Frequency of Post-Concussion Syndrome in Korean Patients with Minor Head Injury

  • Purpose: To determine the incidence of post-concussion syndrome (PCS) in Korean patients after minor traumatic brain injury. Methods: We conducted an observational cohort study of a convenience sample of patients presenting to the emergency department of a major academic Korean hospital. Patients who visited the Emergency Department for head trauma were screened. A researcher questioned the subject regarding his or her symptoms. Subjects were contacted by phone approximately 2 weeks after their Emergency Department visit and questioned about subsequent symptoms and subsequent visits for medical care. Results: Only 8% of subjects reported any post-concussion symptoms. Only 0.4% had three or more symptoms which might have met criteria for PCS. The median peak onset of symptoms was 3 days after injury. Conclusion: The incidence of PCS is Korean patients is much lower than that documented for patients in the United States or other western countries. On the other hand, this study results could give an idea that mild trauma could also cause the PCS. Further study is needed to replicate this finding and investigate possible explanations for this difference.

Research Trend Analysis on Job Stress of Hospital Nurses (병원 간호사의 직무스트레스에 관한 연구 동향 분석)

  • 1) Purpose : The purpose of this research is to identify the trend of job stress of hospital nurses based on last 10 years of domestic research and to suggest directions for systematic future research. 2) Methodology : 101 studies between 2006 and 2016 were selected using key words such as registered nurse and job stress from Academic Information Service(Riss4u), the National Assembly Library and Korean studies Information Service System(KISS). 3) Findings : The researches on job stress of hospital nurses were increasing. The result showed that job stress increased for shift nurses. Especially, nurses of small-medium sized hospital showed remarkable effects to organization such as turnover intention. In addition, although the importance of approach to management is emphasized, there is little research on these aspects. 4) Practical Implications : It is necessary to develop systematic research on job stress directly effecting turnover intention of hospital nurses from hospital management aspect. The development of job stress measuring tools reflecting the characteristics of the medical institution, and the human resources management plan to resolve the imbalance in manpower for nurses should be required.

Association of Violent Experience and Resilience with Burnout in Emergency Medical Technicians (119구급대원의 폭력경험과 탄력성이 소진에 미치는 영향)

  • Purpose: This descriptive correlational study investigated the effects of violent experience and resilience on burnout in emergency medical technicians (EMTs). Methods: The participants were 160 EMTs working in fire stations (safety center and local center) located in U, B, and D city. The collected data were analyzed by descriptive statistics, independent t-test, ANOVA, $Scheff{\acute{e}}$ test post-hoc analysis, Pearson correlation coefficients, and multiple regression analysis using IBM SPSS Statistics 20.0. Results: The number of EMTs getting into ambulances was significantly associated with violent experience, resilience, and burnout. Position of EMTs was significantly associated with both resilience and burnout. In the multiple regression analysis, the subscales of durability and optimism in resilience were significantly associated with burnout after controlling for confounding variables. Conclusion: This study indicated that improving durability and optimism in resilience was important to cope with experience of violence and to prevent burnout in EMTs.

  • After reviewing literatures of Han dynasty or earlier that recognize tumor, bump or mass, we concluded as follows. The recognition or study on tumor, bump or mass started at least Seoju(西周) Dynasty or earlier, and academic system was developed since earlier Han Dynasty(漢代). Until Han Dynasty, main characters that recognize Tumor, bump or mass include Jong(腫), Yang(瘍), Yu(瘤), Yeong, Chang (瘡), Ong(癰), Jeo(疽), Jeok(積), Chwi(聚), Byeok(癖), Jing, and Ga, Hyeon that firstly appeared in Southern and Northern Dynasties(南北朝), and Am(癌) that firstly appeared in Song Dynasty[宋代] can also be considered as significant characters. The main perception of cancer(腫瘍) until Han Dynasty was started by understanding the concept of the word Yu(瘤) and developed through visual, tactile sensation and symptom observation of following 2 categories, One is tumor, btm1p or mass which is expressed in outer body like Yeong, Jong(腫), Chang(瘡), Yang(瘍), Ong(癰), and Jeo(疽), The other is tumor, bump or mass which is expressed internally like Jeok(積), Chwi(聚), Byeok (癖), Jing, and Ga.

