Objective : Erysipelas is an acute inflammation caused by pyogenic bacteria. This mainly involves the upper part of dermis. It begins as erythematous patches with tenderness, followed by fever, headache, chills and fatigue etc. It may results in edema, obstruction of lymphatics and sepsis. So this experiment is carried out for test whether the Bojeasodok-um subtracted Scrophulariae Radix, Lasiosphaera seu Calvatia, Isatidis Radix added indigo Naturalis, Lithospermi Radix have an anti-inflammatory effect and have suppression effect on immunocyte in the state of inflammation which induced by Erysipelas. Method : Experimental animals made use of 4-5 week-age(weight 20-25g) ICR(male) mouse. In the breeding farm, the lighting time was controlled from 7:00 am till 7:00 pm, the temperature was controlled So we concluded that BS is prospected as an anti-inflammatory agent to cure inflammation induced bywithin 18-23$23^{\circ}C$ and water and food were not limited.The freezing lyophilization powder which were extracted from Bojesodok-Um divided low dose group(200mg/kg-BSL) and high dose group(500mg/kg-BSH) and after melting in water, it was orally administered to the mouse. Compared with inflammation induced group which were induced by triggering-inflammation reagent Carageenan and Zymosan and normal contrast group, we measured the edema decrement effect,macrophage and spleen cell activation. Result : 1. BS has suppress inflammatory reaction induced by Carageenan. 2. BS has suppress increasing activation of abdominal cavity macrophage in the Carageenan and Zymosan induced inflammation. 3. BS has suppress increasing activation of spleen cell in the Carageenan and Zymosan induced inflammation. Based on the above result, BS was improved its suppression effect to the inflammatory reaction through the suppression of spleen cell and macrophage activation. So we concluded that BS is prospected as an anti-inflammatory agent to cure inflammation induced by Erysipelas.
The Ponciri fructus immaturus (Poncirus trifoliata Rafinesque) has been used in oriental medicine for uterine contraction, stomachache, abdominal distension and cardiovascular diseases. Two main compounds, poncirin and naringin were successfully analyzed by high performance liquid chromatography (HPLC) and carried out method validation according to ICH guideline. A successful resolution and retention times were obtained with a $C_{18}$ reversed phase column, at an $1m{\ell}min^{-1}$ flow rate, with a gradient elution of a mixture of methanol, water and acetonitrile. Poncirin and naringin showed good linearity ($R^2$ > 0.999) in relatively wide concentration ranged. The recovery of each compound was 95.81 ~ 101.48% with R.S.D. values less than 1.0%. The application of ultrasound-assisted extraction was shown to be more efficient in extracting poncirin and naringin from Ponciri fructus immaturus. The predicted optimal poncirin and naringin yield were poncirin 2.15%, naringin 1.65% under an extraction temperature of $40^{\circ}C$, an extraction time of 10 min in a solvent of 70% methanol.

Purpose: Intraabdominal abscess is one of the most common reasons for re-hospitalization after gastrectomy. This study aimed to develop a model for estimating the probability of intraabdominal abscesses that can be used during the postoperative period. Materials and Methods: We retrospectively reviewed the clinicopathological data of 1,564 patients who underwent gastrectomy for gastric cancer between 2010 and 2012. Twenty-six related markers were analyzed, and multivariate logistic regression analysis was used to develop the probability estimation model for intraabdominal abscess. Internal validation using a bootstrap approach was employed to correct for bias, and the model was then validated using an independent dataset comprising of patients who underwent gastrectomy between January 2008 and March 2010. Discrimination and calibration abilities were checked in both datasets. Results: The incidence of intraabdominal abscess in the development set was 7.80% (122/1,564). The surgical approach, operating time, pathologic N classification, body temperature, white blood cell count, C-reactive protein level, glucose level, and change in the hemoglobin level were significant predictors of intraabdominal abscess in the multivariate analysis. The probability estimation model that was developed on the basis of these results showed good discrimination and calibration abilities (concordance index=0.828, Hosmer-Lemeshow chi-statistic P=0.274). Finally, we combined both datasets to produce a nomogram that estimates the probability of intraabdominal abscess. Conclusions: This nomogram can be useful for identifying patients at a high risk of intraabdominal abscess. Patients at a high risk may benefit from further evaluation or treatment before discharge.
