The Evaluation of Accident Management Strategy Involving Operator Action

  • This paper presents a new approach to the evaluation of an accident management strategy when an operator action is involved. This approach classifies the failure in implementing a given strategy into 4 possible mechanisms, and provides their corresponding quantification methods : 1) the failure to formulate correct intention by operators, 2) the failure to take an adequate action following a correct diagnosis, 3) the failure of a system operation following an adequate action, and 4) the failure due to a delayed action. The proposed method was applied to assess a cavity flooding strategy that uses containment spray system (CSS), and the result shows that the method is more appropriate in evaluating accident management strategies when human action is involved.

  • This Paper introduced a proper time evaluation for railway emergency response staffs using the Therblig Analysis. We defines what Activities the emergency response staffs should take when the fire occurs on the railway. This has been possible through the development of the scenario that is focused on the fire accident and it also has been driven to minimize the spread and damage of the accident. Gilbreth's Therblig Analysis has been used here to derive the reasonable time that would take for the emergency response staffs to respond to the fire accident on the railway system. We developed Emergency Response Scenario against Accidents and defined the Activity of the main Action for Emergency Response staffs.

  • To terminate the progression of the sever accident and mitigate the accident consequences, corium coaling has been suggested as one of most important design features considered in the swore accident mitigation. Till now, some kinds of cooling methodologies have been identified and, specially the corium cooling at the reactor cavity has been considered as one of the most promising cooling methodologies. Moreover, several design requirements related to the cerium cooling at the reactor cavity have been also suggested and applied to the design of the next generation reactor. In this study technical description are briefly described for the important issues related to the cerium cooling at the reactor cavity, i.e. cavity area, cavity flooding system, etc., and simple evaluation for those items have been performed considering present technical levels the experiment and analytical works..

The characteristics and clinical outcomes of trauma patients transferred by a physician-staffed helicopter emergency medical service in Korea: a retrospective study

  • Purpose: Helicopter transport with medical teams has been proven to be effective, with improvements in patient survival rates. This study compared and analyzed the clinical characteristics and treatment outcomes of trauma patients transported by doctor helicopters according to whether patients were transferred after a clinical evaluation or without a clinical evaluation. Methods: This study retrospectively reviewed data from the Korean Trauma Data Bank of trauma patients who arrived at a regional trauma center through doctor helicopters from January 1, 2014, to December 31, 2022. The patients were divided into two groups: doctor helicopter transport before evaluation (DHTBE) and doctor helicopter transport after evaluation (DHTAE). These groups were compared. Results: The study population included 351 cases. At the time of arrival at the trauma center, the systolic blood pressure was significantly lower in the DHTAE group than in the DHTBE group (P=0.018). The Injury Severity Score was significantly higher in the DHTAE group (P<0.001), and the accident to trauma center arrival time was significantly shorter in the DHTBE group (P<0.001). Mortality did not show a statistically significant between-group difference (P=0.094). Surgical cases in the DHTAE group had a longer time from the accident scene to trauma center arrival (P=0.002). The time from the accident to the operation room or from the accident to angioembolization showed no statistically significant differences. Conclusions: DHTAE was associated with significantly longer transport times to the trauma center, as well as nonstatistically significant trends for delays in receiving surgery and procedures, as well as higher mortality. If severe trauma is suspected, air transport to a trauma center should be requested immediately after a simple screening test (e.g., mechanism of injury, Glasgow Coma Scale, or Focused Assessment with Sonography in Trauma), which may help reduce the time to definitive treatment.

Preliminary Radiation Exposure Dose Evaluation for Workers of the Landfill Disposal Facility Considering the Radiological Characteristics of Very Low Level Concrete and Metal Decommissioning Wastes (극저준위 콘크리트, 금속 해체방폐물의 방사선적 특성을 고려한 매립형 처분시설 방사선작업자 예비 피폭선량 평가)

