Severe congenital neutropenia mimicking chronic idiopathic neutropenia: a case report

  • Severe chronic neutropenia is classified as severe congenital, cyclic, autoimmune, or idiopathic. However, there is a lot of uncertainty regarding the diagnosis of severe congenital neutropenia (SCN) and chronic idiopathic neutropenia, and this uncertainty affects further evaluations and treatments. A 20-year-old man presented with fever and knee abrasions after a bicycle accident. On admission, his initial absolute neutrophil count (ANC) was 30/µL. He had no medical history of persistent severe neutropenia with periodic oscillation of ANC. Although his fever resolved after appropriate antibiotic therapy, ANC remained at 80/µL. Bone marrow (BM) aspiration and biopsy were performed, and a BM smear showed myeloid maturation arrest. Moreover, genetic mutation test results showed a heterozygous missense variant in exon 4 of the neutrophil elastase ELANE: c597+1G>C (pV190-F199del). The patient was diagnosed with SCN. After discharge, we routinely checked his ANC level and monitored any signs of infection with minimum use of granulocyte colony-stimulating factor (G-CSF), considering its potential risk of leukemic transformation. Considering that SCN can be fatal, timely diagnosis and appropriate management with G-CSF are essential. We report the case of a patient with SCN caused by ELANE mutation who had atypical clinical manifestations. For a more accurate diagnosis and treatment of severe chronic neutropenia, further studies are needed to elucidate the various clinical features of ELANE.

A Study on the Setting of Regional Oil Recovery Capacity On Water in Korea (우리나라 지역별 해상 기름회수능력 설정에 관한 연구)

  • In this study, the regional states of 7 items are analyzed, the regional risks are calculated by using normalized data & analysis hierarchy process to set regional oil recovery capacity. Area on-water oil recovery capacity, $7,500k{\ell}$, is separated and regional on-water oil recovery capacity is determined, based on calculated regional degree of risk. Excessive current oil recovery capacities, setting in Incheon, Gunsan, Mokpo, Busan region, are as a result distributed to the other regions in each area. In case of central region, Daesan is increased as much as $1,475k{\ell}$, Yeosu is increased as much as $375k{\ell}$, Ulsan is increased as much as $475k{\ell}$. The regional on-water oil recovery capacity, considering both the cause of accident aspects and marine environmental & economic aspects, is estimated as more balanced distribution model, compared to current standard of on-water oil recovery capacity.

  • Reviewing the statistics of harbor shipment of hazardous and noxious substance and the past spill accidents of HNS enabled us to assess the potential risks of transportation of HNS through the major harbors in Korea. Ulsan, Yeosu, Daesan port are relatively evaluated high risk in fire, health and environment disaster. Through the analysis of domestic preparedness to HNS accidents, the supplement of expertise resource to respond the vulnerability of the explosion, the fire and the physiological hazard, is required. Based on this risk assessment and review of the advanced case of Japan in building up HNS response system, a resource model at a national level was proposed which differentiates the sea areas for the proper allocation of resources to respond effectively to HNS accidents in the future.

  • In this paper, the traceability system for rice which is based on RFID technology is introduced. The traceability system plays an important role in the quality management for rice and rice-related food since this system can provide the information of all food chain (production, treatment, distribution and sales etc.) and more quickly identify the cause of the rice and rice-related accident. The main result of the this paper is the basic structure for the traceability system based on RFID technology. To show the implementation case based on this basic structure, the effectiveness of the traceability system is verified. In addition, the prerequisites condition for effective implementation is proposed.

Analysis of premature death of Sprague-Dawley rats in carcinogenicity studies

  • To help the interpretation of causes of death, it is critical that the background incidence of factors contributing to death be recorded and archived. Information was gathered from the control groups of 19 rat carcinogenicity studies. All cases of death occurring within the 2-year period were reviewed. Out of 1124 males and 1084 females, 720 male (64.1%) and 689 female (63.6%) decedents were recorded. There was no difference in the probability of survival between two sexes. Analysis of factors contributing to death revealed that 400 males (48.7%) had neoplastic changes, 189 males (23.0%) had non-neoplastic lesions, and 232 males (28.3%) died from unknown causes. In females, these figures were 627 (76.4%), 62 (7.6%) and 132 (16.0%), for neoplastic, non-neoplastic and unknown findings, respectively. It could be suggested that the risk of death by non-neoplastic reasons was higher in the males than in the females, whereas females were more likely to be affected by tumours. In the neoplastic causes of death, pituitary tumours were the most common in both sexes, followed by mammary tumours in females, and haemopoietic tumours in males. In non-neoplastic cause of death, renal diseases were the most common in both sexes, followed by skin diseases and cardiovascular diseases in males, and skin diseases and poditis in males. A relatively large number of animals (28.3% in males and 16.0% in females) were found dead, without any significant clinical or histologically identifiable cause. Most of the animals with pituitary tumours were killed in extremis and the proportion of females (70.1%) being greater than males (46.8%). There were no case which died by accident, and also only minimal incidence which died by bleeding procedures.

