Purpose : The purpose of this study was to examine the causes, clinical courses and outcomes in children with acute respiratory distress syndrome(ARDS), and evaluate the physiologic variables as prognostic factors in the patients. Methods : Retrograde medical chart review was carried out in 24 patients who were diagnosed with ARDS at the pediatric intensive care unit(PICU) during 20-month period. Results : The incidence of ARDS among all PICU admission was 3.7 percent and the mortality rate was 37.5 percent, which was 14.8 percent of overall deaths in PICU. The most common causes of ARDS were pneumonia and sepsis. We found significant differences between survivors and nonsurvivors in $PaO_2/FiO_2$ ratio(P/F ratio), alveolar arterial oxygen gradient and oxygenation index(OI) on the second day from the onset of ARDS. Therapies for ARDS such as high frequency oscillator ventilation(HFOV), recruitment maneuver and low dose corticosteroid improved the P/F ratio and OI, especially in survivors. Conclusion : The mortality rate of children with ARDS was 37.5 percent; an important cause of death in PICU. HFOV, recruitment maneuver and low dose corticosteroid seemed to be effective in pediatric ARDS. The P/F ratio, alveolar arterial oxygen gradient and OI on the second day from the onset of ARDS may be useful as prognostic factors.
Background : In order to elucidate one of the pathogenic mechanisms of ARDS associated with pulmonary surfactant and oxidant injury, acute lung injury was induced by N-nitroso N-methylurethane (NNNMU). In this model, the role of phospholipase $A_2$ ($PLA_2$), surfactant, gamma glutamyl transferase (GGT) and morphology were investigated to delineate one of the pathogenic mechanisms of ARDS by inhibition of $PLA_2$ with high dose of dexamethasone. Method: Acute lung injury was induced in Sprague-Dawley rats by NNNMU which is known to induce acute lung injury in experimental animals. To know the function of the alveolar type II cells, GGT activity in the lung and bronchoalveolar lavage was measured. Surfactant phospholipid was measured also. $PLA_2$ activity was measured to know the role of $PLA_2$ in ARDS. Morphological study was performed to know the effect of $PLA_2$ inhibition on the ultrastructure of the lung by high dose of dexamethasone. Results : Six days after NNNMU treatment (4 mg/kg), conspicuous pulmonary edema was induced and the secretion of pulmonary surfactant was decreased significantly. In the acutely injured rats' lung massive infiltration of leukocytes was observed. At the same time rats given NNNMU had increased $PLA_2$ and GGT activity tremendously. Morphological study revealed bizarre shaped alveolar type II cells and hypertrophied lamellar bodies in the cytoplasm of the alveolar type II cells. But after dexamethasone treatment (20 mg/kg, for six days) in NNNMU-treated rats, these changes were diminished i.e. there were decrease of pulmonary edema and increase of surfactant secretion from alveolar type D cells. Rats given dexamethasone and NNNMU had decreased $PLA_2$ and GGT activity in comparison to NNNMU induced ARDS rats. Conclusion : Inhibition of $PLA_2$ by high dose of dexamethasone decreased pathological findings caused by infiltration of leukocytes and respiratory burst. Based on these experimental results, it is suggested that an activation of $PLA_2$ is the one of the major factors to evoke the acute lung injury in NNNMU-induced ARDS rats.

Chlorinated aliphatic hydrocarbons (CAHs) especially perchlorethylene (PCE) and trichlooethylene (TCE) are common groundwater contaminants in Korea. PCE and TCE were often reductively dechiorinated in an aquifer. Several isolates dechlorinate PCE to TCE or cis-1,2 dichloroethylene (c-DCE) were obtained from contaminated and pristine sites in USA and Europe. However in Korea, no information on indigenous microorganism being involved in reductive dechlorination of PCE and TCE is available and different dechlorinating microorganisms might be reside in Korea, since geochemical, and hydrogeological conditions are different, compared to those in the other sites. So we evaluate that: 1) if reductive dechlorinating microorganisms are present in PCE-contaminated site in Korea, 2) if so, what kinds of microorganisms are present; 3) to what extent PCE is reductively dechlorinated. As a results in some PCE-contaminated aquifers in Korea other dechlorinating microorganisms but Dehalococcoides ethenogenes may be responsible for PCE dechlorination. More detailed molecular works are required to evaluate that different dechlorinating microorganisms would reside in Korea.
A model is developed to predict the long-term beach evolution near the long groin considering the combined effects of variation of sea level, wave refraction and diffraction. A numerical solution for this problem is solved by considering the equation as a system subject to the boundary condition for longshore transport rate. One possible method is the centered Crank-Nicolson type implicit scheme. The results which ard obtained by applying this numerical model at Songdo beach, Pohang are as follows. Owing to the approximation used in the calculation of the refraction and diffraction coefficients, the discrepancy between the predicted and actual shoreline occurs to the interior of long groin. However, the shape of shoreline at the exterier of long groins agrees well.

