Adolescent obesity and the elevation of aminotransferase levels (청소년 비만과 혈청 아미노 전이 효소 증가)

  • Purpose : The prevalence of obesity and nonalcoholic steatohepatitis(NASH) was increased in adolescents. This study was performed to observe the prevalence of elevated aminotransferase levels in adolescents and to assess the correlations between aminotransferase levels and obesity related parameters(body mass index, waist circumference, hip circumference, waist to hip ratio and insulin level). Methods : We obtained weight, height, waist circumference and hip circumference from 2,417 male and 1,219 female adolescents. Mean age was $15.7{\pm}0.7$ years old. We measured fasting insulin, aspartate aminotransferase(AST) and alanine aminotransferase(ALT). Obese and overweight were defined as body mass indices(BMI) of more than the 95th, and 85th-94th percentiles, respectively, for age and sex. Results : The number of adolescents with obesity is 324(8.9 percent). 414(11.4 percent) subjects belonged to the overweight group. The average ALT level of obese, overweight and control groups were significantly different(obese : $32.1{\pm}34.3U/L$, overweight : $19.6{\pm}13.6U/L$, control : $12.7{\pm}6.7U/L$, P<0.001). The average AST level was also different according to group(obese : $27.8{\pm}16.5U/L$, overweight : $22.8{\pm}8.6U/L$, control : $20.8{\pm}8.5U/L$, P<0.001). AST and ALT were correlated with anthropometric parameters and insulin level. After multiple regression analysis, waist circumference was the significant predictive value for AST(r=0.234, P<0.001). Waist circumference, BMI and insulin levels were significant predictive values for ALT(r=0.435, P<0.001). Conclusion : The prevalence of abnormal aminotransferase was higher in the obese and overweight groups than control group. Waist circumference was useful to predict abnormal aminotransferase levels.

Relationships with Alanine Aminotransferase Activity, and Anthropometric and Biochemical Measures in Obese Children (단순 비만아에서 간효소치와 신체 계측 및 생화학 측정치와의 관련성)

  • Purpose: To investigate the physical and biochemical parameters related with elevated serum alanine aminotransferase (ALT) levels in obese children. Methods: One hundred forty-two obese or overweight children who visited the out-patient clinics of Chungnam National University Hospital between January 2006 and August 2008 were enrolled. Physical measures and biochemical tests were performed in all patients. Liver sonography was performed in 43 patients. They were divided into the following 2 groups based on ALT levels: group I, normal ALT levels (n=65); and group II, elevated ALT levels (n=77). We compared the physical measures, biochemical results, and ultrasonographic findings of the livers in both groups. Other causes of elevated serum ALT levels were ruled out. Results: The male-to-female ratios were 1.6:1 in group I and 7.6:1 in group II. Among physical parameters, the waist circumference-to-height ratio and hip circumference-to-height ratio were significantly higher in group II (p=0.001 and 0.046, respectively). Among biochemical parameters, aspartate aminotransferase (AST), ${\gamma}$-glutamyltransferase (${\gamma}$-GT), and total cholesterol levels were significantly higher in group II (p<0.001, 0.001, and 0.001, respectively). The AST/ALT ratio was <1 (mean, 0.55) and statistically lower in group II. There was a positive correlation between the serum ALT level and ${\gamma}$-GT (p<0.001, r=0.750), and a positive correlation between the serum ALT level and the waist circumference-to-height ratio in group II (p<0.001, r=0.401). Conclusion: The results suggest that the waist circumference-to-height ratio and ${\gamma}$-GT may be associated with elevated alanine aminotransferase activity in obese children.

  • The purpose of this study was to obtain reference data of parameters for hematological health diagnosis in cultured freshwater fish and also evaluate application of automatic dry-type chemistry analyzer (FUJI DRI-CHEM 3000) used to those blood tests. A blood profile of total 200 fish for rainbow trout (Onchorhynchus mykiss), israel carp (Cyprinus carpio), tilapia (Oreochromis niloticus) and eel (Anguilla japonica) cultured in Inland Fisheries Research Institute of NFRDI was determined by hemoglobin (Hb) and plasma chemistry tests: total protein (TP), albumin (ALB), alkaline phosphatase (ALP), blood urea nitrogen (BUN), lactate dehydrogenase (LDH), triglyceride (TG), total cholesterol (TCHO), creatinine (CRE), aspartate aminotransferase (AST), alanine aminotransferase (ALT), glucose (GLU). The values of ALT, TG, LDH, ALB, TCHO, AST and ALP were outside from the minimum and/or maximum of the established detectable range of the analyzer. ALT and TG were not detectable in the range of 67%~61.5%. LDH, ALB and TCHO were not detectable in the range of 36~17%. AST and ALP were not detectable in the range of 5.5~0.5%. However, the values of BUN, CRE, GLU, Hb and TP were below the detectable limits of the analyzer.