Pulsed Nd : YAG laser has been used to treat various soft tissue lesions. However, there have been warnings of using laser to treat peri-implantitis because the laser can alter the surface of implant and can rise its temperature so that it might be harmful to implant itself and its surrounding tissue. However, the study in vivo has not been done yet. The purpose of this study was to evaluate the biocompatibility to the pulsed Nd:YAG lased dental implant. 16 HA coated implants and 16 titanium plasma sprayed implants were used. They were divided 4 groups as unlased control, 0.3 watt, 1.0 watt and 2.0 watt lased groups after lasing. And they were implanted in the peritonial walls of 12 rabbits. The rabbits were sacrificed 6 weeks after implantation and the LM and EM specimems were made. 1. In the unlased implant group, both HA coated and titanium plasma sprayed implants were surrounded by fibroblasts and collagen fibers. No macrophages and inflammatory cells were seen. 2. In HA coated implants 0.3 watt, 1.0 watt lased group had a little inflammation, but in 2.0 watt lased group had scattered macrophages, a significantly larger number of chronic imflammatory cells were seen. 3. In titanium plasma sprayed implant 0.3 watt, 1.0 watt lased group had a little inflammation, but in 2.0 watt group had severe inflammation as in the conditions of HA coated group. 4. The inflammatory reaction of both lased HA coated and titanium plasma sprayed implant groups increased as the increase of the power of the laser.
Copper sulfate is a copper compound used widely in the chemical and agriculture industries. Most intoxication occurs in developing countries of Southeast Asia particularly India, but rarely occurs in Western countries. The early symptoms of intoxication are nausea, vomiting, diarrhea, and abdominal cramps, and the most distinguishable clue is bluish vomiting. The clinical signs of copper sulfate intoxication can vary according to the amount ingested. A 75-year old man came to our emergency room because he had taken approximately 250 ml copper sulfate per oral. His Glasgow Coma Scale (GCS) score was 14 and vital signs were blood pressure 173/111 mmHg, pulse rate 24 bpm, respiration rate 24 bpm, and body temperature $36.1^{\circ}$ .... Arterial blood gas analysis (ABGa) showed mild hypoxemia and just improved after 2 L/min oxygen supply via nasal cannula. Other laboratory tests and chest CT scan showed no clinical significance. Three hours later, the patient's mental status showed sudden deterioration (GCS 11), and ABGa showed hypercarbia. He was arrested and his spontaneous circulation returned after 8 minutes CPR. However, 22 minutes later, he was arrested again and returned after 3 minutes CPR. The family did not want additional resuscitation, so that he died 5 hours after ED visit. In my knowledge, early deaths are the consequence of shock, while late mortality is related to renal and hepatic failure. However, as this case shows, consideration of early definite airway preservation is reasonable in a case of supposed copper sulfate intoxication, because the patients can show rapid deterioration even when serious clinical manifestation are not presented initially.
In this study, we have fabricated two types of non-invasive fiber-optic respiration sensors that can measure respiratory signals during magnetic resonance (MR) image acquisition. One is a nasal-cavity attached sensor that can measure the temperature variation of air-flow using a thermochromic pigment. The other is an abdomen attached sensor that can measure the abdominal circumference change using a sensing part composed of polymethyl-methacrylate (PMMA) tubes, a mirror and a spring. We have measured modulated light guided to detectors in the MRI control room via optical fibers due to the respiratory movements of the patient in the MR room, and the respiratory signals of the fiber-optic respiration sensors are compared with those of the BIOPAC$^{(R)}$ system. We have verified that respiratory signals can be obtained without deteriorating the MR image. It is anticipated that the proposed fiber-optic respiration sensors would be highly suitable for respiratory monitoring during surgical procedures performed inside an MRI system.
Objectives The purpose of this study is to find out whether or not there are differences of effects when we count breaths on inspiration or expiration, and how differently the effects show up according to gender or Sasang Constitution Methods A group of 79 participants were enrolled and diagnosed Sasang Constitution by Sasang Constitution Diagnosis System of the Korea Institute of Oriental Medicine. Physical responses to respiratory changes were measured by Biofeedback sensors that were attached to the participants while they were seated comfortably on a sofa and breathing normally without counting for 5 minutes, counting on inspiration for 5 minutes, and counting on expiration for 5 minutes. Results and Conclusions 1. Mean heart rate and respiration rate decreased in association with breathing counting on inspiration and expiration more than breathing normally without counting. Especially respiration rate was stabilized during counting on inspiration and mean heart rate was stabilized during counting on expiration. 2. Interaction between breathing methods and gender did not appear. In comparison between gender, LF/HF ratio was higher in male during breathing normally without counting and counting on inspiration. Body temperature was higher in male during counting on inspiration and expiration. 3. Interaction between breathing methods and Sasang Constitution did not appear. In comparison between Sasang Constitution, mean heart rate was higher in Soyang-in, Soeum-in than Taeeum-in and abdominal amplitude was lower in Taeeum-in than Soeum-in.
