Effects of overdenture attachment systems with different working principles on stress transmission: A three-dimensional finite element study

  • PURPOSE. The aim of the present study was to compare the stress distributions on the dental implants, abutments, and bone caused by different overdenture attachment types under functional chewing forces. MATERIALS AND METHODS. The 3D finite element models of the mandible, dental implants, attachment types, and prostheses were prepared. In accordance with a conventional dental implant supported overdenture design, the dental implants were positioned at the bone level in the canine teeth region bilaterally. A total of eight models using eight different attachment systems were used in this study. All the models were loaded to simulate chewing forces generated during the centric relationship (450 N), lateral movement (400 N), protrusive movement (400 N), and also in the presence of a food mass unilaterally (200 N). Stress outputs were obtained as the maximum principal stress and the equivalent von-Mises stress. RESULTS. In all attachment types, higher stress values were observed in the abutments, dental implants, and bone in the magnet attachments in different loading conditions. The highest stress values were observed among the magnet systems in the components of the Titanmagnetics model in all loading conditions (stresses were 15.4, 17.7, and 33.1 MPa on abutment, dental implant, and bone, respectively). The lowest stress value was observed in the models of Zest and O-Ring attachments. CONCLUSION. The results of the present study implied that attachment types permitting rotation and tolerating various angles created lower stresses on the bone, dental implants, and abutments.

Micro-CT를 이용한 맞춤형 CAD-CAM 지대주의 적합성 분석 (Fit analysis of CAD-CAM custom abutment using micro-CT)

  • 목적: Biomet 3i 시스템과 Astra 시스템의 내측 연결형 임플란트 고정체에 국내에서 제작된 맞춤형 CAD-CAM 지대주를 체결하여 구성 성분 간의 적합성과 나사의 안정성을 평가하고자 하였다. 재료 및 방법: 24개 3i 임플란트 중 12개의 임플란트에 기성 지대주를 연결하고, 나머지 임플란트에는 맞춤형 CAD-CAM 지대주를 연결하여 각각 Group 1과 Group 2로 분류하였다. 동일 개수와 동일한 방법으로 Astra 임플란트를 각각 Group 3와 Group 4로 분류하였다. 각각의 고정체에 지대주를 장착하여 각 계면 사이의 적합성을 micro-CT로 관찰하고 초기 풀림 토크를 측정한 후 평가하였다. 결과: 고정체-지대주의 접촉길이는 Group 1과 Group 2 사이 뿐만 아니라 Group 3와 Group 4 사이에도 유의한 차이가 없었다(Mann-Whitney test, P>.05). 하지만, 지대주와 나사의 계면 및 고정체와 나사의 계면에서 Group 2와 Group 4가 Group 1와 Group 3에 비해 각기 더 큰 접촉양상을 보여주었다(Mann-Whitney test, P<.05). 또한, Group 2와 Group 4가 Group 1과 Group 3에 비해 각기 더 낮은 나사의 초기 풀림 토크를 기록하였다(Student t-test, P<.05). 결론: CAD-CAM지대주는 임상에 적용할 만한 적합성을 지니고 있으나, 광범위한 임상 적용을 위해서는 풀림 토크가 향상되어야 할 것으로 사료된다.

임플랜트 지대주에 따른 나사 풀림의 연구 (THE INFLUENCE OF IMPLANT FIXTURE-ABUTMENT CONNECTION DESIGN ON SCREW LOOSENING)

  • Purpose: Current trend in implant dentistry is changing from external connection to internal connection. To evaluate the splinting of external and internal connection implant on screw loosening, 2-units prosthesis was fabricated with BioPlant $System^(R)$ of external connection type and Lifecore STAGE-1 Single Stage Implant $System^(R)$ of internal connection type. Material and Method: Experimental group is classified into three groups. 1) $G_1-EE$: 2-units prosthesis was fabricated with two Bioplant $System^(R)$ of external connection type. 2) $G_1-EI$: 2-units prosthesis was fabricated with one BioPlant $System^(R)$ of external connection type and one Lifecore STAGE-1 Single Stage Implant $System^(R)$ of internal connection type. 3) $G_1-II$: 2-units prosthesis was fabricated with two Lifecore STAGE-1 Single Stage Implant $System^(R)$ of internal connection type. In fabricating 2-units prosthesis, two hexed abutments are recommended when two implants are installed parallel, otherwise one hexed abutment is used on major occlusal force area and one nonhexed abutment is used on the other area. Since it is rare to find two implants being parallel, it is hard to fabricate prosthesis with passive adaptation using two hexed abutments. It is much more difficult to acquire passive adaptation when using hex abutment compared to nonhex abutment. To evaluate the influence of hexed and nonhexed abutment on screw loosening, 2-units prosthesis was fabricated with hexed and nonhexed abutment. Experimental group is classified into three groups. 1) $G_2-HH$: 2-units prosthesis was fabricated with two hexed abutments. 2) $G_2-HN$: 2-units prosthesis was fabricated with one hexed abutment and one nonhexed abutment. 3) $G_2-NN$: 2-units prosthesis was fabricated with two nonhexed abutments. Result: The results of comparing the detorque value after loading on a each prosthesis periodically are as follows. 1. In splinting group of external and internal connection implant, $G_1-II$ group demonstrated the biggest detorque value, followed by $G_1-EI$ group and $G_1-EE$ group. 2. There is no notable significance between external connection implant of $G_1-EI$ group and $G_1-EE$ group and also no significance between internal connection implant of $G_1-EI$ group and $G_1-II$ group. 3. $G_2-HH$ group showed higher detorque value than $G_2-HN\;and\;G_2-NN$ group. From the results, we can concluded that using both external connection and internal connection implant together is clinically acceptable and in order to acquire a good passive adaptation in fabricating 2-units implant prosthesis we can use two nonhexed abutments.

