Supravalvular Pulmonic Stenosis with Pulmonary Hypertension in a Bedlington Terrier Dog

  • A 1.3 year-old Castrated male Bedlington terrier (10.0 kg of body weight) was presented with heart murmur and occasional coughing. Diagnostic imaging studies revealed supravalvular pulmonic stenosis (maximal velocity 2.91 m/s) from abnormal membranous structure (aperture) distal to the pulmonary valve in the main pulmonary artery. Further study also revealed pulmonic regurgitant (1.82 m/s of peak velocity, 13.2 mmHg of pressure gradient) jets started from the abnormal membranous structure. Since the dog had no obvious clinical signs related to heart failure, no interventional therapy including balloon dilation was considered in this case. Instead, medical treatment for preventing further deterioration of clinical signs related to PS was done with enalapril. This case report described a rare case of type III supravalvular PS in a dog, which has never been reported. Further deterioration of clinical signs has yet been recognized after medical treatment.

디젤발전기 엔진 상태 진단 기술의 발전소 적용 연구 (A Study on the Power Plant Application of Engine Condition Diagnosis Technology for Diesel Generator)

  • Diesel generator of nuclear power plant has a role for supply of emergency electric power to protect reactor core system in event of loss of off-site power supply. Therefore diesel generator should be tested periodically to verify the function that can supply specified frequency and voltage at design power level within limited time. For this purpose, appropriate maintenances in case that abnormal conditions were found are required in allowed time. In this paper, results of development of engine condition diagnosis technology and study on power plant of its technology for diesel generator are described.

  • Glaucoma is an eye disease which is caused by abnormal high lOP (Intra Ocular Pressure). High lOP is caused by the aqueous humor which is produced consistently but not drained due to malfunction of the trabecular system which has a role of draining the aqueous humor into the venous system. Currently, there are some methods to treat glaucoma, Among these, the use of implants is increasing in these days due to many problems In other methods. However, conventional implants are passive implants and have critical disadvantage. Therefore, it is needed to develop an active implant which is composed of a valve actuator, pressure sensor, controller, and power supply. In this paper, we make experiment with the fabricated valve actuator in In-vitro experiment, and estimate the in-vivo result using the experimental result and investigate the possibility of the fabricated valve.

이상 고수온에 반응하는 이매패류 참굴(Crassostrea gigas)의 패각운동을 활용한 생물모니터링시스템 연구 (A Study on Bio-Monitoring Systems using Shell Valve Movements of Pacific Oysters (Crassostrea gigas) in response to Abnormal High Water Temperature)

  • 이상 고수온을 감지하기 위한 생물모니터링 시스템(BMS) 연구를 위해, 4단계의 수온(5, 10, 20와 $30^{\circ}C$)에서 참굴 패각운동을 측정하였다. 모든 참굴은 실험시작 전에 3일 동안 절식을 통하여, 먹이섭이 및 배출에 따른 패각운동의 요인을 제거하였다. $5^{\circ}C$ 실험구에서는 패각운동이 관찰되지 않았지만, 수온의 증가와 함께 패각운동은 증가하였다($10^{\circ}C$: $6.31{\pm}2.18times/hr$, $20^{\circ}C$: $22.0{\pm}10.0times/hr$). $30^{\circ}C$에서는 $5^{\circ}C$와 같이 패각운동이 전혀 보이지 않았던 실험구와 $20^{\circ}C$와 유사한 패각운동이 실험구가 나타났다. 이는 $30^{\circ}C$이상에서도 $20^{\circ}C$와 같은 신진대사를 보이는 개체군이 있었으나, 대부분이 신진대사의 활력의 감소에 기인하여 폐각상태가 지속되는 것으로 나타났다. 따라서 참굴 양식장에 고수온 감지를 위한 참굴 패각운동 BMS를 설치한다면, 경계단계는 빠른 패각운동(약 30.0회/hr 이상)일 때, 심각단계는 수시간 이상 폐각상태일 때, 조기경보(early warning)를 내릴 수 있을 것이다. 따라서 참굴 패각운동을 활용한 BMS는 이상고수온의 조기경보에 대하여 효과적으로 활용이 가능할 것으로 판단된다.

선천성 심질환에 대한 중재적 치료술의 최근 진전 (Recent advances in transcatheter treatment of congenital heart disease)

  • Over the last several decades there has been a remarkable change in the therapeutic strategy of congenital heart disease. Development of new tools and devices, accumulations of experience, technical refinement have positively affected the outcome of interventional treatment. Many procedures including atrial septostomy, balloon valvuloplasty, balloon dilation of stenotic vessel with or without stent implantation, transcatheter occlusion of abnormal vascular structure, transcatheter closure of patent arterial duct and atrial septal defect, are now performed as routine interventional procedures in many institutes. In diverse conditions, transcatheter techniques also provide complementary and additive role in combination with surgery. Intraoperative stent implantation on stenotic vessels, perventricular device insertion, and hybrid stage 1 palliative procedure for hypoplastic left heart syndrome have been employed in high risk patients for cardiac surgery with encouraging results. Transcatheter closure of ventricular septal defect has been performed safely showing comparable result with surgery. Investigational procedures such as percutaneous valve insertion and valve repair are expected to replace the role of surgery in certain group of patients in the near future. Continuous evolvement in this field will contribute to reduce the risk and suffering from congenital heart disease, while surgery will be still remained as a gold standard for significant portion of congenital heart disease.

