EFFECT OF THE SURFACE MODIFICATIONS AND THE USE OF WASHER ON THE REVERSE TORQUE OF THE IMPLANT PROSTHETIC GOLD RETAINING SCREW

  • The screw loosening is one of the complications that happen frequently in dental implant prostheses. The purpose of this study was to evaluate the changes of reverse/loosening (opening) torque of the screw according to the surface modifications by sandblasting and 24K gold electroplating as well as to determine the possibility of the clinical use of a washer in dental implant. The reverse torque of 4 experimental conditions(control, sandblasted, use of washers, electroplasted) was measured by digital torque gauge (Model MGT50Z, Mark-10 Corp., 458 West John Street Hicksville, NY 11801 USA). Electronic torque controller (Nobel Biocare DEA 020) was used in fastening the gold screws into abutment replicas. Mixed Linear Model Analysis method was used for statistical analysis. To examine the changes of screw thread surface, microphotographs were taken by Olympus PME-3 metallurgic microscope (Olympus Optical Co. Ltd., Tokyo, Japan). Within the limitations of this study, the following results were drawn: 1. The surface modifications of the gold screws and the use of a washer have significantly affected the reverse torque value compared to the control group (P<0.01). 2. Sandblasting and electroplating treatments demonstrated significantly higher reverse torque value than that of control group. 3. The use of a washer may be one of the useful clinical methods that prevent the screw loosening. However, further studies are necessary for the material selection and design of the washer.

매질 경계면의 온도 변화를 이용한 교량 세굴 및 수위 측정방법 개발 (Development of Measuring Method for Bridge Scour and Water Level Using Temperature Difference Between Medium Interfaces)

  • 유체의 흐름에 의해 교량의 교각 및 교대 주변의 하상재료가 유실되는 현상을 말하는 교량 세굴은 교량 파괴의 주요원인이다. 그리고 홍수시 교량의 수위는 교량자체 안전뿐만 아니라 교량을 이용하는 사용자의 안전을 위해도 반드시 확인해야 한다. 따라서 교량의 안전관리를 위해 세굴과 수위계측은 필수적이라 할 수 있다. 본 연구는 기존의 세굴계측의 한계를 극복할 수 있는 새로운 교량세굴 측정방법을 개발하고 현장 검증을 통해 개발 기술의 우수성과 타당성을 확인하였다. 새롭게 개발된 세굴 및 수위 측정방법은 교각에 수직으로 계측장치를 설치하고 일정간격으로 설치된 온도 센서의 온도를 측정하여 수면 위치와 수중 지면의 위치를 추정하는 방법으로 대기온도와 수온, 수온과 수중 지온의 차이를 분석하여 수면 및 수중 지면의 위치를 추정하는 방법이다. 개발 기술은 절대적인 온도 차이뿐만 아니라 대기와 수중 그리고 지중의 일변화량이 서로 다르다는 점을 이용하여 수면의 위치와 수중 지면 위치를 보다 정확하게 추정할 수 있다. 개발 기술의 검증을 위해 소하천 및 실험실, 그리고 교량 교각에서 개발기술을 시범 적용하고, 매질별 온도분포 특성, 시간에 따른 온도변화 추이 등을 분석하여 개발기술이 효과적으로 교량 세굴과 수위를 계측할 수 있는 방법임을 확인하였다.

임플란트의 생존과 변연골 소실에 영향을 미치는 인자들 (Factors associated with the survival and marginal bone loss of dental implants: a 5-year retrospective study)