The Abnormal Rates of Blood Pressures and Blood Biochemical Properties with BMI in Health Checkup Examinees (건강검진 수검자들의 BMI에 따른 혈압, 혈액 및 혈액생화학적지표의 비정상치 분포)

  • The purpose of this study was to investigate the association of blood pressures, hematological and biochemical properties of blood according to the degree of obesity in health checkup examinees. Study subjects were 3,731 adults of 20 years and over (2,312 males, 1,419 females), who underwent health package check-up at the Korea Health Management Association from Jan. 2007 to Dec. 2009. As a result, the rates according to the degree of obesity of study subjects were 4.6% in low weight group, 44.3% in normal weight group, 25.0% in over weight group and 26.1% in obesity group. The over weight group were significantly higher in 40's age group than other groups, and the obesity group were significantly higher in 50's age group than other groups. The abnormal rates of SBP, DBP, TG, TC, HDL-C, LDL-C, ALT, AST in male were significantly higher than that of female. With respect to degree of BMI, the abnormal rates of SBP, DBP, TG, TC, HDL-C, LDL-C, ALT, AST, GGT, ALP were significantly increased with higher level of BMI. The age-adjusted odds ratio in the abnormal level of SBP, DBP, TG, TC, HDL-C, LDL-C, AST, ALT and GGT were significantly increased in overweight group and obese group than that of normal weight group.

Clinical study of the 15patients survived after acute paraquat intoxication (급성 파라콰드중독후 생존한 15예 환자의 임상적 고찰)

  • From January 1994 to April 1997, there was 15 survivals who was admitted to the department of internal medicine, Wonkwang Oriental Medicine' Hospital in Cheun-ju, after ingestion of paraquat, and treated with Oriental and western medicine therapy. For the patients, I investigated clinical symptoms, gastroduodenoscopy, intake by oral and parenteral route, and output by urine and stool, serum ALP, AST. ALT, Bilirubin, BUN, Creatinine level and urine analysis. On admission day, the LFT level was as follows. The serum mean ALP, AST, ALT, Total Bilirubin and Direct Bilirubin was 10.05${\pm}$2.75 KAU, 66.67${\pm}$9.88 IU/L, 43.80${\pm}$7.74 IU/L, 1.89${\pm}$1.22 ㎎/dl and 1.10${\pm}$1.14 ㎎/dl respectively. After that day, administered Gamdutang and checked the mean LFT level regullary. Until the 3rd day, the mean ALP, AST, ALT, Total Bilirubin and Direct Bilirubin level was 11.0l${\pm}$3.16 KAU, 56.47${\pm}$7.19 IU/L, 59.00${\pm}$7.57 IU/L, 2.54${\pm}$1.78 ㎎/dl, 1.64${\pm}$1.59 ㎎/dl respectively. From 4th day to 7th day, the mean ALP; AST, ALT, Total Bilirubin and Direct Bilirubin level was 12.51${\pm}$3.49 KAU, 77.85${\pm}$7.17 IU/L, 58.00${\pm}$9.09 IU/L, 2.54${\pm}$1.97 ㎎/dl, and 1.80${\pm}$1.81 ㎎/dl respectively. From 8th day to 10th day, the mean ALP, AST, ALT, Total Bilirubin and Direct Bilirubin level was 12.43${\pm}$3.14 KAU, 41.13${\pm}$6.49 IU/L, 50.40${\pm}$7.17 IU/L, 1.66${\pm}$1.90 ㎎/dl and 1.14${\pm}$1.50 ㎎/dl respectively. From 11th day to 14th day, the mean ALP, AST, ALT, Total Bilirubin and Direct Bilirubin level was 12.30${\pm}$3.25 KAU, 31.07${\pm}$3.85 IU/L, 43.33${\pm}$5.49 IU/L, 1.62${\pm}$1.95 ㎎/dl, 1.17${\pm}$1.71 ㎎/dl respectvely. On admission day, the mean RFT level as follows. Serum BUN and Creatinine level was 28.73${\pm}$5.19 ㎎/dl and 1.82${\pm}$1.27 ㎎/dl respectively. After that day, administered Gamdutang and checked the mean RFT level regullary. Until the 3rd day, the mean BUN and Creatinine level was 32.12${\pm}$5.65 ㎎/dl and 2.31${\pm}$0.45 ㎎/dl respectively. From 4th day to 7th day, the mean BUN and Creatinine level was 31.07${\pm}$5.47 ㎎/dl and 1.92${\pm}$0.79 ㎎/dl respectively. From 7th day to 10th day, the mean BUN and Creatinine level was 17.47${\pm}$3.57 ㎎/dl and 1.33${\pm}$0.59 ㎎/dl respectively. From 11th day to 14th day, the mean BUN and Creatinine level was 11,93${\pm}$3.16 ㎎/dl, 1.27${\pm}$0.38 ㎎/dl respectively.