Improved Detection of Metastases by Step Sectioning and Immuno-Histochemical Staining of Axillary Sentinel Nodes in Patients with Breast Carcinoma

  • Background: The object of this study was to examine whether a new protocol including step-sectioning and immunohistochemistry (IHC) staining of axillary sentinel nodes (SN) would lead to detection of more metastases in patients with breast cancer. Materials and Methods: Sixty-nine tumor free sentinel lymph nodes were examined. Step frozen sectioning was performed on formalin fixed SN and stained both by hematoxylin and eosin (H and E) and cytokeratin markers using IHC. Any tumoral cell in IHC stained slides were considered as a positive result. Metastases up to 0.2 mm were considered as isolated tumor cells and 0.2 up to 2 mm as micrometastasis. Results: Mean age of the patients was $48.7{\pm}12.2$ years. Step sectioning of the SN revealed 11 involved by metastasis which was statistically significant (p<0.001). Furthermore, 15 (21.7%) of the patients revealed positive results in IHC staining for pan-CK marker and this was also statistically significant (p=0.001). Ten patients had tumoral involvement in lymph nodes harvested from axillary dissection and 4 out of 15 lymph nodes with positive result for CK marker were isolated tumor cells. However, 4 of 10 patients with tumor positive lymph nodes in axillary dissection were negative for CK marker and in contrast 6 of the pan-CK positive SN were in patients with tumor-free axillary lymph nodes. Conclusions: Both IHC and step sectioning improve the detection rate of metastases. Considering the similar power of these two methods, we recommend using either IHC staining or step sectioning for better evaluation of harvested SNs.

A Scientometric Analysis of 20 Years of Research on Breast Reconstruction Surgery: A Guide for Research Design and Journal Selection

  • Background Breast reconstruction refers to the rebuilding of a woman's breast using autologous tissue or prosthetic material to form a natural-looking breast. It is increasingly offered to women undergoing mastectomy for breast cancer. However, there is no systematic analysis available for the expanding area of research on breast reconstruction. Methods A bibliometric method was used to obtain a view of the scientific production about breast reconstruction by data extracted from the Institute for Scientific Information (ISI). Specific parameters were retrieved from the ISI. Articles about breast reconstruction were analyzed to obtain a view of the topic's structure, history, and document relationships using HistCite software. Trends in the most influential publications and authors were analyzed. Results The number of articles was constantly increasing. Most highly cited articles described the methods of flap construction in the surgery. Other highly cited articles discussed the psychological or emotional aspects of breast reconstruction, skin sparing mastectomy, and breast reconstruction in the irradiated breast. Conclusions This was the first breast reconstruction scientometric analysis, representing the characteristics of papers and the trends of scientific production. A constant increase in the number of breast reconstruction papers and also the increasing number of citations shows that there is an increasing interest in this area of medical science. It seems that most of the research in this field is focused on the technical aspects of surgery.

Analysis of Hospital Admissions Related to Adverse Drug Events Using ADE Signals

  • Adverse drug events (ADEs) are the most common type of adverse events in medical practice. Hospital admissions related to ADEs cost high and should be monitored to prevent them. While concerns about the ADEs are increasing, the frequency and characteristics of admissions related to ADEs have not been reported in Korea. The objective of the study was to assess the rate of hospital admission related to ADEs and their characteristics through ADE signal-based retrospective reviews of medical records. As results, a total of 1,420 patients had ADE signals suggesting potential ADEs from 3,494 patients who discharged from an academic medical center over one month period. Six pharmacists independently assessed the presence of ADEs after the review of patients' medical records. Among the 3,494 discharges, 62 admissions (1.8%) were found to be realted to ADEs. Of admissions with ADEs, 83.9% were moderate (category F by the NCC MERP classification), 37.2% were preventable, and 85.5% were type A reaction. The most frequent suspected drugs causing ADEs were antineoplastics (48.9%), and the most frequent ADE signal detecting hospital admissions related to ADEs was white blood cell count (24.2%). Hospital admissions related to ADEs were found through screening the ADE signals. The ADE signal-based retrospective review could be a practical approach for identifying hospital admissions related to ADEs.

