A Case of Clonorchis Sinensis with Multiple Low Echogenic Nodules in a Child (소아에서 진단된 다발성 저음영 결절 형태를 보인 간흡충증 1례)

  • Clonorchiasis, known as the parasitic fluke in the intrahepatic bile ducts, occurs throughout Korea, Japan, Taiwan, and Vietnam with high frequency. Most of the infections follow the consumptions of raw or undercooked freshwater fish, which explains its rare occurrence to childhood even in those areas mentioned earlier above. The authors have experienced it with an 8 year old boy. Abdominal ultrasonogram revealed multiple low echogenic nodules in the right lobe of liver and lymph nodes proliferation around the porta hepatis area. And abdominal C.T. scan also found multiple low attenuated nodules with delayed enhancement in liver parenchyma with hepatomegaly and multiple conglomerated lymph nodes in porta hepatis. Both abdominal ultrasonogram and C.T. scan didn't show any dilatation of peripheral intrahepatic bile ducts. In repeated stool examination, Clonorchis sinensis eggs were found in feces and the result of the skin test for Clonorchis sinensis was positive. The patient had a history of having eaten raw pond smelt back in Cheongju a month before the symptom onset. On the basis of the patient's history and the fact that Clonorchis eggs were found in feces 3 weeks after eosinophilia and symptoms were noted, the authors estimated that the findings of the patient's abdominal sonogram and C.T. scan were the radiologic findings of the acute phase of clonorchiasis, of which no report had ever been made before. So the authors report a case of clonorchiasis with atypical radiologic findings in an 8 year old boy.

A Case study of Urinary Disturbance Treated with Soeumin's Bojungikgi-tang (소음인(少陰人) 보중익기탕(補中益氣湯)으로 호전된 요실금(尿失禁) 환자 치험 1례)

  • Objectives The purpose of this study was to evaluate the effects of Soeumin's Bojungikgi-tang, which is based on the Sasang constitutional medicine, for the patient who has suffered from Urinary Disturbance. Methods A 29 year-old woman who suffered from Urinary Disturbance but had no significant organic lesion, was enrolled in this study. The patient had the symptoms including urinary incontinence, pollakiuria, painful urination, discomfort after urinating, general body weakness, chilling sign, and abdominal pain. We diagnosed her as Soeumin Yang Depletion pattern and treated with Soeumin's Bojungikgi-tang. Results After 12 days of admission treatment with Soeumin's Bojungikgi-tang, remarkable improvement was seen on her total number of urination and the Qualiveen Questionnaire score. Conclusions This clinical case indicates that Soeumin's Bojungikgi-tang is effective in treatment of a Soeumin Mang-yang Syndrome, including Urinary Disturbance.

Delayed Iatrogenic Diaphragmatic Hernia after Left Lower Lobectomy

  • A 66-year-old patient undergoing regular follow-up at Samsung Medical Center after left lower lobectomy visited the emergency department around 9 months postoperatively because of nausea and vomiting after routine esophagogastroduodenoscopy at a local clinic. Abdominal computed tomography showed the stomach herniating into the left thoracic cavity. We explored the pleural cavity via video-assisted thoracic surgery (VATS). Adhesiolysis around the herniated stomach and laparotomic reduction under video assistance were successfully performed. The diaphragmatic defect was repaired via VATS. The postoperative course was uneventful, and he was discharged with resolved digestive tract symptoms.

External Use of Hwangyeonhaedog-tang Gagambang in the Treatment of Lower Abdominal Dermatitis - A Case Report (황연해독탕가감방 전탕액 도포로 치료한 하복부피진 1례)

  • A 69-year old female stroke patient was admitted on August 7, 1998. She had seborrheic dermatitis on her lower abdomen for 25 years. The patient received many treatments but was unable to find a cure. During her hospitalization for stroke, the patient had trouble sleeping. The doctors hypothesized the dermatitis to be the cause of the sleeping disorder and attempted to treat it. The doctors chose Hwangyeonhaedog-tang Gagambang for treatment and smeared it on the patient's abdomen. After ten days of treatment, the dermatitis disappeared and did not reoccur. The patient's abdomen continued to show no symptoms of dermatitis after being released for 2 years.

Elucidation of Pyeongwi-san from the Pathological Point of View (병리학적 관점에서 본 평위산(平胃散)의 소고)

  • This study was focused on a certain clinical symptom, which could applicated by Pyeongwi-san, and the radical treatment. Pyeongwi-san is applicable to dampness retention in the spleen and stomach, accompanied by abdominal fullness, anorexia, nausea and regurgitation. Main action of Pyeongwi-san is to dry dampness, invigorate the spleen, promote the flow of gi, and regulate the stomach. The herbal remedy drying dampness like Pyeongwi-san however, is just indicatio symptomatica therefore radical treatment must be used together. Jeunginmaegchi, a medical book written by Gyeong-Myeong Jin, who was lived in Myeong dynasty. This book contains many clinical applicable symptoms and constituents derived from Pyeongwi-san. The herbal prescriptions composing Pyeongwi-san is composed of Changchul, Hubak, Jinpi, Gamcho. The major action of these herbs is drying dampness. Therefore, it shoud be seriously consider that curative therapy must eliminate the cause of endogenous dampness.