Protective effects of Jijang-kimchi extracts on diabetes mellitus and alcoholic liver injury in laboratory rats (랫드에서 김치가 당뇨병 및 알코올성 간 손상 예방에 미치는 효과)

  • The current study was carried out to investigate the effect of oral administration for 30 days of the Jijang kimchi extracts on prevention of diabetes, alcoholic liver injury and reduction of blood lipids in laboratory rats with alcoholic liver injury and diabetes induced by streptozotocin. In a diabetic model animals, the blood lipid profile, ALT, and AST levels were lower in kimchi extract groups compared to DC (diabetes control) group, and blood glucose level of DCJK (DC+oral administration with Jijang kimchi extracts) group was lower than that of DCCK (DC+oral administration with commercial kimchi extracts) group. Insulin levels were increased in order of NC (normal control), DCJK > DCCK > DC groups. In alcoholic liver injury model animals, ALT, AST and bilirubin were lowed in order of AC (alcohol group received 1 bottle of soju) > ACCK (1 bottle of soju plus oral administration with commercial kimchi extracts) ACJK (AC plus oral administration with Jijang kimchi extracts) > NC groups. In the clinical pathologic findings of liver tissue, AC group was severely injured, and tended to be improved in groups eating a 1 bottle of soju plus oral administration with kimchi extracts, especially Jijang kimchi extract group. The results suggest that eating Jijang kimchi can improve insulin secretion ability while lowering blood lipid profile, blood sugar and ALT, AST, and bilirubin levles in diabetic and alcoholic liver injury model animals.

Influence of Protaetia brevitarsis Extract on Liver Damage Induced by Carbon Tetrachloride and Ethanol in Rats (에탄올 및 사염화탄소의 긴 손상에 미치는 흰점박이꽃무지 추출물의 영향)

  • Protaetia brevitarsis has been utilized as an ingredient of the description for the treatment of patients with chronic hepatic diseases in oriental medicine. This study was attempted to investigate whether Protaetia brevitarsis extract(PBE) protects or modulates the liver injuries induced by carbon tetrachloride or ethanol in Sprageue-Dawley rate. The liver injuries of rats induced by the treatment of carbon tetrachloride or ethanol were manifested by the observation of the significant changes in liver weight, serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) activities, serum thiobarbituric acid reactive substances (TBARS), and microsomal detocification enzymes(cytochrome P_450), cytochrome b$_{5}$, and cytochrome b$_{5}$ reductase).The effect of PBF on the liver damage induced by the chemicals was evaluated with the extent modulated in change of biochemical parameters above. Exposure to ethanol alone resulted a significant change in the ration of liver per body weight, ALT activity, and microsomal detoxification enzymes (cytochrome P_450, cytochrome b$_{5}$, and cytochrome b$_{5}$ reductase), but did not significantly changes in the levels of serum AST activity and TBARS. Pretreatment coith PBE did not modulate the alteration of the ratio of liver to body weigth, and the activities of serum aminotransferascs (AST. ALT), TBARS, and micro somal detoxification enzyme (cytochrome p_450, cytochrome b$_{5}$,and cytochrome b$_{5}$ reductase. These result suggested that PBE has not appreciable therapeutic effect on carbon tetrachloride or ethanol induced hepatotoxicity.

Protective Effect of White-Skinned Sweet Potato (Ipomoea batatas L.) from Indonesia on Streptozotocin-Induced Oxidative Stress in Rats (흰 쥐에서 streptozotocin으로 유발된 산화적 스트레스에 대한 인도네시아산 white-skinned sweet potato (WSSP, Ipomoea batatas L.)의 보호효과)

