A Patient with Spinal Accessory Neuropathy after Self Neck Massage Treated with TMJ Balancing Therapy: A Case Report (경부 자가 마사지 후 척수부신경병증을 호소하는 환자의 턱관절균형요법 치험례)

  • The purpose of this study is to describe the effects of temporomandibular joint balancing therapy on a patient with a spinal accessory neuropathy. A patient reported sternocleidomastoid muscle and trapezius muscle weakness after self-massage. The patient was instructed to wear the Accurate Balancing Appliance (ABA) for at least 8 hours per day and maintain an upright posture. The patient received cervical spine manipulation on day 2, 4, and 6. The patient's progress was tracked based on the angle at which the head turned to the right from the midline when she lifted her head while lying down, as well as the muscle strength and pain in the right trapezius muscle. The patient recovered spontaneously after 7 days. The ABA and cervical spine manipulation assisted the patient in reducing fatigue and discomfort during daily activities.

A Case Study on the Effect of Deep Bite Treatment Using TMJ Balancing Therapy on Growth (턱관절균형요법을 이용한 과개교합 치료가 성장에 미치는 영향 치험례)

  • This case is intended to present the effectiveness of TMJ balance therapy, which normalizes the yin-yang balance of the cranial nervous system, including the meridian system. Temporomandibular joint balance therapy was performed on growing children with occlusal abnormalities, and clinical results were measured through specific observation of height and weight. The patient showed improvement in deep bite and satisfactory increase in growth after treatment. From these observations, it can be inferred that temporomandibular joint balance therapy has a positive effect not only on the brain nervous system but also on the action of pituitary hormones. For a more accurate evaluation, clinical and biological research on temporomandibular joint balance therapy (TBT) is needed.

  • Objectives: This case is to report the effect of TMJ balancing therapy (TBT) and combined Korean Medicine (CKM) on a 15-year-old male patient with suspected cervical dizziness of unknown causes. Methods: He was treated with TBT and CKM included Yin-yang Balancing Therapy of TMJ, pelvic balancing therapy, cervical chuna therapy once a week for 3 months. He continued to wear accurate balancing appliance (ABA) in the mouth, did 12-way cervical gravity straightening exercise and took banhabakchulchunma-tang-gamibang for 3 months. The effects of treatment was evaluated using the numerical rating scale (NRS). Results: After the treatments, dizziness was relieved from NRS 7 to NRS 1. Conclusions: This case report suggests that TBT and CKM treatments can relieve suspected cervical dizziness of unknown causes.

The Effect of Postural Yinyang Correction of Temporomandibular Joint for Temporomandibular Disorder

  • Background: The purpose of this study was to determine the effect of Postural Yinyang correction of the temporomandibular joint (functional cerebrospinal therapy) on temporomandibular disorder. Methods: Medical records of 21 outpatients were reviewed who were diagnosed with temporomandibular joint disorder, unspecified (K0769) and treated at the Department of Acupuncture & Moxibustion, Dong-Eui University Korean Medicine Hospital from May $1^{st}$, 2017 to April $30^{th}$, 2018. Patients received more than 10 treatments of upper cervical manipulation and performed self-exercise therapy more than 3 times a day and wore an accurate balancing appliance in the oral cavity for more than 8 hours per day. To estimate the efficacy, visual analogue scale (VAS), numerical rating scale (NRS), maximum mouth opening (MMO), symptom intensity scale (SIS), max SIS (MSIS), symptom frequency scale (SFS), mandibular function impairment questionnaire (MFIQ) and 5-point Likert scale were used. Results: NRS and MSIS were significantly improved during each period. VAS, MMO, SIS, and SFS were significantly improved during each period, except the period from the $8^{th}$ to $10^{th}$ visit. MFIQ score was significantly improved during the period from the $1^{st}$ to $10^{th}$ visit. In the 5-point Likert scale, the results showed a high patient satisfaction with the treatment. Conclusion: These results showed that functional cerebrospinal therapy using an accurate balancing appliance, may be useful for reducing the symptoms of temporomandibular disorder.

