말에서 흉복부의 초음파상 (Ultrasonographic Evaluation of the Thorax and Abdomen in Horse)

  • Ultrasonogram for kidney, spleens liver, intestine and heart was evaluated 11 Chriu horses and 14 Thoroughbred horse. The kidney was determined at both flan and both 17th intercostal spaces. The spleen was determined at the left 13-17 intercom spacer and the liver was determined at the right 7-13 intercostal space. The heart was determined at the right 4-6 intercostal space and left 3-6 intercostal space. The length, of rig kidney at the Thoroughbred horse, Thoroughbred foul, Cheju horse and Cheju foul were 16.2, 12.6, 13.1 and 11.2 mm, respectively. The width of right kidney at the Thoroughbed horse, Thorughbred foul, Cheju horse and Cheju foul were 5.4, 4.3, 4.6 and 4.2 mm, respectively. The depth of right kidney at the Thoroughbred horsed Thoroughbred foul, Cheju horse and Chrju foul were 5.2, 4.4, 4.5 and 4.3 mm, respectively. Similar ultrasonographic measurements were obtained for the left kidney, The left ventricular end-diastolic diameter at the Thoroughbred horse, Thoroughbred fouls Cheju horst and Cheju foul were 107, 83, 85 and 73 mm, respectively. The left ventricular end-systolic diameter were at the Thoroughbred horse, Thoroughbred foul, Cheiu horse and Cheiu foul were 63, 52, 53 and 45 mm, respectively. Also, the interventricular septum in end-diastole, interventricular septum in end-systoles left ventricular wall end-diastoles left ventricular wall end-systoles right ventricular end-diastole diameter, aorta and left atrium at t Thoroughbred horse, Thoroughbred foul, Chriu horse and Chriu foul were measured. Experimental renal stone and enterolith of colon were observed by ultrasonography.

외상성 복벽 탈장 환자에서 biologic mesh를 이용한 치료 (A Case of Traumatic Ventral Hernia Repair with a Porcine Dermal Collagen Graft (Permacol))

  • Resection of the bowel is necessary for the repair of a ventral hernia after recovery from trauma in some cases. In such instances, polyester or polypropylene meshcannot be used due to the possibility of infection; we had to use biological mesh instead. We report a case in which a traumatic hernia was repaired with Permacol (Covidien, Norwalk, CT, USA). A 42-year-old male patient had been injured by a factory machine seven months prior to admission. At that time, he had abdominal wall injury and small bowel perforation. His abdominal wall had been a defect after operation. A CT scan of the abdomen showed that the left abdominal wall, which is lateral to left rectus abdominis muscle had only one muscle layer, an external oblique muscle, and that a previous abdominal incision had a defect along the entire incision. During the exploration, 10 cm of small bowel was removed due to firm adhesion to the previous surgical scar. Permacol mesh was applied and fixed with transfascial fixations and tacks by using the intraperitoneal onlay mesh technique. There were no complications after the surgery and the patient was discharged without any problems.

신생아 위 자연천공에 대한 임상적 고찰 (Clinical Review of Spontaneous Gastric Perforation in the Newborn)

  • Spontaneous gastric perforation in the newborn is a rare disease that requires early diagnosis and prompt surgical treatment. Between 1988 and 2001 at the Department of Pediatric Surgery. Kyungpool National University Hospital, 9 cases of spontaneous gastric perforation were treated. Seven were males and two females. The mean gestational age and birth weight were 36.7 weeks and 2,455 g respectively. All patients presented with severe abdominal distention and pneumoperitoneum on cross table lateral film of the abdomen. Perforations were located on the anterior wall along the greater curvature of the stomach in six and on the posterior wall along the greater curvature in two. One case showed two sites of perforation on the anterior and posterior wall along the greater curvature. Six patients were managed with debridement and primary closure and the others with debridement and partial gastrectomy. Peritoneal drainage was not performed. There were four deaths; two from sepsis due to leakage from the anastomotic site, one as a result of acute renal failure, and the other by associated respiratory distress syndrome. Spontaneous gastric perforation in the newborn is usually located along the greater curvature. Elevated intragastric pressure is a possible cause of the perforation. Poor prognosis is related to associated diseases and prematurity.