  • 목적: 이 연구의 목적은 임플란트의 생존율과 주위 변연골 수준을 후향적으로 평가하여, 술자의 임상적 경험을 포함한 요인들이 미치는 영향을 분석하고자 하였다. 연구 재료 및 방법: 2002년 1월부터 2009년 3월까지 강릉원주대학교치과병원 치주과에서 전공의가 2단계법으로 식립한 임플란트 중 5년 이상의 기록이 있는 146명의 420개 임플란트를 대상으로, 임플란트 탈락여부, 주위 골 소실량, 성별, 연령, 2형 당뇨, 흡연, 지대주 연결 형식, 표면 처리 방법, 직경, 식립 부위, 식립 당시 전공의 연차, 덮개 나사의 조기 노출, 보철학적 합병증, 동반된 골 이식 술식 등의 영향을 평가하였다. 결과: 최종 보철물 장착 후 5년간 누적생존율은 94.9% 이었다. 이원로지스틱회귀분석 결과, 흡연과 덮개 나사의 조기 노출이 실패율을 유의하게 증가시켰다. 다중회귀분석 결과, 흡연, 임플란트 지대주 연결 형식, 및 표면 처리 방법이 임플란트 주위 변연골 수준 변화에 유의한 영향을 주었다. 술자의 임상경험을 반영하기 위한 식립 당시의 전공의 수련 연차는 임플란트의 실패와 유의한 연관성은 보이지 않았다(P = 0.171). 결론: 흡연, 덮개 나사의 조기 노출, 지대주 연결 형식 및 표면 처리 방법이 임플란트의 성공과 유의한 상관관계를 보이는 것으로 생각된다.

Bacterial adhesion and colonization differences between zirconia and titanium implant abutments: an in vivo human study

  • Purpose: Several parameters have been described for determining the success or failure of dental implants. The surface properties of transgingival implant components have had a great impact on the long-term success of dental implants. The purpose of this study was to compare the tendency of two periodontal pathogens to adhere to and colonize zirconia abutments and titanium alloys both in hard surfaces and soft tissues. Methods: Twelve patients participated in this study. Three months after implant placement, the abutments were connected. Five weeks following the abutment connections, the abutments were removed, probing depth measurements were recorded, and gingival biopsies were performed. The abutments and gingival biopsies taken from the buccal gingiva were analyzed using real-time polymerase chain reaction to compare the DNA copy numbers of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and total bacteria. The surface free energy of the abutments was calculated using the sessile water drop method before replacement. Data analyses used the Mann Whitney U-test, and P-values below 0.05 find statistical significance. Results: The present study showed no statistically significant differences between the DNA copy numbers of A. actinomycetemcomitans, P. gingivalis, and total bacteria for both the titanium and zirconia abutments and the biopsies taken from their buccal gingiva. The differences between the free surface energy of the abutments had no influence on the microbiological findings. Conclusions: Zirconia surfaces have comparable properties to titanium alloy surfaces and may be suitable and safe materials for the long-term success of dental implants.

타이타늄 합금에 다양한 두께로 코팅된 TiN 피막의 기계적 성질 (MECHANICAL PROPERTIES OF TIN COATED FILM WITH VARIOUS COATING THICKNESS ON TITANIUM ALLOY)

  • Statement of problem: Titanium nitride(TiN) coatings are the most general and popular coating method and used to improve the properties of metallic surface for industrial purposes. When TiN coating applied to the abutment screw, frictional resistance would be reduced, as a results, the greater preload and prevention of the screw loosening could be expected. Purpose: The purpose of this study was to investigate mechanical properties of TiN coated film of various coating thickness on the titanium alloy surface and to evaluate proper coating thickness. Material and method: 95 Titanium alloy (Ti-6Al-4V) discs of 15 mm in diameter and 3 mm in thickness were prepared for TiN coating and divided into 7 groups in this study. Acceding to coating deposition time (CDT) with TiN by using Arc ion plating, were divided into 7 groups : Group A (CDT 30min), Group B (CDT 60min), Group C (CDT 90min), Group D (CDT 120min), Group E (CDT 150min), Group F(CDT 180min) and Group G (no CDT) as a control group. TiN coating surface was observed with Atomic Force Microscope(AFM), field emission scanning electron microscopy(FE-SEM) and examined with scratch tester, wear tester. Result: 1. Coating thickness fir each coated group was increased in proportion to coating deposition time. 2. Surface of all coated groups except Group A was homogeneous and smooth. However, surface of none coated Group G had scratch. 3. Adhesion strength for each coated group was increased in proportion to coating deposition time. 4. Wear resistance for each coated group was increased in proportion to coating deposition time. 5. Surface roughness in Group A, B, C was increased in proportion to coating deposition time. But, surface roughness in Group D, E, F was showed decreased tendency in proportion to coating deposition time. Conclusion: According to coating deposition time, mechanical properties of TiN coated film were changed. It was considered that 120 minutes coating deposition time ($1.32{\mu}m$ in coating thickness) is necessary.