  • Objectives : The purpose of this study is to find out the effect of Postural Yinyang Correction of TMJ(Functional Cerebrospinal Therapy; FCST) on Bell's palsy. Methods : We reviewed the medical records of 41 patients who were treated for Bell's palsy at the Dept. of Acupuncture and Moxibustion, Dong-Eui University from January $1^{st}$, 2016 to August $31^{st}$, 2016. The patients were divided into 2 groups: Group A(n=21), and Group B(n=20). Patients in Group A were treated by Korean Medical Treatments with FCST using ABA(Accurate Balancing Appliance), a standard intra-oral appliance. Patients in Group B were treated by Korean Medical Treatments without FCST. To estimate the efficacy in recovering palsy, we analyzed the House-Brackmann Grading Scale and Yanagihara's Unweighted Grading System. Results : In both groups, H-B grades were improved significantly during each period except the period from the first visit to one week later(P01). The improvement of H-B grade during each period in Group A is higher than Group B except the period from one week later to two weeks later, but statistically insignificant. In Group A, Y-score was significantly improved during each period except during P01. In group B, Y-score was significantly improved during each period except during P01 and the period from the first visit to two weeks later. The improvement of Y-score during each period in Group A is higher than Group B, but statistically insignificant. Conclusions : FCST using Standard Intraoral Appliance may be helpful in treating Bell's Palsy, but the further researches are needed.

The Effect of Korean Medical Treatments with Functional Cerebrospinal Therapy on Patients from Traffic Accidents with Temporomandibular Joint Disorder (기능적 뇌척주요법을 병행한 한방치료가 턱관절 장애가 있는 교통사고 환자에 미치는 영향)

  • Objectives : The purpose of this study is to determine the effect of Functional Cerebrospinal Therapy(FCST) on patients from traffic accidents with Temporomandibular Joint Disorder. Methods : We reviewed the medical records of 33 patients who were treated for injuries stemming from traffic accidents at the Dong-Eui University Korean Medicine Hospital from December 29th, 2015 to August 29th, 2016. The patients were divided into 2 groups: Group A(n = 15) and Group B(n = 18). In Group A, patients received Korean medical treatments with FCST using an Accurate Balancing Appliance(ABA) - a standard intra-oral appliance. In Group B, patients received Korean medical treatments without FCST. To estimate the efficacy of the treatments in decreasing pain, we analyzed the Visual Analogue Scale(VAS), Neck Disability Index(NDI) and Oswestry Disability Index(ODI). Results : In Group A, VAS of headache(VAS H), VAS of neck & nuchal pain(VAS N), VAS of shoulder pain(VAS S), and VAS of low back pain(VAS L) were significantly improved during each period. In Group B, VAS H was significantly improved during each period, except the period from the first visit to two weeks later. VAS N, S, L were significantly improved during each period, except the period from the first visit to one week later. The total VAS improvement during each period in Group A was significantly higher than Group B. In both groups, NDI and ODI were improved significantly during each period. The improvement of NDI and ODI during each period in Group A was significantly higher than Group B. Conclusion : According to the results, FCST using ABA may be an effective treatment for patients from traffic accidents.

The Effect of Korean Medical Treatment with Postural Yinyang Correction of Temporomandibular Joint on Chronic Low Back Pain (턱관절 자세 음양 교정술을 병행한 한방치료가 만성요통에 미치는 영향)

  • Objectives : The purpose of this study was to examine whether or not Postural Yinyang Correction of TMJ(Temporomandibular joint) using Standard intraoral appliance could decrease chronic low back pain. Methods : We reviewed the medical records of 40 patients with TMJ disorder aged from 30 to 70 years from 124 patients who were treated in chronic low back pain at Department of Acupuncture & Moxibustion of Korean Medicine Hospital of Dong-Eui University from November $1^{st}$, 2015 to June $5^{th}$, 2016. The patients were divided into 2 groups: Group A and Group B. In Group A, we treated the patients with acupuncture, sweet bee venom treatment and FCST(Functional Cerebrospinal Therapy) through Standard intraoral appliance from ABA(Accurate Balancing Appliance), Full body exercise therapy, and Upper cervical alignment balance therapy. In Group B, we treated the patients with acupuncture and sweet bee venom treatment. To estimate the effectiveness in controlling pain, we analyzed the Visual analog scale(VAS). To evaluate functional change of patients, we analyzed the Oswestry low back disability questionnaire(ODI). Results : In both groups, VAS and ODI decreased significantly. In the VAS, the result of Group A is significantly more effective than that of Group B. In the ODI, the result of Group A is statistically less effective compared to that of Group B, but not significantly. Conclusions : According to the results, Postural Yinyang Correction of TMJ using Standard intraoral appliance may be effective treatment for low back pain, but the further researches are needed.