A Case of Intussusception with Acute Appendicitis

  • In children presenting to hospital with gastrointestinal symptoms, diseases such as intussusception and acute appendicitis require particular attention and careful examination. Early diagnosis and proper treatment are important because of possible severe complications such as peritonitis and death. Intussusception and appendicitis share similar clinical manifestations. More importantly, the presence of acute appendicitis together with intussusception in children is very rare. We describe an interesting case of a 38-month-old boy who presented with abdominal pain in the right lower quadrant. His vital signs were stable and laboratory test findings showed no specific alterations. We detected tenderness in the right lower quadrant. A computed tomography scan showed an ileocolic intussusception with no strangulation and diffuse wall thickening of the appendix trapped within the intussusception. The patient underwent an appendectomy and manual reduction.

Reoperation of Failed Tricuspid Mechanical Prosthetic Valve Due to Pannus Formation (Pannus 형성으로 인한 삼첨판막 재치환술)

  • We performed a reoperation of failed tricuspid mechanical valve in a 63-year-old female patient because the overgrown endothelial pannus had entrapped the prosthetic leaflets. Four years ago, the patient underwent mitral and tricuspid valve replacements with 31 and 33 mm Carbomedics, respectively. The patient showed symptoms of neck vein distention, abdominal distention and peripheral edema. The chest film, echocardiography and cineangiography confirmed the diagnosis of tricuspid valve. During the operation, we found the entrapped leaflets of the tricuspid valve in a partially closed state and the endothelial pannus had overgrown into the leaflets. Carpentier-Edward bovine pericardial valve was inserted and the patient was discharged with no significant events.

A Case Report of Chengsim Yeunja-Tang for Ankylosing Spondylitis associated with Severe Ulcerative Colitis (청심연자탕(淸心蓮子湯)으로 치료하여 호전된 강직성 척추염을 동반한 중증 궤양성 대장염 1례)

  • A 33 year-old male who had been diagnosed with severe ulcerative and ankylosing spondylitis was treated and observed. Even though he had been treated with western medicine, he still complained of severe diarrhia, hemafecia, abdominal pain, general weakness, mild fever($37.8^{\circ}C$) and lower back pain. On radiologic examination, the lumbar view revealed the sacroliac joint space are irregular with adjacent bony sclerosis on both sides. He was treated with Chengsim Yeunja-Tang. Having been treated for 99 days, severity of ulcerative colitis changed from severe to mild ade, lower back pain reduced from nine to three, and all other symptoms improved.

Fatal Aortic Tumor Embolism Presenting as Acute Paraplegia

  • We report a case of fatal aortic tumor embolism presenting as acute paraplegia. A four-year-old girl was referred from a local hospital with sudden paraplegia and a poor medical condition. A neighbor had noticed her fall from a bike, and she could not walk. She had no previous illness. Emergency spine MRI revealed no remarkable findings. During the process of evaluation, her general condition deteriorated progressively. Chest and abdominal CT showed a large mass in the left lung field, and a diagnosis of aortic occlusion was made. An emergency transfemoral embolectomy was attempted. However, the patency of the aorta was not recovered. On pathological examination of tissues taken from the embolectomy, a pleuro-pulmonary blastoma was found. The patient died 22 hours after the onset of her symptoms. We describe a possible mechanism for the tumor embolism. To the best of our knowledge, this is the first case report of aortic occlusion caused by an embolic malignancy, presenting as acute paraplegia.

A Case Report of Rectal Cancer Treated by Sojukjeongwon-san. (대장암 환자의 소적정원산 및 한방치료 관찰 1례)

  • Objectives : The purpose of this study was to investigate the effect of Sojukjeongwon-san (이하 SJJWS) on a rectal cancer patient. Methods : This clinical case was analyzed on a patient who was admitted on August 11th and discharged on September 7th, 2006. We recordedhis abdominal pain, urine, anal pain, bleeding, mucous stools, frequency and hardness and amount of the stools, weight, lab follow-ups and so on. Results : After the treatment, the chief complaints of the patient, such as rectal pain, bloody excrement, loss of weight, etc., had improved. Conclusion : In this study we suggest that SJJWS reduced symptoms for a rectal cancer patient and improved the patient's quality of life.