공진 주파수 분석법에 의한 임플랜트의 안정성 측정에 관한 연구 (A STUDY ON THE MEASUREMENT OF THE IMPLANT STABILITY USING RESONANCE FREQUENCY ANALYSIS)

  • Statement of problem : Successful osseointegration of endosseous threaded implants is dependent on many factors. These may include the surface characteristics and gross geometry of implants, the quality and quantity of bone where implants are placed, and the magnitude and direction of stress in functional occlusion. Therefore clinical quantitative measurement of primary stability at placement and functional state of implant may play a role in prediction of possible clinical symptoms and the renovation of implant geometry, types and surface characteristic according to each patients conditions. Ultimately, it may increase success rate of implants. Purpose : Many available non-invasive techniques used for the clinical measurement of implant stability and osseointegration include percussion, radiography, the $Periotest^{(R)}$, Dental Fine $Tester^{(R)}$ and so on. There is, however, relatively little research undertaken to standardize quantitative measurement of stability of implant and osseointegration due to the various clinical applications performed by each individual operator. Therefore, in order to develop non-invasive experimental method to measure stability of implant quantitatively, the resonance frequency analyzer to measure the natural frequency of specific substance was developed in the procedure of this study. Material & method : To test the stability of the resonance frequency analyzer developed in this study, following methods and materials were used : 1) In-vitro study: the implant was placed in both epoxy resin of which physical properties are similar to the bone stiffness of human and fresh cow rib bone specimen. Then the resonance frequency values of them were measured and analyzed. In an attempt to test the reliability of the data gathered with the resonance frequency analyzer, comparative analysis with the data from the Periotest was conducted. 2) In-vivo study: the implants were inserted into the tibiae of 10 New Zealand rabbits and the resonance frequency value of them with connected abutments at healing time are measured immediately after insertion and gauged every 4 weeks for 16 weeks. Results : Results from these studies were such as follows : The same length implants placed in Hot Melt showed the repetitive resonance frequency values. As the length of abutment increased, the resonance frequency value changed significantly (p<0.01). As the thickness of transducer increased in order of 0.5, 1.0 and 2.0 mm, the resonance frequency value significantly increased (p<0.05). The implants placed in PL-2 and epoxy resin with different exposure degree resulted in the increase of resonance frequency value as the exposure degree of implants and the length of abutment decreased. In comparative experiment based on physical properties, as the thickness of transducer increased, the resonance frequency value increased significantly(p<0.01). As the stiffness of substances where implants were placed increased, and the effective length of implants decreased, the resonance frequencies value increased significantly (p<0.05). In the experiment with cow rib bone specimen, the increase of the length of abutment resulted in significant difference between the results from resonance frequency analyzer and the $Periotest^{(R)}$. There was no difference with significant meaning in the comparison based on the direction of measurement between the resonance frequency value and the $Periotest^{(R)}$ value (p<0.05). In-vivo experiment resulted in repetitive patternes of resonance frequency. As the time elapsed, the resonance frequency value increased significantly with the exception of 4th and 8th week (p<0.05). Conclusion : The development of resonance frequency analyzer is an attempt to standardize the quantitative measurement of stability of implant and osseointegration and compensate for the reliability of data from other non-invasive measuring devices It is considered that further research is needed to improve the efficiency of clinical application of resonance frequency analyzer. In addition, further investigation is warranted on the standardized quantitative analysis of the stability of implant.