Research on Service Development Plans for the National Center for Medical Information and Knowledge: Comparison and analysis with the U.S. National Library of Medicine (국립의과학지식센터 서비스 발전 방안을 위한 연구 - 미국 국립의학도서관과의 비교·분석을 통해 -)

  • This study was conducted with the purpose of providing suggestions for improvement through a comparison and analysis of the services of the U.S. National Library of Medicine, the world's largest medical library, and the National Center for Medical Information and Knowledge, Korea's national medical library. Core services that need to be improved are topic-specific services, community services, services by user type, educational services, technology, facility/space services, research support services, and marketing and public relations and cooperation services. Specialized libraries are also increasingly interested in topic-specific services and public services. Efficiency in access through services for each type of user is needed, and various types of educational services that do not limit the target audience are also needed. Marketing through AI, virtual reality, and technology, facility, and space services to support the disabled, research support services centered on research ethics, research grants, and programs, and collaborative services with domestic and international libraries, academic societies, institutions, and local communities in other related fields and publicity are also needed.

A Study for the Enhancement of Accessibility to Community Home Nursing Care Services - The Home Nursing Care Program of Seoul Nurse Association - (지역사회에서의 가정간호 접근성 제고 방안 - '서울시간호사회' 가정간호사업 분석을 토대로 -)

  • Recently, there has been an increasing need for long-term care and comprehensive health care services in community settings. The Ministry of Health and Welfare introduced the Hospital-Based Home Nursing Care Program in 2000. Before this initiative, there was a Home Nursing Demonstration Center, affiliated with the Seoul Nurse Association, had offered home nursing services with the financial support from the local government. since 1993, the Center's nursing staff has been engaged in a general hospital in an effort to provide home nursing care services within Korea's health care system. The purpose of this study was to analyze and identify characteristics of community-based home nursing care supplied by a community-based home nursing team engaged in a general hospital. Also. visit nursing care services provided by public health centers were evaluated in terms of accessibility and supply versus demand, to enhance the accessibility of low-income patients living in Seoul to home nursing care services. Data were collected from home nursing insurance reimbursement claims submitted by the community-based home nursing care team from March 1 to October 30 in 2001 and a questionnaire survey on home-visit nursing services of 25 public health centers in Seoul. The subjects consisted of 197 patients and 12 public health centers. The result were as follows. First, medical institution's community-based home nursing care program was better in technical quality than health-center-based home-visit nursing care. In addition. the pattern of the subject patients was similar to that of hospital-based home nursing care program. Second, there was a high demand for community-based home nursing care while only a small number of home-visiting nurses served at public health centers in Seoul. As a result, many patients could not receive adequate care. Finally, we suggest that community-based home nursing care program should be introduced in the national health system to meet the at-home care needs of severely ill low-income patients. Furthermore, to better utilize home nursing and visit-nursing care resources and offer continued care for patients in community settings, an efficient referral network should be built among related institutions. This would require improvement of reimbursement system and amendment of the law related to health insurance system and community-based home nursing care services.

Young Women with Breast Cancer in the United States and South Korea: Comparison of Demographics, Pathology and Management

  • Background: Breast cancer diagnosed in young women may be more aggressive, with higher rates of local and distant recurrence compared to the disease in older women. Epidemiologic evidence suggests that Korean women have a lower incidence of breast cancer than women in the United States, but that they present at a younger age than their American counterparts. We sought to compare risk factors and management of young women with breast cancer in Boston, Massachusetts (US) with those in Seoul, South Korea (KR). Materials and Methods: A retrospective review was performed of consecutive patients less than 35 years old with a diagnosis of breast cancer at academic cancer centers in the US and KR from 2000-2005. Patient data were obtained by chart review. Demographic, tumor and treatment characteristics were compared utilizing Pearson's chisquare or Wilcoxon rank-sum tests where appropriate. All differences were assessed as significant at the 0.05 level. Results: 205 patients from the US and 309 from KR were analyzed. Patients in US were more likely to have hormone receptor positive breast cancer, while patients in KR had a higher rate of triple negative lesions. Patients in US had a higher mean body mass index and more often reported use of birth control pills, while those in the KR were less likely to have a sentinel node procedure performed or to receive post mastectomy radiation. Conclusions: Patients under 35 diagnosed with breast cancer in the US and KR differ with respect to demographics, tumor characteristics and management. Although rates of breast conservation and mastectomy were similar, US patients were more likely to receive post mastectomy radiation. The lower use of sentinel node biopsy is explained by the later adoption of the technique in KR. Further evaluation is necessary to evaluate recurrence rates and survival in the setting of differing disease subtypes in these patients.
