaction were found. Cronbach's ${\alpha}$ coefficient were .726~.850, .537~.887, .764~.943 respectively. For the statistical analysis, SPSS 15.0 and AMOS 7.0 were utilized. The relationship between research variables were examined by the frequency, explore factor, reliability, corelation, structural equation modeling analysis. The significance level of all test was p<.05. The findings were as follows: First, leisure competence did have a positive influence on leisure flow. Second, leisure competence didn't have influence on leisure satisfaction. Final, leisure flow did have positive influence on leisure satisfactions.

Stability and Bioavailability on Prodrug of 5-Flurouracil (I).   • To assess its suitability as a prodrug of 5-fluorouracil (5-FU), 1-glycyloxymethyl-5-FU HCl (GFU), a 5-fluorouracil derivative having a glycyloxymethyl group at the N-l position was synthetized. Its physicochemical properties and hydrolysis kinetics, in aqueous solution of pH $1{\sim}10$ and in the presence of human plasma or rat liver homogenate were studied. Its acute toxicity and antitumor activity against sarcoma 180 were also examined, GFU showed higher lipid/water partition coefficient than 5-FU. The calculated $pK_{\alpha}$ values of 5-FU and GFU were 8.02 and 7,20, respectively. The decomposition rates of GFU in aqueous solution showed a pH-dependence over the pH range used, which could be ascribed to solvent catalysed hydrolysis reaction at pH lower than 4,16 and to specific hydroxide ion hydrolysis reaction at pH higher than 4,16, The half-life of GFU was 6,9 min in 80% human plasma solution and less than 3 min in rat liver homogenate at $37^{\circ}C$, The $LD_{50}$ value of 5-FU was 240 mg/kg while that of GFU was 440.6 mg/kg (226 mg as 5-FU). Both of 5FU and GFU showed a strong antitumor activity, Therapeutic ratios of 5-FU and GFU were 3.07 and 3.55, respectively.

  • The purpose of this study is to investigate the relationship of job stress and job satisfaction according to leisure activity type of nurses. The subjects were from August 1, 2017 to August 15, 2017 for nurses working in general hospitals in B, K city. Data were analyzed by frequency, percentage, mean and standard deviation, t-test, ANOVA, and Pearson's Correlation coefficient. Job stress was slightly lower than the middle level and job satisfaction was slightly higher than the middle level. Job stress was negative correlated with the number of leisure activities per week in types of leisure activity, and job satisfaction was not related. In conclusion, in order to reduce job stress of nurses, it is necessary to increase the number of leisure activities per week and further study is needed for various nurses in a wide area.