좌심방 양심실 연결증 [DOLA] (Double Outlet Left Atrium: A rare form of the atrioventricular septal defect with malposition of the atrial septum)

  • The case of a patient with abnormal position of the atrial septum resulting in a left atrium with two atrioventricular valves and a disconnected right atrium is presented with review of related articles. Anatomic details showed atrial situs solitus and a left sided cardiac apex. The right atrium received both venae cavae and a coronary sinus. No AV valve was found in the right atrium, and the floor of this chamber was placed above the posterior wall of right ventricular chamber. The atrial septum with secundum ASD was displaced to right anteriorly at its lower portion and inserted to right of tricuspid annulus. The tricuspid and mitral valve configuration was that of so-called partial ECD, i.e. mitral cleft with large anterior mitral leaflets. The ventricular septum was intact and both ventricular chambers were equally well developed with normal relationships. Surgical repair of this anomaly was performed by resecting the abnormally positioned lower part of the atrial septum, repairing the cleft of the anterior mitral leaflet, and septating the atrium for diverting the systemic and pulmonary venous blood to RV and LV, respectively.

허혈성 승모판막 폐쇄부전의 생체역학적 특성 분석: 비대칭적 유두근 변위와 판륜 확장의 영향 (Evaluation of the Biomechanical Characteristics of Ischemic Mitral Regurgitation: Effects of Asymmetric Papillary Muscle Displacement and Annular Dilation)

  • Ischemic mitral regurgitation (IMR) is the primary mitral valve (MV) pathology in the aftermath of myocardial infarction as a consequence of regional left ventricular (LV) remodeling. We investigated the effect of asymmetric papillary muscle (PM) displacement and annular dilation on IMR development. Virtual MV modeling was performed to create a normal human MV. Asymmetric PM displacement, asymmetric annular dilation, and the combination of these two pathologic characteristics were modeled. Dynamic finite element evaluation of MV function was performed across the complete cardiac cycle for the normal and three different IMR MV models. While the normal MV demonstrated complete leaflet coaptation, each pathologic MV model clearly revealed deteriorated leaflet coaptation and abnormal stress distributions. The pathologic MV model having both asymmetric PM displacement and annular dilation showed the worst leaflet malcoaptation. Simulation-based biomechanical evaluation of post-ischemic LV remodeling provides an excellent tool to better understand the pathophysiologic mechanism of IMR development.

좌측 상공정맥을 동반한 삼심방증 1례 보고 (Cor triatriatum with left superior vena cava[Report of a case])

  • Cor triatriatum is a rare congenital malformation of the heart in which a septum stretches in a transverse plane through the left atrium, thus creates two left atrial subchambers. The upper one connects with the pulmonary veins, and the lower connects with the left ventricles. Due to the rarity of, and difficulty in diagnosing car triatriatum, data on the surgery of the disease are of necessity and very limited. A case of cor triatriatum combined with atrial septal defect and persistent left superior vena cava was experienced in November, 1984 in Chonnam University Medical School. There was a transverse septum in the left atrium below atrial septal defect, all pulmonary veins were drained into the upper chamber of the left atrium which connected with the right atrium via atrial septal defect and the lower chamber via an oval opening[8mm] in the abnormal septum and the lower chamber was connected with the left atrial appendage, and the left ventricle via mitral valve. There was persistent left superior vena cava drained to left atrium and coronary sinus. The abnormal transverse septum within the left atrium was completely excised and the atrial septal defect was repaired with Woven Dacron patch. The post-operative course was not eventful and the patient was discharged to home with good result on the 15th postoperative day, and has been in good condition upto now.

흡기관 분사 방식 수소 연료 기관의 성능 및 배출물에 관한 연구 (The Performance and Emission of the Intake Port Injection Type Hydrogen Fueled Engine)

  • Using the solenoid driven gas injection valve, Hydrogen fuel supply system was made. It was attached to a single cylinder research engine and intake port injection type hydrogen fueled S.I. engine was constructed. Engine performance, emission characteristics, and abnormal combustion were studied through the engine test performed with the variations of fuel-air equivalence ratio and spark timing. Compared with gasoline, hydrogen burns so fast that cylinder peak pressure and temperature are higher and NO is emitted more at full load condition. IN the case of intake port injection type engine, COVimep becomes lower due to the well-mixing of air and fuel, and engine output is lower owing to the low volumetric efficiency. As fuel-air equivalence ratio goes up, the combustion speed increases, and COVimep decreases. NO emission peaks slightly lean of stoichiometric. As spark timing advances and fuel-air equivalence ratio goes up, the cylinder peak pressure and temperature become higher, so abnormal combustions take place easily.

  • Recently, with the development of IoT communication technology, products and services that detect and inform the surrounding environment under the name of smart plugs are being developed. In particular, in order to prepare for fire or gas leakage accidents, products that automatically close and warn when abnormal symptoms occur are used. Most of them use methods of collecting, analyzing, and processing information through networks. However, there is a disadvantage that it cannot be used when the network is temporarily in a failed state. In this paper, sensor information was analyzed using deep learning, and a model that can predict abnormal symptoms was learned in advance and applied to MCU. The performance of each model was evaluated by developing firmware that can judge and process on its own regardless of network and applying a predictive model to the MCU after 3 to 120 seconds.