The purpose of this study is to survey the importance of items for the physiological and pathological symptoms for estimating a health condition according to Sasang constitution to establish the SHI(Sasangin Health Index) which will reflect the concept of inherent vitality. We surveyed expert opinion with questionnaires from 20 Sasang constitution specialists. The questionnaire was composed of 57 items on physiological and pathological symptoms and specialist opinions. Each item was marked from A to E according to importance in evaluating health state in each constitution, and if the mean score of an item was over 3.0, the item was regarded as important. Important indexes among physiological symptoms were diet, digestion condition, perspiration condition, frequency of defecation, heat and cold response, and temperature of drinking water in Soeumin, pathologic perspiration and defecation condition in Soyangin, repast, perspiration condition, and the amount of drinking water in Taeeumin, and urination frequency in Taeyangin. Important indexes among pathological symptoms were sighing, indigestion, and abdominal pain in Soeumin, oral condition, chest distress, brash, and amnesia in Soyangin, eye condition, palpitation, and edema in Taeeumin, and vomiting and incapacity of the lower limbs in Taeyangin. There are different health evaluating index and priority order in it according to Sasang constitution.

Hemolymph is oxidized and darkens visibly during the collection from silkworm due to the activity of tyrosinase in it. Toxic quinones are produced by the oxidation and consequently inhibit the cell growth. Heat treatment can be used to prevent the oxidation ; however, the oxidation may occur during the collection of hemolymph before it is heat-treated. It makes the hemolymph collection difficult especially on a large-scale preparation. Hemolymphs collected from 257 different strains of silkworms were examined to select the slowly oxidized hemolymphs. Hemolymphs collected from mutant strains such as Lemone, TBO, Cre, Y4, and wEb showed relatively slow color changes. Oxidation rates of the hemolymphs were measured by the absorbance change using a spectrophotometer. The hemolymph of wEb showed the slowest oxidation. The absorbance of this mutant hemolymph reached the saturation value at 20$^{\circ}C$ in 450 min, whereas the total oxidation time of the wild-type (Baekokjam) hemolymph at the same temperature was 120 min. We tested if this mutant hemolymph is useful as a medium supplement for insect cell culture. Cell growth rate and final cell concentration in the medium supplemented with the wEb hemolymph were almost same as those in the medium supplemented with the wild-type hemolymph. Hemolymph is collected on a small scale by clipping the abdominal leg; however, this method is not appropriate fro large scale preparation. Centrifugation after chopping the silkworm hemolymph by a blending mixer is a more appropriate procedure for large scale collection. Slowly oxidized wEb hemolymph resulted in higher cell concentration than the wild-type hemolymph when hemolymph was collected by the large scale preparation method.
The diagnosis of the main complications resulting from lipoabdominoplasty has not yet been standardized. Infrared thermal imaging has been used to assess possible complications, such as necrosis and changes in micro- and macro-circulation, based on perforator mapping techniques, among others. The objective of this study was to present two clinical cases involving thermal imaging monitoring of the healing process of lipoabdominoplasty in the immediate postoperative evaluation and its preliminary results. Infrared thermography was performed 24 hours after the operation and on postoperative days 5, 25, and 27. In clinical case 1, it was found that the delta-R (∆TR)-defined as the difference in minimum temperature between the highest and lowest points in the SA3 region (caution suction area) following the classification established by Matarasso-was 0.4℃ at 24 hours after surgery and decreased to 0.1℃ on a postoperative day 5. There were no complications in this case. In contrast, in clinical case 2, the ∆TR was 1.7℃ at 24 hours after surgery (upon hospital discharge) and remained high, at 2.2℃, on postoperative day 5. A higher ∆TR was found in the second patient, who developed necrosis of the surgical wound. The ∆TR thermal index may be a new tool for predicting possible complications, complementing the clinical evaluation and therapeutic decision-making.