전부 피개관의 치아 형성 시 축면 경사각에 대한 조사 (Clinical Convergence Angle of Prepared Tooth for full Veneer Crowns)

  • 삭제된 치아의 경사도는 치관 수복물의 유지와 저항에 있어 매우 중요한 요소이다. 하지만 다양한 구강 내 환경과 술자의 능력에 따라 이상적인 경사도를 얻는 것은 쉽지 않다. 따라서 이상적으로 삭제된 치아의 경사도와 실제 임상에서 삭제된 치아의 경사도를 조사하고자 한다. 본 실험의 연구 자료는 K대학 부속 치과병원 중앙 기공실을 통하여 보철과 전공의가 시술한 환자의 모형과 일반 기공소를 통해 일반 개업의가 시술한 환자의 모형을 수집한다. 연구 방법은 각 치아의 근원심면과 협설면을 디지털 카메라를 통해 촬영한 뒤 'ImageJ' 프로그램 상에서 치아의 삭제 각도를 측정 한 뒤 항목별( 1. 술자 집단별 분류, 2. 치열궁 내에서의 위치에 따른 분류, 3. 삭제 양식에 따른 분류 )로 분류하여 비교 분석한다. 이번 연구를 통해 조사한 결과 국내 임상가의 지대치 형성시 축면 경사각의 평균은 $15.02^{\circ}$ 표준 편차는 ${\pm}10.13^{\circ}$였다. 1. 술자 집단별 분류에 대한 분석 일반 치과의 집단의 측면 경사각과 보철과 전공의 집단의 축면 경사각에서 유의성 있는 차이를 보였다(p<0.05). 2. 치열궁 내에서의 위치에 따른 분석 치아의 근원심면과 협설측면에 따라 일반 치과의 집단의 경우 유의성 있는 차이가 없었으며(p>0.05) 보철과 전공의 집단에서는 유의성 있는 차이를 보였다(p<0.05). 치아의 전후적인 위치에 따라 일반 치과의 집단의 경우 전치부와 소구치부 사이에 유의성 있는 차이가 없었으며 (p>0.05) 두 부위와 대구치부 사이에서는 각각 유의성 있는 차이를 보였다(p<0.05). 보철과 전공의 집단의 경우 전치부와 소구치부 사이에 유의성 있는 차이가 없었으며(p>0.05) 두 부위와 대구치부 사이에서는 각각 유의성 있는 차이를 보였다(p<0.05). 상악과 하악으로 분류하여 분석한 경우 일반 치과의 집단에서 근원심면과 전체적인 축면의 경사각에 각각 유의성 있는 차이가 없었으며(p>0.05) 협설측면에서는 유의성 있는 차이를 보였다(p<0.05). 보철과 전공의 집단에서도 근원심면과 전체적인 축면의 경사각에 각각 유의성 있는 차이가 없었으며(p>0.05) 협설측면에서는 유의성 있는 차이를 보였다(p<0.05). 악궁을 좌측과 우측으로 분류하여 분석한 경우 일반 치과의 집단과 보철과 전공의 집단에서 각각 유의성 있는 차이가 없었다(p>0.05). 3. 삭제 양식에 따른 분석 단일 치관을 위한 지대치와 3본 고정성 부분 의치를 위한 지대치로 분류하여 분석한 결과 일반 치과의 집단에서 근원심면의 경우 단일 치관을 위한 지대치의 축면 경사각과 3본 고정성 부분 의치를 위한 지대치의 축면 경사각에 유의성 있는 차이를 보였으며(p<0.05) 협설측면과 전체적인 축면의 경우 각각 단일 치관을 위한 지대치의 축면 경사각과 3본 고정성 부분 의치를 위한 지대치의 축면 경사각에 유의한 차이가 없었다(p>0.05). 보철과 전공의 집단에서는 협설측면의 경우 단일 치관을 위한 지대치의 축면 경사각과 3본 고정성 부분 의치를 위한 지대치의 축면 경사각에 유의성 있는 차이가 없었으며(p>0.05) 근원심면과 전체적인 축면의 경우 각각 단일 치관을 위한 지대치의 축면 경사각과 3본 고정성 부분 의치를 위한 지대치의 축면 경사각에 유의한 차이를 보였다(p<0.05). 이상에서 임상에서 치아 삭제시 이상적인 축면 경사각을 얻기 힘들며 이는 술자의 경험과 악궁 내에서의 위치, 삭제 양식에 따라 영향을 받을 수 있다.