  • The importance of the security management system for the aviation infrastructure cannot be overemphasized. What is especially important on the security management system for it is the assessment that is detaild and systematic. This article presents a framework based on a Hanulcha-type security management system model for a Information security of the Aviation infrastructure. This system checks, estimates and analyzes the goal of security with effect, especially in case of the security-accident on the aviation infrastructure because this system model gives the integrated security assessment method.

A Study on No-Fault Arbitration in U.S.'s Automobile Insurance - Focus on the Case of New York State - (미국 자동차보험에 있어서 무과실보험의 중재에 관한 고찰 - 미국 뉴욕주를 중심으로 -)

  • No-fault automobile insurance system is a statutory scheme to provide automobile accident victims with compensation for certain expenses arising from personal injuries occurring in car accidents. New York State has enacted No-Fault Law to ensure that the injured in automobile accidents be paid rapidly by their own insurance company for medical expenses, lost earnings regardless of fault, replacing common law system of reparation for personal injuries under tort law. Its primary purpose is to facilitate compensation without the need to exhaust time-consuming litigation over establishing the existence of fault and the extent of damages. No-Fault Law allows arbitration as a method for settling the no-fault insurance disputes. No-fault arbitration, however, differs in a significant way from general arbitration system. First, No-Fault Law provides the parties with the option to submit any dispute involving no-fault automobile insurance to arbitration. Second, no-fault arbitration attempts to speed its procedure incorporating various methods. Third, the parties are required to seek review of arbitral awards by master arbitrator prior to seeking court's review. Fourth, the parties have right to bring de novo action in court if master arbitrator's award exceeds $5,000. Given the current state of law in Korea, it may not be easy to introduce no-fault arbitration system into Korea in the context of automobile insurance disputes settlement as its law has a long-established reparation system based on tort liability and no-fault arbitration system has its own features that differ from general arbitration system. Nonetheless, it could be suggested that no-fault arbitration be introduced in other fields which require speedy dispute resolution and a third party's decision to settle the disputes. The optional right of submitting disputes to arbitration as provided by No-Fault Law of New York State may offer a ground to supprot the effectiveness of an optional arbitration agreement.

  • In this paper, we propose an efficient location tracking scheme using wireless signals for various situations in marine environment that requires location information for many reasons such as lifesaving, accident prevention, and facility management. Our proposed location tracking scheme not only monitors user's location, but also minimizes the risk under the user's limited battery power budget. The position of a user can be obtained at base stations from the strengths of the received signals from the user. In this case, it may require to prevent the user from getting out of the predetermined safe area. For each location in the safe area, we define a risk function, which is influenced by many factors such as location accuracy, depth, flow rate, and geometry. Our proposed scheme is based on finger printing technique and aims at minimizing the average risk of each user in the safe area.

  • Dangerous cargo in maritime transportation is increasing in international trade. The types and forms of dangerous cargo are very diverse, complex, and the scope is expanding widely. For this reason, it is increase risk of accident threatens the safety of ships and other cargoes, as well as serious damage. Carriers' require special care and handling of dangerous cargo and have a duty of care for safe transport. The shipper is obliged to notify the carrier of the nature and characteristics of the dangerous cargo prior to loading on the ship, the responsibility of the carrier varies depending on the notification or not. This study compares and analyzes the concept and classification of dangerous cargoes, the provisions of the Hague rules, Hamburg Rules and Rotterdam Rules about Carriers' Dangerous Cargo Liability Regulations, after reviewing case studies. We intend to provide information to dangerous cargo handling, the carrier.

Elastoplastic FEM analysis of earthquake response for the field-bolt joints of a tower-crane mast

  • Safety measures for tower cranes are extremely important among the seismic countermeasures at high-rise building construction sites. In particular, the collapse of a tower crane from a high position is a very serious catastrophe. An example of such an accident due to an earthquake is the case of the Taipei 101 Building (the author was the project director), which occurred on March 31, 2002. Failure of the bolted joints of the tower-crane mast was the direct cause of the collapse. Therefore, it is necessary to design for this eventuality and to take the necessary measures on construction sites. This can only be done by understanding the precise dynamic behavior of mast joints during an earthquake. Consequently, we created a new hybrid-element model (using beam, shell, and solid elements) that not only expressed the detailed behavior of the site joints of a tower-crane mast during an earthquake but also suppressed any increase in the total calculation time and revealed its behavior through computer simulations. Using the proposed structural model and simulation method, effective information for designing safe joints during earthquakes can be provided by considering workability (control of the bolt pretension axial force and other factors) and less construction cost. Notably, this analysis showed that the joint behavior of the initial pretension axial force of a bolt is considerably reduced after the axial force of the bolt exceeds the yield strength. A maximum decrease of 50% in the initial pretension axial force under the El Centro N-S Wave ($v_{max}=100cm/s$) was observed. Furthermore, this method can be applied to analyze the seismic responses of general temporary structures in construction sites.