  • The Kori Unit 1 nuclear power plant, which is planned to be dismantled after permanent shutdown, is expected to generate a large amount of various types of radioactive waste during the dismantling process. For the disposal of Very-low-level waste, which is expected to account for the largest amount of generation, the Korea Radioactive waste Agency (KORAD) is in the process of detailed design to build a 3-phase landfill disposal facility in Gyeongju. In addition, a large container is being developed to efficiently dispose of metal and concrete waste, which are mainly generated as Very low-level waste of decommissioning. In this study, based on the design characteristics of the 3-phase landfill disposal facility and the large container under development, radiation exposure dose evaluation was performed considering the normal and accident scenarios of radiation workers during operation. The direct exposure dose evaluation of workers during normal operation was performed using the MCNP computer program, and the internal and external exposure dose evaluation due to damage to the decommissioning waste package during a drop accident was performed based on the evaluation method of ICRP. For the assumed scenario, the exposure dose of worker was calculated to determine whether the exposure dose standards in the domestic nuclear safety act were satisfied. As a result of the evaluation, it was confirmed that the result was quite low, and the result that satisfied the standard limit was confirmed, and the radiational disposal suitability for the 3-phase landfill disposal facility of the large container for dismantled radioactive waste, which is currently under development, was confirmed.

  • In this study, Event Corrective Action Supporting System (ECAS) is developed for the accident evaluation in nuclear power plant. The ECAS system can be used in supporting regulator and/or operator under event situation in nuclear power plants. The ECAS system consists of 5 modules including failure location module, failure analysis module, failure integrity evaluation module, system vulnerability evaluation module, and reporting and operating experience feedback module. The ECAS system will be used as sub module of Knowledge-Based Event Evaluation Network (K-EvENT) which is developing for the against the accident in nuclear power plants.

  • This study propose a model for railway safety evaluation with which the safety of whole railway system can be evaluated. The evaluation model is to generate a safety index which quantitatively represent the degree of railway safety. Safety index is proposed a function of three indexes; an accident index, safety management index, and safety culture index. This paper describes the first result from the study on the safety target which will be a key starting point toward the development of safety evaluation model. It is recommended that the safety target be composed of several sub-targets that are apportioned to constituent components. It is concluded that the classification of safety target influence on deciding components or attributes that constitute each sub-indexes; an accident index, safety management index, and safety culture index. Based on this study, a railway safety evaluation model will be developed in the next study.

In particular, this study confirmed theoretically how domestic media biased frame when it presents problem definition, causing interpretation, moral evaluation, and post-prescription on the ferry Sewol accident, In addition, the bias of the frame was analyzed comparing what is the difference between the conservative newspapers and liberal newspapers. Findings are as follows. First, in diagnosis of ferry Sewol accident, news slanted fragmentation frame>personalization frame>authority-disorder frame. The Chosun Ilbo focus on fragmentation bias, meanwhile Hankyoreh focus on the authority disorder relatively. Second, in accident evaluation, responsibility frame> moral frame> problem-solution frame. The Chosun Ilbo focus on responsibility frame and moral frame. But Hankyoreh focus on responsibility frame and problem-solution frame. Third, in the matter of responsibility, government frame>personal frame>organizational frame. Chosun Ilbo biased responsibility of the government and individuals, while the Hankyoreh is relatively more emphasis on government responsibility and the responsibility of the organization also showed. Fourth, in problem solving, thematic frame and episodic frame bias appeared as rough and level. Chosun Ilbo showed episodic frame, Hankyoreh showed thematic frame. News frame and bias as well as ideological differences of media on ferry Sewol accident was discussed in the context of the social dimension.

Motorcycle Accident Model at Roundabout in Korea using ZAM (ZAM을 이용한 국내 회전교차로 오토바이 사고모형)

  • The goal of this study is to develop the accident models of motorcycle at roundabouts. In the pursuing the above, this study gives particular attentions to developing the appropriate models using ZAM. The main results are as follows. First, the evaluation of various developed models by the Vuong statistic and over-dispersion parameter shows that ZINB is analyzed to be optimal among Poisson, NB, ZIP(zero-inflated Poisson) and ZINB regression models. Second, the traffic volume, width of central island and width of approach are evaluated to be important variables to the accidents. Finally, the common variables that affect to the accident are selected to be traffic volume and width of approach. This study might be expected to give some implications to the accident research on the roundabout by motorcycle.

  • This review is based on articles published in Japanese Journal of Health Physics on "Radiation-exposed medical care and waste" related to Tokyo Electric Power Company Incorporated (TEPCO)'s Fukushima Daiichi Nuclear Power Station accident. Here, we have considered three original articles; one technical data, one special article, one 50th anniversary article, one preface, three topics, and two cases of From Japan to the World (J to W). These articles have reported the system and standards that were established after the accident. Moreover, they have summarized rare experiences such as the fumbling response at the time and the evaluation of samples in that disaster. These articles constitute valuable records of the situation.