회전삽입로를 이용한 국소의치에 관한 임상적연구 (A Clinical Study on Rotational Path Removable Partial Denture)

  • Path of insertion(1) can be defined that the direction of movement of an appliance from the point of initial contact of its rigid parts with the supporting teeth to the place of final rest. Krol(2) described that in the conventional path of insertion, all the rests are seated more or less simultaneously but in the use of the rotational path one segment of the partial denture is seated first then the remainder of the prosthesis is rotated into position. The rotational path of insertion is limited primarily to the tooth borne prosthesis. Its great advantages are the elimination of anterior clasps to improve ethetic and reduction of tooth coverage to minimize plaque accumulation. Either a rigid minor connector or proximal plate provides retention through its intimate contact with a proximal tooth surface below the height of contour as indicated at a o-degree tilt. A specially designed rest in conjunction with this retentive component satisfies the basic requirements of clasp design. The purpose of this study was a clinical evaluation of rotational path removable partial dentures. Author delivered rotational path removable partial dentures to three different cases of patients and evaluated function of the dentures, difficulties of removal and insertion of the dentures and supporting structures of the abutment teeth by means of clinical and X-ray examinations for eighteen months. According to the examination data author came to the conclusion that the prognosis of the rotational path removable partial dentures was excellent.

Clinical and scanning electron microscopic analysis of fractured dental implants: a retrospective clinical analysis

  • Many longitudinal studies have reported the successful osseointegration of dental implants, with survival rates approaching 90-95%. However, implants regarded as a "success" may have also failed to undergo osseointegration. A variety of complications and failures have been observed, including implant fracture - a rare and delayed biomechanical complication with serious clinical outcomes. Given the increasing popularity of dental implants, an increase in the number of failures due to late fractures is expected. This study sought to determine the rate of implant fractures and factors associated with its development. This retrospective evaluation analyzed implants placed at Wonkwang Dental Hospital (from 1996 to the present). In our study we found that the frequency of dental implant fractures was very low (0.23%, 8 implant fractures out of 3,500 implants placed). All observed fractures were associated with hybrid-surface threaded implants (with diameter of 4.0 or 3.75 mm). Prosthetic or abutment screw loosening preceded implant fracture in a majority of these cases.

The path of placement of a removable partial denture: a microscope based approach to survey and design

  • This article reviews the topic of how to identify and develop a removable partial denture (RPD) path of placement, and provides a literature review of the concept of the RPD path of placement, also known as the path of insertion. An optimal RPD path of placement, guided by mutually parallel guide planes, ensures that the RPD flanges fit intimately over edentulous ridge structures and that the framework fits intimately with guide plane surfaces, which prevents food collecting empty spaces between the intaglio surface of the framework and intraoral surfaces, and ensures that RPD clasps engage adequate numbers of tooth undercuts to ensure RPD retention. The article covers topics such as the causes of obstructions to RPD intra-oral seating, the causes of food collecting empty spaces that may exist around an RPD, and how to identify if a guide plane is parallel with the projected RPD path of placement. The article presents a method of using a surgical operating microscope, or high magnification (6-8x or greater) binocular surgical loupes telescopes, combined with co-axial illumination, to identify a preliminary path of placement for an arch. This preliminary path of placement concept may help to guide a dentist or a dental laboratory technician when surveying a master cast of the arch to develop an RPD path of placement, or in verifying that intra-oral contouring has aligned teeth surfaces optimally with the RPD path of placement. In dentistry, a well-fitting RPD reduces long-term periodontal or structural damage to abutment teeth.