Bowel Wall Thickening on Computed Tomography in Children: A Novel Method of Measurement and Its Clinical Significance

  • Purpose: The clinical implications of bowel wall thickening (BWT) on abdominal computed tomography (CT) among children are unknown. We aimed to suggest a new method for measuring BWT and determining its clinical significance in children. Methods: We retrospectively analyzed 423 patients with acute abdomen who underwent abdominal CT; 262 were classified into the BWT group. For this group, the pediatric radiologist described the maximal bowel wall thickness (MT), normal bowel wall thickness (mm) (NT), and their ratios for each segment of the bowel wall. Results: In the thickened bowel walls, the thickness differed significantly between the small bowel (6.83±2.14 mm; mean±standard deviation) and the colon (8.56±3.46 mm; p<0.001). The ratios of MT to NT in the small bowel (6.09±3.17) and the colon (7.58±3.70) were also significantly different (p<0.001). In the BWT group, 35 of 53 patients had positive fecal polymerase chain reaction results; 6 patients infected with viruses predominantly had BWT in the small intestine, while the terminal ileum and the colon were predominantly affected in 29 patients with bacterial infections. In the initially undiagnosed 158 patients with BWT, the symptoms improved spontaneously without progression to chronic gastrointestinal disease. Conclusion: This study provides a clinical reference value for BWT in the small intestine and colon using a new method in children. The BWT on abdominal CT in children might indicate nonspecific findings that can be observed and followed up without additional evaluation, unlike in adults.

Subcutaneous Sparganosis on Abdomen Mimicking Multiple Lipomas

  • Human sparganosis is a food-borne zoonosis mainly caused by the plerocercoid belonging to the genus Spirometra. The most common clinical sign of sparganosis is a subcutaneous mass in the trunk including abdominal or chest wall. The mass may be mistaken for a malignant tumor, thereby causing difficulty in terms of diagnosis and treatment. A 66-year-old woman visited our clinic for the removal of a lipoma-like mass. It was movable, hard, and painless. we identified 2 white mass, measuring $0.2{\times}4cm$ and $0.2{\times}1cm$. Pathologic finding indicated the white mass was a sparganum. She recalled having eaten a raw frog approximately 60 years before. A 35-year-old who lived North Korea was also presented to our clinic with an asymptomatic nodule on her abdomen. Intraoperatively, we found sparganum approximately 24 cm size. Subcutaneous masses are associated with clinical signs of inflammation or they may mimic a soft tissue neoplasm. While the incidence rate of sparganosis has decreased with economic development and advancements in sanitation, surgeons still encounter patients with sparganosis in the clinical setting. Therefore, a careful history is required in order to diagnose sparganosis.

The three-point crossover technique for immediate reconstruction of the umbilicus

  • The umbilicus is an important aesthetic component of the abdomen; therefore, its absence is both cosmetically and psychologically distressing to the patient. However, loss of the umbilicus during abdominal surgical procedures is often unavoidable. Umbilical reconstruction is aimed at obtaining a natural, three-dimensional appearance. We propose a simple method for immediate umbilical reconstruction with good long-term results. This technique was used successfully on a patient who underwent tumor excision. A 49-year-old woman presented with a large mass, measuring 5.8×4.0 cm, on her umbilicus. The mass, an epidermal cyst, developed after laparoscopic uterine myomectomy 5 years earlier. Complete excision of the mass resulted in a large defect, and immediate umbilical reconstruction was planned. Our procedure involved apposing and anchoring two opposing flaps onto the abdominal wall, so that the umbilicus would retain its depth over a long period of time. Negative-pressure wound therapy was applied for 72 hours as a mild compressive dressing. No complications were encountered. The healing process was uneventful and the aesthetic outcome was pleasing; a natural-appearing navel was created. The patient was satisfied with the end result. This technique provides a permanent and sufficient depression for the umbilicus.

Organ dose conversion coefficients in CT scans for Korean adult males and females

  • Dose monitoring in CT patients requires accurate dose estimation but most of the CT dose calculation tools are based on Caucasian computational phantoms. We established a library of organ dose conversion coefficients for Korean adults by using four Korean adult male and two female voxel phantoms combined with Monte Carlo simulation techniques. We calculated organ dose conversion coefficients for head, chest, abdomen and pelvis, and chest-abdomen-pelvis scans, and compared the results with the existing data calculated from Caucasian phantoms. We derived representative organ doses for Korean adults using Korean CT dose surveys combined with the dose conversion coefficients. The organ dose conversion coefficients from the Korean adult phantoms were slightly greater than those of the ICRP reference phantoms: up to 13% for the brain doses in head scans and up to 10% for the dose to the small intestine wall in abdominal scans. We derived Korean representative doses to major organs in head, chest, and AP scans using mean CTDIvol values extracted from the Korean nationwide surveys conducted in 2008 and 2017. The Korean-specific organ dose conversion coefficients should be useful to readily estimate organ absorbed doses for Korean adult male and female patients undergoing CT scans.