  • Sweet potato (Ipomoea batatas L.) is widely used in Indonesia and other countries as a traditional medicine for the treatment of diabetes mellitus (DM). The MeOH extract of white skinned sweet potatoes (WSSP) was administered orally in doses of 100 and 200 mg/kg body weight in streptozotocin (STZ)-induced diabetic rats. Experimental diabetes was induced by a single dose of STZ (45 mg/kg, i.p.) injection. Oxidative stress was measured by tissue lipid peroxide (LPO) levels, serum aspartate transaminase (AST), alanine transaminase (ALT), total triglyceride (TG), total cholesterol (TC) and by antioxidative enzymatic activities of superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GPx) and glutathione S-transferase in the liver. An increase in blood glucose, LPO level, AST, ALT, TG and TC levels was observed in the STZ-induced diabetic rats. Administration of MeOH extract of WSSP at a dose of 200 mg/kg for two weeks caused a significant reduction in blood glucose, LPO levels, AST, ALT, TG and TC levels in the STZ-induced diabetic rats. Furthermore, oral administration of MeOH extract showed significant improvement in the activities of antioxidant enzymes (SOD, GPx, and CAT) compared to STZ-induced diabetic rats. In conclusion, the obtained results clearly indicate the role of oxidative stress in the induction of diabetes, and that the protective effects of MeOH extracts of WSSP could be used to benefit diabetic patients.

Effect of coffee and green tea consumption on liver enzyme and metabolic syndrome in Korean (한국인에서 커피 및 녹차의 섭취빈도가 간염증 수치 및 대사증후군에 미치는 영향)

  • We investigated whether coffee and green tea consumption reduced the risk of elevated alanine aminotransferase(ALT)/aspartate aminotransferase(AST) activity and the prevalence of metabolic syndrome. Participants were 5,283 adults, aged 19-79 years, in the Third Korean National Health and Nutrition Examination Survey, with excessive alcohol consumption, overweight, viral hepatitis, metabolic syndrome. Increased coffee and green tea consumption was not associated with decreased serum ALT. However, amount of coffee consumption had negative correlation with serum AST activity. Moreover, coffee consumption reduced the prevalence of metabolic syndrome (p for trend <0.001). Hypertension, impaired glucose metabolism and dyslipidemia was involved as subgroup of metabolic syndrome. Comparing persons who drank more than 2 cups per day with less than 1 cup per day, the prevalence of all subgroups was declined significantly. In this large, national, population-based study, consumption of coffee was associated with lower the risk of metabolic syndrome.

Effects of Storing Time on the Values of the Clinical Biochemistry in Sprague-Dawley(SD) Rats (랫드 혈청의 저장기간에 따른 혈액생화학치 변화)

  • The present study was undertaken to compare the variation on serum biochemical values by storage in the rats. Sera were prepared from 30 Sprague-Dawley rats of each sex. 5 aliquots from each serum were placed in a -80$^{\circ}C$ freezer with the exception of I aliquots which was analyzed immediately. The analysis was performed on the following months; 1, 2, 3, 6, and 12 months after freezing. The parameters measured) were aspartate aminotransferase(AST), alanine aminotransferase(ALT), alkaline phosphatase(ALP), blood urea nitrogen(BUN) creatinine(CRE), glucose(GLU), total cholesterol(TCHO), triglyceride (TG), total protein(TP), albumin(ALB), total bilirubin(TBIL), calcium(Ca$\^$++/), inorganic phosphorus(IP), creatine kinase (CK), phospholipid(PL), albumin-globulin ratio(A/G), sodium(Na$\^$+/), potassium(K$\^$+/), and chloride(Cl$\^$-/) The statistical analysis with Repeated Measures ANOVA, did not show statistical significance in the parameters of AST, ALT, BUN, TG, CK, A/G, Na$\^$+/ of 1 month freezed sera, in those of AST, TG, CK, K$\^$+/) of 2 month freezed sera, in those of AST, ALT, BUN, CRE, TCHO, TP, TBIL, CK, PL, Na$\^$+/), K$\^$+/), Ct on month fteezed sera, in those of Cl$\^$-/ of 6 month fteezed sera, and in those of ALT, TG, ALB of 12 month freezed sera in male SD rats. On the other hand, it did not show statistical significance in the parameters of AST, ALT, ALP, BUN, GLU, TCHO, TG, TBIL, CK, PL, A/G, Na$\^$+/ of 1 month freezed sera, in those of AST, TCHO of 2 month freezed sera, in those of AST, BUN, CRE, TCHO, TP, TBIL, CK, PL of 3 month freezed sera, in those of TCHO, IP, PL of 6 month freezed sera, and in those of ALB of 12 month freezed sera in female SD rats. On the basis of the results, although there are some statistical variations in the biochemical values of the sera, it is suggested that if sera are analysed at the same time before 12 months storage in a -80 $^{\circ}C$ freezer, the storing time does not affect the biochemical evaluation of the sera in SD rats.