False-tripping of MCCB(Molded Case Circuit Breaker) and ELB(Earth Leakage Circuit Breaker), using to protect overload and ground-fault accidents in power system raise direct and indirect losses like as information loss, facilities damage, slipup of goods production in industry place. Cause of false-tipping of MCCB and ELB is various and cause analysis of false-tripping is mostly made up by inference because trip is not reappeared. This paper investigated and analyzed causes and influences of false-tripping of MCCB and ELB by making up a question to safety managers of two hundred and eighteen electrical facilities for private use. Also, the necessity of experimental study on the effect of harmonics on false-tipping of MCCB and ELB was presented.
We build up distributed database of medical picture and design and realize H/W & S/W of special image workstation. We build up high speed image transmission system for distributed database and retrieval of various medical pictures in w ard through image transmission system and realize integrated image diagnosis. This system improves medical service by speedy diagnosis and enables more precise diagnosis by integrated image diagnosis through distributed database. In economical view this system curtails huge cost of film processing and transmission, which make medical expense cheaf, because it does not use film. We built up PACS in pediatric hospital of Seoul National University Hopital and tested the system with various medical pictures and showed that speedy integrated image diagnosis is possible.
The thioxanthylium ion ard anthracenium ion have coplanar geometries. The empty $P_z$-orbitals of the positive centers in the cations, therefore, overlap with the ${\pi}$-orbitals of the fused benzen rings. These cations have aromatic characters, obeying Huckel's (4n + 2)${\pi}$ rule. The contribution of the sulfur atom to aromatic stabilization was estimated by comparing the ${^19F}$-nmr chemical shifts of the thioxanthylium ions and anthracenium ions.
This paper presents a technique that can obtain an optimal solution for the Security-Constrained Economic Dispatch (SCED) problems using the Interior Point Method (IPM) while taking into account of the power flow constraints. The SCED equations are formulated by using only the real power flow equations from the optimal power flow. Then an algorithm is presented that can linearize the SCED equations based on the relationships among generation real power outputs, loads, and transmission losses to obtain the optimal solutions by applying the linear programming (LP) technique. Finally, the application of the Primal Interior Point Method (PIPM) for solving the optimization problem based on the proposed linearized objective function is presented. The results are compared with the Simplex Method and the Promising results ard obtained.

Thermal cracks ard occured when thermal stress due to the hydration of cement exceeds the tensile strength of concrete. Since cracking causes poor durability of concrete, the effect of thermal cracking should be included for the desing and construction of massive concrete structures. In this study, an experiments are performed for the investigation of time dependent temperature and thermal stress of massive concrete structure at early ages. In order to measure temperatures and thermal stresses, concrete stress meter, embedded strain meter, non-stress meter, and thermocouples are used. Based on the analyses of measured thermal stress data, measured values by concrete stress meter are more reliable than those by embedded strain meter and non-stress meter, And measured values by concrete stress meter are compared with the calculated values by FEM program developed by DICT (DWTS2D). Calculated values by DWTS2D show good agreement with measured values.
Introduction: Khon Kaen Cancer Registry (KKCR) was established in 1984. KKCR aims to collect all cancer cases in Khon Kaen Province. The poorly qualified data may lead to distort the cancer burden and misinterpretation of policy maker. Objective: To assess data quality in childhood cancer between 1990 and 2007 in Khon Kaen Province, Thailand. Materials and Methods: Data of childhood cancer cases aged less than 20 years diagnosed during 1990-2007 were retrieved from the population-based data set of KKCR. All childhood cancer data were verified before data entry. Internal consistency, percentage of morphological verification (MV%) and cancer cased of the basis of diagnosis by death certificate only (DCO%) were evaluated. The age-adjusted rate (ASR) was calculated by standard method. Results: The data of childhood cancer from KKCR is acceptably qualified which reflects the quality of the whole registration.