Effect of Electrical Muscle Stimulation Belt for Abdominal Muscles Activation

  • Objective: The purpose of this study is to observe the change in the thickness of abdominal muscles when electrical muscle stimulation (EMS) is applied to the abdomen during rest and abdominal muscle exercise to investigate the effect of EMS applied to the abdomen on the superficial and deep muscles thickness. Design: Cross sectional design. Methods: Twenty healthy subjects participated in this study. Subjects were performed resting position, resting position with EMS, curl-up and curl-up with EMS. The electrode of the EMS belt is attached to the abdominal wall between the 12th rib and iliac crest. The thickness of abdominal muscles including rectus abdominis (RA), external oblique (EO), internal oblique (IO), and transverse abdominis (TrA) were captured in each position by ultrasound image during expiration. All subjects were performed four positions randomly. Data were analyzed using repeated ANOVA with the level of significance set at 𝛼=0.05. Results: The muscle thickness of RA, EO, IO and TrA were significantly different at each position (p<0.05). The thickness of all abdominal muscles increased significantly when curl-up than curl-up with EMS. Both RA and EO thickness were significantly increased at resting position than resting position and EMS were combined(p<0.05). But IO and TrA thickness were decreased at resting position when EMS were combined. Conclusions: The results suggest that EMS activates superficial abdominal muscles RA and EO. Therefore, abdominal strengthening exercise combined EMS can activate abdominal muscles and can be applied to various patients and rehabilitation in clinical practice.

트로카 삽입 부위에 생긴 림프관 확장을 동반한 기생 평활근종: 증례 보고 (Parasitic Leiomyoma with Lymphatic Dilatation in Trocar Port-Site of Abdominal Wall: A Case Report)

  • 자궁 근종은 여성에서 가장 흔한 양성 골반 강 내 종양이며 증상이 있을 경우 수술적 제거를 하게 된다. 내시경 수술이 발달하게 되면서 포트 삽입 부위의 기생 평활 근종들이 보고되기 시작하였다. 40세의 여자 환자가 왼쪽 아랫배에서 만져지는 종괴를 주소로 외래로 내원하였고 2년 전 내시경 근종 제거를 받은 과거력이 있었다. 조영증강 CT와 MRI를 시행하여 해당 종괴에 대한 평가를 시행하였으며 수술적 제거를 시행하였다. 병리 검사에서 해당 종괴는 기생 평활 근종으로 최종 진단되었는데 이는 이전 내시경 근종 제거술을 시행한 환자에서 수술부위에 만져지는 종괴를 주소로 내원하였을 때 반드시 기생 평활 근종을 감별 진단으로 고려하여야 함을 시사한다.

Nineth Rib Syndrome after 10th Rib Resection

  • The $12^{th}$ rib syndrome is a disease that causes pain between the upper abdomen and the lower chest. It is assumed that the impinging on the nerves between the ribs causes pain in the lower chest, upper abdomen, and flank. A 74-year-old female patient visited a pain clinic complaining of pain in her back, and left chest wall at a 7 on the 0-10 Numeric Rating scale (NRS). She had a lateral fixation at T12-L2, 6 years earlier. After the operation, she had multiple osteoporotic compression fractures. When the spine was bent, the patient complained about a sharp pain in the left mid-axillary line and radiating pain toward the abdomen. On physical examination, the $10^{th}$ rib was not felt, and an image of the rib-cage confirmed that the left $10^{th}$ rib was severed. When applying pressure from the legs to the $9^{th}$ rib of the patient, pain was reproduced. Therefore, the patient was diagnosed with $9^{th}$ rib syndrome, and ultrasound-guided $9^{th}$ and $10^{th}$ intercostal nerve blocks were performed around the tips of the severed $10^{th}$ rib. In addition, local anesthetics with triamcinolone were administered into the muscles beneath the $9^{th}$ rib at the point of the greatest tenderness. The patient's pain was reduced to NRS 2 point. In this case, it is suspected that the patient had a partial resection of the left $10^{th}$ rib in the past, and subsequent compression fractures at T8 and T9 led to the deformation of the rib cage, causing the tip of the remaining $10^{th}$ rib to impinge on the $9^{th}$ intercostal nerves, causing pain.