BIOMECHANICS OF ABUTMENTS SUPPORTING REMOVABLE PARTIAL DENTURES UNDER UNILATERAL LOADING

  • Statement of problem. In distal extension removable partial denture, the preservation of health of abutment teeth is very important, but abutment teeth are subjected to unfavorable stress under unilateral loading specially. Purpose. The purpose of this study was to investigate the biomechanical effects of mandibular removable partial dentures with various prosthetic designs under unilateral loading, using strain gauge analysis. Material and methods. Artificial teeth of both canines were anchored bilaterally in a mandibular edentulous model made of resin. Bilateral distal extension removable partial dentures with splinted and unsplinted abutments were fabricated. Group 1: Clasp-retained mandibular removable partial denture with unsplinted abutments Group 2: Clasp-retained mandibular removable partial denture with splinted abutments by 6-unit bridge. Group 3: Bar-retained mandibular removable partial denture Strain gauges were bonded on the labial plate of the mandibular resin model, approximately 2 mm dose to the abutments. Two unilateral vertical experimental loadings (30N and 100N) were applied subsequently via miniature load cell that were placed at mandibular left first molar region. Strain measurements were performed and simultaneously monitored from a computer connected to data acquisition system. For within-group evaluations, t-test was used to compare the strain values and for between-group comparisons, a one-way analysis of variance (ANOVA) was used and Tukey test was used as post hoc comparisons. Results. The strain values of group 1 and 2 were tensile under loadings. In contrast, strain values of group 3 were compressive in nature. Strain values increased as the applied load in increased from 30N to 100N (p<.05) except for right side in group 1. Under 30N loading, in left side, group 1 showed higher strain values than groups 2 and 3 in absolute quantity (p<.05). And group 2 showed higher strain values than group 1 (p<.05). In right side, group 1 and 2 showed higher strain values than group 3 in absolute quantity (p<.05). Under 100N loading in left side, group 1 showed higher strain values than groups 2 and 3 in absolute quantity (p<.05). And group 2 showed higher strain values than group 1 (p<.05). In right side, group 1 and 2 showed higher strain values than group 3 in absolute quantity (p<.05). Under 30N loading, group 2 and 3 showed higher strain values in right side than in left side. Under 100N loading, right side strain values were higher than left side ones for all groups. Conclusion. Splinting of two isolated abutments by bridge reduced the peri-abutment strain in comparison with unsplinted abutments under unilateral loading. Bar-retained removable partial denture showed the lowest strain of three groups, and compressive nature.

Effect of base isolation on the seismic response of multi-column bridges

  • A nonlinear model for time-step analysis of bridges subjected to two orthogonal horizontal components of earthquake motions was developed. The focus of the study was on elastomeric isolators with or without lead cores. The hysteretic behavior of the isolators, the columns, abutments, and shear keys was taken into account. The nonlinear analysis showed that, contrary to linear theory prediction, the use of isolators does not necessarily increase the displacement of the superstructure. Furthermore, it was shown that properly designed isolators can reduce the ductility demand in RC bridge columns substantially.

Integral Bridge System with Geosynthetic-Reinforced Backfill

  • A new type bridge combining an integral bridge and a pair of geosynthetic-reinforced soil (GRS) retaining walls having full-height rigid (FHR) facings, called the GRS integral bridge, is proposed. The geosynthetic reinforcement layers are connected to the FHR facings (i.e., RC parapets) that are integrated with a girder without using any girder-support. GRS integral bridges are basically much more cost-effective in construction and long-term maintenance while having a much higher seismic stability than conventional-type bridges having a girder via movable and fixed supports on a pair of cantilever abutments. GRS integral bridges are better than bridges using GRS retaining walls as abutments and also than conventional integral bridges with unreinforced backfill. To validate the above, a series of static cyclic lateral loading tests of the facing and a series of shaking table tests were performed on smallscaled models of different bridge types.

  • In this paper, we proposed effective algorithm that do Availability of communication network account using total probability formula. Use path tracing method about PSTN linked with MSC in WCDMA considering occasion that do not consider mobility of terminal and Availability did account. First, express minimum path series gathering by structure function after network that do modeling by stochastic graph for communication network saves all minimal path sets between particular two abutments and we gain Availability value in angular variable after express structure function by Boolean operation form and Availability between two abutments did account.

유리단 국소의치 환자에서 T-scan system을 이용한 중심교합시에 교합접촉 양상 연구 (COMPUTERIZED EVALUATION OF OCCLUSAL CONTACTS IN CENTRIC CLOSURE IN DISTAL EXTENSION PARTIAL DENTURE CASES)

  • This clinical study was to analyze occlusal contacts in maximum intercuspation on distal extension partial denture and to compare tooth contact state between the denture teeth and abutment teeth by time mode and force mode using the T-scan system. The subjects ware twenty-one adult patients with upper natural teeth and lower distal extension partial denture. Conclusion : 1. The patient with simultaneous occlusal contacts both denture tooth and abutment and bilaterally in Kennedy Class I cases was one-sixth, but there was no one with symmetric occlusal and equal force among bilateral denture teeth and abutments. 2. The five-fifteenth of Kennedy Class I case patients resulted in simultaeous occlusal contacts bet-ween denture teeth and abutments but no one has the symmetric occlusal contacts and unifarm force between denture teeth and abutment teeth.
